1.ANALYSIS OF PLATELET ACTIVATION AND ITS CLINICAL SIGNIFICANCE IN OLDER HYPERTENSIVE PATIENTS

To investigate the changes of activated platelet marker and its significance in older hypertensive patients, glycoprotein Ⅱb/Ⅲa(PAC 1)and P selectin(CD62P) were used as molecular markers for activated platelets,which were analyzed by flow cytometric 3 color immunofluorescence. The results showed that PAC 1 expression on platelet membrane surface in older hypertensive patients was higher than that in healthy individuals and was not related with mean blood pressure.However, the expression of CD62P on platelets membrane surface did not differ in the two groups.These results suggest that PAC 1 may be regarded as a sensitive and specific molecular marker for activated platelets. Platelet activation occurrs in older hypertensive patients, and is of great importance for the antiplatelet therapy in hypertensive patients.
2.Influence of two different data collecting methods on the patients' evaluation of quality nursing care

Objective To investigate the inpatients' evaluation of quality nursing care and disucss different data collecting methods on patients's evaluation of quality nursing care. Methods Patients measure up to standard (82 cases) were divided into the questionnaire group and the interview group with 41 patients in each group. The questionnaire investigation was carried out in the questionnaire group by applying 4 themes of quality nursing care. The interview group used structured interview instead. The results of the two methods were compared and analyzed. Results The score of quality nursing care of the questionnaire group was higher than that of the interview group (P<0.01) in evaluating health education method. In humanistic environment patients in the questionnaire group thought nurses could protect their privacy, respect their personality and treat them and their family members kindly. Besides visiting institution was reasonable. All of these were the demonstration of quality nursing care. The above results were different from those of the interview group (P<0.01). Conclusions Patients had comprehensive requirement on quality nursing care. Nurses should, use this as reference, regulate their service behavior and improve nursing quality.
3.Primary PCI for acute myocardial infarction caused by acute left main coronary artery occlusion

Objective To study mortality of acute myocardial infarction caused by acute left main coronary artery occlusion.The objectives of this analysis were to determine the effect of primary PCI and the impact of cardiogenic shock on acute myocardial infarction caused by acute left main coronary artery occlusion.Methods From 1999 to 2005,of 752 consecutive patients with acute myocardial infarction,16 patients had acute left main coronary artery occlusion with TIMI flow≤2.All patients were given primary PCI.Results Of these 16 patients,9(56.25%)died in hospital,and 7(43.75%)discharged.In the survival group reperfusion was successful in 100% of patients,as opposed to 44.44% in the mortality group(P=0.019).Cardiogenic shock was overt in 12(75.00%)patients,42.86% of the survival group and 100% of the mortality group(P=0.020).Shock patients had higher in-hospital mortality than stable patients(75 % vs 0%,odds=4.0,95%CI 1.50~10.66,P=0.019).Conclusion Patients presenting with AMI caused by acute left main coronary artery occlusion and cardiogenic shock have poor survival regardless of primary PCI.Nevertheless,primary PCI is a feasible and effective procedure,and it may save lives in this clinical setting.
4.Modified Yupingfeng Powder in Adjuvant Therapy of Allergic Rhinitis:A Systematic Review

OBJECTIVE:To systematically evaluate therapeutic efficacy and safety of Yupingfeng powder in adjavant therapy of allergic rhinitis,and to provide evidence-based reference for clinical treatment. METHODS:Retrieved from Wanfang database,VIP,CJFD,CBM,Cochrane Library,EMBase and PubMed,RCTs about Yupingfeng powder combined with chemical medicine (trial group)vs. chemical medicine alone in the treatment of allergic rhinitis were collected. Meta-analysis was conducted by using Rev Man 5.2 statistical software after data extraction and quality evaluation according to Cochrane system evaluation method 5.1.0. RESULTS:A total of 15 RCTs were included,invovling 1366 patients. The results of Meta analysis showed that response rate [OR=3.95,95%CI(2.80,5.58),P<0.001],recurrence rate [OR=0.37,95%CI(0.22,0.63),P<0.001] and the incidence of ADR [OR=0.15,95%CI(0.06,0.35),P<0.001] in trial group were significantly higher than control group,with statistical significance. CONCLUSIONS:Yupingfeng powder in adjuvant therapy of allergic rhinitis show good therapeutic efficacy and can reduce recur-rence rate with good safety.
5.Preliminary study of salivary gland mass qualitative diagnosis with contrast-enhanced ultrasound

Objective To investigate the qualitative diagnosis of salivary gland mass with contrastenhanced ultrasound(CEUS).Methods The CEUS manifestations in 78 cases with salivary gland mass were observed after intravenous bolus injection with contrast agent SonoVue and confirmed by histology.Results Among 78 salivary gland masses,there were 29 cases with pleomorphic adenomas (37.2%),19with Warthin's tumors (24.4 %),7 with basal cell adenomas (8.9 %),and 11 with the other benign masses (14.1%),12 with malignant tumors (15.4%).The intensity of contrast-enhanced masses,whether the enhanced mass margin was clear,whether the peripheral enhancement rim was complete and whether the mass was enlarged were the diagnostic criteria to differentiate the benign and malignant tumors.The accuracy,the sensitivity,specificity,positive predictive value and negative predictive value were 87.2%,95.2%,56.3%,89.4%,75.0% respectively,and the results also displayed positive likelihood ratio and negative likelihood ratio were 2.178,0.085 respectively.Although they presented with the highest incidence among benign tumors,their CEUS manifestations showed remarkable statistical differences when the pleomorphic adenoma was compared with Warthin' s tumor and basal cell adenoma respectively in enhancement intensity(P <0.01) while there were no statistical differences between Warthin's tumors and basal cell adenomas (P >0.05).Conclusions CEUS manifestations of salivary gland mass were helpful to the differential diagnosis of various salivary gland tumors.
7.Clinical effects of tirofiban intra-coronary artary inject on patients with ST segment elevation myocardial infarction no-reflow and acute thrombosis after emergency intra-coronary artary stenting

