1.Role of PI3K/Akt signaling pathway and autophagy in reduction of adriamycin-induced myocardial injury by sevoflurane in rats

Objective To evaluate the role of phosphatidylinositol 3-kinase/protein-serine-threonine kinases (PI3K/Akt) signaling pathway and autophagy in reduction of adriamycin-induced myocardial injury by sevoflurane in the rats.Methods Thirty-six healthy male Sprague-Dawley rats,weighing 200-250 g,were randomly divided into 6 groups (n =6 each) using a random number table:control group (group C),adriamycin-induced myocardial injury group (group Dox),sevoflurane group (group Sev),LY294002 inhibitor group (group LY),solvent control group (group dimethyl sulfoxide [DMSO]),and 3-MA inhibitor group (group 3-MA).Adriamycin 4 mg/kg was injected intraperitoneally once a week for 3 weeks in all the groups except group C.The rats were mechanically ventilated for 2 h in C and Dox groups.The rats inhaled 2.4% sevoflurane for 2 h in group Sev.In group LY,0.3 mg/kg LY294002 was injected via the tail vein at 10 min before anesthesia,and the rats inhaled 2.4% sevoflurane for 2 h.In group DMSO,the equal volume of DMSO was injected,and the rats inhaled 2.4% sevoflurane for 2 h.After the blood samples were collected from the heart,the rats were sacrificed,and myocardial specimens were obtained for determination of cardiac troponin Ⅰ (cTnI) concentrations in serum (by enzyme-linked immunosorbent assay),expression of total Akt (t-Akt),phosphorylated Akt (p-Akt),mammalian target of rapamycin (mTOR),phosphorylated mTOR (p-mTOR) and autophagy marker microtubule-associated protein light chain 3 Ⅱ (LC3 Ⅱ) (by Western blot),and cell apoptosis (by TUNEL).Apoptosis index (AI) was calculated.Results Compared with group C,the expression of p-Akt and p-mTOR was significantly down-regulated,and the expression of LC3 Ⅱ,AI,and serum cTnI concentration were significantly increased in the other five groups (P< 0.05).Compared with group Dox,the expression of p-Akt and p-mTOR was significantly up-regulated,and the expression of LC3 Ⅱ,AI,and serum cTnI concentration were significantly decreased in group Sev (P<0.05).Compared with group Sev,the expression of p-Akt and p-mTOR was significantly down-regulated,and the expression of LC3 Ⅱ,AI,and serum cTnI concentration were significantly increased in group LY,and the expression of LC3 Ⅱ was significantly down-regulated,and serum cTnI concentrations were significantly decreased in group 3-MA (P<0.05).Conclusion Sevoflurane can activate PI3K/Akt signaling pathway and inhibit autophagy,thus reducing adriamycin-induced myocardial injury in rats.
2.Dietary fat intake and fatty acid composition

Objective To investigate dietary fat intake and fatty acid composition in Qingdao residents.Methods Cluster random sampling method was used to select 189 adults (53 males and 136 females) between March and to July 2010.The food intake habit and the relationship between nutrient,fat,fatty acid composition and serum lipid concentrations were analyzed.Results Per capita intake of cereal was 200 g,potato 26 g,poultry meat and offal 85 g,seafood 73 g,nuts 23 g,and oils 40 g.Protein accounted for 16% of total calorie intake,carbohydrate 48%,and fat 37%.Oils accounted for 43% of fat intake,and nuts 11%.High fat intake was mainly contributed to intake of oils and nuts.Those with dyslipidemia showed higher nutritent intake and body mass index.ConclusionQingdao residents show higher dietary fat intake and lower long-chainω-3 (n-3) PUFAs consumption.Long-chain n-3 PUFAs may be negatively correlated with dyslipidemia.
3.Present situation and application control of hospital medical devices

Some countermeasures were put forward to solve the problems in the application of hospital medical devices. It's pointed out that the application of hospital medical devices tends to be standardized with the progress of medical reformation, the attention on medical devices management, the supervision and etc.
4.Expression of serum thrombopoietin in patients with hemophagocytic syndrome

