1.Effects ofα-lipoic acid combined with motilin in treatment of patients with diabetic gastroparesis

Objective To explore the curative effects and safety of α-lipoic acid combined with motilin in treatment of patients with diabetic gastroparesis and its influence on gastrointestinal dynamics and gastrointesti -nal hormone.Methods 108 patients with diabetic gastroparesis were included and they were divided into the ob -servation group and the control group according to the random indicator method , with 54 cases in each group .All patients were given routine hypoglycemic therapy .On this basis, patients in the control group were given motilin while patients in the observation group were given α-lipoic acid combined with motilin .After 14 days, the cura-tive effects, the rate of gastric emptying, oral-colonic transit time(OCTT), and gastrointestinal hormone including gastric dynamic element ( MTL ) and somatostatin ( SS ) , stomach gastrin-releasing ( GAS ) and adverse reactions during treatment were compared between the 2 groups.Results The total effective rate was 85.19% and 64.81% respectively in the observation group and the control group , and the difference had statistical significance (P<0.05).Compared with those before treatment , gastric emptying rate in the 2 groups were significantly in-creased after treatment , and OCTT were significantly extended .The difference had statistical significance ( P<0.05).Compared with those before treatment , MTL and GAS in the 2 groups were significantly decreased while SS were significantly increased.The difference had statistical significance(P<0.05).The changes of MTL, SS, and GAS were more obvious in the observation group than in the control group , and the difference had statistical significance ( P<0.05 ) .The incidence of adverse reactions in the observation group was 11.11% and it was 7.41%in the control group.The difference had no statistical significance (P>0.05).Conclusions The cura-tive effects of α-lipoic acid combined with motilin in treatment of diabetic gastroparesis is distinct .Its mechanism may be related to improving gastrointestinal disorders , and adjusting gastrointestinal hormone .

3.Change of soluble P-selectin and von Willebrand factor levels in traumatic patients

Objective To investigate the change of soluble P-selectin (sPsel)and von Willebrand factor (vWF)levels after ad-mission in traumatic patients and their relation with the coagulation indexes levels,coagulation disorders and prognosis.Methods 82 cases of severe trauma in ICU of Affiliated Tongji Hospital were prospectively selected and detected plasma sPsel,VWF anti-gen,protein C,activity of coagulation factor Ⅶ and routine coagulation indexes on admission and on every day within the first week after admission.The 30 d fatality rate was recorded.Results The sPsel and vWF levels on admission in the patients with coagula-tion disorders were lower than those in the patients without coagulation disorders (P <0.05)and significantly correlated with the coagulation indexes (protein C and coagulation factor Ⅶ)levels (P <0.05).The vWF level within 3 d after admission in the death patients was significantly lower than that in the survival patients,but which on 7 d after admission in the death patients was signifi-cantly higher than that in the survival patients (P <0.05 );no significant difference in sPsel level within 1 week after admission were found between the survival patients and the death patients.Conclusion Among severe traumatic patients in ICU,the low lev-els of sPsel and VWF on admission are associated with the coagulation disorders,the significant rise of vWF level on 7 d after ad-mission is associated with the increase of the 30 d fatality rate.

Cognitive impairment in diabetes ( CID) is one of the complications of diabetes .The features of mild or moderate cognitive disorder and the decread abilities in memory and studying are the main symptoms of CID .But it′s mechanism is still un-known .NF-κB is the original signal activator , it can activate other signal pathway of cell dysfunction .With the influence of hyperglyce-mia and oxidative stress, the expression of NF-κB is enhanced, that leads to the changes of the expression of NOS , MMP-9, TNF-α, then, these changes lead to the apoptosis of hippocampus neuron cell .This review focuses on NF-κB in order to provide evidences in studying CID by reviewing the relationship between NF-κB and it′s relative factors of NOS, MMP-9, TNF-αand CID.
5.Influence of miR-21 control Smad7 expression and lung cancer A549 cell line proliferation

Objective To explore miR-21 regulation of Smad7 in lung cancer A549 cell line as well as its impact on the proliferation of lung cancer A549 cell line. Methods miR-21 mimic and inhibitors in lung cancer A549 cell line were transfected by using Lipofectamine 2000 Reagent. After 48 h, Western blot and qRT-PCR were applied to assess the expression of protein and mRNA of Smad7 . MTT assay was used to determine the proliferation influence of the transfected lung cancer A549 cell line. Results Western blot and qRT-PCR showed that A549 cell trans-fected miR-21 mimic exhibited down-regulated Smad7 protein and mRNA expression, and A549 cell transfected miR-21 inhibitor exhibited up-regulated Smad7 protein and mRNA expression. The A549 cell proliferation activity decreased significantly after transfected miR-21 inhibitors. Conclusion miR-21 inhibitors can increase Smad7 pro-tein and mRNA expression, and suppress the proliferation activity of lung cancer A549 cell significantly.
6.The correlation of miR-106a expression with clinical characteristics and prognosis of colon cancer

Objective To detect the correlations of miR -106 a expression with clinical features of pa-tients with colon cancer ,and to explore the significance of miR -106 a as a prognostic factor .Methods One hun-dred and five patients with primary colon cancer were retrospectively enrolled in this study including their clinical information and slices of FFPE.miR-106a expression was detected by qRT -PCR,and analyzed the correlation between the level of miR -106a and clinical features and survival .Results The level of miR -106a in tumor tissue was higher than adjacent normal tissue (1.142 vs.0.685,P<0.001).miR-106a was correlated with TNM stage and lymphnode metastasis .The higher miR -106a expression,the poorer survival patients were .The hazard risk was increased 3.390 folds(95%CI for HR:1.028 ~11.178)when compared high expression group with the low.Conclusion The aberrant expression of miR -106a may be associated with colon cancer .It will po-tentially be a therapeutic target and prognostic factors .
7.Research progress on miR-17-5p in tumors