ObjectiveTo evaluate the efficacy and safety of tirofiban in treatment of ST segment elevation acute myocardial infarction(STEAMI) no-reflow and acute thrombosis after emergency percutaneous coronary intervention(PCI). MethodsForty patients which were made definite diagnosis of STEAMI were intra-coronary artary injection fortirofiban after emergency PCI stenting occured no-reflow and acute thrombosis. First,the dose of 0.4μg·kg-1·min-1 was given from intra--coronary artary injection of tirofiban within three minutes, after 30min the dose were given 0.1μg·kg-1·min-1 for 48 hours. ResultsThe no re-flow and acute thrombosis was completely disappeared within five minutes,at the time,side effect with in one week was not observed. ConclusionsTirofiban treatment by direct injection in coronary arteries combined with emergency PCI, can increase the repeffusion rate of infarction related vessel in AMI patients,and improve TIMI reflow. This reperfusion method was effective and safe.
8.Study on the correlation of serum insulin-like growth factor-1 and radioactive iodine uptake rate in patients with nodular goiter

Objective To study the correlation of the serum insulin-like growth factor-1(IGF-1) level and the radioactive iodine uptake rate in patients with nodular goiter,to explore a simple and saftymethods of identifying different kinds of thyroid nodule.Methods Sixty diagnosed cases was selected intohot nodule group(30 cases)and cool nodule group(30 cases),and 30 healthy people were selected into thehealthy control group.Measured the IGF-1,FT3,FT4,TSH level of each group by radioimmunoassay,and measured the radioactive iodine uptake rate by radionuclear technique.Results To compare with healthy control group,the levels of serum IGF-1,FT3,FT4 and the radioactive iodine uptake rate of hot nodule group were increased obviously,but the serum TSH was decreased significantly,and the level of IGF-1 had positive correhtion with the radioactive iodine uptake rate(r=0.835)in the same group.Meanwhile,the 1 evel of IGF-1 had negative correlation with the level of TSH(r=-0.326).In cool nodule group,the levels of serum IGF-1,FT3,FT4,TSH had no significant difference oompared with healthy control group(P>0.05).Conclusion The level of IGF-1 and radioactive iodine uptake rate in patients with nodular goiter has certain correlation.
9.Comparative analysis of ultrasonographic feature and histopathology in salivary Warthin's tumor.

OBJECTIVETo promote the diagnostic accuracy of salivary Warthin's tumor by summarizing the ultrasonographic features of salivary Warthin's tumor.
METHODSSixty-nine lesions (58 patients with salivary Warthin's tumor) were retrospectively studied by ultrasonography and confirmed by histopathology.
RESULTSThe ultrasonographic manifestations of salivary Warthin's tumor demonstrated in three patterns.Type I: Solid mass(34 lesions, 49.3%), the mass was solid, grid-shaped echo inside. The mass was pathologically composed of such various tissues as epithelial cells, lymphatic organization, fibrous tissue which were compactly and evenly arranged, and little mucous formed the cyst cavity in which there projected epithelial papilla. Type II: Cystic-solid mixed mass(30 lesions, 43.5%), there was no flake-like echo, cystic-solid boundary was clear. Histopathological study revealed glandular epithelium made up irregular catheter and cyst cavity with yellowish mucous, glue-frozen-like stuff or brown colloidal substances. Type III: Cystic mass (5 lesions, 7.2%), the mass manifested separated cystic echo crumbs. The histopathological study showed salivary Warthin's tumor with infarctions and infections. The coincidence of ultrasound and histopathology diagnose was 79.7%.
CONCLUSIONThe ultrasonographic features of salivary Warthin's tumor are mainly based on the organization of histopathological study. The grasp of various ultrasonographic imagings and pathological changes is beneficial for increasing the accuracy rate of diagnosis higher.

10.Association of cathepsin L with coronary heart disease and its risk factors.

OBJECTIVE:
To explore the relationship of cathepsin L (CatL) with coronary heart disease (CHD), severity of coronary stenosis and risk factors of CHD.
METHODS:
A total of 137 CHD patients and 48 controls were included in the study, to determined the serum levels of CatL, high sensitive C reactive protein (hs-CRP), fasting glucose (FBS), total cholesterol, triglyceride, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol (HDL-C), apolipoprotein A1(Apo-A1) and apolipoprotein B. All the subjects were invited for a coronary angiography, using the sum of the Gensini scores to assess the severity of coronary artery stenosis.
RESULTS:
Serum CatL levels were significantly higher in CHD patients (5.63 +/= 0.12 microg/L) than non-CHD subjects (3.93 +/= 0.22 microg/L, P<0.01). CatL was an independent risk factor of CHD in Logistic regression analysis [Exp(B)=2.341, 95%CI 1.567 approximately 3.496, P<0.01]. Serum CatL levels were associated positively with the Gensini scores(r=0.228, P<0.01); In fact, CatL was an independent correlator of Gensini scores (P<0.05). CatL inversely associated with HDL-C (r=-0.228, P<0.01) and ApoA1(r=-0.187, P<0.05), and positively with FBS(r=0.161, P<0.05).
CONCLUSION
CatL is involved in the pathogenesis of CHD. Serum CatL levels could reflect the severity of coronary luminal narrowings. CatL might participate in glucose and lipid metabolic disorders.
