1.CT and MRI findings of combined hepatocellular carcinoma and cholangiocarcinoma

Objective To investigate the imaging findings of combined hepatocellular carcinoma and cholangiocarcinoma(cHCC-CC) with dynamic MRI and spiral CT.Methods Twenty-three patients of cHCC-CC were evaluated.All patients had surgery and diagnoses were confirmed by histopathology.Eighteen patients had pre- and post-contrast scans with spiral CT.Five patients had MR with dynamic multi-phase contrast scanning.Imaging findings in all patients were retrospectively reviewed.Results CT of eighteen patients showed low-density masses with ill-defined margin (n =15 ),lymphadenopathy( n =5 ),invasion of blood vessels ( n =7 ),satellite lesions ( n =5 ),enhancing pseudocapsule ( n =9 ),and dilatation of intrahepatic ducts (n =1 ).MRI of five patients showed lesions with low signal on T1 WI and high signal on T2WI,lymphadenopathy(n =1 ),invasion of blood vessels (n =1 ),enhancing pseudocapsule (n =4).Dynamic CT or MRI enhanced scanning showed areas of heterogeneous enhancement in the interior or periphery of the tumor on the arterial phase,areas of inhomogeneous high signal ( corresponding regions of low signal on the arterial phase) on the portal venous phase,areas of mixed signal intensity due to retention of contrast agents on the delayed phase.Conclusions Combined hepatocellular carcinoma and cholangiocarcinom demonstrate imaging features on conventional and dynamic MRI and spiral CT,which can be helpful to improve the diagnostic accuracy of this disease.
2.Effect evaluation of staging and individualized health education on alleviation of anxiety and depression symptoms of fracture patients

Objective To discuss the staging and individualized health education on alleviation of anxiety and depression of fracture patients.Methods 60 fracture patients who showed anxiety and depression symptoms were randomly divided into the experimental group and the control group with 30 patients in each group.The experimental group used the special-designed staging and individualized health education plan to intervene the anxiety and depression symptoms of patients.The control group used the routine health education program.The Hamilton Anxiety Scale (HAMA)and Hamilton Depression Scale (HAMD)were used to assess the patients' anxiety and depression degree before and after intervention.Results HAMA and HAMD score at discharge were lower than on admission in both groups.HAMA and HAMD score at discharge in the experimental group were significantly lower than the control group.Conclusions The systematic,staging and individualized health education program has significant effect on the anxiety and depression symptom of fracture patients.It can effectively alleviate the symptoms of anxiety and depression,satisfy the requirement of patients for health education,stimulate the patients' initiative to coordinate with the treatment,establish the confidence to fight with disease and help patients form a good behavior manner,and promote patients' recovery.

4.Analysis of Infection-related Factors for Coronary Artery Bypass Grafting

Objective:To analyze infection-related factors for Coronary artery bypass grafting(CABG).Method: The cases undergoing operations with CABG from January to June in 2006 were 614.The records of the two sets of statistical indicators were kept and the data were analyzed to find the infection-related factors.Result:There were three factors related to infection:the measure of anaesthesia,the time of hospitalization before surgical operations and the duration of prophylac- tic antibiotics treatment.Diabetes mellitus was not a risk factor in our study.Conclusion:In view of patients'health condi- tion,prophylactic antibiotics can be used to prevent infection in surgical operations.Shortening the time of hospitalization before surgical operations and adopting off-pump coronary artery bypass grafting can decrease the infection.CABG is as safe for diabetic patients as for non-diabetic patients.
5.Impact of drug intervention on cardiomyopathy in STZ-induced type 1 diabetic rats

