1.Anatomical factors of wedge pressure on cervical nerves in cervical intervertebral foramen of cervical nerve and sulcus of spinal nerve

BACKGROUND: Whether nerve root is involved or not in cervical spondylosis of nerve root type is related not only to stenosis of bone canal and inflammatory disorder and swelling in peripheral soft tissues, but also to the relevant size of external diameter of nerve root.OBJECTIVE: To probe into the anatomical factors of cervical spondylosis of nerve root type with occurrence rate of involved cervical nerve in cervical intervertebral foramen and sulcus of spinal nerve.DESIGN: Single sample observation was designed.SETTING: Department of Anatomy of Chengde Medical College.PARTICIPANTS: Totally 120 sides of 60 adult corpses were employed with formaldehyde fixation (28 cases of male and 32 cases of female).METHODS: Sixty adult corpses were measured with the vernier of 0.02 mm spinal nerve and the transverse diameter of anterior branch (A-diameter) of superoinferior diameter (S - I diameter) of cervical intervertebral foramen and its spinal nerve root. The ratios were calculated between A-diameter cervical nerve and E-diameter of the corresponding sulcus of spinal nerve and between A - P diameter and S - I diameter of spinal nerve root and those of corresponding intervertebral foramen respectively. The rates of occurrence were concluded for those cervical nerves whose A-diameter larger than or equal to E-diameter of sulcus of spinal nerves and for those spinal nerve roots whose rations larger than or equal to those of corresponding intervertebral foramens.cervical nerve in C3-6 larger than or equal to E-diameter of sulcus of spinal nerve roots in C3-6 larger than or equal to those of corresponding intervertebral foramens.RESULTS: Totally 120 sides of 60 corpses were measured (56 cases of male wedge pressure in intervertebral foramen of cervical nerve root for both male and female, of which, that of female was remarkably superior to the male was 6. 3% for wedge pressure of anterior branch of cervical spinal nerve in sulcus in spinal nerve for both male and female, of which, that of male was remarkably superior to the female(8.9%, 3.9%, χ2 = 6.65, P < 0. 01 ).CONCLUSION: The involvement of cervical nerve is related not only to the stenosis of bone canal but also to the injury of peripheral soft tissues. By the ratios between E-diameter of nerve root and intervertebral foramen and between A-diameter of cervical nerve and E-diameter of sulcus of spinal nerve, those ≥ 1 can be obtained directly as the rate of occurrence of nerve compression.
2.Effect of Tongru Sanpitang on Hyperplasia of Mammary Glands in Model Rats

Objective To research the mechanisms and the link of therapy and the optimal dosage of Tongru Sanpitang on hyperplasia of mammary glands in model rats. Methods Rats were randomly divided into 6 groups:negative contronl group, model group, Rupixiao Tablets group and Tongru Sanpitang low, middle, high dose group. Model was made with complex method of Progesterone and Estradiol benzoatehad injected into abdominal. In the middle of experiment, rats were given different drug respectively. Estradiol, follicle stimulating hormone, luteinizing hormone, the expression of estrogen receptor, weight of the ovary and uterus and visceral coefficient were determined. Results Follicle stimulating hormone, luteinizing hormone, the expression of estrogen receptor in medicine group decreased. The ovary and uterus in electron microscope, weight of organs and organ coefficient were improved. Conclusion Tongru Sanpitang may adjust reproductive endocrine function by decreasing sensitivity of mammary glands to estradiol and adjust whole function in the treatment of hyperplasia of mammary glands. There are dose-effect relationship.
3.Comparison of the occurrence rates of cervical nerve entrapment at cervical intervertebral foramen and spinal nervous sulcus

