1.Infection-associated Factors of Peritoneal Cavity Infection by Enterococcus

OBJECTIVE To study the role of Enterococcus in peritoneal cavity infection and the infection -associated factors, by which the prophylactic measures are put forward. METHODS The role of Enterococcus in peritoneal cavity infection and the infection-associated factors were discussed with the analysis of the investigated 33 cases with the peritoneal cavity infection by Enterococcus. RESULTS The peritoneal cavity infection was mainly caused by Enterococcus faecalis, for which penicillin and ampicillin were the first choice of empirical therapy. For regime of the combination use of antibiotics might be feasible for the infection caused by the speeics with unshowed high-level resistance to aminoglycosides. Many related risk factors on peritoneal infection caused by Enterococcus were included such as the colonization of the drug resistant strain of Enterococcus promoted by cephalosporins Ⅲ and the abuse of large amount of antibiotics as a selective pressure on the resistant strain. CONCLUSIONS Much attention should be paid on the role of Enterococcus in peritoneal infection clinically. The effective therapy is associated with the eradication of the pathogen. and the better control measures against infection.
2.Clinicopathological analysis of 306 cases with endometrioid cancer in northern Shandong area

Objective To explore the clinicopathology characteristics of endometrial adenocarcinoma and non-endometrial adenocarcinoma.Methods The clinical data of 306 patients with endometrioid cancer were analyzed retrospectively.Results The patients with endometrial adenocarcinoma accounted for 90% of all patients,and the patients with non-endometrial adenocarcinoma accounted for 10%.Compared with the patients with endometrial adenocarcinoma,the average age of the patients with non-endometrial adenocarcinoma was older[(59.32 ± 7.77)years],the constitution ratio of postmenopausal patients was higher(90%),the clinicopathologic stage was later(P =0.000),the grade of histology was higher(P =0.000),the myometrial invasion was deeper(P =0.014).The patients with nonendometrial adenocarcinoma were prone to occur lymph metastasis(P =0.006),and the recurrence rate was higher(P =0.000).Conclusion There is significant defference between endometrial adenocarcinoma and non-endometrial adenocarcinoma,the patients with non-endometrial adenocarcinoma has poor pronosis,and we should pay more attention to it.
3.Therapic effect of lauromacrogol sclerotherapy on infantile hemangioma

Objective To investigate the effect of intralesional lauromacrogol therapy on infantile hemangioma.Methods 30 cases all received intralesional injection of lauromacrogol.The changes of tumor size,texture and color were monitored and recorded.The adverse effects after medication were observed and managed accordingly.The adverse effects after medication were observed.The times of treatment were determined according to degree of reduction of tumor volume.To assess the efficacy,a 4 scales system was adopted.Results All patients had completed the treatment and followup.The overall response was scale Ⅰ in 2 cases (6.7 %),scale Ⅱ efficacy in 4 patients (13.3 %),scale Ⅲ efficacy in 5 cases (16.7 %) and scale Ⅳ in 19 cases (63.3 %).The smaller the tumor,the fewer the times of treatment,and the better the results.The effects of treatment were better if patient could be treated by intralesional lauromacrogol therapy in early stage.No severe adverse effects were observed during 6 months of follow-up.Conclusions Intralesional injection of lauromacrogol sclerotherapy is a safe and effective way to treat infantile hemangioma.
4.The Vertebral Artery and Basilar Artery Haemodynamics of Sudden Deafness

