1.Clinical study on Jin’s three-needle plusXiang Ju Capsules for allergic rhinitis

Objective:To observe the clinical effects of Jin’s three-needle plusXiang Ju Capsules for allergic rhinitis (AR). Methods:Fifty patients with AR were randomly divided into an acupuncture-medication group and a medication group by the random digital table, 25 cases in each group. The acupuncture-medication group was treated with dorsal three-needle and nasal three-needle in Jin’s three-needle therapy, once every day, plus oral administration of Xiang JuCapsules, 4 pills each time, and three times per day. The medication group was only treated with oral administration ofXiang JuCapsules, 4 pills each time, and three times per day. For both groups, 10-day treatment constituted a course. The therapeutic effects were observed after continuous 2 courses. The symptoms and signs are scored before and after treatment, and the therapeutic effects were assessed based upon the changes of the patient’s symptoms and signs. Results:After treatment, the total effective rate was 96.0% in the acupuncture-medication group and 72.0% in the medication group. The difference in the total effective rate between the two groups was statistically significant (P<0.05). Conclusion:The therapeutic effect was remarkable in treating AR of Jin’s three-needle plusXiang JuCapsules, better than oral administration of Xiang JuCapsules alone.
2.The establishment of the method of precise lung slicing and the evaluation of neutral red assay for measuring viability of lung slices

Aim To establish the metho d of precise lung slicing and examine the validity of neutral red(NR) assay for measuring lung slices viability.Method Inflated with 1% low melting agaro se solution, lung lobes were cut into slices by shaking microtome. With the thic kness of 400,500,600,700 ?m,lung slices were cultured in medium pH 6.8,7 .0,7.2, 7.4, respectively. After 1 h pre-incubation, lung slices were cont inuously submerged in 24-well plate, incubated for 0,2,4,6 h,respectively. NR assay, MTT assay, LDH leakage and SOD activity were used to assess the slices viability under different slices thickness, medium pH and culturing time. Result When the slice thickness was 600 ?m and medium pH was 7.0, t he viability of slices maintained best and steady for 6 h. 3.Preparation and evaluation of silymarin supersaturation self-emulsifying drug delivery system

Objective To prepare the supersaturation self-emulsifying drug delivery system(S-SEDDS) containing silymarin and to evaluate its basic properties.Methods With the time of self-emulsifying,the consequence of color visual examination and particle size as parameters,the optimum formulations of silymarin SSEDDS were screened by solubility test,compatibility tests and pseudo ternary phase diagrams.The silymarin concentration was determined by HPLC.The in vitro dissolution characteristics of silymarin S-SEDDS were investigated with silymarin SEDDS as control.Results The optimum silymarin S-SEDDS was composed of medium chain triglycerides(MCT) 40%,Cremophor RH40(ethoxylated hydrogenatedcastor oil) 48%,Labrasol 12%.The time of self-emulsifying was less than 3 min,the average particle diameter was 49.6 nm,the adding amount of hydroxypropyl methylcellulose(HPMC) was 50 mg/g,and the average content of silymarin was 39.3 mg/g.The in vitro dissolution test of silymarin S-SEDDS showed that the presence of a small amount of cellulosic polymer effectively sustained a metastable supersaturated state by retarding precipitation kinetics.Conclusion The designed formulation of silymarin S-SEDDS is reasonable and provides a strong foundation for further development of new preparations.
4.Effects of Digoxin on Cardioventricular Function in Rats after Simulated Weightlessness

Objective To investigate the protective effects of Digoxin on cardioventricular function in rats after simulated weightlessness.Methods Eighty rats were divided randomly into 5 groups:control(Con)group,tail-suspended+sham feeding(TS+SF)group,tail-suspended+Digoxin 2 wk(TS+D2)group and tail-suspended+Digoxin 3 wk(TS+D3)group.The TS+SF group was given the distilled water.At the end of tail-suspended for 3 weeks,the heart function of 10 rats in each group was examined with echocardiography.The level of atrial natriuretic polypeptide(ANP)in serum was measured with immunoradiometric assay in other 6 rats.Results Compared with TS+SF group,the contractile function of LV improved in TS+D2 and TS+D3 groups(P0.05).Conclusion Digoxin can improve muscle contractile and blood ejection of LV and retard the tendency of atrophy in simulated weightlessness.
5.Effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes

Objective To explore the effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes. Methods A total of 202 elderly patients with type 2 diabetes in our hospital were selected,and were randomly divided into observation group and control group according to the random number table, with 101 cases in each group. The control group were given healthy related education during hospitalization , and post-discharge telephone follow-up every month, lasting 10~15min each time.The observation group were additionally given timeliness motivation theory-based continuing care by telephone follow-up each month , including emotional arousal , need motivation , examples stimulating and benefit motivation, lasting 6 months. Their self-care ability and healthy behavior in the two groups were compared. Result The self-care ability in the observation group was significantly significantly better than that of the control group (P<0.001), and had significantly much more healthy behaviors than the latter (P < 0.001). Conclusion Timeliness motivation theory-based continuing care can improve the self-care ability and healthy behavior in elderly patients with type 2 diabetes.
6.Two-dimensional speckle tracking imaging evaluation of left ventricular global longitudinal strain in normal children

Objective To explore the clinical value of two-dimensional speckle tracking imaging (2DE-STI) by measuring global longitudinal strain (GLS) in normal children.Methods Totally 176 normal children underwent cardiac ultrasound examination,the left ventricular GLS were measured,and the difference of GLS between gender was analyzed.And the changes of GLS with body surface area (BSA) and age were discussed.Results GLS had no significant difference between male and female ([-24.90 ± 2.06]% vs [-24.93 ± 2.01]%,t =0.83,P =0.934).GLS had significant differences among different BSA groups (F=3.84,P =0.003),and the differences of GLS in BSA> 1.0 group and the other five groups were statistically significant (all P<0.05).GLS had statistically significant differences among different age groups (F=4.81,P=0.001),and the differences of GLS in >9 years old group and the other four groups were statistically significant (all P<0.05).Conclusion Left ventricular GLS presents certain regularity with children's growth.
7.The left ventricular cavity blood flow energy loss in patients with dilated cardiomyopathy as detected by vector flow mapping

Objective To study the characteristics of blood flow in isovolumic systole(IVC)of left ventric-ular(LV)in patients with dilated cardiomyopathy by flow-vector imaging(VFM). Methods Color flow Doppler imaging was performed from the apical long-axis view in 58 patients with dilated cardiomyopathy who was admitted in our hospital from October 2015 to November 2016 and 56 healthy volunteers as control. Doppler flow data ob-tained during IVC were analyzed offline with vector flow mapping.Results At the beginning of IVC,the swirl area (15 ± 3)cm2and flow(36 ± 8)cm2/s in the control group were significantly less than those in the observation group (27 ± 8)cm2,(45 ± 12)cm2/s,all P=0.000.At the end of IVC,the vortex flow was similar in both groups[(28 ± 4)cm2/s vs.(29 ± 11)cm2/s,P=0.617)],and the vortex area in the control group was significantly smaller than that in the observation group[(16 ± 3)cm2vs.(24 ± 9)cm2,P=0.000)].During the IVC,the area of the vortex in the control group increased slightly but not significantly(P = 0.073),but the flow rate decreased significantly (10 ± 27%). The area and volume of the vortex in the observation group decreased significantly with the flow de-creased by 29 ± 54%(P=0.000 vs.the control group).When the aortic valve was open,the average blood flow veloc-ity in the middle and basal parts of the LV compartment in the observation group was significantly lower than that in the control group(P<0.05).In multivariate models,a decreased LV ejection fraction was the only independent pre-dictor of the percentage decrease in area of the vortex during the IVC(P < 0.001),and a larger QRS width(P =0.026)and LV end-systolic long diameter(P = 0.004)were independent predictors of the percentage decrease in flow volume of the vortex. Conclusion The VFM technique enables a better assessment of the pathophysiological state of LV lumen energy loss during IVC in patients with dilated cardiomyopathy by vortex imaging.
8.Comparison of Transesophageal Echocardiography and Transthoracic Echocardiography in Preoperative and Postoperative Diagnosis of Paediatrics Total Correction of Tetralogy of Fallot