BACKGROUND: Nerve root could be directly or indirectly entrapped due to lateral backward prolapse of nucleus pulposus of intervertebral disc, as well as parenchymal inflammatory tumefaction of the soft tissues in nerve root tube. But the factors and their severity related to cervical nerve entrapment at cervical intervertebral foramen and spinal nervous sulcus still need further studies.OBJECTIVE: To investigate the relationship between the occurrence rate of cervical nerve entrapment at cervical intervertebral foramen and spinal nervous sulcus and the severity of cervical syndrome.DESIGN: A single sample study.SETTING: Department of Anatomy, Department of Geriatrics of the Affiliated Hospital, and Department of Teaching Affairs, Chengde Medical College.PARTICIPANTS: The research was conducted in the Department of Anatomy, Chengde Medical College. Totally 120 sides of 60 cadavers(28 males and 32 females) were provided by the Department of Anatomy.METHODS: The outside diameters of cervical intervertebral foramen and spinal nerve root, as well as the width of external opening of spinal nerve sulcus and transverse diameter width of the anterior branch of intra-spinal nerve were measured, and the data were dealt with statistically.MAIN OUTCOME MEASURES: The ratio of transversal diameter of the forepart of cervical nerves to the width of spinal nerve sulcus; the ratio of the peripheral diameter of cervical nerve roots to the size of intervertebral foramen; the occurrence rate where the ratio was ≥ 1.RESULTS: The ratio between cervical nerve root and intervertebral foramen was significantly larger than that between spinal nerve root and spinal nerve sulcus( t = 2.66, P < 0.01 ). The occurrence rate of cervical nerve entrapment at the intervertebral foramen(24.6% ) was higher than that in spinal nerve sulcus(6.3% ), and the difference had a considerable statistical significance(x2 =6.95, P < 0.01).CONCLUSION: It is suggested that the spinal nerve may be entrapped in the cervical intervertebral foramen and the spinal nervous sulcus, and the spinal nerve lesions in the cervical intervertebral foramen was more serious than in the spinal nervous sulcus. Therefore, in clinic, when diagnosing and treating cervical spondylosis, while nerve entrapment at the intervertebral foramen is fully considered, the possibility of nerve entrapment at the spinal nervous sulcus should not be neglected.
4.Optimum concentration and dose of ropivacaine for caudal block in neonates undergoing laparotomy under general anesthesia: evaluation of postoperative outcome

Objective To investigate the optimum concentration and dose of ropivacaine for caudal block in the neonates undergoing laparotomy under general anesthesia.Methods One hundred pediatric patients of both sexes,aged 9-30 days,weighing 2.5-4.5 kg,scheduled for elective pyloromyotomy,were randomly divided into 5 groups (n =20 each) using a random number table:control group (group C),0.10% ropivacaine 1.0 ml/kg group (group 0.1% L1),0.15 % ropivacaine 1.0 ml/kg group (group 0.15 % L1),0.10 % ropivacaine 1.2 ml/kg group (group 0.10 % L2),and 0.15 % ropivacaine 1.2 ml/kg group (group 0.15 % L2).Anesthesia was induced with sevoflurane and cisatracurium.The pediatric patients were tracheally intubated and mechanically ventilated.Remifentanil was infused intravenously at 0.2-0.3 μg· kg-1 · min-1 in group C.In 0.10 % L1,0.15 % L1,0.10 % L2 and 0.15%L2 groups,the corresponding concentrations and doses of ropivacaine were injected into the sacral canal under the guidance of ultrasound.The operation was started at 15 min after administration and sevoflurane was inhaled and the end-tidal concentration of sevoflurane was maintained at 0.8-1.0 MAC.Before induction (T1),at pyloric muscle retraction (T2),and at 4,8,12 and 24 h after operation (T3-6),blood samples were collected from the central vein for determination of plasma concentrations of cortisol and interleukin-6 (IL-6).Pain was assessed using CRIES score at T3-6.When CRIES scores > 3,10% chloral hydrate 0.5 ml/kg was given by retention enema for analgesia,and the requirement for chloral hydrate was recorded.The emergence time,extubation time,duration of stay in post-anesthesia care unit (PACU) and hospital discharge time were recorded.Bradycardia and hypotension during operation,and development of motor block of lower extremities,infection and dehiscence of incision,vomiting,and urinary retention after operation were also recorded.Results Compared with group C,no significant changes were found in the emergence time,extubation time,duration of stay in PACU,hospital discharge time,plasma concentrations of cortisol and IL-6,the requirement for chloral hydrate,and the incidence of bradycardia,hypotension,motor block of lower extremities,and infection and dehiscence of incision in 0.10% L1 and 0.15 % L1 groups,the emergence time,extubation time,duration of stay in PACU,hospital discharge time were significantly shortened,and the plasma concentrations of cortisol and IL-6,requirement for chloral hydrate,and the incidence of hypotension and infection of incision were decreased in 0.10% L2 and 0.15% L2 groups,and the incidence of vomiting and urinary retention was increased in 0.15% L1 and 0.15% L2 groups.Compared with group 0.10% L2,the incidence of vomiting and urinary retention was significantly decreased,and no significant changes were found in the other parameters mentioned above in 0.15% L2 group.Conclusion The optimum concentration and dose of ropivacaine are 0.10% and 1.2 ml/kg,respectively,for caudal block in the neonates undergoing laparotomy under general anesthesia.
5.Survey on the genes conferring plasmid-mediated quinolone resistance and those encoding β-lactamases in Escherichia coli isolates

