1.Application of image sensor in wireless endoscopic micro-electro-mechanical-system

Objective To discuss the application of image sensor in wireless endoscopic micro-electro-mechanical-system (MEMS). Methods Through the comparison of various types of image sensors, and in the consideration of the environment and requirement inside the body, CMOS image sensor which can output analog image signal with high density of integration, low power consumption, small profile was chosen as the sensor component of a wireless endoscope. The profile and power consumption of some specific models of this type of image sensor were compared and analyzed.Results The problems, such as ground connection, crystal configuration and the layout of circuit board, were reasonably solved during the design of image sensor's schematic circuit diagram. The image capture module based on OV7930 sensor was designed and manufactured. Images were successfully captured during the animal experiments.Conclusion The application and design of image sensor in a wireless endoscopic MEMS are feasible.
2.Immune tolerance induced by cytokines in allogeneic hematopoietic stem cell transplantation

Allo-HSCT is a potentially curative therapy for many hematological malignancies,the basis of which is graft-versus-leukemia(GVL) effect.However,acute graft-versus-host disease(GVHD) is responsible for 15% to 40% of mortality and is the major cause of morbidity after transplantation.Therefore,separation of GVHD and GVL partially or completely could improve the transplant outcomes.This study focuses on the effects of G-CSF on T cells in peripheral blood stem cell grafts and bone marrow grafts and its mechanisms after in vivo G-CSF application.The separation of GVHD and GVL effect and the mechanisms of which after in vivo G-CSF and interleukin-11(IL-11) treatment of healthy donors were investigated.The main contributions of this research are listed as follows:(1) Immune tolerance of T cells was induced simultaneously in peripheral blood stem cell grafts and bone marrow grafts after in vivo G-CSF application;(2) T cell hyporesponsiveness and polarization of T cells from Th1 to Th2 were maintained after mixture of G-CSF-mobilized peripheral blood grafts(G-PB) and G-CSF-primed bone marrow grafts(G-BM) according to different proportions in vitro;(3) Treating donor with G-CSF and IL-11 decreased GVHD and retained GVL effect;(4) The incidence of acute GVHD was decreased after Allo-HSCT using G-PB and G-BM as allografts;(5) In combination with other techniques,the HLA barriers were overcomed using G-PB and G-BM as allografts;(6) The incidences of acute GVHD were significantly decreased and the GVL effects were retained or enhanced in relapsed patients after treatment by G-CSF-mobilized peripheral blood graft infusion compared with those received steady-state peripheral blood lymphocyte infusion,indicating that GVHD and GVL could be partially separated in clinical settings.Based on our results,we would conclude that the issues on the deficiency of donors are resolved,and novel strategies offered for the prophylaxis and treatment of patients with hematological malignancies who relapse after Allo-HSCT.Further studies on the mechanism of the separation of GVL and GVHD are warranted.
3.Effects of rhG-CSF mobilization on immunological characteristics of peripheral blood grafts and steady-state bone marrow

Objective To explore the difference of immunological characteristics between recombination human granulocyte colony-stimulating factor(rhG-CSF)mobilized peripheral blood grafts(G-PB)and steady-state bone marrow grafts(SS-BM).Methods From April to October 2003,G-PB and SS-BM of 15 related donors were collected.T cell subgroups,dendritic cells(DC),monocytes and the expression of CD28 costimulatory molecules on T cells were determined by multicolor flow cytometry.The lymphocyte proliferation ability and the quantities of interleukin-4(IL-4)and interferon-?(IFN-?)secreted by T cells were determined using MTT assays and sandwich ELISA.Results The absolute numbers of monocytes,CD3+,CD4+ and CD8+ T cells,and the ratios of CD4/CD8 in G-PB were significantly higher than those in SS-BM,respectively(P0.05).The quantities of IFN-? and IL-4 secreted by T cells per micromilter in G-PB was significantly higher than those in SS-BM(P0.05).The absolute numbers of DC1 and DC2 in G-PB were significantly higher than those in SS-BM(P0.05).Conclusion It is concluded that the difference of immunological characteristics between G-PB and SS-BM may explain the lower incidence of GVHD and lower relapse rate after SS-BM and G-PB transplantation.
4.Effect of rhG-CSF mobilized peripheral blood and bone marrow after being mixed in different proportions on immunologic properties of grafts

