1.Application of continuous renal replacement therapy to the patients with acute renal failure after liver transplantation

Objective To study the curative effect of continuous renal replacement therapy (CRRT) to the patients with acute renal failure after liver transplantation. Methods From Mar. 2004 to Jan. 2005, the continuous renal replacement therapy was applied to 7 patients with acute renal failure after liver transplantation. The average time of CRRT was 36.1h. The volume of fluid displacer during CRRT was 2-4L/h and the volume of blood flow was 150~250ml/min. Normal heparin was used as anticoagulant. Results For the 7 patients, 4 improved, 2 died. The contents of blood urea nitrogen (BUN) and blood Cr of all the 7 patients have obviously declined(P0.05). The haemodynamic variables were stabilized during CRRT and no obvious side-effect related to CRRT was found. Conclusions CRRT improved the prognosis to the patients with acute renal failure after liver transplantation, showed a favorable tolerance, and may be applied as a emergency measure to the patients with acute renal failure.
2.Effect of continuous renal replacement therapy on the elderly patients with acute renal function failure

ObjectiveTo investigate the effect of continuous renal replacement therapy (CRRT) on the elderly patients with acute renal function failure.Methods11 elderly patients with acute renal function failure were treated with continuous renal replacement therapy. The average time of CRRT was 24.8 h. The ultrafiltration rate during CRRT was 2~4 L/h and blood flow rate was 150~250 ml/min. Normal heparin was used as anticoagulant.ResultsAmong 11 patients, 7 were mend, 3 died. The blood urea nitrogen (BUN) and creatinine (Cr) levels of all patients obviously declined (P<0.01). The haemodynamic variables of all patients were stabilized during CRRT and no obvious side-effect related to CRRT was found.ConclusionThe treatment of CRRT can improve the sufferer prognosis to the elderly patients with acute renal function failure, not restricted by age, and is the effective means to the elderly patients with acute renal function failure.
3.Photoelastic analysis of the biomechanical behavior of the bone interface of Tension More implants.

OBJECTIVETo photoelastically investigate the difference in load distribution of Tension More (TM) implants with different conical angle designs.
METHODSThe following five groups of implants of different conical angles were designed: cylinder implant, upper 1/3 TM implant (taper length of 3 mm); 1/2 TM implant (taper length of 5 mm); lower 1/3 TM implant (taper length of 7 mm); and bottom TM implant (taper length of 10 mm). The implants were centrally located in individually photoelastic models consisting of a simulated trabecular bone and a 1 mm-thick layer of cortical bone. Vertical and 45° oblique static loads were applied at the center of the superstructures. The resulting stresses were monitored photoelastically and recorded photographically. RESULTS With vertical loading, the cylinder implant showed higher stress levels in the cortical bone and trabecular bone than the upper 1/3 TM implant, 1/2 TM implant, and lower 1/3 TM implant. The four groups of TM implants showed lower stress levels in the cortical bone than the cylinder implant under oblique loads. The least favorable stress concentration in cortical bone was observed in the upper 1/3 TM implant under vertical and oblique loads.
CONCLUSIONTM implants of rational conical angle designs seem to be effective in stress distribution. For all designs and load directions, the upper 1/3 TM implant is the most favorable around the crest.

Objective To evaluate the CT findings of invasive pulmonary aspergillosis(IPA) in patients with leukemia.Methods Plain CT scanning of chest was performed in 9 cases of leukemia in company with IPA proved by sputum culture.5 of them underwent over twice CT re-scanning,CT findings of IPA were analysed.Results The lesions were seen more common in the bilateral upper lobe and dorsal segment of lower lobe,mainly located in the peripheral and sub-pleural lung field.Of the 9 cases,the lesions appeared as multi-nodular shadows in 5,single nodule in 2 and sub-pleural consolidation with ground-glass opacity around them in 2,of these 2 cases,one accompanied by multi-nodules.Halo-sign around the nodules in 6 cases and air cresent sign or cavity in 6 were seen.There were scattered ground-glass opacities and patchy attenuations in the bilateral lung in 4 cases.The adjacent pleura of lesion was slightly thinkened in 5 cases.Conclusion The incidence of IPA in leukemia patients is rather high,and CT features of IPA are of certainly characteristics.
6.Antibiotic Pretreatment and Bacterial Colonization in Respiratory Tract in Liver Transplanted Patients

