1.Electro-acupuncture at points of Zusanli and Quchi exerts anti-apoptotic effect through the modulation of PI3K/Akt signalling pathway

Objective To evaluate the neuroprotective effect of electro‐acupuncture(EA) on cerebral ischemia‐reperfusion (IR) injury and deeply investigate the relationship between this neuroprotective effect and PI3K/Akt pathway .Methods A total of 60 Male adult SD rats were randomly divided into five groups:sham control group(SC ,n=12) ,ischemia control group ,(IC group , n=12) electro‐acupuncture group(EA group ,n= 12) ,EA + DMSO group(DMSO group ,n= 12) and EA + LY294002 group (LY2940002 group ,n=12) .Rats in the IC group、EA group、DMSO group and LY2940002 group underwent 2 h of middle cerebral artery occlusion(MCAO) ,followed by 72 h of reperfusion .DMSO or LY294002 was injected into the lateral ventricle at 30 min be‐fore the ischemia .Rats of the IC group ,EA group ,DMSO group and LY2940002 group received electro‐acupuncture at 24 h after the operation and continued until animals were sacrificed at 72 h after the operation ,and the stimulation parameters were set as fol‐lows :disperse wave ,30 min of each time ,once a day .Neurological deficit scores were assessed at 24 h ,48 h and 72 h after the IR in‐jury .The infarct volume was determined at 72 h after the IR injury using TTC staining .Apoptotic cells in the cerebral cortex were quantified under confocal fluorescence microscope using TUNEL staining .The expression of PI3K ,Akt ,p‐Akt ,Bad and p‐Bad at the protein level was assessed by Western blotting .Results Significant differences were observed in terms of neurological deficit scores between and among the EA group ,DMSO group and other groups at 48 h and 72 h after the cerebral IR injury(P<0 .05) .At 72 h after the cerebral IR injury ,the infarct volume of the IC group and LY294002 group was significantly larger than those of the EA group and DMSO group(P<0 .05) .Compared with the IC group and LY294002 group ,EA treatment greatly reduced the number of apoptotic nerve cells(P<0 .01) .The expression of PI3K ,p‐Akt and p‐Bad at the protein level in the EA group and DMSO group were higher than those of the IC group and LY294002 group(P<0 .05) .Conclusion EA treatment at points of Zusanli(ST36) and Quchi(LI11) exerted the neuroprotective effect on the cerebral IR injury through the modulation of PI3K/Akt pathway .
2.Effect of motivational interview on the self-care behaviors in type 2 diabetic patients

Objective To explore the effect of motivational interviewing on the self-care of type 2 diabetic patients . Methods Totally 96 inpatients with type 2 diabetes were randomly divided into the control group and the intervention group , each group containing 48 patients. The control group received routine diabetes education, while the intervention group received face-to-face and one-by-one motivational interviewing intervention for three months. The patients′self-care level were evaluated by using diabetes self-care scale after intervention. Result After intervention, the score on the self-care behaviors including diet control, exercise therapy, medication , blood glucose monitoring and foot care were significantly higher in the intervention group than those in the control group (P < 0.01). Conclusion Motivational interviewing can improve the ability of type 2 diabetic patients to control the glycemic level.
3.The analysis on the causes and treatment stretagies of Stanford A aortic dissection

Objective To analyze the reasons of perioperative hypoxia in Stanford A aortic dissection,and summarize its management strategies.Methods From Dec.2005 to Jul.2011,sixty four patients underwent surgery for acute type A aortic dissection,of which 9 cases were with chronic dissection and 55 cases with emergent ones.Preoperative oxygen fraction ratio( PaO2/FiO2 )in 51 cases was lower than 200 mm Hg.All of them underwent the surgery with the help of deep hypothermia cardiac arrest technique.Results Three cases died.Thirty-three cases could not live without ventilation during the first 72 h because of continuous hypoxia ( PaO2/FiO2 < 200 mm Hg).One case underwent tracheotomy and auxiliary ventilation for 9 days.The rest were live without ventilation after auxiliary ventilator for 72 - 120 hrs.The data showed that postoperative hypoxia was related to preoperative hypoxia (oxygen fraction ratio < 200 mm Hg),BMI,acute onset,hypothermia cardiac arrest time,and transfusion more than 3000 ml ( P < 0.05 ).Conclusion Great attention should be paid to the perioperative hypoxia-related factors in Stanford A dissection,which will be helpful to improve prognosis.
4.Monitoring of microembolism and cerebral blood flow before, after and during carotid angioplasty and stenting with filter devices