5.Effect of modified periocular injection on relieving pains by local application of drugs

Objective To study the effect of modified periocular injection on the pains induced by local application of drugs. Methods Sixty patients treated with periocular injection were divided randomly into experimental group and control group in equal number. The control group was treated with orthodox periocular injection,while experimental group with modified periocular injection, immersing the pinhead into lidocaine solution before injection.The pain level of patients was evaluated by self-designed pain scale. Result The pain level of modified periocular injection was lower than that of orthodox periocular injection statistically (Z=3.799, P<0.001).Conclusion Modified periocular injection can alleviate pains induced of local application of drugs obviously and increase their compliance,so it deserves clinically popularizing.

Objective To investigate the correlation of monocyte counts prior to the collection of recombinant human granulocyte colony-stimulating factor(rhG-CSF)primed bone marrow grafts(G-BM)with the quantities of CD34+ cells in the grafts.Methods From June 2004 to July 2007,fifty-three healthy donors were treated with rhG-CSF[5 ?g/(kg?d)]injected subcutaneously for five consecutive days.Bone marrow grafts and peripheral blood grafts were harvested on the 4th and 5th day,respectively.The quantities of CD34+ cells and blood routine prior to the collection of the G-BM were determined by flow cytometry and blood analyzer XL2100,respectively.Results The monocyte counts in peripheral blood(PB)of the 53 healthy donors were 896?424 per microliter.The counts of CD34+ cells per microliter and quantities of total CD34+ cells in the bone marrow grafts were 84?45 and(8.22?4.84)?107,respectively.Pearson correlation analysis indicated that the counts of monocyte in the PB correlated positively with the counts of CD34+ cells per microliter(r=0.573,P
7.Minimum alveolar concentration of sevoflurane in neonates

Objective To determine the minimum alveolar concentration (MAC) of sevoflurane in Chinese neonates.Methods Thirty ASA Ⅰ or Ⅱ neonates,aged ≤ 28 days,with normal body weight,scheduled for elective surgery under general anesthesia,were enrolled in the study.Anesthesia was induced with inhalation of 6.00% sevoflurane in oxygen.The infants were tracheal intubated and mechanically ventilated.The inhaled concentration of sevoflurane was adjusted to achieve the preset end-tidal concentration and maintained at this level for 20 min.Skin incision was then performed.The concentration of sevoflurane was determined by modified Dixon's up-and-down method.The initial end-tidal concentration of sevofluren was 3.00%.Each time the concentration increased/decreased by 0.25 % in the next infant according to the infant's response.Successful skin incision was defined as no body movement during skin incision.The MAC,ED95 and 95 % confidence interval of sevoflurane were calculated using logistic regression analysis.Results The MAC and ED95 (95 % confidence interval) of sevoflurane required for successful skin incision were 2.82% (2.66%-2.98%) and 3.39% (2.89%-3.89%),respectively,in neonates.Conclusion The MAC of sevoflurane is 2.82 % in Chinese neonates and lower than the present reference values previously described in foreign reports.
8.Defense mechanisms and childhood abuse of patients with borderline personality disorder

