1.Cyclooxygenase-2 promotes angiogenesis possibly by increasing vascular endothelial growth factor expression in gallbladder carcinoma

Objective To investigate the relationships between the expression of cyclooxygenase-2 (COX-2) , vascular endothelial growth factor (VEGF) and the degree of vascularization, clinicopathological feature, survival time of the patients with gallbladder carcinoma. Methods Routine paraffin-embeded sections of gallbladder carcinoma tissues in 64 cases were evaluated for COX-2, VEGF expression and MV by the streptavidin-peroxidase complex immunohistochemical method. Results COX-2, VEGF immunoreactivity were observed in 72%,and 55% cases, respectively. The average MVC was (57?14)/ HP. The status of MVC was closely correlated with Nevin staging, tumor differentiation and lymph node metastasis (P well differentiated,P0.05). There was a positive correlation between COX-2 expression and clinical stages. The positive rate of COX-2 was higher in cases of Nevin stages S4-S5 (82%) with lymph node metastasis than in those of Nevin stages S1 -S3 (50%) and without metastasis (P0.05 ). 2.A second ERCP for the failure of pre-cut ERCP

Objective To study the therapeutic value of a second ERCP for the patients with failure of pre-cut ERCP.Methods A total of 167 cases of pre-cut ERCP failure were recruited to the study,among which 109 cases were diagnosed as common bile duct stones and/or benign papillary stenosis,and 58 cases as biliopancreatic lesion.ERCP failed with standard intubation for more than 20 minutes,even with pre-cut or fenestration.A second ERCP was preformed after rest of 3-5 days.The position sequence of intubation for most patients was horizontal,the front and rear.Results The success rate was 79.6% (133 cases) for the patients with a second ERCP,85 patients received the procedures via the horizontal intubation,36 via anterior,and 12 via posterior intubation.The treatments were performed after successful completion of the endoscopic cannulation.One patient had retroperitoneal infection with duodenal perforation,another patient had severe pancreatitis,who were cured by the intervention methods.Conclusion The success rate of a second ERCP is high with proficient intubation skills.
3.Role of aquaporin-4 on cerebral ischemia/reperfusion injury in rats

Objective To investgate the role of aquaporin-4(AQP4) in secondary cerebral edema after ischemia/reperfusion injury in rats.Methods When the models of reversible middle cerebral artery occlusion were established, the alterations of cerebral edema and BBB were evaluated by measuring water and Eval's Blue (EB) contents of cerebral tissue, and the expression of AQP4 in brain was observed by Western Blot at different time point after reperfusion. 4.Application Experience of Pulse-activating Injection in Hypotension Caused by Tracheal Intubation

Pulse-activating injection has the functions of supplementing qi and nourishing yin, and recovering pulse from a collapse state. It is applied in hypotension caused by serious infection or hypovolemia extensively. Pulse-activating injection can be also used to treat hypotension caused by tracheal intubation with good effects. Appropriate application according to state of illness is very important, such as controlling infection and supplementing blood-volume, etc. When applied in cardia insufficiency, vasodilating agent should be used together to regulate dropping rate.

Start to explore the theory conditions around the theme why to build the modern basic theory of tradi-tional chinese medicine and what kind of theory we want to establish . On the basis of making clear the related concepts such as theory, knowledge, scientific theory and scientific knowledge, discuss the theoretical development pathways and evaluation criteria, compare the development mode of Chinese and Western medicine, providing a refer-ence for evaluation and future development of the modern basic theory of traditional chinese medicine. According to Carl G. Hempel's bridge principle, summarize the five bridge principles and a logic protection principle of the research and foundation of the modern basic theory of traditional chinese medicine, and explain its role and signifi-cance. Put forward a theoretical framework of the modern basic theory of traditional chinese medicine Three levels and five grades like a tree structure, demonstrate the scientific connotation of 11 principles and concepts under the frame. Analyze and demonstrate its scientific rationality according to the role and evaluation criteria of scientific theory mentioned above, clearing direction for future development.
6.Transient transfection of angiopoietin-1 into vascular endothelial cells by ultrasound combined with microbubbles

Objective To investigate the effects of ultrasound and microbubbles on transient transfection of angiopoietin-1(Ang-1) into vascular endothelial CRL1730 cells and the effects of cell migration. Methods CRL1730 cells were divided into four groups:a control group (without any treatment), an Ang-1 group (with Ang-1 added only), a microbubble group (with only microbubbles added) and an ultrasound with microbubbles group (Ang-1, microbubbles and 1 MHz ultrasound irradiation for 30 s). All were cultivated for 24 h. The expression of Ang-1 mRNA and protein were measured using RT-PCR and Western blotting. The cells' migration ability was detected using the Transwell method. Results There was no difference in the expression of Ang-1 mRNA or protein among the control group, the Ang-1 group and the microbubbles group, but the expression of Ang-1 mRNA and protein was increased significantly in the ultrasound plus microbubbles group. Cell migration ability was also enhanced significantly in the ultrasound and microbubbles group compared with the other groups. Conclusion A combination of ultrasound and microbubbles might successfully transfect Ang-1 genes into vascular endothelial cells, and the migration ability of the transfected cells is also enhanced.

