1.Study on the content of health education in discharged patients with acute myocardial infarction based on transcultural nursing

Objective To construct the content of health education based on transcultural nursing in discharged patients who were acute myocardial infarction (AMI) and had different cultural backgrounds,and to instruct the nurses to choose and formulate the individual health education program and prescription according to patients' cultural background.Methods Twenty experts who worked in tertiary A hospitals for 15 years,had secondary senior positions,were bachelor degree or above in nursing,cardiovascular and social science were chosen and received the expert consultation of Delphi method through the self-designed framework consulting table with three-level core content of the health education.The items on the content of health education at every level were quantitatively analyzed.Results The Cr was 0.865,and the Cs was 0.840,and the Ca was 0.890.The kandall coefficients of concordace at every level were respectively 0.425,0.443,0.408,and chi-square test showed that the difference was statistically significant (P < 0.01).The number of items on the core content of health education through two rounds of expert consultation was determined including 11 items Grade Ⅰ,42items Grade Ⅱ and 126 items Grade Ⅲ.Conclusions The expert opinion of items on the content of health education in discharged patients with AMI at every level is intensive,and the reliabihty of the result is high,and which provides the scientific basis for carrying out effective health education in discharged patients who are AMI and have different cultural backgrounds.
2.Demands of self-monitoring in patients with chronic heart failure: a qualitative research

Objective To explore the self-monitoring needs of patients with chronic heart failure, so as to understand their needs for self-monitoring and provide references for clinical nursing healthcare education. Methods From October 2016 to January 2017, totally 9 cases of heart failure patients who met the inclusion criteria in an affiliated hospital were enrolled in this study. In-depth interviews were conducted and analyzed by phenomenological data analysis to form the main topic. Results At last, 5 subjects were extracted, namely, the needs for self-monitoring knowledge, the needs for suitable monitoring tools selection, the needs for self-monitoring skill, the willingness of self-monitoring and the needs for support from affection and friends. Conclusions The long-term treatment and rehabilitation of chronic heart failure patients was not only to improve symptoms and prolong life, but also to build a diversified health education and follow up model, to establish a professional health education team; to construct a rational service support system, to provide professional services for chronic heart failure patients, so as to meet the self-monitoring needs, help with symptom recognition and ultimately improve patients' quality of life.
3.Differences in near-infrared fluorescence imaging and histological analysis of cheek mucosa in golden hamsters with different pathological states

Objective To explore and analyze the feasibility of using indocyanine green(ICG)near-infrared fluores-cence(NIF)imaging technology for the early diagnosis of oral potential malignant disorders and oral squamous cell car-cinoma.Methods 7,12-Dimethylbenz[a]anthracene in acetone solution was used to induce various pathological models of buccal mucosal lesions(mild/moderate dysplasia,severe dysplasia,squamous cell carcinoma)in golden hamster.ICG-NIF was conducted for the quantitative analysis of the fluorescence signal of lesion tissue,and evaluation of the diagnos-tic and discriminative capabilities of the ICG-NIF technology for mucosal lesions in various pathological states.Immuno-histochemical staining was perform to examine the mi-crovessel density(MVD)and microlymphatic vessel den-sity(MLVD)of mucosa in various pathological states and explore the histological reasons underlying the differ-ences in fluorescence signals.Results The results of ICG-NIF fluorescence quantitative analysis reveal the higher fluorescence intensity of mucosal lesions in the experimental group compared with that of the normal mucosa on the control side,with statistical differences(P<0.05).Moreover,the more severe the malignancy of mucosal lesions in the experimental group,the higher the fluorescence intensity.According to histopathological analysis,the malignant pro-gression of mucosal lesions in golden hamsters was accompanied with an increase in MVD(P<0.05)and a decrease in MLVD(P<0.05).Conclusion The abnormal proliferation of mucosal lesions in golden hamsters exhibits a difference in ICG-NIF fluorescence signal compared with normal mucosal tissue.Fluorescence quantitative analysis methods can provide assistance in differentiation and show potential for clinical applications.

