1.Influence of acute hyperglycemia on the assessment of cardiac function with N-terminal pro-brain natriuretic peptide

Objective To investigate the relationship between N-terminal pro-brain natriuretic peptide (NT-proBNP) and tissue Doppler imaging (TDI) derived cardiac function index (Tei index) in patients with acute coronary syndrome under different plasma glucose levels and to evaluate the influence of hyperglycemia on the preciseness of cardiac function assessment with NT-proBNP.Methods Consecutive patients with acute coronary syndrome admitted to the department of cardiology in Guangdong General Hospital were prospectively enrolled.Based on their plasma fasting glucose level,patients were divided into hyperglycemia group (fasting plasma glucose ≥ 6.1 mmol/L) and euglycemia group (fasting plasma glucose < 6.1 mmol/L).All the patients underwent transthoracic echocardiagraphy and tissue Doppler imaging (TDI) investigations.Blood samples were obtained within 24 hours of hospitalization for measurment of NT-proBNP level.Relationship between TDI-Tei index and the level of NT-proBNP in the two groups were analyzed respectively.Results The TDI-Tei index,the systolic index and the diastolic index were all significant higher in the hyperglycemia group (n =27) than those in the euglycemia group (n =35)(0.68±0.14) vs.(0.61 ±0.10),P =0.03; (0.29±0.07) vs.(0.26±0.05),P =0.045; (0.38±0.08) vs.(0.35 ±0.050,P =0.03,respectively.In both groups,TDI-Tei and In NT-proBNP showed significant linear regression.In the hyperglycemia group,TDI-Tei =0.175 + 0.068 In NT-proBNP,R2 =0.702,P < 0.01.In the euglycemia group,TDI-Tei =0.185 + 0.060 In NT-proBNP,R2 =0.405,P < 0.01.Conclusions (1) Compared with patients suffering from an acute coronary syndrome with euglycemia,the global cardiac function of patients with hyperglycemia is poorer; (2) NT-proBNP correlates significantly with TDI-Tei in both hyperglycemia and euglycemia patients with acute heart syndrome.It is appropriate to assess global cardiac function with NT-proBNP in patients suffering from ACS complicated with hyperglycemia.
2.Research progress on exercise rehabilitation in female patients with stress urinary incontinence

Pelvic floor muscle training (PFMT) is the first-line intervention for non-surgical treatment of female patients with stress urinary incontinence (SUI). However, due to the particularity of the physiological position of the pelvic floor muscles, 13% to 53% of patients are unable to perform PFMT correctly after the initial visit. In view of this, scholars at China and abroad put forward some other sports rehabilitation methods and achieve good clinical results. In this paper, exercise rehabilitation methods for female SUI patients were reviewed, including PFMT, core muscle training, Pilates, yoga, physical activities, etc., in order to provide reference for medical personnel to formulate scientific and feasible exercise rehabilitation programs for female SUI patients.
3.Expression and clinical significance of moesin and E-cadherin in invasive carcinoma of breast, no specific type

Objective To investigate the correlation of moesin and E-cadherin with biological behavior of breast cancer and its mechanism by comparing expression of moesin and E-cadherin in breast invasive carcinoma of no specific type (BIC-NST),breast ductal carcinoma in situ (BDCIS) and normal breast tissues adjacent to carcinoma.Methods Breast cancer cases of the Huizhou Municipal Center People Hospital were collected between Jan 2008 and Dec 2010,expression of moesin and E-cadherin in 104 cases of BIC-NST,84 cases of BDCIS and 53 cases of normal breast tissues adjacent to carcinoma were detected by tissue-microarray and SP immunohistochemical staining.Western blot was used to detect moesin expression of 16 BIC-NST fresh tissues.Results Expression rate of moesin in BIC-NST and BDCIS were significantly higher than normal tissues(P < 0.01),but the expression rate of E-cadherin in BIC-NST and BDCIS were significantly lower than those of normal tissues(P < 0.01).Expression rate of moesin in BIC-NST grade Ⅲ group was significantly higher than that of the grade Ⅰ group.There was a significantly positive correlation between histological grade and moesin expression(P < 0.05).However,E-cadherin expression rate in BICNST grade Ⅲ group was significantly lower than that in grade Ⅰ group,and there was a significantly negative correlation between histological grade and E-cadherin expression (P < 0.05).Moreover,no significant correlation was observed between moesin and E-cadherin expression in BDCIS tissues.Expression of moesin in clinical stage Ⅱ + Ⅲ BIC-NST was significantly higher than that in stage Ⅰ (P < 0.01).Expression of moesin was significantly associated with lymph node metastasis (P < 0.01).But no significant correlation was observed between moesin expression and age,tumor size and vascular invasion.However,expression of E-cadherin in clinical stage Ⅱ + Ⅲ BIC-NST was significantly lower than that in stage Ⅰ (P < 0.01).Expression of E-cadherin was significantly associated with lymph node metastasis and vascular invasion (P < 0.01).But no significant correlation was observed between E-cadherin expression,age and tumor size.There was a negative correlation between expression of moesin and E-cadherin in BIC-NST(P =0.021)and BDCIS(P =O.032).Conclusion Higher moesin and lower E-cadherin signal transduction is closely related to the recurrence and development of breast carcinoma,therefore moesin and E-cadherin might provide new targets for gene therapy in breast carcinoma.