Objective To investigate the expression of serum thrombopoietin (TPO) in patients with hemophagocytic syndrome (HPS) and to evaluate its clinical significance. Methods The clinical data of HPS patients from Beijing Friendship Hospital,Capital Medical University during the period from September 2008 to July 2010 were collected. They were divided into infection-associated HPS,tumor-associated HPS and rheumatological disease associated HPS according to different causes. The serum concentration of TPO in HPS patients and 26 healthy controls was measured with enzyme-linked immunosorbent assay (ELISA). Then the correlations between TPO and laboratory indicators was analyzed. Results The level of serum TPO in 26 cases of HPS was significantly lower than that of control group (P =0.001) and the variance was not statistically significant in different causes of HPS groups using ANOVA (P =0.183). The TPO levels in HPS patients showed positive correlations with serum creatinine,after we detected their TPO levels and platelets,red blood cells,hemoglobin,serum creatinine,urea nitrogen,triglyceride,fibrinogen,ferritin,NK cell activity and sCD25 levels on the same day. Conclusion TPO together with various cytokines may affect the formation of blood cells in HPS patients,and its reduction may be associated with impaired liver function. The underlying mechanism remains to be further explored.
5.Extracurricular activities in general practice teaching for medical students

One hundred and forty-four medical students were enrolled in the study, the participants were randomly divided into experimental group and control group with 72 in each. All students attended the theoretical lecture of general medicine. Students in experimental group took part in the extracurricular teaching activities including the nutritional course and practice, the clinical case study and problem based learning ( PBL), students in control group did not have the extracurricular activities. The results showed that although there was no difference in general testing scores between two groups, the problem solving ability and assay-writing ability of experimental group were significantly higher than those of control group.
6.Clinical observation of iron removal treatment with week-therapy of chronic iron overload

Objective To explore the clinical application and curative effect of iron removal treatment with week-therapy in chronic iron overload related to blood transfusion.Methods Twenty patients who were diagnosed as chronic iron overload were retrospectively studied.The amelioration of liver and pancreatic fanctions in patients with chronic iron overload treated with week-therapy of iron chelating agent through intravenous drip.Results The changes in the level of serum ferritin [2693.7±709.9) μg/L] were not significant after treatment at six months,hut the cilinical symptoms were relieved obviously.The levels of serum ferritin in patients after the treatment at twelve months were significantly different from the level before treated [(2083.7±714.4) μg/L vs (2771.5±725.2) μg/L,t =3.35,P < 0.01],and the decreases were significant in sixteen patients.The liver functions in seven patients were improved and pancreatic functions were better in five patients.Conclusion The study suggests that iron removal treatment with week-therapy is effective for chronic iron overload,and improves the functions of damaged organs.
7.Exploration of Pharmaceutical Service Fee Implementation and Online Pharmaceutical Service Prospect Based on a Questionnaire Survey

Objective: To investigate the public opinion on pharmaceutical service and explore the implementation of pharmaceutical service fee and the prospect of online pharmaceutical service in our country.Methods: Questionnaires which related to pharmaceutical service fee and online pharmaceutical service were distributed to patients and their relatives or friends, and then recycled.The data including the recycling of questionnaires, information of respondents and answers to the questions were statistically analyzed.Results: Totally 63.8% respondents considered pharmaceutical service fee was reasonable and the fee should be borne by the government finance and health care insurance.Totally 85.5% respondents thought launching online pharmaceutical service was necessary, which included online retails of medicine, consultation, medication guidance, decoction of Chinese traditional medicines, medicine delivery and so on, especially pharmacy consultation.Conclusion: Outpatients have high demands of pharmaceutical service and most of them accept online pharmaceutical service with different understandings.Pharmaceutical service fee is an inevitable trend in the development of pharmaceutical industry.More publicity and guidance should be given to improve people's acceptance of pharmaceutical service and relevant cost.Relevant departments should take full advantages of Internet and provide diversified pharmaceutical service.
8.Clinical features and treatment of rare etiologies of upper gastrointestinal bleeding