Objective To investigate the pathological changes of myocardium in diabetic rats and the effect of aspirin, perindopril, prednisone on the pathological changes of myocardium in type 1 diabetic rats. Methods Thirty SD rats were randomly divided into the following groups: ( 1 ) diabetes mellitus (DM)group ( n = 20): treated with intraperitoneal injection of streptozocin (STZ) 60 mg/kg at one time, diluted in sodium citrate buffer. (2) normal control group (n = 10): intraperitoneal injection of sodium citrate buffer. The blood glucose and the weight were measured regularly for 8 weeks. The 20 diabetic model rats were randomly divided into four groups (n = 5 individually), from the 4th day, the rats were given normal saline dissolved aspirin, perindopril, prednisone gavage respectively, and the normal control group and diabetic group were given an equal volume of saline daily gavage. After 8 weeks, the pathological changes in the cardiac muscle tissue were observed under light microscope and electron microscope. Results Three days after the injection of STZ, the blood glucose in the DM group was higher than 16. 7 mmol/L,20 rats were all modeling successfully. The model rats suffered from polydipsia, polyphagia, polyuria and weight loss significantly. HE staining showed diabetic myocardial cell swell, hypertrophy, degeneration and significant nuclear enlargement; the myocardial fiber fractured,dissolved, contraction wavy and disorganized fibre infiltrated by inflammatory cells. Under electron microscope, diabetic myocardial muscle fibers arranged irregularly, fractured and dissolved; the glycogen between the muscle fiber accumulated, mitochondria swelled, most of the ridges in mitochondria were not clear, cell plate widened. Compared with the diabetic group, the above-mentioned pathological changes reduced in the drug intervention group,and the changes in perindopril group was most significant. Conclusion Injection of STZ 60 mg/kg could successfully induce diabetic in rats and held stable blood glucose level. After 8 weeks, diabetes could cause servere damages in myocardium in the rats, but aspirin, perindopril, prednisone could reduce the cardiomyopathy in diabetic rats,in which perindopril had more positive effect.
6.Analysis of Factors Affecting Adverse Reactions of Statins

Objective:To discuss the regularity of adverse drug reactions induced by statin.Method:82 cases from published articles were retrieved from 1998 to 2008.According to the given standard,the data were statistically analyzed. Result:77 articles carried 82 case reports concerning statins inducing ADRs in the period of 10 years.52 cases were caused by simvastatin.ADRs of statins could involve multiple organs and systems,mainly myopathys.Severe reaction could result in renal failure and even death.Conclusion:A rational use of statins should be strengthened in the clinical practice and strict monitoring in order to prevent and reduce the occurrence of serious adverse drug reactions.
7.Experimental Study on Hypoglycemic Effect of Yumixu(Zea mays)

The water extract frem the style of Zea mays L.showed a therapeutic effect on diabetes caused by alloxan, and obviously reduced blood glucose level of heperglycemia induced by glucose or epinephrine in mice, but no affect in normal mice, when it was administrated orally. In addition, it could decrease cholesterin content.
8.Application of parenteral nutrition in critically ill patients undergoing cardiac surgery: a retrospective study

Objective To summarize the clinical application of parenteral nutrition (PN) in Fuwai Hospital. Methods The clinical applications of PN in 132 critically ill patients undergoing cardiac surgery in our hospital were retrospectively investigated and analyzed. Results The application of PN in Fuwai hospital was basically appropriate and standardized. Energy supply in PN formulation was reasonably low, but still met the energy needs of patients in the fasting period. The nutritional composition and energy/nitrogen ratio in the formulation were basically rational. However, problems including the lack of double energy in partial formulation, low proportion of glucose-supplied energy due to low glucose-lipid ratio, and insufficient supply of insulin still existed. One (0.76%)patient was discharged according to his own decision; 22 ( 16.67% ) patients died, which were not directly associated with PN; and the other 109 (82.58%) patients were cured and discharged. Conclusion Application of PN for critically ill patients undergoing cardiac surgery must strictly follow the operation indications and rationally select nutritional components and proportions to ensure its safety and effectiveness.
9.The influence of fusing conditions on the surface structure and morphology of SP1 SiO_2 in dental resin composites

Objective: To evaluate the influence of fusing conditions on the surface structure and morphology of the SP1 SiO2. Methods:The SP1 SiO2 samples were sintered under 500, 650, 800, 950 and 1 100 ℃ at a rate of 250 ℃/h and maintained for 10, 30 min, 3 h respectively. The surface structure was characterized by FT-IR. Results:The characteristic peak of Si-OH bond at 958 cm-1 took off gradually with the rising temperature and prolonged time. Under the conditions of 950 ℃ maintained for 10 min and 30 min, and 650 ℃ maintained for 3 h, it disappeared completely. The particles of the SP1 SiO2 became larger while the size distribution became narrow. Conclusion: Fusing conditions can affect the Si-OH content and the size of the SP1 SiO2, as well as the size of distribution.[

Objective To Describe the queuing mathematical model and analyze the reasonable hospital application pattern of electronics voluntary system.Methods By scientifically arranging medical care personnel and equipment,patients queuing process was optimized.Results We successfully applied the outpatient real-time computer information management system of electronics voluntary based on this mathematical model.Conclusion It is a possible way to optimize the queuing process and improve the efficiency in electronic triage system based on queuing theory.