Objective To understnad the defense mechanisms of patients with borderline personality disorder (BPD) and to explore its relationship with childhood abuses.Methods One hundred and forty-two outpatients with BPD (group A),63 outpatients with other personality disorders (group B) and 20 outpatients without any Axis Ⅱ diagnoses (group C) were enrolled and completed Chinese versions of the Childhood Experience of Care and Abuse Questionnaire (CECA.Q) and the Defensive Style of Questionnaire (DSQ).Multivariate analysis of variance and Pearson correlation analysis were used for data analysis.Results Compared to group B and C,the score of defense mechanisms reported by group A were significantly increased in all eight premature defense mechanisms,such as projection(F =26.5,P < 0.05),passive aggression (F =18.1,P < 0.05),acting out (F =28.3,P < 0.05),complaint (F =16.7,P <0.05),fantasy(F =16.3,P < 0.05),splitting (F =28.8,P < 0.05),withdrawal (F =8.3,P < 0.05),somatization(F=7.6,P < 0.05),and part of the middle defense mechanisms,such as omnipotence-devaluation(F =9.2,P < 0.05),isolation(F =4.7,P < 0.05) and identification (F =10.0,P < 0.05).At the same time,the BPD patients got lower scores in the mature defense mechanisms,such as sublimation (F =4.1,P < 0.05) and repression(F =3.9,P < 0.05).Those who experienced physical abuse use more complaint (F =6.9,P < 0.05),denial mechanisms (F =5.2,P < 0.05) and less anticipation one (F =4.6,P < 0.05).Those who experienced sexual abuse use more somatization (F =4.4,P < 0.05) and isolation mechanism (F =3.6,P < 0.05).Conclusions BPD patients would like to use more immature and middle defense mechanisms and less mature ones.Emotional,physical and sexual abuse from parents and other adult care takers have a correlation with immature defense mechanisms,which indicates the importance of care about parental rearing patterns and mental health of children.
9.Association between helicobacter pylori infection and dyslipidemia in different gender population

Objective To assess the association between helicobacter pylori (HP) infection and dyslipidemia in different gender population.Methods We conducted cross-sectional analysis using data of 1921 cases of demographic characteristics,anthropometry,life style,lipid profile,etc.,from the subjects who received health examination from January 2010 to June 2012 in Department of Geriatrics,Tongji Hospital.Diagnosis of HP infection was achieved by using 14C-Urea Breath Test (14C-UBT).The participants were divided into HP infection positive group and HP infection negative group by 14C-UBT.Results In female subjects,the levels of total cholesterol/high-density lipoprotein cholesterol (TC/HDL-c) were higher in HP positive group than HP negative group (P < 0.05),but there was no obvious difference between the levels of low density lipoprotein cholesterol (LDL-c) in HP positive group and HP negative group.However,in the male subjects,the levels of LDL-c and TC/HDL-c were significantly increased in HP positive group than HP negative group (P < 0.05).HP positive group had a greater risk for high TC/HDL-c both in female and male subjects.In female subjects,the risk for high TC/HDL-c in HP positive group was 1.90 times of that in HP negative group (95% CI,1.06 ~ 3.38).In male subjects,the risk for high TC/HDLc in HP positive group was 1.56 times of that in HP negative group (95% CI,1.21 ~ 2.00).But only in male subjects,the risk for high LDL-c in HP positive group was 2.33 times of that in HP negative group (95 % CI,1.34 ~ 4.06).Conclusions We observed that HP infection was probability associated with dyslipidemia.
10.Fragmented QRS scale and brain natriuretic peptide predict infarction in patients with acute myocardial infarction and heart attacks

Objective To investigate the acute myocardial infarction (AMI) patient's electrocardiogram appearing fragment QRS wave (fQRS) and brain natriuretic peptide (BNP) level and scope of coronary lesions,severe cardiac complications,and the cor relation of cardiac death.Methods For selected patients with AMI,whether based on electrocardiogram (ECG) appeared in fQRS group and non fQRS groups; immunofluorescence technique was used to detect the plasma BNP level in patients with AMI.Two groups of patients with serious cardiac events and coronary artery lesions scope were observed; Coronary artery lesion count and BNP level were recorded.Results The incidence of fragment QRS wave in patients with AMI was 34.0%,fQRS wave group height/three degree atri oventricular block,ventricular tachycardia/ventricular fibrillation,cardiac shock,cardiac death rate was higher than non fQRS wave group (P <0.05).fQRS wave group of plasma BNP and left ventricular end-diastolic diameter,the double branch lesion,multivessel lesions were significantly higher than that of non fQRS group (P < 0.01) ; left ventricular ejection fraction,the single lesion was sig nificantly lower than non fQRS wave group (P <0.01).The BNP levels in single,double,and multivessel lesions in the group with the increase of the lesion count were increased.Conclusions The AMI patients with fQRS easily complicated with severe arrhythmia,and case fatality rate was high,the prognosis was poor.fQRS on electrocardiogram (ECG) and BNP level had a certain relationship with range and degree of coronary artery lesions,degree of indexes might be used as a prediction of coronary lesions,and multivessel lesions had certain prediction value.
