Objective:To explore the effect of mind mapping based on evidence-based theme in health education of patients undergoing thoracentesis and drainage.Methods:From September 2020 to December 2021, 78 patients with pleural effusion and thoracentesis drainage diagnosed by infection in Beijing Chaoyang Hospital affiliated to Capital Medical University were selected by convenience sampling as the research object. The patients from September 2020 to March 2021 were set as the control group, and the patients from May to December 2021 were set as the observation group, with 39 cases each. The control group was given traditional health education, while the observation group received evidence-based health education with mind mapping. The health education knowledge examination score, self-care ability, adverse event rate and nursing satisfaction were compared between the two groups.Results:The scores of health education knowledge examination in the observation group were higher than those in the control group before , 24 hours after catheterization and before decannulation, with statistically significant differences ( P<0.01) . The total score and each dimension score of self-care ability in the observation group were higher than those in the control group, with statistically significant differences ( P<0.01) . The incidence of adverse events in the observation group was lower than that in the control group, and the score of Nursing Satisfaction Questionnaire was higher than that in the control group, with statistical differences ( P<0.05) . Conclusions:The mind mapping on the basis of evidence-based themes can effectively improve patients' health education knowledge and self-care ability, reduce the incidence of adverse events, and increase nursing satisfaction.
3.Study on protective effect and mechanism of phenolic acid components from Salvia deserta Schang on oxidative injury of HK -2 cells

OBJECTIVE To study the protective effect of phenolic acid components from Salvia deserta Schang on the oxidative stress injury of human renal tubular epithelial cells HK -2 induced by high glucose and high fat . METHODS HK-2 cells were divided into control group ,model group ,canagliflozin group (positive control group ,15 μmol/L),purified product of phenolic acids from S. deserta Schang group (10.8 μg/mL),4 monomers group (salvianic acid ,protocatechuic aldehyde ,caffeic acid,rosmarinic acid ,50 μmol/L). In addition to the control group ,cell injury model of high glucose and high fat was established in other groups (500 μmol/L palmitic acid+ 30 mmol/L glucose for 48 h)and cultured for 48 h. The cell apoptotic rate ,the contents of malondialdehyde (MDA)and glutathione (GSH),and the activity of superoxide dismutase (SOD)were detected in each group ; the expression levels of nuclear erythroid 2-related factor 2(Nrf2),Kelch-like ECH -associated protein 1(Keap1)protein,heme oxygenase-1(HO-1)and NADH :quinone acceptor oxidoreductase 1(NQO1)were determined in above 5 groups(except for salvianic acid ,protocatechuic aldehyde ,caffeic acid ). RESULTS Compared with control group ,the apoptotic rate of HK -2 cells in model group was increased significantly (P<0.01);the content of MDA was increased significantly (P<0.01),while the content of GSH and the activity of SOD were decreased significantly ;protein expressions of Nrf 2,NQO1 and HO -1 2018D01C169) were significantly down -regulated(P<0.01),while the protein expression of Keap 1 was up -regulated significantly (P<0.01). Compared with model group , the apoptotic rate and the content of MDA were decreased significantly in administration groups(P<0.01);the content of GSH in administration groups and the activity of SOD in purified product of phenolic acids group,protocatechuic aldehyde group and rosmarinic acid group were increased significantly (P<0.01). The protein expressions of Nrf2,HO-1 and NQO 1 in purified product of phenolic acids group as well as the protein expression of Nrf 2 in rosmarinic acid group were up -regulated significantly (P<0.01),while the protein expression of Keap 1 was down -regulated significantly in purified product of phenolic acids group and rosmarinic acid group (P<0.01). CONCLUSIONS The phenolic acids components from S. deserta Schang can relieve oxidative stress injury of renal tubular epithelial cells induced by high glucose and high fat ,the mechanism of which may be associated with activating Keap 1/Nrf2 signaling pathway and inhibiting oxidative stress response .
4.HIV/AIDS antiretroviral treatment and its influencing factors in Yining of Xinjiang