Objective To explore different characteristics of the vertebral artery and basilar artery haemody-namics in different frequencies to provide a theoretical basis for clinical treatment .Methods 90 cases of sudden deaf-ness were induded and according to hearing curve ,the cases were divided into three groups with 30 cases for each while the control group had 30 subjects .All cases were examined by TCD ,the VA ,BA test .Results Compared with group of median and low frequency sudden deafness ,the abnormal blood flow rate were found with decreased hearing(P<0 .05) .High and the full frequency range showed the abnormal blood flow rate ,although results were different(P<0 .05) .There were more high velocity detection rates among patients of high frequency group than the other groups .The full-frequency group's blood flow detection rate increased significantly more than the first two groups of patients with sudden deafness .PI ,RI of sudden deafness increased slightly ,but there was no statistically significant difference compared with control group (P>0 .05) .High frequency hearing loss compared with the con-trol group patients with sudden deafness had a clear abnormal velocity (P<0 .05) ,characterized by high velocity . There was no statistically significant difference in blood flow rate among low and median frequency group ,full-fre-quency group and control group except for Vs of BA in low and median frequency group .Conclusion Vertebral and basilar arterial circulation disorders had present more significance in the incidence of sudden deafness ,evident espe-cially in high and all frequency sudden deafness .Early initiation of TCD examination can understand the change of the vertebral and basilar artery hemodynamics ,providing high clinical application values .
5.Factors associated with prevalence, awareness, treatment and control of hypertension among adults in Dagang Oil Field:A cross-sectional survey

Objective To investigate factors associated with prevalence, awareness, treatment and control of hypertension in Dagang Oil Field, and to provide the scientific evidence for the intervention measurements. Methods A cross-sectional, population-based survey was conducted in Dagang Oil Field from 2008 to 2010. Totally, 16 553 adults, 8 174 men (49.38%) and 8 379 women (50.62%), completed all of the questionnaires, measurement examination and blood specimen collection. Results The prevalence of hypertension was 27.30%. Multivariate logistic regression analysis showed that male, old age, overweight or obesity, family history of hypertension, marriage, education attainment, snoring, high heart rate, fasting blood glucose, serum total protein, and hyperuricemia were associated with hypertension. The awareness, treatment, control rates among all hypertensive subjects, and control rate among treated hypertensive participants were 52.65%, 40.07%, 15.51%, and 38.78%, respectively. Awareness was more common in those who were 45-64 years old , high medical costs, family history of hypertension, and smoking currently. Odds ratio (OR) (95% CI) of awareness was 5.24 (2.78 to 9.88), 24.11 (11.91 to 48.84) and 27.46(13.92 to 54.18) for medical costs 1-59.9 yuan/month, 60-199.9 yuan/month, and≥200 yuan/month, respectively. In contrast, exercising 2-4 times/week and longer sleep duration were associated with lower awareness. Medication treatment was more common in those who had old age, high medical costs, hypertension family history, coronary heart disease, prefer vegetable, while less common in those who exercise occasionally, sleep longer and drink alcohol currently. OR (95%CI) of treatment was 10.89 (3.76 to 31.56), 54.07 (19.20-152.24), and 87.31 (31.54 to 241.75) for medical costs 1-59.9 yuan/month, 60-199.9 yuan/month, and ≥200 yuan/month, respectively. Controlled hypertension was more common in those who had old age, high medical costs, exercise 2-4 times/week, coronary heart disease, prefer crude fibre diet, but less common in those who drink alcohol currently, have higher heart rate, and higher education attainment. OR (95%CI) of controlled hypertension was 13.59 (6.50-28.44), 14.67 (7.01-30.68), and 17.43 (8.34-36.39) for medical costs 1-59.9 yuan/m, 60-199.9 yuan/m, and≥200 yuan/m, respectively. Conclusion Despite high rate of awareness, treatment and control of hypertension still need to be strengthened.
6.Effect of continuous aspiration of subglotlic secretions on the prevention of ventilator-associated pneumonia in mechanically ventilated patients:a prospective, randomized, controlled clinical trial

