1.An experimental study on the embolus-capturing efficiency of a new-designed retrievable artery filter

Objective To develop a physical in vitro model of the human carotid artery(CA) and analyze the factors that influence the embolus-capturing efficiency of the retrievable artery filters. Methods The flow model closely simulated the physiological parameters of the human carotid artery. The influence of the following experimental parameters on the capturing rate was analyzed with a multiple logistic regression model: embolus diameter and length, CA diameter, flow quantity, type of filter, and type of testing. Results At the 5% level,except for flow quantity,a significant influence on the capturing rate could be demonstrated for the 6 factors mentioned above, the embolus diameter was the strongest influencing factor. In CA with diameters of 4,5,6,7 mm, the 8-wire filter captured (90?7)%, (91?8)%, (81?8)%,and (68?9)% of 0.5 mm emboli and (93?4)%, (95?6)%, (95?6)%,and (82?11)% of 1 mm emboli. In all, the smaller, the interlacing opening of the filter, the higher the capturing rate, and the presence of an embolus in a filter would decrease the capturing rate for the second embolus. Conclusion Retrievable artery filter is a promising cerebral protection device. The physical in vitro model is an effective alternative in studying the influence factors of the capturing rate of the filters.
2.Effects of leptin antibody on the expression of transforming growth factor-β1 mRNA in hypertrophic scar model in rabbit ears

Objective To estimate the effect of leptin antibody on transforming growth factor (TGF)-β1 mRNA expression in hypertrophic scar model in rabbit ears.Methods Fifteen New Zealand white rabbits were included in this study.Three circular incisions which measured 7 mm in diameter and reached the perichondrium,were made in each ear of these rabbits to establish 90 models of hypertrophic scar.After the operation,these models were randomly and equally divided into 3 groups to be treated with topical sodium chloride physiological solution for 40 days (saline group),topical leptin antibody of 2 ng/ml for 40 days (leptin antibody group),and topical leptin antibody of 2 ng/ml for 14 days followed by injection of leptin antibody of 2 ng/ml once a week for 3 weeks (combination group).Scar tissue was resected from these rabbit ears at 40 days after the operation,followed by the determination of scar elevation index,histopathological examination by using haematoxylin and eosin staining,and quantification of TGF-β31 mRNA expression by real-time fluorescence-based PCR.SPSS 13.0 software was used for data processing.Statistical analysis was carried out by one-way analysis of variance.Results A significant decrease was observed in scar elevation index (2.33 ± 0.33 and 2.35 ± 0.22 vs.3.33 ± 0.41,both P <0.05) and TGF-β1 mRNA expression in the leptin antibody group and combination group compared with the control group,whereas no significant difference was observed between the leptin antibody group and combination group in either of the two parameters.Pathologically,there was an apparent proliferation of capillaries in the saline group with numerous irregularly and densely arranged fibroblasts with large nuclei,while relatively few fibroblasts with small nuclei,which were arranged in a more regular way,were observed in the leptin antibody group and combination group.Conclusion Leptin antibody treatment can reduce the expression of TGF-β1 mRNA in hypertrophic scar tissue in rabbit ears.
3.Alkyl Pheols in Tap Water in Shenyang

Objective To observe the pollution states of environmental estrogen-alkyl pheols in tap water of Shenyang. Methods With solid phase extraction and gas chromatography/mass spectroscope (GC/MS) ionic selective methods, the concentration of pollutants of alkyl pheols were measured in 21 tap water samples of water works of Shenyang. Results The main pollutants of alkyl pheols in tap water of water works in Shenyang were 4-t-octyl phenol, nonyl phenol and bisphenol A. The respective maximum concentration was 690.5 ng/L (ranged 29.5-690.5 ng/L), 2 791.6 ng/L (ranged 212.1-2 791.6 ng/L) and 161.0 ng/L (ranged 14.7-161.0 ng/L). The varieties and concentrations of pollutants of alkyl pheols showed a significant difference among water samples from different water works and the seasonal changing of them was demonstrated in the waters made in the same water works. Conclusion The problem on the drinking water contamination with alkyl pheols in Shenyang should be paid more attention by government.
4.Approach of extra-articular oblique osteotomy of olecranon for the surgical management of intercondylar fracture of humerus