Purpose To evaluate clinical value oftransesophageal echocardiography (TEE) on total correction of tetralogy of Fallot (TOF) through preoperative and postoperative monitoring of TEE in paediatrics total correction of TOF and comparison with preoperative and postoperative transthoracic echocardiography (TTE).Materials and Methods Thirty-two children patients receiving total correction of TOF were retrospectively analyzed.TEE was adopted to measure atrial and ventricular cavity size,diameter of right ventricular outflow tract,residual shunt surrounding ventricular septum,diameter of pulmonary artery trunk,orifices flow rate and differential pressure of pulmonary artery and differential pressure of right ventricular outflow tract for all patients preoperative and intraoperative.Meanwhile,all these were compared with value measured by TTE before and after the surgery.Results Among the 32 TOF patients,preoperative TEE figured out one revised diagnosis (3%) of TTE and one supplementary diagnosis (3%).Coincidence rate of disease diagnosis of preoperative TEE and surgical outcome of the 32 patients was 100% (32/32).Intraoperative TEE examination found out 7 (21.9%) with residual surgical issues.Differential pressure of right ventricular outflow tract measured by preoperative and intraoperative TEE and value measured by preoperative and postoperative TTE was of statistical significance (P<0.05).Right ventricular outflow tract measured by intraoperative TEE was obviously reduced by approximately 28% compared with the value measured by postoperative TTE.Correlation of atrial and ventricular cavity size,diameter of right ventricular outflow tract and orifices flow rate of pulmonary artery measured by intraoperative TEE and value measured by postoperative TTE were well (r=0.75-0.87,P<0.05).Differential pressure of right ventricular outflow rate measured by intraoperative TEE was positively correlated with value measured by postoperative TTE (r=0.518,P<0.05).Difference of atrial and ventricular cavity size,diameter of right ventricular outflow tract orifices flow rate and differential pressure of pulmonary artery and differential pressure of right ventricular outflow rate measured by intraoperative TEE and consistency scattergram of value measured by postoperative TTE was of no statistical significance (P>0.05).Conclusion Application of TEE for total correction of TOF monitoring is of important practical value in correction and supplement of preoperative diagnosis and immediate postoperative surgical efficacy assessment.
9.Expression of ALC1 in tissues of esophageal squamous cell carcinoma and its effect on ESCC cell proliferation, invasion, and migration

Objective: To investigate the expression of ALC1 protein during esophageal squamous cell carcinoma (ESCC) development and progression, so as to explore its association with clinicopathological characteristics and overall survival of ESCC patients, and the effect of ALC1 overexpression on malignant biological behavior of esophageal squamous cells. Methods: Immunohistochemistry (IHC) was used to detect ALC1 protein expression in 245 primary ESCC tissues and their paired normal esophageal mucous membranes, and to determine its correlation to gender, age, tumor cell differentiation, invasion, TNM stage, lymph nodes metastasis, and overall surviv-al rate of ESCC patients. MTT assay, colony formation assay, transwell invasion, and wound healing assay were used to observe the ef-fect of ALC1 on ESCC cell proliferation, invasion, and migration. Results: The expression ratio of ALC1 in esophageal squamous cell car-cinoma was higher compared with that in their paired normal esophageal mucous membranes (41.6% vs . 21.2% , P<0.05). Upregula-tion of ALC1 was associated with ESCC invasion, TNM stage, and lymph node metastasis (P<0.05). The overall survival of ESCC patients with ALC1 overexpression was significantly lower than that in patients with downregulated ALC1 expression (P=0.002). Therefore, ALC1 may promote the proliferation, invasion, and migration of ESCC cells. Conclusions: ALC1 upregulation may play an important role in the progression and development of ESCC. Upregulation of ALC1 leads to poorer disease prognosis, and could promote the prolifera-tion, invasion, and migration of the KYSE30 ESCC cells. Therefore, ALC1 may have potential prognostic value for ESCC patients.

Clinical pathway is an important quality management tool for regulating medical behavior both at home and abroad, and an important means of controlling medical costs in the reform of medical insurance payment methods.The author reviewed the current development status of clinical pathways both at home and abroad, focusing on summarizing the development experience of foreign countries, and analyzing the shortcomings in the development of clinical pathways in China from the perspectives of formulation, implementation, and evaluation. It is proposed that China should establish and improve the regulatory and incentive mechanisms for clinical pathways, accelerate the construction of supporting medical security systems, explore new incentive transmission models, attach importance to the role of patient participation in the formulation and implementation of clinical pathways, and so on, in order to provide reference for promoting the efficient development of clinical pathways in China.