Objective To evaluate the effect of continuous aspiration of subglottic secretions (CASS) on the prevention of ventilator-associated pneumonia (VAP) in mechanically ventilated patients. Methods Patients ventilated mechanically at the ICU from October, 2004 to April,2006 were randomly divided into 2 groups: one group received CASS and the other did not (NASS group). CASS was performed immediately after admission for patients in the CASS group. The diagnosis d VAP was made based on clinical presentations, and the evaluation of YAP was done using simplified version of the clinical pulmonary infection score (CPIS). The general status of the patients, days of ventilated treatment, the volume of daily aspirated aubglottic secretions, the morbidity and timing of VAP, days of stay in ICU and mortality within 28 days of hospitalization were recorded. Results One hundred and one patients were included in the study. There were 48 patients in the CASS group who were treated with mechanical ventilation more than 48 hours,and 43 patients in the NASS group. There was no significant difference in the general status of the patients and days of ventilation between 2 groups with the averaged score of APACHE Ⅱ being 20.8± 6.1. The average of CPIS was of 5.6±1.0 when VAP was diagnosed. The mean volume of aspirated subglottic secretions within the first 24 hours in the CASS group (n=48) was (27.2±21.2)ml. The morbidity of VAP in the CASS and the NASS groups was 25.0% and 46. 5% respectively (P=0.032), and the length of time before the onset of VAP in these 2 groups was (7.3±4.2) days and (5.1±3.0) days respectively (P=0.100). There was a significant increase in the percentage of gram-positive cocci from the lower respiratory tracts in the NASS group compared with that in the CASS group (P=0.004). In the CASS group, the volume of the first daily aspirated subglottic secretions in patients with VAP was significantly less than that in patients without VAP(P =0.006). The morbidity of VAP in patients with failed early aspiration (the volume of first daily aspirated secretions≤20 ml) was significantly higher than that in patients in whom the aspiration was effective (P<0.01). The length of mechanical ventilation in patients with VAP was significantly longer than that in patients without VAP(P=0.000). The in-hospital mortality in patients with VAP was significantly higher than that in patients without VAP(P=0.009), and the mortality in 28 days after admission in patients with VAP was significantly higher than that in patients without VAP(P=0.035).Conclusion Effective continuous aspiration of subglottic secretions could significantly reduce the morbidity of early-onset VAP.
7.The role of I-Ab/I-E expression in dendritic cells of acute lung injury mice

Objective To observe the expression of I-Ab/I-E on circulating,lung and splenic dendritic cells (DC) in acute lung injury (ALI) mice.Methods Twenty-four C57BL/6 mice were randomly divided into four groups:control group,ALI 6 h,ALI 12 h and ALI 24 h group.Blood,lungs and spleens were harvested after lipopolysaccharide or phosphate butter solution administration.The expression of I-Ab/I-E on DC was assessed by flow cytometry (FCM).IL-6 level in the lung was measured by enzymelinked immunosorbent assay (ELISA).Lung wet weight/body weight (LW/BW) was recorded to assess lung injury.Meanwhile,pathological changes were examined under optical microscope.Results (1) lipopolysac charide-induced ALI mice resulted in a significant increase in lung LW/BW ratio.(2) Histologically,widespread alveolar wall thickening caused by edema,marked and diffuse interstitial infiltration with inflammatory cells,and severe hemorrhage in the interstitium and alveolus were observed in the ALI groups.(3) The level of IL-6 in lung tissue was significantly enhanced in ALI mice.(4) FCM analysis showed that I-Ab/I-E expressions on lung DC [(73 ±9)%],and splenic DC [(81 ±8)%] were significantly higher than that on circulating DC [(24 ± 7) % ; P < 0.05] in control mice.(5) In ALI mice,the expressions of I-Ab/I-E on peripheral blood DC were (34 ± 17)% at 6 h,(51 ± 16)% at 12 h,(50 ± 17)% at24 h respectively; I-Ab/I-E expressions on lung DC were (82 ± 14)% at 6 h,(88 ±6)% at 12 h,(90 ±10)% at 24 h respectively; the expressions of I-Ab/I-E on splenic DC were (88 ± 8)% at 6 h,(89 ± 4)% at 12 h,(93 ± 9)% at 24 h respectively,which were also significantly higher than those on the peripheral blood DC (P < 0.05).(6) The I-Ab/I-E expressions on circulating DC in ALl mice at 12 h and 24 h was significantly higher than that on circulating DC in control mice (P < 0.05).(7) The I-Ab/I-Eexpressions on lung DC and splenic DC in ALI mice at 24 h were significantly higher than those on lung DC and splenic DC in control mice (P < 0.05).(8) There was a significant correlation of I-Ab/I-E expression on respiratory DC with the IL-6 level and lung injury score in LPS-induced ALI group (P < 0.05).Conclusions There is a dynamic characteristic in the expression I-Ab/I-E on circulating,lung and splenic DC populations in ALI mice.I-Ab/I-E on pulmonary DC seems to play an important role in the pathogenesis of ALI.
8.The pilot study of combined detection of thyroid stimulating hormone and free thyroxine in screening for ;congenital hypothyroidism in neonates