OBJECTIVE To investigate the epidemiological conditions of bacterial colonization in respiratory tract,and antibiotic resistance under the antibiotic pretreatment in liver transplanted patients. METHODS From Oct 2002 to Jan 2005,121 liver transplanted patients were admitted to ICU.Among them,45 patients who were intubated more than 48 hours were included in this study.All of the patients received antibiotic pretreatment after operation.Sputum specimens were collected after 24 hours and 48 hours in ICU via the intubated tube with bronchoscopy. RESULTS Thirty nine pathogens were isolated from 36 patients during 48 hours.Pseudomonas aeruginosa was the predominant pathogen.Only 8 bacilli were susceptible to the antibiotics. CONCLUSIONS Although antibiotic pretreatment performed,bacterial colonization in respiratory tract emerges early in liver transplanted intubated patients.Antibiotic pretreatment may make the susceptible bacteria escape.
7.Risk factors influencing the prognosis of critical ill patients with ARDS

ObjectiveTo observe the risk factors influencing the prognosis of critical ill patients with acute respiratory distress syndrome (ARDS).MethodsThe clinical data of 67 critical ill patients with ARDS from 2003 to 2004 were reviewed retrospectively.ResultsThe acute physiology and chronic health evaluation (APACHEⅡ) scores and age were all differed significantly between survivors and non-survivors groups (P<0.05). The earlier of respiratory supported, the lowest of mortality (P<0.05).Conclusion APACHEⅡ scores, age and the time of starting respiratory support are main risk factors for mortality of critical ill patients with ARDS.
8.Cardiac repair effect of AKT transfected amniotic fluid mesenchymal stem cells on ischemic reperfusion ;injured myocardium in rabbits

Objective:To explore cardiac repair effect of AKT gene transfected amniotic fluid mesenchymal stem cells (AFMSC) transplantation on ischemic reperfusion injured myocardium in rabbit model . Methods :AKT overex‐pressed lentiviral vector was established to transfect AFMSC ;New Zealand rabbits were randomly divided into low glucose DMEM (L‐DMEM) group (group A) ,AFMSC group (group B) and AKT transfected AFMSC group (AKT‐AFMSC group ,group C) .Then rabbit myocardial ischemia reperfusion injury model was established .Before reper‐fusion ,0.2ml L‐DMEM ,AFMSC and AKT‐AFMSC was directly injected into infarct area and surrounding myocar‐dial epicardium in corresponding group . On 21 days after operation , left ventricular end‐diastolic dimension (LVEDd) ,left ventricular shortening fraction (LVFS) ,left ventricular ejection fraction (LVEF) ,infarct size and pathological changes were compared among three groups .Results:Compared with L‐DMEM group and AF‐MSC group ,there was significant reductions in LVEDd [ (16.37 ± 0.84) mm ,(15.06 ± 1.01) mm vs .(13.51 ± 0.85) mm] and infarct size [ (37.3 ± 2.1)% ,(26.6 ± 0.7)% vs .(18.1 ± 1.2)% ] ,and significant rise in LVFS [ (29.18 ± 2.36)% ,(33.65 ± 2.81)% vs .(36.89 ± 3.02)% ] and LVEF [ (58.62 ± 3.47)% ,(67.42 ± 3.03)% vs .(72.02 ± 2.89)% ] , P< 0.05 or < 0.01. Pathological injury significantly relieved in AKT‐AFMSC group than those of group A and group B .Conclusion:Compared with amniotic fluid mesenchymal stem cells ,AKT‐transfected amniotic fluid mesenchymal stem cells possesses better effects of repairing injured myocardium and improving myocardial function in ischemic reperfusion injured myocardium .
9.Comparison of radiation field irradiated by different focusing modes of γ-knife

