1.Affinity maturation of human scFv anti-TSLP in vitro

Objective:To single amino acid mutation of the full human scFvs against TSLP to enhance its affinity.Methods: The specific scFvs against TSLP was screened in our previous study and here the three-dimensional structures of TSLP and anti-TSLP scFvs were simulated by Discovery Studio system,then the molecular docking was made.The amino acids of binding epitope were randomly mutated and the mutated amino acids were selected which could remarkably improve the affinity of scFvs.The primers were designed based on the sequence of mutation amino acids and the scFv sequences were mutated by the overlapping extension PCR.The DNA of mutated scFvs was ligated with the expression vector pLZ16 and transformed into E.coli DH5αF′.Then the scFvs were expressed and the scFvs with improved affinity were selected by ELISA and BIAcore.Results: The five scFvs with single amino acid mutation were screened out by DS system,which could elevate the affinity of scFvs.The mutated anti-TSLP-scFvs were amplified by PCR,which size was about 1 000 bp.The mutated scFvs with correct sequence were expressed,and the mutated scFvs with improved affinity were detected by ELISA and BIAcore.The affinity of selected mutated scFv (M4) has been about 10 times higher than the scFv nonmutation.Conclusion: The affinity of anti-TSLP-scFv has been improved successfully.
2.Transcatheter arterial embolization with Glubran-2 glue for treating hemorrhage after percutaneous transhepatic cholangial drainage

Objective To observe the effect of transcatheter arterial embolization(TAE)with Glubran-2 glue for treating hemorrhage after percutaneous transhepatic cholangial drainage(PTCD).Methods Data of 17 patients with hemorrhage after PTCD who underwent TAE with Glubran-2 glue were retrospectively analyzed.The technical success rate,clinical success rate and complications were observed.The red blood cell(RBC)and hemoglobin(Hb)on the day of TAE and the next day of TAE were compared,also the glutamic-pyruvic transaminase(GPT)level before TAE,on the next day of TAE,on the second and the fourth day after TAE,respectively.Results The offending vessel of bleeding was successfully embolized in all 17 cases,both technical success rate of TAE and clinical success rate of hemostasis were 100%.There was no serious complication such as liver abscess,septicemia nor pulmonary embolism.No significant difference of RBC nor Hb was found between the day of TAE and the next day of TAE(both P>0.05).GPT before TAE was lower than the next day of TAE and the second day after TAE(P<0.05),while no significant difference of GPT was found before TAE and 4 days after TAE(P>0.05).Conclusion TAE with Glubran-2 glue for treating hemorrhage after PTCD was safe and effective.
3.Clinical application of XperCT combined with needle-guided Glubran-2 glue for small pulmonary nodule localization in thoracoscopic pulmonary nodule resection

Objective To discuss the application value of XperCT combined with needle-guided Glubran-2 glue for small pulmonary nodule localization in thoracoscopic pulmonary nodule resection.Methods The clinical data of 67 patients,who received XperCT combined with needle-guided Glubran-2 glue for small pulmonary nodule localization before thoracoscopic resection of a single small pulmonary nodule at the Jining Municipal First People's Hospital of China between June 2018 and February 2023,were retrospectively analyzed.The size of the pulmonary nodule,the maximum vertical distance from the visceral pleura to the lesion,the technical success rate of localization,the number of puncturing times,the complications,the time spent for operation,and the postoperative pathological diagnosis were recorded.Results The average size of the small pulmonary nodules in the 67 patients was 8.7 mm,and the average vertical distance from the visceral pleura to the lesion was 19.4 mm.Successful preoperative localization of nodule was accomplished in all patients.The average number of puncturing times was 1.1,and no serious complications occurred.The average time spent for operation was 12.7 min.Definite pathological results were obtained in all 67 patients.Conclusion XperCT combined with needle-guided Glubran-2 glue for small pulmonary nodule localization carries advantage of accurate localization with fewer complications.Therefore,this technique is a highly-efficient and quickly-accomplished positioning method,and it is highly valuable in clinical practice.(J Intervent Radiol,2024,33:623-626)
4.Effectiveness of defocus lenses with multi-zone positive optical defocus design versus high aspheric micro-lens design in controlling myopia

Objective To compare the effectiveness of Defocus Incorporated Multiple Segments(DIMS)and High Aspheric Lenslet(HAL)defocus lenses in controlling myopia.Methods A total of 214 children and adolescents who were fitted with defocus lenses(DIMS or HAL)in our hospital from January to June 2023 were selected,including 100 cases in DIMS group and 114 cases in the HAL group.Changes in parameters such as spherical lens power,cylindrical lens power,spherical e-quivalent(SE),corneal curvature(K),axial length(AL),binocular accommodative response(BCC),positive/negative relative accommodation(PRA/NRA),distance/near latent phoria(DLP/NLP),and accommodative convergence to accommodation ratio(AC/A)were compared between the two groups before wearing and after wearing the lenses for 6 months,and the effectiveness of myopia control between DIMS and HAL was further compared.Results After wearing the lenses(DIMS or HAL)for 6 months,the average increase in axial length was(0.09±0.14)mm,the average in-crease in spherical lens power was(-0.11±0.23)D,the average increase in astigmatism was(-0.07±0.16)DC,and the average increase in spherical equivalent was(-0.15±0.24)D,with statistically significant differences observed for all these changes(t=9.24,-7.27,-6.60,-9.03,respectively,P<0.001).Compared with parameters before wearing the lenses,statistically significant differences were noted after wearing for 6 months in spherical lens power,cylindrical lens power,SE,BCC,PRA,DLP,average K,and AL(P≤0.001),except for NRA(t=-0.12,P>0.05),NLP(t=-0.28,P>0.05),and AC/A(t=0.33,P>0.05).The differences in parameters between the two groups after wearing the lenses for 6 months showed a statistically signifi-cant difference only in the AL difference(t=4.38,P≤0.001),with no statistically significant differences observed for other parameter differences(P>0.05).Conclusion Wearing defocus lenses has a positive effect on controlling myopia,delaying axial length elongation,and maintaining binocular visual function balance.
