Objective To investigate the expression and correlation of cell cycle protein-D1 and Ki-67 in laryngeal cancer tis-sues.Methods Immunohistochemical streptavidin-perosidase ( SP) staining method was used to detect the expression of Cyclin D 1 and Ki-67 proteins in laryngeal squamous cell carcinoma tissues and their correlation between Cyclin D 1 and Ki-67 was analyzed .Re-sults Among 56 cases of Cyclin D1 positive over-expressed laryngeal squamous cell carcinoma tissues , 41 cases of Ki-67 had positive over-expression.Among 51 cases of Ki-67 low-expressed tissues, 35 cases of Cyclin D1 had low-expression.Spearman rank correlation test showed that expressions of Cyclin D 1 and Ki-67 in laryngeal squamous cell carcinoma tissues had a significant positive correlation ( rs =0.620, P <0.01).Conclusions Cyclin D1 and Ki-67 expressions in laryngeal squamous cell carcinoma have a significantly positive correlation .

Objective To investigate the correlation between subacromial space and acromial shoulder impingement syndrome (SIS).Methods The clinical and imaging data of 40 patients with SIS were collected and retrospectively reviewed. The shape of acromion and tears of rotator cuff of all patients were classified.A cromion-Humerus (A-H) distance was measured. Results Among 40 patients, there were 9 cases of type Ⅰ,12 cases of type Ⅱ,19 cases of type Ⅲ.32 patients had subacromial space narrowing.7 cases had calcification in the supraspinatus tendon and 10 patients had acromioclavicular joint ostcoarthritis. There were 17 cases of type Ⅰ,15 cases of type Ⅱ,18 cases of type Ⅲ of tears of rotator cuff.6 patients had subacromial bursa thickening,5 patients had subacromial-deltoid sac fluid and 10 patients had the joint capsule fluid.2 patients had jointlabrum avulsion,3 patients had the supraspinatus muscle atrophy and 4 patients had Bankart Lesion. There was no significant difference was found in A-H distance measured with X-ray and MRI(P =0.137 0).There was positive correlation between the A-H distance and class of rotator cuff(r =0.545,P =0.000 1).Conclusion There exists some correlation between subacromial space and SIS.The diagnosis and class of SIS can be assessed according to narrowing range of subacromial space.

Objective To understand the effectiveness evaluation research of tumor MDT,and analyze the development status and differences of evaluation tools at home and abroad,to provide reference for the subsequent summary evaluation and continuous improvement of tumor MDT,and the strengthening of MDT supervision. Methods Four literature databases at home and abroad were searched to obtain relevant literatures, and literature screening and systematic review were conducted. Results A total of 87 literatures were included, including 26 literatures in Chinese and 61 literatures in English;the most published years were 2020;the main countries of the first authors were the UK.Foreign evaluation tools focus on the key elements of structure and process, while evaluation systems in China focus on the index content at the result level. Conclusion In China, the scientific and comprehensive selection of tumor MDT evaluation indicators needs to be improved, the analysis of influencing factors on the structure and process of MDT needs to be strengthened, and the extrapolation of the existing evaluation systems need to be verified. It is suggested to strengthen the evidence support of evaluation index selection, attach importance to the evaluation of process links, promote the in-depth study of the influencing factors of tumor MDT,and further encourage the empirical application of the existing evaluation system.

ObjectiveTo assess the effectiveness of tele-medicine and self-management goal(SMG) setting technique used in the diabetes management in the community setting. Methods It is a control-group study.415 type 2 diabetic residents were recruited from the Shanghai Wuliqiao community based on existing medical records. The subjects were divided into two groups, the study group was cared by general practitioners (GPs) specialists cooperation through the tele-medicine mechanism, the other was a control group. For the study group,a cooperation pathway between community health care centers and general hospitals were established.Standardized training and guidelines were provided to community health workers, regarding the setting of management goals of blood glucose and blood pressure, treatment plan, patient education, and SMG techniques.Fasting blood glucose ( FBG ) and 2 h postprandial blood glucose (2hBG) in the study group were monitored, followed by community health workers visiting monthly with seminars for diabetes education. At the baseline and the 12tb month,FBG,2hBG,HbA1C,blood pressure, triglyceride, total cholesterol, body mass index,waist-hip ratio were determined in each group.A survey was conducted to evaluate the costs of diabetes treatments, the knowledge base related to their disease, lifestyle, and the awareness of the new care model. The rates of achieving the goal of blood glucose, blood pressure, and HbA1Ccontrol were calculated.Internet case discussion between GPs-Specialists and referral to certain specialists were implemented when some patients did not reach the control goal.ResultsBy the 12 month follow up,FBG,2hBG,HbAIc,blood pressure of the study group were lower than the baseline, and as well as the control group with statical significance (P<0.05).There are other improvcments:diabetes knowledge (29.1% vs 5.5% ),healthy diet (9.6% vs -10.4% ),blood glucose monitoring (30.3% vs 10.8% ),support for diabetes care in community (35.7% vs 9.4% ),and the preference of the new model (63.8% vs 17.9% ) with statistical significauce (P<0.01 ).As for the medical costs, the study group's monthly costs were consistently lower than the control's.( -3.39Yuan vs 32.26 Yuan,P<0.05).ConclusionsThe new diabetes care model based on GPs-Specialists tele-medicine and SMG in community opens the door to the community based care model formulation in regard to the health quality and costs control. The deployment of more technologies and management techniques could be explored further to improve the outcomes of community based chronic disease care model.

Objective:To construct a value evaluation framework for high-value medical consumables, providing a guidance for medical insurance access and hospital access management scenarios in China. Methods:It conducted literature review, qualitative in-terviews and quantitative surveys.A total of 12 experts were invited for qualitative interviews, while 100 experts from four fields of health technology assessment, medical insurance, hospital management, and clinical practice participated in the quantitative survey.Through those process,it generated the composition of the value framework and the scoring of each item.Differences in ratings be-tween different scenarios and experts were analyzed through chi-square tests. The recommendation level for each item was graded.Re-sults:A comprehensive value evaluation framework for high-value medical consumables was established, which included 6 core dimen-sions,comprised 16 items for secondary dimensions and 50 items for tertiary dimensions. It showed significant differences between the medical insurance access and hospital access scenarios,as well as among different fields of experts in the same scenario.furthermore, grading the items in two scenarios. The medical insurance access scenario had 8 highly recommended items, and the hospital access scenario had 24 highly recommended items. Conclusion:Value evaluation should encourage multi-dimensional assessments and inter-disciplinary participation, continually improving the management of high-value medical consumables in medical insurance and hospital access.
