Epstein-Barr virus is a kind of herpes virus that has oncogenic potential and is related to the tumorgenesis process of various malignancy. Important advances have been obtained on the study of the structure and functions of Epstein-Barr virus(EBV) latent membrane protein 1(LMP1) and signal transduction pathways mediated by LMP1 in recent years. LMP1 could regulate cellular growth, proliferation, transformation, differentiation, transference and apoptosis by means of signal transduction pathways such as NF-?B, AP-1, JAK/STAT, Ets1, MAPK/ATF2 and cyclin dependent kinase.

Objective To study the cell metabolism changes of hippocampal region in patients with vascular dementia (VD) after stroke by 1 H-magnetic resonance spectroscopy (1 H-MRS),and to observe the relationship between cell metabolism changes and clinical symptoms after donepezil hydrochloride treatment.Methods Enrolled 18 patients with VD after stroke,16 males and 2 females,with a mean age of (76.9±2.7) years,13 patients without VD after stroke,11 males and 2 females,with a mean age of (76.2±3.6) years and 14 health volunteers as control group,12 males and 2 females,with a mean age of (75.1+2.4) years.The mini-mental state examination (MMSE),activities of daily living (ADL) and Hamilton depression scale (HAMD) were used to assess the effects on cognitive function,ability of daily living and behavior symptoms.1 H-MRS examination of bilateral hippocampus was conducted in all subjects.Metabolisms of N-acetylaspartate (NAA),choline (Cho) and creatine (Cr) were examined and the ratios of NAA/Cr and Cho/Cr were calculated.The patients with VD were prescribed donepezil and given 1 H-MRS assessment half a year later.Results The patients with VD had a significantly lower NAA/Cr ratio in bilateral temporal hippocampus area than did health controls and the patients without VD (F=4.23,4.98,both P＜0.05),and a higher Cho/Cr ratio than did health controls and the patients without VD in left hippocampus (P＜0.05).The NAA/Cr and Cho/Cr ratios in patients without VD had no significant changes compared to health controls.There was a positive relationship between MMSE score and NAA/Cr ratio,and negative relationship between MMSE score and Cho/Cr ratio in the bilateral hippocampus area in patients with VD.After 6 months of treatment of donepezil hydrochloride,the MMSE and ADL scales significantly increased (both P＜0.05),and NAA/Cr ratio had no significant change in hippocampal region (t=-2.02,-2.04,both P＞0.05),whereas Cho/Cr had significant decrease (t=2.86,2.23,both P＜0.05).Conclusions The data suggest that the lower NAA/Cr ratio and higher Cho/Cr ratio in hippocampus area might be helpful in distinguishing VD and no-VD in patients with stroke.Donepezil improves general cognition function in patients with VD,which may be associated with regulating the cholinergic pathogenesis of VD in vivo and in vitro.

Researches have indicated that matrix metalloproteinases (MMP) and vascular endothelial growth factors (VEGF) play a key role in the metastasis of nasopharynx carcinoma (NPC). They promote tumor cell infiltration and metastasis by proteolysis of extracellular matrix proteolysis of tumor cells and induction of angiogenesis and lymphangioenesis. Researches on the mechanism of MMPs and VEGFs on NPC may help to further understand the metastasis of NPC and develop new methods for the diagnosis and treatment of NPC and other tumors as well.

Objective To improve the cleaning quality of reusable medical instruments continuously through root cause analysis,reduce healthcare-associated infection(HAI)and the wear and tear of instruments.Methods The major factors influencing the cleaning quality of instruments were analyzed and clarified by fishbone diagram,contin-uous quality improvement was carried out based on 5 terminal factors,the rate of rusting,stain,repeated cleaning, damage,and clinical department satisfaction before and after the implementation of continuous improvement were compared.Results The qualified rate of instrument clean increased from 94.24% before implementing measures to 97.60% after implementing measures,the rate of rusting,stain,repeated cleaning,damage decreased from 3.39%, 2.37%,5.76%,and 2.08% to 1 .55%,0.85%,2.40%,and 0.48% respectively,there were significant difference among different groups(all P <0.001 );the score of satisfaction rate to central sterile supply department elevated from(93.87±3.87)before implementing measures to (98.08 ±0.59)after implementing measures(t =6.80,P <0.001).Conclusion Root cause analysis is important in improving cleaning quality of reusable instruments continu-ously,as well as reducing HAI and the wear and tear of instrument,it can save medical cost.

