2.Effects of Health Education Prescription on the Reduction of Intravenous Dripping Leakage in Obstetrics and Gynecology Department of Our Hospital

OBJECTIVE:To investigate the influence of health education prescription on the reduction of intravenous dripping leakage in obstetrics and gynecology department of our hospital. METHODS:Totally 300 inpatients selected from obstetrics and gy-necology department of our hospital Jun.-Nov. 2015 were divided into observation group and control group according to random number table,with 150 cases in each group. Control group received health education of daily operation during intravenous dripping. In observation group,intravenous dripping treatment card came with a health education prescription of intravenous dripping,which facilitate the patients and families to consult;the nurses carried out health education for patients every day during dripping in accor-dance to the prescriptions;clinical pharmacists accompanied resident physicians to make patient round and conducted random health education inspection. Awareness rate of health education prescription content,satisfactory rate of treatment,dripping rate of Magne-sium sulfate injection and Cefazolin sodium for injection,the incidence of ADR were compared between 2 groups. The intravenous dripping leakage were observed in 2 groups. RESULTS:The awareness rate of health education prescription content and satisfactory rate of treatment were 94.0% and 96.0% in observation group,which was significantly higher than 70.35% and 85.75% of control group,with statistical significance (P<0.05). The dripping rate of Magnesium sulfate injection <40 drops/min accounted for 97.6% in observation group,which was significantly higher than 91.6% of control group;the incidence of ADR was 0.3% in ob-servation group,which was significantly lower than 1.3% of control group. The dripping rate of Cefazolin sodium for injection <60 drops/min accounted for 92.3%,which was significantly higher than 85.0% of control group;the incidence of ADR was 0.4%in observation group,which was significantly lower than 1.0% of control group,with statistical significance(P<0.05). The intra-venous dripping leakage rate was 5.5%in observation group,which was significantly lower than 9.5%of control group,with statis-tical significance(P<0.05). CONCLUSIONS:Health education prescription can significantly improve satisfactory degree of treat-ment and reduce ADR and leakage during intravenous dripping
3.Application of extrapedicular infiltration anesthesia in unipedicular percutaneous vertebroplasty or percutaneous kyphoplasty

Objective To introduce extrapedicular infiltration anesthesia as an improved method of local anesthesia which applied to unipedicular percutaneous vertebroplasty or percutaneous kyphoplasty.Methods From March 2015 to March 2016,20 patients in our hospital received percutaneous vertebroplasty or percutaneous kyphoplasty with 1% lidocaine local infiltration anesthesia and extrapedicular infiltration anesthesia.The visual analogue score of patients during the operation and whether they needed additional sedative anesthesia were evaluated.The anaesthetic effect of nerve root block was observed.Results The visual analogue score of all the patients ranged from 1 point to 3 point,averagely (2.5 ± 0.7) point.Among the 20 patients,there were 2 cases of 1 point,7 cases of 2 point and 11 cases of 3 point.No patients required additional sedative anesthesia,and no nerve root block effects were observed.Conclusion Extrapedicular infiltration anesthesia provides good local anesthetic effects without significant complications,which deserved further use in unipedicular percutaneous vertebroplasty and percutaneous kyphoplasty.
4.Research of anatomical imaging on operative window related to L1 ~L2 oblique lumbar interbody fusion

tomography angiography ( CTA) and T12-S1 vertebral computed tomography three-dimensional reconstruction were selected .The operative win-dows of L1 ~L2 OLIF were observed:the vascular window ,bare window ,psoas major window ,ideal operative window and actual operative win-dow.The operative windows ’ percentage accounted for ideal operative window were calculated ,the actual operative window based on an actual operative window of <1 cm,≥1 cm were statistically analyzed ,and the positions of the left renal artery and renal vein in front of operative window of L1 ~L2 OLIF were observed.Results The actual operative window was <1 cm in 2 cases (3.3%) and ≥1 cm in 58 cases (96.7%).In 58 cases,the difference was significant(P=0.008) in gender and men were more than women.The vascular window,bare win-dow and psoas major window accounted for the ideal operative window by 45%,43%and 12%,respectively ,and the actual operative window accounted for the ideal operative window by 55%.The left renal artery and renal vein's walking planes were at between the middle 1/3 of L1 to up 1/3 of L2 .There were 31 cases (51.7%) of the left renal artery being behind the left renal vein .Conclusion The regional anatomy of the operative window of L1 ~L2 OLIF has its own peculiarities,and not all L1 ~L2 levels are suitable for OLIF.The left renal vessels’ walk-ing planes were in front of L 1 ~L2 .Before L1 ~L2 OLIF surgery,surgeons should analyze the imaging anatomimy through imaging .
5.Implementation and effect evaluation of teacher training for Resident Planning Training

Objective To understand the effect of teacher training to provide reference for teachers' teaching methods in the standardization of clinical resident training.Method The teachers carried out five days of teacher training.Teacher training adopted the method of theory teaching and group discussion.The main content of the training had a "daily rounds with teaching","the proper use of ORIME evaluation","feedback of sandwich type","a minute tutor teaching method","SOAP method to report cases".Before the training,100 residents were randomly selected to evaluate the teaching in teaching teachers through self-designed questionnaire.After the training,the questionnaire survey was conducted among 100 residents again.All data were statistically analyzed by SPSS 22.0.The measurement data were represented by (mean or + standard deviation),and the date were compared by t test.Results Residents believed that after the teacher training,teachers' teaching level improved (P<0.05).The three aspects of "general and phased learning goals","training students' clinical thinking ability through questions","requiring students to report cases and give guidance and feedback" were the most obvious.The pre training score was (3.28 ± 0.92) (3.31 ± 1.12) (3.55 ± 0.85),and the corresponding score was after training (3.98 ±0.85) (4.19 ± 0.85) (4.11 ± 0.74).Conclusion From the comparison of the survey results before and after the residents,the teachers' teaching modes,skills and methods have been improved after receiving teachers' training.
6.Genetic analysis of a rare fetus with mandibulofacial dysostosis Guion-Almeida type.

OBJECTIVE:
To delineate the clinical and genetic features of a fetus with micrognathia, low-set ears, microtia, polyhydramnios and anechoic stomach by ultrasonography.
METHODS:
Whole exome sequencing (WES) was carried out to detect genetic variant in the fetus, for which routine chromosomal karyotyping and chromosomal microarray analysis (CMA) yielded no positive finding. Candidate variants were verified by Sanger sequencing and bioinformatic analysis.
RESULTS:
WES revealed that the fetus has carried a de novo nonsense c.2302C>T (p.Q768X) variant in exon 23 of the EFTUD2 gene, which was detected in neither parent. The variant was unreported previously and may lead to premature termination of the translation of EFTUD2 protein at the 768th amino acid. Bioinformatic analysis predicted the amino acid to be highly conserved and may alter the structure and function of the EFTUD2 protein.
CONCLUSION
The c.2302C>T variant of the EFTUD2 gene probably underlay the mandibulofacial dysostosis Guion-Almeida type in the fetus. Discovery of the novel variant has enriched variant spectrum of the EFTUD2 gene and provided a basis for genetic counseling and prenatal diagnosis for the family.