Objective To compare the single-shot fields irradiated by three focusing modes of γ-knife and explore the approaches for improving the quality of stereotactic radiosurgery.Methods GAFCHROMIC(R) EBT3 mode flushing-free film was used to measure the single-shot fields irradiated by multi-source static focusing modes,multi-source single-axis rotating focusing mode and single-source double-axis rotating focusing mode of γ-knife.Also the uniformity and penumbra of the single-shot fields were compared.Results The 2D dose distribution of the single-shot fields irradiated by three focusing modes of γ-knife was different.In the axis (x,y,z),the rang of penumbra axial length ratios of multisource static focusing modes,multi-source single-axis rotating focusing mode and single-source double-axis rotating focusing mode were 0.13-0.48,0.17-0.33 and 0.28-0.54,in the diagonal direction of the wings plane (NSD,PSD),were 0.31-0.39,0.38-0.43 and 0.54-0.72,respectively;the penumbra axial length ratio of single-source double-axis rotating focusing mode was bigger than in multi-source static focusing modes and multi-source single-axis rotating focusing mode.On the no-wings plane,the area ratios of 80% dose curve enveloped and 50% dose curve enveloped(A80%/A50%)were 0.40,0.47 and 0.19,on the wings plane,were 0.61,0.53 and 0.35,respectively.The field uniformity of multi-source static focusing modes and multi-source single-axis rotating focusing mode were superior to single-source doubleaxis rotating focusing mode.Conclusions Considering dose distribution of the single-shot fields,the multi-source static focusing modes devices and the multi-source single-axis rotating focusing mode devices should be preferred,when important tissues and organs are adjacent to the target areas.Compared with single-source double-axis rotating focusing mode,both multi-source static focusing modes and multi-source single-axis rotating focusing mode could make more target areas to be surrounded by high dose region.
10.Exploratory experimental study on rabbit model of atrial fibrillation by wireless telemetering and stimulation technology

Objective To explore the establishment of a rabbit model of atrial fibrillation by wireless telemetering and stimulation technology.Method An implantable telemetering stimulator which was independently designed and devel-oped was hypodermically implanted in New Zealand rabbits.The implantable telemetering stimulator was made with the core development and design of MSP single-chip microcomputer of TI Corporation ( Texas Instruments) and RF wireless trans-ceiver chip CC2250 of TI Corporation.The design of the implantation system was optimized to cater to the exploratory ex-periment to establish atrial fibrillation model of New Zealand rabbits.The implanter was implanted into the abdominal sub-cutaneous tissue of the New Zealand rabbits, the collecting electrodes were placed in the oxter subcutaneous tissues of the left and the right upper extremities, and the two stimulating electrodes were sutured at the left auricle and the left atrium. The signals were collected and stimulated by the wireless transceiver.The I-lead ECG electrical signals were continuously monitored on the body surface by a Powerlab physiological recorder.High frequency ( >20 Hz) suprathreshold stimulus ( intensity 2 mA, pulse width 1 ms) was emitted by specialized stimulation software of a computer program by the interval ( stimulating for 2 s and pausing for 2 s) .In case of atrial fibrillation during intervals, the stimulation could be stopped by hand.In case of sinus rhythm, the stimulation could be continued.Results The implantable telemetering stimulator can work stably in vivo ( including collecting stimulated electrocardio signal and emitting stimulations) for 30 days.Atrial fibril-lation can be induced after stimulating in vivo of the New Zealand rabbits for 3 weeks, with a duration of >48 h.Conclu-sions Applying implantable telemetering stimulator can build a New Zealand istead of beagles model of atrial fibrillation which is more consistent with welfare optimization and substitution principle for laboratory animals.