Objective To investigate the current status and its influencing factors of ART coverage and VL inhibition rate of HIV/AIDS in Yining City, Xinjiang, and to provide reference for AIDS prevention and treatment. Methods The ART data for 2017-2019 years in Yining, Xinjiang was retrieved from the National AIDS Antiretroviral Treatment Information System, to analyze the changing trend of ART coverage rate and VL inhibition rate of HIV/AIDS. Logistics regression model was used to analyze the influencing factors of untreated HIV/AIDS and uninhibited VL. Results The coverage rate of antiviral treatment in Yining city from 2017 to 2019 was 73.39%, 78.06% and 87.03%, respectively. The inhibition rates of VL were 83.91%, 85.05% and 86.09%, respectively, showing an increasing trend year by year. Female, other transmission routes, positive sexual partner detection, and testing and special investigation reduced the risks of untreated HIV/AIDS, while other domicile locations, non-marital and non-commercial heterosexual contact, and unawareness of their own VL increased the risks of untreated HIV/AIDS. Female, 26-35 years old, 36-45 years old, 46~55 years old, >55 years old, primary school, junior high school, high school or technical secondary school, junior college or above, and male-male sex behavior reduced the risks of unsuppressed VL of HIV/AIDS, while other domicile locations, non-marital and non-commercial heterosexual contact, and unawareness of their own VL increased the risks of unsuppressed VL in HIV/AIDS. Conclusion The ART coverage rate and VL inhibition rate in Yining, Xinjiang rise year by year, approaching the expected target. Targeted education and supervision should be conducted to promote the realization of “90-90-90” in 2020 and “95-95-95” target in 2030.
5.Characteristics of exposure to microplastics via ingestion and inhalation among Chinese children

Objective To evaluate the characteristics of exposure to micro- and nano-plastics (MPs) via food ingestion and inhalation among Chinese children including 6-7 years old school children in Pudong New Area of Shanghai. Methods Articles published until March 2021 were searched on PubMed, Web of Science, Scopus, China National Knowledge Infrastructure (CNKI), Wanfang and Weipu databases. The exposure levels of MPs by food ingestion and inhalation pathways were estimated by referring to the Chinese children food consumption and breathing rates data. The characteristics of children’s exposure to MPs in China were compared with that in the United States. In addition, the consumption of bottled water, tap water and salt intake among children aged 6-7 years was determined in a cohort of children and adolescents in Pudong New Area of Shanghai. The characteristics of exposure to MPs through food intake and inhalation among them were investigated. Results A total of 5 786 samples from 38 studies were retrieved. Annual intake of MPs among Chinese children was estimated to range from 159 400 to 204 637 MPs from food ingestion, depending on age and sex. When inhalation exposure is also considered, the annual MPs intake increased to 164 635 to 213 032 MPs, which were larger than the values in American persons. Ingestion of MPs posed the largest health risk to male Chinese adults. With the questionnaire survey data, the estimated annual total exposure amount through food intake alone is 116 272‒120 334 MPs for 6 to 7 years old school children in Pudong New Area. With addition of inhalation exposure, the annual total exposure amount is 121 300‒125 983 MPs, which is lower than the average exposure level in the whole country. The discrepancy could be mainly due to the different drinking habits between the local children and others. Overall, tap drinking water, seafood,bottled water, and air represented four substantial vectors of daily MPs exposure in Chinese children. Conclusion Ingestion and inhalation of MPs pose higher health risk to Chinese children in comparison to American children. The level of exposure to MPs among 6 to 7 years old school children in Pudong New Area of Shanghai is lower than that in the same age group of children across the whole country. The attempt to lower the MPs levels in drinking water and seafood would represent the most effective means to help reduce the MPs exposure risk to Chinese children.
6.Increased expression of coronin-1a in amyotrophic lateral sclerosis: a potential diagnostic biomarker and therapeutic target.