Objective:To study the distribution and antibiotics resistance of the main pathogens of neonatal purulent meningitis in different regions of China.Methods:A retrospective descriptive clinical epidemiological study was conducted in children with neonatal purulent meningitis which admitted to 18 tertiary hospitals in different regions of China between January 2015 to December 2019. The test results of blood and cerebrospinal fluid, and drug sensitivity test results of the main pathogens were collected. The distributions of pathogenic bacteria in children with neonatal purulent meningitis in preterm and term infants, early and late onset infants, in Zhejiang Province and other regions outside Zhejiang Province, and in Wenzhou region and other regions of Zhejiang Province were analyzed. The chi-square test was used for statistical analysis.Results:A total of 210 neonatal purulent meningitis cases were collected. The common pathogens were Escherichia coli ( E. coli)(41.4%(87/210)) and Streptococcus agalactiae ( S. agalactiae)(27.1%(57/210)). The proportion of Gram-negative bacteria in preterm infants (77.6%(45/58)) with neonatal purulent meningitis was higher than that in term infants (47.4%(72/152)), and the difference was statistically significant ( χ2=15.54, P=0.001). There were no significant differences in the constituent ratios of E. coli (36.5%(31/85) vs 44.8%(56/125)) and S. agalactiae (24.7%(21/85) vs 28.8%(36/125)) between early onset and late onset cases (both P>0.05). The most common pathogen was E. coli in different regions, with 46.7%(64/137) in Zhejiang Province and 31.5%(23/73) in other regions outside Zhejiang Province. In Zhejiang Province, S. agalactiae was detected in 49 out of 137 cases (35.8%), which was significantly higher than other regions outside Zhejiang Province (11.0%(8/73)). The proportions of Klebsiella pneumoniae, and coagulase-negative Staphylococcus in other regions outside Zhejiang Province (17.8%(13/73) and 16.4%(12/73)) were both higher than those in Zhejiang Province (2.9%(4/137) and 5.1%(7/137)). The differences were all statistically significant ( χ2=14.82, 12.26 and 7.43, respectively, all P<0.05). The proportion of Gram-positive bacteria in Wenzhou City (60.8%(31/51)) was higher than that in other regions in Zhejiang Province (38.4%(33/86)), and the difference was statistically significant ( χ2=6.46, P=0.011). E. coli was sensitive to meropenem (0/45), and 74.4%(32/43) of them were resistant to ampicillin. E. coli had different degrees of resistance to other common cephalosporins, among which, cefotaxime had the highest resistance rate of 41.8%(23/55), followed by ceftriaxone (32.4%(23/71)). S. agalactiae was sensitive to penicillin, vancomycin and linezolid. Conclusions:The composition ratios of pathogenic bacteria of neonatal purulent meningitis are different in different regions of China. The most common pathogen is E. coli, which is sensitive to meropenem, while it has different degrees of resistance to other common cephalosporins, especially to cefotaxime.
5. Effects of 1-bromopropane on brain ultrastructure in male rats

OBJECTIVE: To explore the effects of sub-acute inhalation of 1-bromopropane( 1-BP) on the ultrastructure of cerebral cortex,hippocampus,cerebellum,and brainstem in male rats. METHODS: Specific pathogen free healthy male Wistar rats were randomly divided into control group and exposure group with 6 rats in each group. The rats of exposure group received 1-BP vapor at a concentration of 5 000 mg/m3. The rats in the control group were given fresh air in a dynamic inhalation chamber system for 4 weeks(6 hours/day,5 days/week). After the end of the exposure,the cerebral cortex,hippocampus,cerebellum and brainstem of rats were collected and the ultrastructural changes were observed under transmission electron microscope( TEM). RESULTS: After 3 weeks of exposure to 1-BP,the rats in the exposure group began to have unresponsiveness and decreased muscle strength in hind limbs. The body weight of exposure group was lower than that of control group from the 1 st to the 4 th week( P < 0. 05). TEM results showed destroyed structure of the myelin sheath in the region of cerebral cortex, hippocampus, cerebellum and brainstem, and irregular nucleus, vacuolar degeneration,increased lysosome of endoplasmic reticulum,mitochondrion swelling of neuron cells,karyopyknosis of astrocytes and vacuolation in the neurite of astrocytes located in the blood brain barrier( BBB). CONCLUSION: 1-BP sub-acute inhalation exposure could damage the myelin,neuron,astrocyte and BBB in male rats. The demyelination of nerve fiber and decreased permeability of BBB was particularly noticeable.