Objective To explore the surgical management and its results of intercondylar fracture of humerus through approach of extra-articular oblique osteotomy of olecranon. Methods From March 1995 to October 2003, 26 patients of intercondylar fracture of humerus were treated surgically through approach of extra-articular oblique osteotomy of olecranon, and the fracture was reduced and fixed with "8" steel wire. The patients were 15 men and 11 women with a mean age of 43.2 years (range 19-68 years). 14 fractures occurred on the left extremity and 12 on the right. The fractures resulted from traffic accident or fall in 23 and other trauma in 3. According to AO/ASIF classifation, type C1 fracture was found in 3, C2 in 5 and C3 in 18. 5 patients suffered open fracture(Custilo scaleⅠin 3, and Ⅱ in 2). Contusion of skin occurred in 3 and multiple injuries in 5, while primary injury of ulnar nerve occurred in 4. The rehabilitation exercises began at 2-7 d post-operatively. The healing of the osteotomy was evaluated with physical examination and plain X-ray film, and the function of elbow was assessed according to Cassebaum scale. Results 21 patients were followed from 9 months to 8 years(average, 18 months). Of these 21 cases, all the osteotomies healed in 6.2 weeks averagely, no nonunion, delayed union, separation or malunion was found. Heterotopic ossification occurred in 4 cases, and obviously limited locomotion of elbow in 2. Of the 4 patients with primary injury of ulnar nerve, no breaking was found during exploration, and the nerve was displaced anteriorly. 3 of these 4 patients recovered completely, and mild sensory disturbance and dyskinesis remained in 1. The function of the elbow showed excellent in 12, good in 6, fair in 1 and poor in 2. The locomotion of the elbow averaged 110?. Conclusion The use of the approach of extra-articular oblique osteotomy of olecranon for surgical management of intercondylar fracture of humerus has many advantages, it provides satisfactory stability with simple technical procedure avoiding intra-articular invasion, it facilitates rehabilitation exercises and provides good results with low complication rate.
5.Effect of Dietary Medium-and Long-chain Triacylglycerols on Body Fat Accumulation in Subjects with Different Levels of Hypertriglyceridemia

Objective To investigate the effect of medium- and long-chain triacylglycerols on body fat accumulation in subjects with different levels of hypertriglyceridemia. Methods 112 subjects with hypertriglyceridemia were randomly divided into two groups: one group was demanded to adopt long-chain triacylglycerol (LCT) oil as their daily use and the other one to medium- and long-chain triacylglycerol (MLCT) oil for consecutive 8 weeks. Anthropometric measurements of body weight, body mass index (BMI), waist circumference (WC), hip circumference(HC), WHR (ratio of WC/HC), triceps skinfold thickness (TSF), subscapular skinfold thickness (SSF), body fat weight, body fat percentage, total fat area, visceral and subcutaneous fat area in abdominal and concentration of blood triglycerides (TG) were measured at the initial and final time of the study. In both groups, the cutpoint of TG 2.3 mmol/L was used to distinguish the different levels of hypertriglyceridemia. Results Compared with LCT group, body weight, BMI, WC, body fat weight, total fat area, visceral and subcutaneous fat area in abdominal in those of TG≤2.3 mmol/L in MLCT group were significantly decreased (P 2.3 mmol/L between the two groups. Furthermore, the concentrations of TG in hypertriglyceridemia subjects of different levels in MLCT group were both significantly lower than those in LCT group after 8 weeks (P 2.3mmol/L.
6.Treatment of nonunion after Pilon fracture with fibula approach fixation of tibia and ankle joint fixation

