1.Observation of the curative effect of colonoscopy combined with laparoscope in the treatment of colonic polyps

Objective To explore the curative effect of colonoscopy combined with laparoscope in the treatment of colonic polyps.Methods The clinical data of 48 cases with colonic polyps were retrospectively analyzed.According to the operation pattern,the patients were divided into observation group(23 cases)and control group(25 cases).The observation group received colonoscopy combined with laparoscopy for radical surgery of colon polyps,the control group used the traditional open surgery to remove polyps.The operation time,bleeding volume, exhaust and defecation time,hospital days and cost as well as postoperative complications were observed.Results The two groups were successfully completed surgery.The operative time,bleeding volume of the observation group were (78.3 ±8.2)min and (1 3.1 ±4.5)mL respectively,which of the control group were (1 1 5.5 ±1 0.1 )min, (63.6 ±1 8.1 )mL,the differences between the two groups were statistically significant(t =1 3.93,1 3.01 ,all P <0.05).The anal exhaust time and defecation time of the observation group were (25.1 ±6.5)h and (42.5 ±7.6)h, which were significantly shorter than those of the control group[(36.6 ±8.1 )h,(82.3 ±1 1 .5)h],the differences were statistically significant(t =5.39,1 4.01 ,all P <0.05).The hospitalization time,hospital costs of the observation group were (5.5 ±0.6)d,(1 3 842.5 ±875.5)yuan,which of the control group were (8.2 ±1 .5)d,(1 8 992.5 ± 1 001 .5)yuan,the differences between the two groups were statistically significant(t =8.05,1 8.89,all P <0.05). Pulmonary infection in 1 case,anastomotic fistula in 2 cases,1 case of intestinal obstruction in the observation group, and in the control group,pulmonary infection occurred in 2 cases,2 cases of intestinal obstruction,the postoperative complications between the two groups had no statistically significant difference(P >0.05).Conclusion For simple colonoscopy treatment difficulties of colon polyps,colonoscopy,laparoscopy combined treatment is minimally invasive and can improve the safety,strictly control surgical indications,can give full play to the double mirror combined advantage.
2.Experiment of nanobubbles-mediated plasmid green fluorescent protein(GFP) transfer into skeletal muscle in mice

ObjectiveTo explore the effect of nanoscale bubbles transfering gene in skeletal muscle cells in mice.MethodsPlasmid DNA encoding green fluorescent protein (GFP) was mixed with bubbles dissolved in saline and injected into the tibialis anterior (TA) muscle of C57B10 mice with and without ultrasound (US).The ultrasound frequency was 1 MHz and the pulse repetition frequency was 100 Hz with 20% duty cycle.The spatial peak temporal peak intensity (ISPTP) power level was 2 W/cm2.The entire treatment period was 30 seconds.The efficiencies of GFP transgene expression were determined under different experinmental conditions.Mice were sacrificed 1 week after plasmid DNA injection.Fibres with fluorescence green signals were determined as GFP-positive fibres by fluorescence microscopy.Readout was performed on the section with the maximum number of transfected fibers.Results1 )Albumin nanobubbles:in the conditon of with or without ultrasound,albumin nanobubbles had significantly increased gene expression compared with negative control (P <0.05),but significantly decreased gene expression compared with positive control ( P < 0.05 ).2) Phospholipid nanobubbles:In the conditon of without ultrasound,there were no significant differences compared with negative and positive control ( P >0.05).In the conditon of with ultrasound,phospholipid nanobubbles had significantly increased gene expression compared with negative control ( P <0.05).No significant difference was observed between phospholipid nanobubbles and positive control ( P > 0.05).ConclusionsNanobubbles could enhance gene transfection efficiency for skeletal muscle fibres in mice.Nanobubbles with perfluoropropane gas and albumin have potentiality in gene-enhanced transfection.
3.Host immune responses to avian influenza A H5N1 virus in a BALB/c model

Objective To study the cell immunity and eytokines responses to avian influenza A H5N1 virus infections in a BALB/c model to better understand the pathogenesis of H5N1 avian influenza disease. Methods Two hundred and twenty BALB/c mice of the infected group were inoculated with 0.1 ml (10-4.875 TCID50) of A/Goose/Guangdong/NH/2003 ( H5N1 ) virus intra-nasally. Fifty control mice received noninfectious allantoic fluid and another fifty control mice received normal sodium. Blood and spleen samples were collected from the live mice every 24 h during the 14 d post-infection. The changes of CD3 + T cells , CD4 + T cells, CD8 + T cells for cell immunity in blood circulation and spleen were detected by flow cytometry. And the cytokines and antibody responses in blood circulation were detected by ELISA. Necropsy was performed on mice that died during the experiment and those euthanized at end of study. Results Avian influenza A( H5N1) virus infections can make damages to the cell immune system transiently. The CD3 + T cells, CD4 + T cells, CDS + T cells declined at 24 days post infection in blood circulation and declined at 5-8 days in spleen, then recovered to the normal level gradually. The eytokines responses to the infections can be detected: the level of IFN-γ,TNF-α declined, IL-4, IL-18, IL-10 increased, and IL-2 changed little. The antibody increased rapidly from day 7 post infection until the end of the study (day 14 post infection). Conclusion Collectively, avian influenza A(H5N1) virus can cause cell immunity deficiency and an imbalance in the level of eytokines, which may contribute to the unusual severity of disease caused by the H5N1 avian influenza virus.
4.Diaphragm dysfunction and expressions of calcium regulatory proteins in diabetic rats.

