1.Electron microscopic observations on changes of cytoplasmic organelles in the oocytes of Cervus nippon hortulorun

Objective Revealing the developmental regulation of Cervus nippon's oocyte and organelles. Methods In the experiment,follicle systems during both estrum and non- estrum were divided into the primordial follicle,growing follicle and mature follicle according to the Cervus nippon's follicle diameter size,formation of zonapellucida,appearance time of follicular cavity.At the same time,observations on cytoplasmic organelles in development of oocytes were conducted with electron microscopic,eyepiece micrometer and photomicrographic technique(The number of every oocyte observed is 6-8). Results In the primordial follicle and early growing follicle phase,the quantity of mitochondria,golgi apparatus,smooth endoplasmic reticulum and cortical granules increased gradually and all organelles moved to the cortical area.However,in the late growing follicle and mature follicle phase,Golgi apparatus and rough endoplasmic reticulum disappeared,cortical granules began to arrange themselves in line beneath the plasma membrane of the oocyte,mitochondrias dispersed toward the central region of cytoplasm,and almost all the round mitochondria with rare cristae turned into hooded ones, and nucleus compaction occurred.In addition,the short and thick microvilli began to appear from the primary follicle ovocyte,become intensive and slender when secondary follicle's ovocyte;It's until tertiary follicle's ovocyte,microvilli started to shorten and become coarse,and even parts of them contract from the zona pellucida gradually.Conclusion In the development of oocytes, the changes of type,quantity and distribution on mitochondria has a close relation with cells at proliferation,differentiation and metabolism level.Cortical granule has no association with golgi apparatus basically,but smooth endoplasmic reticulum(SER).The nuclei are the sites of RNA synthesis and warehouses,and its densification is the premise of meiosis recovery.
2.Continuous surveillance of antimicrobial resistance among gram-positive coccus from 1999 to 2006 at Beijing Chaoyang Hospital

Objective To investigate the change of antimicrobial resistance among gram-positive cocci from 1999 to 2006 in Beijing Chaoyang Hospital. Methods Antimicrobial susceptibility test was car-ried out by MIC method, data were analyzed by WHONET 5.3 software. Results A total of 6192 clinical isolates were collected. The top four pathogens were Staphylococcus coagulase-negative, Staphylococcus au-reus, Enterococcus faecalis and Enterococcus faecium. The prevalence of methicillin-resistant S. aureas and coagulase-negative were identified in 88.4% and 86.9%, respectively. The resistance rates of S. aureus to penicillin G and ampicillin were more than 90.0% during 1999-2006. Neither strains of S. aureus nor strains of coagulase-negative Staphylococcus were found resistant to vancomycin. Vancomyein-resistant Enterococci (VRE) strains were firstly isolated in 2003, thirteen vancomycin-resistant E.faecalis isolates were identified as VanB genotype by PCR and sequencing. The VanA gene was confirmed by PCR and sequencing in seven-teen vancomycin-resistant E.faecium, vancomycin-resistance E.faecium rate increased sharply. The most ac-tive antibiotics against E. faecalis were vancomycin, ampicillin, penicillin with the susceptible rates of 98.7%, 95.7% and 85.6%, however, the susceptibility rate of E. faecalis to penicillin decreased from 94.3%-84.6%. The resistance rates of E.faecalis to clindamycin was 99.0%. The resistance rates of E. faecium to erythromycin and clindamycin were more than 95.0%. The resistance rates of E.faecium to ampi-cillin, penicillin, ciprofloxacin were more than 90.0%. The most active antibiotics against E. faecium was vancomycin. The susceptibility rate of E.faecium to tetracycline increased from 27.8% to 82.6%. Conclu-sion Vancomycin remains highly active against gram-positive cocci. A total of 30 Enterococci isolates were found resistant to vancomycin.
3.Multi-section ultrasonic diagnosis and classification of congenital clubfoot

Objective:To evaluate and analyze the ultrasonic findings of idiopathic clubfoot and positional clubfoot deformities.Methods:Forty-nine newborn babies with congenital clubfoot were examined in the Department of Ultrasound of the Third Affiliated Hospital of Zhengzhou University from December 2020 to January 2022, Including 21 newborn babies(32 feet) with idiopathic clubfoot, and 28 babies(53 feet) with positional clubfoot. Twenty-two normal infants in the same period and the normal feet of the single clubfoot were selected as control group. The distance between medial malleolus and scaphoids of all feet were measured by ultrasound. The distance from the tangent line of the lateral edge of calcaneus to the midpoint of the lateral edge of the chondroid bone, medial soft tissue thickness and tibial calcaneal angle were measured by ultrasound. The data of idiopathic clubfoot group, positional clubfoot group and control group were statistically analyzed.Results:A total of 71 newborn babies with 142 feet were evaluated.The idiopathic clubfoot group had born and joint changes in the medial, lateral and posterior side, and the differences were statistically significant compared with the control group (all P<0.05). Compared with the control group, there were statistically significant differences in the medial and lateral side of the positional group(all P<0.05). But no significant changes in the posterior side( P>0.05). There were significant differences between medial and posterior side of idiopathic and positional clubfoot group (all P<0.05), but no significant differences in lateral side ( P>0.05). Conclusions:Ultrasonography can clearly display the tarsus bones in clubfoot, and observe the deformity changes of the idiopathic clubfoot and positional clubfoot.
4.Establishment and validation of prediction model of concurrent infection in elderly patients with malignant lymphoma undergoing chemotherapy