[Objective]To investigate the treatment of nonunion after Pilon fracture with fibula approach fixation of tibia and ankle joint fixation and its clinical efficacy.[Method]Six cases of nonunion after Pilon fracture were collected from 1999 to 2004,which included 4 males and 2 females,with the average age of 34.7 years(21~53 years).All fractures were found invasion to the tibia-fibula joint and ankle malformation.Fibula and tibia were opened from posterolateral approach and reconstruction plate was placed laterally on the fibula,with screws crossing the fibula into the tibia directly.Autogenous bone graft was used in fracture position and area between tibia and fibula to gain tibia-fibula fusion.[Result]All cases were followed up from 8 months to 4 years with the average of 22 months.Bone fusion were gained in 5 cases with the average time of 3.5 months.Plate broken was found in 1 case for too early weight bearing.[Conclusion]Fibula approach fixation of tibia and ankle joint fixation has advantages of promoting fracture healing,preventing complications,and correction joint malformation.It is a convenient and reliable method for clinical surgeons.
7.Experience about treatment of segmental and comminuted femoral fractures in multiple injuries

[Objective]To summarize the experience about treatment of segmental and comminuted femoral fractures with intramedullary nail in multiple injuries.[Method]From March 2002 to July 2006,28 patients with segmental and comminuted femoral fractures were treated surgically,13 with AO type C1,9 with C2 and 6 with C3.All of the fractures were treated with close reduction static intramedullary nail fixation.[Result]The postoperative follow-up ranged from 11 to 34 months with an average of 21 months.All of the fractures were healed.Limit load was required for 5 patients who had delayed union without special treatment.After 18-30 months the patients healed up.There were no fat embolisms.No complications such as infection,intramedullary bending or break occurred.[Conclusion]Treatment of femoral fractures with intramedullary nail is a standard method.However,to achieve satisfactory results,comprehensive treatment,careful planning and procedure should be taken for segmental and comminuted femoral fractures in multiple injuries.
8.Children′s cavernous transformation of the portal vein: angiographic features and the impact on surgical treatment

Objective To study the arterial portography features and the impact on surgical treatment of children′s cavernous transformation of the portal vein (CTPV). Methods Angiographic findings of the arterial portography of 6 children with CTPV and its impact on surgical treatment were retrospectively analyzed. Results Abrupt occlusion of the portal vein at the porta hepatis was revealed in 5 out of 6 cases, and a masslike network of intertwined veins around the porta hepatis and many small irregular veins radiating from the network to the liver were demonstrated. Both hepatopetal and reverse flow of collateral venous pathways were defined. Valuable information of correlative vessels for surgical treatment was obtained from all 6 angiographic findings. ConclusionChildren′s CTPV have very characteristic angiographic findings, which can be very helpful for surgical management.
9.Evaluation of angioplasty and stenting for carotid artery stenosis

Objective To promote the application of angioplasty and stenting technique for the treatment of carotid artery stenosis in China. Methods The angioplasty and stenting were performed in 12 cases with carotid artery stenosis, of which 7 were symptomatic. Of 12 cases, 10 were atherosclerotic and the other 2 were of Takayasu′s disease. Results 11 stents were successfully implanted. Minor stroke occurred in 2 cases during the procedure and both were completely recovered with thrombolytic treatment in less than 1 hour. Follow up of the 11 cases 6 months through 4 and half years, no restenosis and ischemic symptoms were revealed. Conclusion Endovascular stent placement for carotid artery occlusive disease is evolving to an accepted alternative to surgery. The use of cerebral protection during the proceture is needed in the future.
10.Effect of brain derived neurotrophic factor precursor block on improvement of motor function in rats subjected to cord transaction

Objective To investigate the role of precursor of brain derived neurotrophic factor (proBDNF) in the improvement of motor function of rats subjected to cord transection. Methods Spinal cord transection was performed at T10 level. The rats were divided into sham operation group and artificial cerebrospinal fluid group. Then, the rats were subjected to proBDNF antibody treatment. The efficiency of proBDNF block was also determined by immunohistochemistry. BBB scale scores were used to e-valuate the effect of ProBDNF block on the motor function of the rats with spinal cord transection. Results Compared with control rats, proBDNF treatment showed a marked down-regulation of proBDNF protein in the spinal cord and could significantly improve the motor function. BBB scores in proBDNF block group was higher than those in artificial cerebrospinal fluid group. Conclusion The proBDNF block is available to the recovery of the motor function in hindlimbs in cord transected rats, suggesting that proBDNF could be as a member of candidate molecules for SCI therapy in the future.