Objective To explore the clinical signiifcance of combined detection of thyroid stimulating hormone (TSH) and free thyroxin (FT4) in dried blood spots in screening for congenital hypothyroidism (CH) in neonates. Methods The TSH and FT4 levels in dried blood spot were measured by time-resolved lfuorescence immunity in live born neonates from June to December 2013. If the screening was positive, the blood was drawn and the serum TSH and FT4 were measured and compared with the results from dried blood spots. Results In a total of 31 199 neonates screened, 12 cases were diagnosed with CH and the prevalence rate of CH was 1/2 600;4 cases were hyperthyropinemia and no pituitary CH was detected. There was no signiifcant difference between TSH or FT4 levels in dried blood spot and those in serum in neonates diagnosed with CH (P>0.05). Conclusions Combined detection of TSH and FT4 in dried blood spot can be used for neonatal screening of CH. It can be applied for early distinguishing CH from hyperthyropinemia, and also helpful for early diagnosis and treatment of central CH.
9.Insulin-secreting cells from induced pluripotent stem cells regulate blood glucose levels in vitro

BACKGROUND:Mouse pluripotent stem cel s are induced to differentiate into insulin-secreting cel s that can effectively improve blood glucose levels in diabetic mice. OBJECTIVE:To detect mRNA and protein levels of insulin-like cel clusters from induced pluripotent stem cel s and to investigate the function of insulin-secreting cel s in vitro and in vivo. METHODS:Mouse induced pluripotent stem cel s cultured in vitro were induced to differentiate into insulin-secreting cel s using combined inducers through three stages. The morphology of endodermal cel s, islet-derived progenitor cel s and mature islet cel s in each stage was observed and relative gene expression levels were detected by PCR. Mature insulin-like cel clusters underwent dithizone staining and functions of insulin released in vitro were observed by ELISA assay. Final y, the insulin-secreting cel s were transplanted into the subrenal capsule of diabetic mice, and then blood glucose levels were observed. RESULTS AND CONCLUSION:The mature spherical insulin-like cel clusters were successful y obtained in vitro, which were in iron red by dithizone staining, and expression of insulin mRNA was determined by PCR. The insulin-like cel clusters could secrete insulin in response to various blood glucose levels by ELISA assay. In addition, after the cel s clusters were transplanted into the subrenal capsule of mice with type 1 diabetes, the blood glucose levels were marbedly improved.
10.Vibration therapy helps relieve spasticity and improve upper limb motor function after stroke

Objective To observe the effect of variable frequency vibration therapy while sitting in an anti-spasmodic posture on spasticity and the motor function of the upper limbs among stroke survivors.Methods Thirty stroke survivors with upper limb spasticity were randomly divided into a treatment group and a control group,each of 15.Both groups were given routine rehabilitation training for 4 weeks while the treatment group was additionally provided with variable frequency vibration training while sitting in anti-spasmodic postures.Before and after the treatment,the modified Ashworth scale (MAS) was used to assess spasticity.The root mean square (RMS) value of the surface electromyogram amplitude of the affected biceps when extended passively and those of the triceps,obliques and multifidus in maximum isometric contraction was measured and recorded.The motor function of the affected upper limbs was evaluated using the active range of motion (A-ROM) of the shoulder,elbow and wrist,as well as a Fugl-Meyer assessment (FMA).Moreover,ability in the activities of daily living (ADL) was assessed using the modified Barthel index (MBI).Results After the treatment,significant improvement was observed in the average MAS,A-ROM,RMS,FMA and MBI results in both groups compared to those before the treatment.Moreover,the results in the treatment group were significantly better than those of the control group,on average.Conclusions Variable frequency vibration therapy while sitting in an anti-spasmodic posture combined with traditional rehabilitation is more effective than the latter alone in relieving spasticity as well as improving motor function and ability in the activities of daily living among stroke survivors with the upper limb spasticity.