Objective To examine the prevalence of plasmid mediated quinolone resistance (PMQR) genes and their correlation with the genes encoding β-lactamases in E.coli isolates.Methods A total of 200 levofloxacin-and/or ciprofloxacin-resistant E.coli isolates were collected from Fujian Medical University Union Hospital during the period from July to December 2013.PCR method was used to screen these E.coli isolates for the presence of qnrA,qnrB,qnrC,qnrD,qnrS,aac(6')-Ib-cr,qepA,oqxAB genes,and the blaTEM,blasnv and blacTx-M genes in the PMQR positive strains.Agar dilution method was utilized to measure the antimicrobial susceptibility of PMQR-positive strains.Phylogenetic analysis was conducted by triplex PCR.Enterobacterial repetitive intergenic consensus-polymerase chain reaction (ERIC-PCR) was used to evaluate the genetic similarity between the PMQR-positive isolates.Results Of the 200 clinical isolates of E.coli,58 (29.0%)were PMQR-positive.And qnr,aac(6')-Ib-cr,oqxAB,and qepA genes were positive in 11 (5.5%),41 (20.5%),16 (8.0%),1 (0.5%) strains,respectively.The genes encoding CTX-M-1,CTX-M-9 and TEM type enzymes was positive in 32 (55.2%),17 (29.3%),and 1 (1.7%) of the PMQR-positive strains,respectively.The blasHv gene was not identified in any isolate.PMQR-positive strains were multi-drug resistant.Phylogenetic analysis indicated that 21 (36.2%),17 (29.3%),11 (19.0%),and 9 (15.5%) of the PMQR-positive strains belonged to group A,group D,group B2 and group B 1,respectively.ERIC-PCR suggested the PMQR-positive isolates belonged to 50 different types.Only one strain was non-typeable.Conclusions Most of the PMQR-related genes in E.coli are aac(6')-Ib-cr,qnr,and oqxAB in our hospital,which are highly relevant to β-1actamase genes.PMQR-positive strains may spread by way of non-clonal dissemination in our hospital.
6.Cordyceps sinensis for acute kidney injury-a systematic review

Objective To evaluate the benefits and potential adverse effects of Cordyceps sinensis as an adjuvant immunosuppressive treatment in acute kidney injury.Methods English language database ( including MEDLINE, EMBASE and CENTRAL) in systematic review searched, using search terms relevant to this review.Chinese language databases ( including CNKI, CBM, WanFang and other resources) in systematic review also searched.It included all randomised controlled trials ( RCTs) and quasi-RCTs evaluating the benefits and potential side effects of Cordyceps sinensis for acute kidney injury, irrespective of blinding or publication language.An inclusion criterion was that immunosuppressive therapy baseline must be the same across arms in each study.Data were collected and analyzed by RevMan 5.0.Methodological risk of bias was assessed using the Cochrane risk of bias tool. Results This review included 7 RCTs (representing a total of 601 patients with acute kidney injury) that assessed the use of Cordyceps.Reporting of methods was incomplete in most studies.It conducted comparisons of Cordyceps versus no treatments on the base of same conventional treatment in both groups.Cordyceps for patients showed a favorable trends toward improving patient survival and reducing renal replace treatment, but the difference did not reach significance.However, Cordyceps significantly reduce the level of serum creatinine shorten the period of oliguria, and increase the osmotic pressure of urine.Conclusion On the base of same conventional treatment in both groups, compared with no treatment, Cordyceps significantly improves the functions of renal tubule and glomerulus.But long-term studies are needed to clarify any benefit-harm trade-off.Future studies also need to be appropriately sized and powered.
7.Application of ultrasound-guided transversus abdominis plane block for operations of inguinal region in infants