Objective To explore the clinical features and treatment of upper gastrointestinal bleeding with rare etiologies.Methods A total of 45 cases with rare etiologies was selected from a group of 1200 patients with upper gastrointestinal bleeding admitted from January 2012 to December 2015.Results The incidence of gastric stromal tumor (24.4%) and Dieulafoy's lesion (20.0%) was the highest.Among rare etiologies of upper gastrointestinal bleeding were Mallory-Weiss syndrome (17.8%) and stomal ulcer and hemorrhage of efferent loops after subtotal gastrectomy (17.8%),as well as left-side portal hypertension (6.7%),liver cirrhosis complicated with variceal bleeding in the descending part of the duodenum (2.1%),esophageal hematoma (2.1%),esophageal mucosa avulsion (2.1%),P-J syndrome (2.1%),duodenal adenocarcinoma (2.1%),and cutaneous malignant melanoma with duodenal metastasis (2.1%).Conclusions Gastroscopy in early stages played a role in etiologies identification,while etiological treatments could substantially reduce recurrent bleeding rate,thus enhanced cure rate.Analysis on rare etiologies promoted the medical understanding,with clinical diagnosis and treatment improved.
9.The clinical characteristics of adult hemophagocytic lymphohistiocytosis treated with haploidentical donor hematopoietic stem cell transplantation

Objective To analyze the clinical characteristics of adult patients with hemophagocytic lymphohistiocytosis (HLH) receiving haploidentical donor hematopoietic stem cell transplantation (HID HSCT).Method We retrospectively reviewed 20 adult patients with HLH from August 2009 to August 2014.The clinical features and outcome were analyzed.Results Conditioning regimens consisted of total body irradiation/etoposide/cyclophosphamide (TBI/VP-16/CTX) and busulfan (Bu)/VP-16/CTX in HLH with anti-thymocyte globulin (ATG) 8 mg/kg.The stem cells were mobilized from donors' peripheral blood.Median time to white blood cell engraftment was 13 (9-27) days.Median time to platelet engraftment was 14 (10-28) days.Mixed chimerism after transplantation developed in 4 patients and no patient presented graft failure.Eight patients developed grade Ⅱ to Ⅲ acute graft-versus-host disease (GVHD),while as chronic GVHD occurred in 9 patients.Among 12 patients with EB virus (EBV) reactivation,2 patients developed post-transplant lymphoproliferative disorder (PTLD),7 were suspected as PTLD and 3 were considered as relapse of primary disease.With a median follow-up of 20 months (range:0.5-108 months) after transplantation,the estimated 2-year overall survival (OS) rate was (60.0 ± 11.0) % in all patients.During the follow-up,12 patients survived,8 died including 5 within 100 days after HSCT.Among 5 non-remission patients before HSCT,4 patients died within 100 days after HCT.Conclusions HID HSCT is an effective treatment for adult patients with HLH to achieve remission and long-term survival.High proportion of mixed chimerism has been seen at early stage after transplantation.EBV reactivation and early transplant-related mortality are common.
10.Clinical study of 6 patients with Epstein-Barr virus associated hemophagocytic lymphohistiocytosis treated with rituximab-containing regimen

Objective To investigate the efficacy of rituximab-containing regimen in Epstein-Barr virus associated hemophagocytic lymphohistiocytosis (EBV-HLH). Methods A retrospective analysis involving 6 EBV-HLH patients who had received treatment with rituximab-containing regimen was performed. The patients who were diagnosed with lymphoma or primary HLH subsequently were not included in the analysis. Results All patients were males. The median age was 27.5 years (range 20-61 years). Two patients received rituximab-containing regimen as primary therapy, and got partial remission (PR) within 2 weeks after the first course of rituximab, but relapsed within 4 weeks. Four patients received rituximab-containing regimen as salvage therapy, but none achieved remission. The 6 patients died due to HLH and complications, such as infection and hemorrhage. Laboratory data including white blood cell count, haemoglobin concentration, platelet count ferritin, alanine transaminase, aspartate transaminase,total bilirubin, fibrinogen and EBV-DNA did not show statistical significance (all P>0.05). Conclusion The efficacy of rituximab as a treatment for EBV-HLH is not as good as that in the previous study, and a prospective clinical trial of rituximab-based monotherapy is needed to answer the question.