OBJECTIVETo study the changes in diaphragmatic function and gene expressions of calcium regulatory proteins in diabetic rats and explore the mechanism of diaphragm dysfunction in diabetes mellitus.
METHODSSD rats were randomly divided into normal control group and diabetic (induced by intraperitoneal STZ injection) group. After 4 and 8 weeks, the body weight and diaphragm to body weight ratio were measured, and the activities of succinic dehydrogenase (SDH) in the diaphragm and blood glucose were assayed. The diaphragm contractility was assessed and the alterations of diaphragm ultrastructure were observed. RT-PCR was used to detect the changes in sarcoplasmic reticulum Ca(2+)-ATPase (SERCA) and phospholamban (PLB) mRNA expressions in the diaphragm.
RESULTSThe diabetic rats showed a significant weight loss with a lowered diaphragm to body weight ratio (P<0.01) and SDH activity (P<0.01). The peak twitch tension and maximum tetanic tension of the diaphragm were significantly lowered and the time to peak contraction and half relaxation time significantly prolonged (P<0.01) in the diabetic rats, which also exhibited a lowered tetanic force in response to stimulus (P<0.01). Transmission electron microscopy revealed obvious ultrastructural changes of the diaphragm in diabetic rats. RT-PCR showed significantly decreased SERCA and increased PLB mRNA expressions in diabetic rat diaphragm (P<0.01), and these changes intensified with time (P<0.01).
CONCLUSIONDiabetes can cause impairment of diaphragmatic ultrastructure, mitochondrial injuries, and lowered SDH activity and ATP production. Decreased SERCA and increased PLB mRNA expressions in diabetes result in reduced Ca(2+) uptake by the diaphragm sarcoplasmic reticulum to induce diaphragm dysfunction.

5.Analysis on mortality and premature death rates of four major chronic diseases in Taizhou, Zhejiang 2011‒2018

ObjectiveTo analyze the characteristics of death and premature death of 4 major chronic diseases (cardiovascular and cerebrovascular diseases, malignant tumors, chronic respiratory diseases and diabetes) in Taizhou City from 2011 to 2018,and provide data basis for the government to formulate chronic disease prevention planning. MethodsThe death data of household registration residents in Taizhou City from 2011 to 2018 were derived from the Chronic Disease Surveillance Information Management System in Zhejiang Province. The death toll ratio of chronic diseases, the mortality rate of chronic diseases, the probability of premature death of chronic diseases were analyzed. The standardization rate was calculated six times in 2010. Population composition of the census. The Joinpoint Regression Program 4.2 software was used for calculating annual percent change (APC) and its statistical test results. ResultsFrom 2011 to 2018, there were 231 724 chronic disease deaths in Taizhou City, with a mortality rate of 486.52/105 and a standardized mortality rate of 381.55/105. The proportion of chronic disease deaths to total deaths was 79.89%, of which males were higher than females and rural areas were higher than urban areas.From 2011 to 2018, the standardized mortality and early death probability of cardiovascular and cerebrovascular diseases, malignant tumors and chronic respiratory diseases in Taizhou showed a downward trend (P<0.05), the standardized mortality of diabetes (P=0.46) and the early death probability (P=0.22) did not decline, and the mortality of all age groups of the above four types of chronic diseases in rural areas was higher than that in urban areas. The mortality of the four types of chronic diseases from high to low are cardiovascular and cerebrovascular diseases, malignant tumors, chronic respiratory diseases and diabetes, and the mortality tends to increase with age. From 2011 to 2018, the probability of premature death from four types of chronic diseases in Taizhou City showed a downward trend, from 13.49% in 2011 to 10.49% in 2018, with an average annual decrease of 2.97%. The difference was statistically significant (t=‒5.83,P<0.05). ConclusionChronic disease death is the main cause of death in Taizhou City. In order to reduce the mortality rate of chronic diseases, effective prevention and control measures for chronic diseases should be carried out, especially the prevention and control of diabetes and male chronic diseases.