Amyotrophic lateral sclerosis (ALS) is the most common motor neuron disease. At present, no definite ALS biomarkers are available. In this study, exosomes from the plasma of patients with ALS and healthy controls were extracted, and differentially expressed exosomal proteins were compared. Among them, the expression of exosomal coronin-1a (CORO1A) was 5.3-fold higher than that in the controls. CORO1A increased with disease progression at a certain proportion in the plasma of patients with ALS and in the spinal cord of ALS mice. CORO1A was also overexpressed in NSC-34 motor neuron-like cells, and apoptosis, oxidative stress, and autophagic protein expression were evaluated. CORO1A overexpression resulted in increased apoptosis and oxidative stress, overactivated autophagy, and hindered the formation of autolysosomes. Moreover, CORO1A activated Ca2+-dependent phosphatase calcineurin, thereby blocking the fusion of autophagosomes and lysosomes. The inhibition of calcineurin activation by cyclosporin A reversed the damaged autolysosomes. In conclusion, the role of CORO1A in ALS pathogenesis was discovered, potentially affecting the disease onset and progression by blocking autophagic flux. Therefore, CORO1A might be a potential biomarker and therapeutic target for ALS.

7.Puncture positioning versus free-of-puncture positioning under three-dimensional navigation in the anatomical segmentectomy for pulmonary nodules: A retrospective cohort study

Objective To explore the feasibility and clinical value of free-of-puncture positioning in three-dimension-guided anatomical segmentectomy for ground-glass nodule (GGN) compared with percutaneous positioning. Methods Clinical data of 268 enrolled patients undergoing anatomical pulmonary segmentectomy from October 2018 to June 2019 were retrospectively collected, including 75 males and 193 females with an average age of 56.55±12.10 years. The patients were divided into two groups, including a percutaneous positioning group (n=89) and a free-of-puncture positioning group (n=179). Perioperative data of the two groups were compared. Results The average CT scan times of the percutaneous positioning group was 3.01±0.98 times, and the numerical rating scale (NRS) score of puncture pain was 3.98±1.61 points. Pulmonary compression pneumothorax (≥30%) occurred in 7 (7.87%) patients and intercostal vascular hemorrhage occurred in 8 (8.99%) patients after puncture. Lung nodules were successfully found and removed in both groups. There was no statistically significant difference between the two groups in the location of nodules (P=0.466), operation time (151.83±39.23 min vs. 154.35±33.19 min, P=0.585), margin width (2.07±0.35 cm vs. 1.98±0.28 cm, P=0.750), or the number of excised subsegments (2.83±1.13 vs. 2.73±1.16, P=0.530). Conclusion Anatomical segmentectomy with three-dimensional navigation avoids the adverse consequences of puncture, which has the same clinical efficacy and meets the requirements of oncology compared with percutaneous positioning. The free-of-puncture positioning method can be used for GGN located in the central region of pulmonary segment/subsegment or adjacent to intersegment veins instead of percutaneous positioning.
8.Neuronal intranuclear inclusion disease: the clinical features and pathological findings of peripheral tissue biopsy in nine cases with genetic diagnosis

Objective:To summarize the clinical features and pathological changes of peripheral tissues from patients with neuronal intranuclear inclusion disease (NIID) diagnosed by genetic tests.Methods:Repeat-primed polymerase chain reaction was used to confirm the GGC repeated expansion in the 5′ untranslated region of the NOTCH2NLC gene in patients with suspected NIID who had visited the Department of Neurology of Peking University First Hospital from January 2018 to February 2020. The clinical data and pathological changes of peripheral tissues from patients with genetically diagnosed NIID were collected retrospectively and analysed. Immunostaining with anti-p62 and anti-ubiquitin antibody was performed on peripheral biopsy specimens.Results:Totally nine patients with NIID who had GGC repeated expansion in the NOTCH2NLC gene were found. Five patients were familial (from three faimilies), and four patients were sporadic. The age of onset was 36-61(51.33±7.12) years. The most common symptoms in this NIID group were episodic emotion and personality change (8/9), paroxysmal disturbance of consciousness (6/9) and intermitant head discomfort (6/9). Other symptoms included cognitive dysfunction, limb weakness, limb sensory disturbance, bladder dysfunction, ataxia, seizures and psychiatric symptoms. Brain magnetic resonance imaging showed high signals along the corticomedullary junction on diffusion-weighted image in eight out of nine patients. Skin biopsied samples from nine patients demonstrated the presence of eosinophilic intranuclear inclusions (IIs), appearing in the nucleus of fibroblasts, fat cells and ductal epithelial cells of sweat glands on hematoxylin-eosin staining. IIs were positive on anti-p62 and anti-ubiquitin immunostaining. Electron microscopy indicated the IIs were composed of a pile of filament materials without membrane. Muscle biopsies from two patients showed no obvious neurogenic or myogenic pathologic changes, except in one patient several rimmed vacuoles fibers were found. In one patient sural nerve biopsy showed severe demyelinating pathological changes. No IIs were found in the muscles and peripheral nerve tissue either by histological examination or by immunohistochemical staining with anti-p62 or anti-ubiquitin, while IIs were found by immunofluorescence staining with both anti-p62 and anti-ubiquitin in three patient′s tissue. Conclusions:The phenotype of this NIID patient group is adult-onset NIID, with episodic encephalopathy as the main clinical manifestation. Skin biopsy has high pathological diagnostic value for NIID. The immunofluorescence staining with anti-p62 and anti-ubiquitin is easier to detect the presence of IIs than histological staining and immumohistochemical staining.
9.The lesion characteristics and predictors of obstructive coronary artery disease with anatomy-function mismatch