One hundred and fifty ASA Ⅰ-Ⅱ infants scheduled to undergo surgery on unilateral inguinal region,were randomly assigned to 3 groups with 50 in each group.In groups Ⅰ and Ⅱ puncture was performed at traversus abdominis plane under ultrasound-guidance,0.4 or 0.5/kg of 0.25% ropivacaine was injected respectively.In group Ⅲ the triangle of Petit was positioned through palpating and then 0.5 ml/kg of 0.25% ropivacaine was injected.HR,RR,SpO2,PETCO2,expired sevoflurane concentration and BIS value at the time of entering the operation room (T1),incision of skin (T2),pulling hernia sac (T3),ending the surgery (T4),waking (T5),the number of pressing the analgesia pump after operation were recorded.The results showed that HR at T2 and T3 of groups Ⅰ and Ⅱ was lower than group Ⅲ (P < 0.05).The were significant differences in case numbers of insufficient intraoperative analgesia among 3 groups(x2 =10.500,P =0.005).The CHEOPS scores and the number of pressing analgesia pump after operation in groups Ⅰ and Ⅱ were lower than those in group Ⅲ (x2 =7.230,P =0.027).Results indicate that ultrasound-guided transversus abdominis plane block is safer and more effective than conventional method for operations of inguinal region in infants; it may reduce dose of local anesthetics and postoperative use of analgesics.
8.Individualized quality of life measurement and its application in health service

9.Proton Magnetic Resonance Spectroscopy Study of Normal Neonatal Brain Development

Objective Single voxel proton spectroscopy technique (1H-MRS)was performed in the healthy newborn brain tissue to investigate the metabolic ratio in the right basal ganglia and the right frontal lobe changes with gestational age, and to improve the understanding of the neonatal brain development. Methods In this study, 69 newborns were selected, including twenty-five premature and forty-four term neonates. By using the Gesell developmental scale to evaluate brain development , they all had normal neurologic outcomes at 12-months corrected age. A point-resolved spectroscopy sequence was used to study the metabolic data obtained from each region of interest. Results N-acetylaspartate: creatine and N-acetylaspartate:choline were positively correlated with gestational age (the right basal ganglia r = 0.27, 0.63, the right frontal lobe r = 0.44, 0.64 respectively, P < 0.05) and choline:creatine was negative (the right basal ganglia r = -0.51,the right frontal lobe r = -0.41, P < 0.05), with a linear regression relationship among variables. In the same gestational age group , the N-acetylaspartate: choline of the right basal ganglia was higher than that of the right frontal lobe , and the metabolic ratios among the anatomic locations were significantly different (P < 0.05). Conclusion Neonatal brain metabolite levels are associated with gestational age , and MRS can monitor newborn brain maturation quantitatively.

Objective To establish the clinical pharmacokinetics research method for Bawei Jiangu Tablet.Methods Drug blood concentration was estimated through analyzing the ingredients and preclinical pharmaceutical research results of Bawei Jiangu Tablet.Results Corresponding cardiac glycoside value in serum of animals treated with oral use of Bawei Jiangu Tablets and cortex periplocae extracts can be detected by drug monitor system of digoxin immunoreaction, which was similar to that in animals treated with digoxin.Corresponding cardiac glycoside value had also been detetected in serum of subjects in phaseⅠclinical tolerance trial,which was related to the time and dosage of administration.Clini- cal negative control trial showed that the reaction was specific.Conclusion Pharmacokinetics study on target of toxic components with cross-reaction of periplocoside is feasible through drug monitor system of digoxin immunoreaction.