As an important gene regulatory molecule, microRNA is closely related with various human diseases. Numerous studies have confirmed that microRNAs function as tumor suppressors or oncogenes in various tumor types. Therefore, microRNA investiga-tion has become a new direction in oncology research. As a member of the miR-17 to-92 cluster, miR-17-5p has been the focus of re-search recently. MicroRNA is involved in many aspects of diseases, such as diabetes mellitus, endometriosis, and a variety of tumors. In this review, the basic characteristics and roles of miR-17-5p in tumors are elaborated.
8.Evaluation of the significance of EBV nucleic acid and serology tests in the management of EBV-related diseases

Objective To investigate the relationship between Epstein-Barr virus (EBV) DNA and EBV serology markers and evaluate the clinical application values in different diseases.Methods Plasma samples from 397 diagnosed EBV infection-associated patients and 120 health donors from May 2014 to November 2015 in Wuhan Tongji Hospital were collected.Real-time fluorescent quantitative PCR was performed to detect the levels of EBV-DNA in peripheral blood mononuclear cell and plasma.ELISA was used to detect VCA IgA,VCA IgM,VCA IgG,EA(D) IgG and EBNA IgG antibodies in plasma.The positive rate of EBV-DNA and EBV antibodies were counted in each group according to the detection threshold.Kappa statistic and Spearman sank correlation test were used to analysis the correlation and uniformity between EBV-DNA and EBV serology indicators.Results The positive rate of VCA IgG in patient and health control was 94.2% (374/397) and 93.3% (112/120) respectively (χ2=0.125,P=0.67);The positive rate of EBNA IgG in patient and health control was 95.4% (379/397) and 95.0% (114/120) respectively (χ2=0.045,P=0.807);but the positive rate of VCA IgM was 5.5% (22/397) and 0% (0/120) respectively (χ2=6.9,P<0.01);The positive rate of VCA IgA was 43.3% (172/397) and 9.2% (10/120) respectively (χ2=49.5,P<0.01);The positive rate of EA(D) IgG was 42.0% (167/397) and 7.5% (9/120) respectively (χ2=49,P<0.01).The positive rate of EBV-DNA was 65.5% (260/397) and 16.7% (20/120) respectively (χ2=88.5,P<0.01);The positive rate of EBV-DNA in plasma was 45.8% (182/397) and 5.0% (6/120) respectively (χ2=66.4,P<0.01).Furthermore,the uniformity and Spearman correlation analysis showed that there was no significant correlation between EBV-DNA and EBV serology indicators.The correlation analysis between PBMC EBV-DNA and VCA IgM,VCA IgA,EA(D) IgG showed the Kappa was 0.073,0.147,0.073,respectively;the correlation analysis between plasma EBV-DNA and VCA IgM,VCA IgA,EA(D) IgG showed the Kappa was 0.144,0.369,0.288,respectively.Thus,the patients were divided into different groups according to the discharge diagnosis,it was observed that the positive rates of EBV-DNA is more than 90% in extra-nodal NK/T cells lymphoma,EBV-associated hemophagocytic lymphoid tissue hyperplasia,chronic active EBV infection and infectious mononucleosis.In nasopharyngeal carcinoma patients,the positive rate of EBV antibodies (VCA IgA and EA(D) IgG) were higher than the detection of EBV-DNA.Conclusions There was no significant correlation between EBV-DNA and EBV serology markers for the same sample.The clinical application values of EBV DNA and EBV serology markers were not identical in nasopharyngeal carcinoma,extra-nodal NK/T cells lymphoma,infectious mononucleosis and EBV-associated hemophagocytic lymphoid tissue hyperplasia.

10.The preliminary study on mortality prediction for patients in surgical intensive care unit with protein C

Objective To determine whether anticoagulation markers can improve mortality prediction in patients of surgical intensive care unit (ICU).Methods A case-control study was adopted,252 patients from Tongji hospital's surgical ICU and 30 healthy control individuals were investigated.The protein C,antithrombin,thrombomodulin,and other coagulation/ inflammatory markers were detected.The Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score were obtained.Markers level comparison among survivors,non-survivors and controls were conducted with single factor variance analysis,Kruskal-Wallis test or Mann-Whitney U test.Results Between survivors and non-survivors after 28-day hospitalization,there were significant difference on protein C levels[(70.2 ±22.7)% vs (48.6 ±29.8)%,t=2.84,P<0.01],APACHE Ⅱ scores[(21.0±8.2) vs (29.5 ±10.9),t =-2.51,P<0.05] and prothrombin times[(12.9-± 3.5) s vs (18.8 ± 10.2) s,t =-2.13,P < 0.05].Combining protein C levels with APACHE Ⅱ score could obtain a higher mortality prediction efficiency in patients of surgical ICU than any single marker (AUC =0.806).That protein C concentration less than 47.5% [OR =6.40,95%confidence interval(CI) 2.526-16.216,P <0.001] and APACHE Ⅱ score (OR =1.123,95% CI 1.012 -1.250,P < 0.05) were the independent risk factors for surgical ICU death.Conclusion Decrease of protein C levels predict increase of mortality risk in patients of surgical ICU,combining protein C with APACHE Ⅱ score can improve the prognostic accuracy for patients of surgical ICU.(Chin J Lab Med,2013,36:339-342)