Objective:To explore the lesion characteristics and predictors of invasive coronary angiography (ICA)-verified obstructive lesions with fractional flow reserve (FFR)>0.80, that is, anatomy-function mismatch.Methods:A total of 515 obstructive vessels in 419 coronary disease patients from 11 Chinese medical centers undergoing coronary CT angiography and ICA and FFR were retrospectively analyzed. All vessels had one target lesion with diameter stenosis ≥50 % by ICA. There were 229 vessels in the match group (FFR≤0.80) and 286 vessels in the mismatch group (FFR>0.80). The lesion characteristics including lesion territory, the distance of the coronary artery ostium to the proximal end of the lesion, minimum lumen area, reference lumen area, plaque length and burden, plaque volume and component volume, remodeling index and plaque morphological complexity were measured and compared between the two groups. Optimal thresholds of quantitative plaque characteristics were defined by Yoden index. Logistic regression analysis was used to analyze the predictors of anatomy-function mismatch. Area under receiver operating characteristic curve (AUC) was used to analyze the ability of different lesion features to predict mismatched lesions.Results:The coronary stenosis, plaque burden and length, plaque volume (including each component volume) in the mismatch group were smaller than those in the match group, and FFR, minimum lumen area were larger (all P<0.05). Left anterior descending artery (LAD) lesion and severe complex plaque were more common in the match group than the mismatch group with a statistically significant difference. Univariate logistic regression analysis showed that LAD lesion, minimum lumen area>4 mm 2, plaque burden and length, plaque calcification volume<27 mm 3, plaque lipid volume<30 mm 3, plaque fiber volume<150 mm 3 and plaque morphological complexity were predictiors of anatomic function mismatched lesions; Multivariate logistic regression showed that the minimum lumen area>4 mm 2 (OR=3.371, 95%CI 1.903-5.973, P<0.001), plaque lipid volume<30 mm 3 (OR=3.014, 95%CI 1.691-5.373, P<0.001), plaque morphological complexity (mild OR=17.772, 95%CI 8.072-39.128, P<0.001, moderate OR=6.383, 95%CI 3.739-10.896, P<0.001) were independent predictors of mismatched lesions. The AUC of the model based on the minimum lumen area, plaque lipid volume and morphological complexity was 0.824, which was superior to either of the plaque feature alone ( P<0.001). Conclusions:The minimum lumen area, lipid volume and plaque morphological complexity are independent predictors of the anatomical-functional mismatch lesions, and the combination can significantly improve the prediction value.
10.Discussion on teaching reform of oral histopathology in multiple classes

In the information age, teaching methods are undergoing tremendous changes, and the traditional teaching methods are difficult to draw students′ attention. As the core course of stomatology, oral histopathology is an important foundation for oral students. Along with the rapid development of medical science, it is imperative to deepen the reform of teaching. This article discussed the diversified teaching methods conducted in oral histopathology course in Zhejiang University School of Stomatology in the past 10 years with the core teaching theory of “learning before teaching” including the exploration design course, case based learning, journal club, etc. Diversified teaching methods activated students′ subjective initiative, which laid a foundation for the next stage of clinical study, and provided a guiding framework for the future teaching reforms.