The efficacy of different kinds of insulin analogue administered by multiple subcutaneous injection or continuous insulin infusion was compared. The two forms of administration all can well control blood glucose. Continuous subcutaneous injection has better compliance.
8.Observation of effect of metformin and exercise therapy for IGT.

Objective To observe the effect of metformin and exercise therapy for IGT. Methods 86 patients with IGT were given metformin and exercise therapy,the range of glucose,BMI and symptoms improvement were observed before and after the therapy. Results After 6 months therapy,the fasting blood glucose and 2 h after oral administration of 75 g glucose levels were significantly reduced(P<0.01=,BMI reduced(P<0.05=.72 cases (83.7%)were back to normal glucose tolerance,14 cases maintained IGT(those unable to adhere to exercise and no diet controlled),0 case became DM. Conclusion Metformin and exercise therapy had good efficacy in curing IGT.
9.Effects of ultrasound-mediated SonoVue microbubbles destruction on the integrity and expression of hAng-1 gene

Objective To test the efficiency of gene transfer and expression mediated by ultrasound and microbubble strategy in 293T cell. Methods SonoVue microbubbles were mixed with pla.smid DNA encoding hAng-1 and green fluorescent protein. The mixture of the DNA and microbubbles was administer to cultured 293T cells by ultrasound exposue. The ultrasound condition was 1. 5 W/cm~2 and 30 s. Forty eight hours later, transfer rate was assessed by fluorescence microscopy and flow cytometry. Cell viability was assayed by Trypan Blue staining. RT-PCR and Western blot analysis was used to examine the expression of hAng-1 mRNA and protein. Agarose gel electrophoresis was used to evaluate the integrity of the plasmid. Results The transfection expression rate of eGFP in 293T cells was markedly increased with the additon of 20% microbubbles and 15 mg/L DNA. Fetal calf serum had no influence on the gene transfer rate. Ultrasound irradiation combined with microbubbles couldn't destroy the integrity of plasmid. Conclusions Ultrasound-mediated microbubbles destruction can increase the transfection and expression of hAng-1 gene in 293T.
10.Feasibility of dose reduction in lumbar spine multi-slice CT examination with automatic tube current modulation

Objective To investigate the feasibility of achieving consistent image quality with dose reduction technology in lumber spine MSCT examination with Z-axis automatic tube current modulation (ATCM). Methods Forty-eight patients diagnosed as lumber intervertebral disc protrution scanned twice by MSCT before and after interventional operations with the same coverage from third lumbar vertebra to first sacral vertebra. The first scan (FM) was with fixed tube current of 320 mAs. The follow-up scan was with ATCM with noise index (NI) of 12.0 HU. At the levels of L3-4, L4-5 and 15-S1, image quality, image noise and radiation dose were measured and analyzed. Image quality and radiation dose were compared by paired t-test and the image noise was compared by ANOVA test. Results The dosage of the ATCM had a 31.3% reduction compared with FM, the average DLP was(187.9±66.4)mGy·cm and(273.4±45.4) mGy·cm respectively, where t = 8.205, P < 0.05. The average noise and their deviations for the FM group were (9.8±2.4) HU,(9.9±2.4) HU, and (11.5±3.2) HU at level of L3-4, L4-5, 15-S1, respectively. With ATCM, the average noise was(12.0±0.8) HU, (11.7±0.6) HU, and (11.7±1.4) HU, respectively. There was statistical difference between the two groups (F = 23.31, P < 0.05). The image quality scores for the FM group were (4.7±0.3), (4.5±0.2), (4.5±0.2) and showed no statistical difference to ATCM group (4.6±0.3), (4.5±0.2), (4.5±0.2) at level of L3-4, L4-5, L5-S1, respectively, where t = 1.000, P > 0.050 Conclusion ATCM technique with the noise index setting at 12.0 HU can achieved a 31.3% dose reduction while keep the consistent image quality for lumbar spine MSCT study.