Objective:To explore the influencing factors of concurrent infection in elderly patients with malignant lymphoma undergoing chemotherapy, construct a risk prediction model and verify its effectiveness.Methods:A total of 92 elderly patients with malignant lymphoma admitted to Department of Hematology in the First Affiliated Hospital of Wenzhou Medical University from January 2019 to August 2021 were selected as the research objects by the convenient sampling method. The age, gender, nutritional risk, activities of daily living (ADL), comorbidities, occurrence of infection, treatment time and prognosis of the patients were collected. The independent risk factors for concurrent infection in elderly patients with malignant lymphoma undergoing chemotherapy were analyzed, and the risk prediction model and line graph were constructed for internal validation by Bootstrap.Results:Logistic regression analysis showed that ADL score, nutritional screening score and Charlson's Weighted Index of Comorbidities (CCI) score were independent risk factors for concurrent infection in elderly patients with malignant lymphoma undergoing chemotherapy ( P<0.05). The prediction model of concurrent infection in elderly patients with malignant lymphoma undergoing chemotherapy was established, namely Z=0.983×CCI score +1.075× nutritional screening score +0.645×ADL score -5.115. The C-index of the prediction model nomogram was 0.785 [95% CI (0.692-0.879) ], indicating good differentiation. According to the correction curve verified internally by Bootstrap, the absolute error of the possibility prediction of the nomogram model was 0.028, which was in good consistency. Conclusions:The risk prediction model of concurrent infection in elderly patients with malignant lymphoma chemotherapy is constructed based on ADL score, nutritional screening score and CCI score, which are easy to obtain and evaluate, and the nomogram of the prediction model is established with good differentiation and consistency.
5.TBC1D15 deficiency protects against doxorubicin cardiotoxicity via inhibiting DNA-PKcs cytosolic retention and DNA damage.

Clinical application of doxorubicin (DOX) is heavily hindered by DOX cardiotoxicity. Several theories were postulated for DOX cardiotoxicity including DNA damage and DNA damage response (DDR), although the mechanism(s) involved remains to be elucidated. This study evaluated the potential role of TBC domain family member 15 (TBC1D15) in DOX cardiotoxicity. Tamoxifen-induced cardiac-specific Tbc1d15 knockout (Tbc1d15CKO) or Tbc1d15 knockin (Tbc1d15CKI) male mice were challenged with a single dose of DOX prior to cardiac assessment 1 week or 4 weeks following DOX challenge. Adenoviruses encoding TBC1D15 or containing shRNA targeting Tbc1d15 were used for Tbc1d15 overexpression or knockdown in isolated primary mouse cardiomyocytes. Our results revealed that DOX evoked upregulation of TBC1D15 with compromised myocardial function and overt mortality, the effects of which were ameliorated and accentuated by Tbc1d15 deletion and Tbc1d15 overexpression, respectively. DOX overtly evoked apoptotic cell death, the effect of which was alleviated and exacerbated by Tbc1d15 knockout and overexpression, respectively. Meanwhile, DOX provoked mitochondrial membrane potential collapse, oxidative stress and DNA damage, the effects of which were mitigated and exacerbated by Tbc1d15 knockdown and overexpression, respectively. Further scrutiny revealed that TBC1D15 fostered cytosolic accumulation of the cardinal DDR element DNA-dependent protein kinase catalytic subunit (DNA-PKcs). Liquid chromatography-tandem mass spectrometry and co-immunoprecipitation denoted an interaction between TBC1D15 and DNA-PKcs at the segment 594-624 of TBC1D15. Moreover, overexpression of TBC1D15 mutant (∆594-624, deletion of segment 594-624) failed to elicit accentuation of DOX-induced cytosolic retention of DNA-PKcs, DNA damage and cardiomyocyte apoptosis by TBC1D15 wild type. However, Tbc1d15 deletion ameliorated DOX-induced cardiomyocyte contractile anomalies, apoptosis, mitochondrial anomalies, DNA damage and cytosolic DNA-PKcs accumulation, which were canceled off by DNA-PKcs inhibition or ATM activation. Taken together, our findings denoted a pivotal role for TBC1D15 in DOX-induced DNA damage, mitochondrial injury, and apoptosis possibly through binding with DNA-PKcs and thus gate-keeping its cytosolic retention, a route to accentuation of cardiac contractile dysfunction in DOX-induced cardiotoxicity.
6.Clinical factors associated with composition of lung microbiota and important taxa predicting clinical prognosis in patients with severe community-acquired pneumonia.

Few studies have described the key features and prognostic roles of lung microbiota in patients with severe community-acquired pneumonia (SCAP). We prospectively enrolled consecutive SCAP patients admitted to ICU. Bronchoscopy was performed at bedside within 48 h of ICU admission, and 16S rRNA gene sequencing was applied to the collected bronchoalveolar lavage fluid. The primary outcome was clinical improvements defined as a decrease of 2 categories and above on a 7-category ordinal scale within 14 days following bronchoscopy. Sixty-seven patients were included. Multivariable permutational multivariate analysis of variance found that positive bacteria lab test results had the strongest independent association with lung microbiota (R2 = 0.033; P = 0.018), followed by acute kidney injury (AKI; R2 = 0.032; P = 0.011) and plasma MIP-1β level (R2 = 0.027; P = 0.044). Random forest identified that the families Prevotellaceae, Moraxellaceae, and Staphylococcaceae were the biomarkers related to the positive bacteria lab test results. Multivariable Cox regression showed that the increase in α-diversity and the abundance of the families Prevotellaceae and Actinomycetaceae were associated with clinical improvements. The positive bacteria lab test results, AKI, and plasma MIP-1β level were associated with patients' lung microbiota composition on ICU admission. The families Prevotellaceae and Actinomycetaceae on admission predicted clinical improvements.