70%). TCD monitored the microembolic signals (MES) and cerebral blood flow of middle cerebral artery (MCA) using M-Mode before, after and during CAS. TCD and color ultrasound duplex were examined and new vascular events recorded during the following-up. Results Except a large numbers of MES arising from the recent contrast injection, the most MES were detected during the phase of predilation (MES number=116) and stent placement (MES number=135, 91, 113, 90, 106, 125). The MES count during the phase of postdilation was relatively reduced (MES number=8, 14, 37, 16). The most MES were characterized by high intensity and over-loading in TCD spectrum. MCA blood flow velocity varied predominantly in phases of predilation (Vsys/Vmean=46/31 to 76/54) and postdilation (Vsys/Vmean=40/26 to 74/49, 0/0 to 114/69, 35/24 to 116/71, 50/36 to 137/86). MES recorded in 2 patients prior to CAS disappeared in post-procedure. MCA blood flow velocity was enhanced or MCA pulsative index increased after CAS. No complications occurred. Tissue debris was collected in one filter device. During 3 to 19 months following-up, only one patient had a new vascular event of cerebral infarction located in the contralateral side of the CAS. Conclusions TCD monitoring might evaluate the microemboli and cerebral blood flow before, after and during CAS with filter devices. Filter devices might reduce embolization by preventing the bigger particulate microembolism into the distal part of the cerebral vessel without influencing cerebral blood flow. The gaseous and the smaller particulate microembolism unfiltrated should not result in clinical symptoms in most cases.

BACKGROUND: In the research of biomaterial, especially bio-coatings field, how to find a good biological and mechanical properties basal body, and to prepare coatings with uniform and compact surface and strong bonding force, is the key factor in clinical Application of artificial prosthesis. OBJECTIVE: To review the progress of bio-coatings.METHODS: Databases of CNKI, VIP, and Science Direct were retrieved with key words of "Biocoatings, HAp, Titanium, Carbon/carbon composites" to search papers published from January 1998 to October 2009. PApers underlying bio-coatings were selected. Totally 216 literatures were initial searched, according to inclusive criteria, 31 literature were summarized. RESULTS AND CONCLUSION: By contrasting the basal bodies made of titanium or carbon/carbon and different coating preparation, we found that the basal body composed of carbon/carbon composites exhibited good biocompatibility, which had similar strength to natural bone. Using hydrothermal method, the composites coating prepared by taking carbon/carbon as basal body and hydroxyApatite, chitosan, or polymethyl methacrylate as coatings provided a new idea for increasing the combing strength between coatings and basal body.
6.Study on Isolation and Structure Characteristics of Immunoactive Polysaccharide RTPS-Ⅰfrom Trichosan-this Radix

Objective:To separate and purify the immunoactive polysaccharide RTPS- I from Trichosanthis Radix and study the physicochemical properties and preliminary structure. Methods:The crude polysaccharides were extracted by water, and then precipita-ted by ethanol. After the deproteinization with trichloroacetic acid, the polysaccharide RTPS-I was separated and purified by using DE-AE-Cellulose, DEAE-Sepharose FF and Sephadex G-200 column chromatography. The analysis of properties and structure was per-formed by using GC, HPGPC, IR and NMR. Results:RTPS-I was a white flocculent solid after vacuum freeze-drying, and soluble in water. It contained three elements (carbon, hydrogen and oxygen) with the specific rotation [α]25D (H2O) of +164. 44 and the relative molecular mass of 17555. The glycosyl in RTPS- I was composed of glucose without uronic acid. The sugar ring was pyranoid ring, and each monosaccharide was linked by α-glycosidic bond. Conclusion: A homogeneous immunoactive polysaccharide RTPS- I from Trichosanthis Radix is isolated for the first time.
7.Minimally invasive percutaneous pedicle screw fixation versus open surgery for thoracolumbar fracture:Cobb’s angle and vertebral height

BACKGROUND:Open reduction and internal fixation cause big trauma and many complications. With the progression of minimal y invasive concept, percutaneous pedicle screw fixation gradual y showed its obvious superiority. <br> OBJECTIVE:To compare clinical outcomes of minimal y invasive percutaneous pedicle screw fixation versus open surgery in the treatment of thoracolumbar fracture. <br> METHODS:From October 2012 to January 2014, 50 cases of thoracolumbar fractures, including 25 cases in the minimal y invasive percutaneous pedicle screw fixation group and 25 cases in the open surgery group, were retrospectively analyzed. The differences in length of skin incision, intraoperative blood loss, operation time, postoperation hospital stay, and visual analog scale scores were compared. Serum creatine kinase activity and C-reactive protein levels were measured before surgery and at 24 and 48 hours after operation. Imaging results were used to observe vertebral height and kyphosis Cobb’s angle changes. <br> RESULTS AND CONCLUSION:Compared with the open surgery group, the length of skin incision was smal er and intraoperative blood loss was less, operation time, bed time and hospital stay were shorter, and pain of the wound was lighter in the minimal y invasive group. No significant difference was found in serum creatine kinase activity and C-reactive protein levels between the two groups. Serum creatine kinase activity and C-reactive protein levels were higher at 24 and 48 hours after treatment compared with before treatment in both groups. Serum creatine kinase activity and C-reactive protein levels were higher in the open surgery group than in the minimal y invasive group at 24 and 48 hours. There were significant differences in vertebral height and kyphosis Cobb’s angle in both groups after treatment compared with before treatment (P<0.01). No significant difference in vertebral height and kyphosis Cobb’s angle was detected between the two groups after treatment (P>0.05). Results indicated that minimal y invasive percutaneous pedicle screw fixation and open surgery in repair of thoracolumbar fractures had similar outcomes. However, the trauma of minimal y invasive percutaneous pedicle screw fixation was apparently less than open surgery.