Objective Three-dimensional(3D) automated ultrasonic volume measurement is becoming favorable in clinical application and turns to be an important direction .The article was to evaluate the consistency of two-dimensional (2D) and three-di-mensional ultrasonography in the diameter measurement of single follicle . Methods 438 single folliculars were respectively meas-ured by the same sonographer in 2D and 3D ultrasonic measurement.2D diameter, 3D mean diameter and 3D virtual ball diameter of every single follicle were recorded , and Bland-Altman method were used to evaluate the consistency of the results by these two meth-ods. Results In the consistency analysis on the diameter measurement of single follicle by 2D and 3D mean diameter measurment methods, d equaled -1.06 mm, and 95% LoA range covered the range of -4.82 mm to 2.70 mm.95% CI of 95% LoA range,-5.21 mm to 3.08 mm, was beyond the settings of clinical acceptable error limit range , -3.56 mm to 1.44 mm, showing poor consis-tency.Likewise, in the research of the consistency in the measurement of 2D diameter and 3D virtual ball diameter in the diameter measurement of single follicle , d equaled -0.07 mm , and 95%LoA range covered the range of -3.11 mm to 2.98 mm.95%CI of 95%LoA range, -3.43 mm to 3.29 mm, was beyond the settings of clinical acceptable error limit range , -2.57 mm to 2.43 mm, showing poor consistency . Conclusion 2D and 3D ultrasound measurements in the diameter measurement of single follicle are not consistent.However, considering the objectivity and accuracy of the results , 3D ultrasonography still has an advantage over 2D ultra-sonography , which can provide more acurate aid for assisted reproductive technology .

Background and purpose:The development of nasopharyngeal carcinoma in rat and transformation of embryo nasophryngeal epithelia induced by N,N'-dinitrosopiperazine (DNP) were interfered by Yiqijiedu formular medicine via inhibiting telomerase. We investigated the impact of Yiqijiedu formula on precancerous lesion of nasal and nasopharyngeal epithelia in TgN (p53mt-LMP1) / HT mice induced by DNP and on its expression level of c-jun and p16 genes. Methods:The fourth generation TgN (p53mt-LMP1) / HT mice aged 5 months were randomly divided into 2 groups:(1)TgN(p53mt-LMP1)/HT cancerous lesion-inducing group (group A) was treated by normal saline with 15ml/kg, and Chinese traditional medicine treating group (group B) was treated by Yiqijiedu formular drug with 12.91 g/(kg?d). C57BL/6J wild-type mice were chosen as negative control group (group C) treated by normal saline with 15ml/kg. Epithelial tissue samples were taken from nasal cavity and nasopharyngnx of mice respectively for pathohistological evaluation by H-E staining procedures, and expression level of c-Jun and p16 in these tissue sampleswere determined by immunohistochemical methods and western blot. OBJECTIVE:To probe into the reasons for drug repercussion in ward area and find a solution.METHODS:The reasons for drug repercussion,the kinds of the returned drugs and the proportion of these drugs in the total in terms of amount of money between Oct.to Dec.in 2006 were analyzed and compared with the amount of money of returned drugs between 2005 and 2006 in our hospital.RESULTS:There were 2 674 cases of drug repercussion between Oct.to Dec.in 2006 in our hospital;the returned drugs totaled 192 kinds,and the drug repercussion was mainly due to patients' discharge,alteration of medical orders and input error.The rate of drug repercussion was 4.67%.CONCLUSION:There were drug repercussion cases in all the departments of our hospital,and the amount of the returned drugs were large,which resulted in increased difficulty of drug quality management and serious hidden dangers for medication safety,therefore,standard administration on which is much needed.

Objective To study the drug resistance situation of Mycobacterium tuberculosis strains isolated from the patients with HIV/AIDS complicating tuberculosis (TB).Methods One hundred and ninety-six inpatients with AIDS complicating TB in our hospital from January 2012 to December 2015 were collected.The drug resistance situation of Mycobacterium tuberculosis strains cultured from sputum/tissue fluid/fiber liquid and pathological tissue submitted samples in the patients with AIDS complicating active TB was analyzed.The BACTEC MGIT960 systems was adopted to perform the bacterial identification.The drug sensitivity test was conducted by using 960 culture testing system.Results One hundred and ninety-six strains of Mycobacterium tuberculosis were cultured,the total drug resistance rate of Mycobacterium tuberculosis was 26.02％.These cases were divided into the ＞100/μL and ≤100/μL groups according to different CD4+ T cells count.The drug resistance situation of Mycobacterium tuberculosis to 4 kinds of first line anti-TB drugs were compared between these two groups.The comparison results found that the resistance rate of Mycobacterium tuberculosis strains to the first line anti-TB drugs had no statistical difference between the two groups (P＞0.05).The resistance rates of first line anti-TB drugs from high to low were isoniazid,rifampicin,streptomycin and ethambutol.The drug resistance rate of Mycobacterium tuberculosis strains to 5 types of drug resistant TB had no statistical difference(P＞0.05).Conclusion The drug resistance rate in the initial patients with HIV complicating TB is consistent with the average initial drugresistance level of TB patients in our country.The TB drug resistance rate in the patients with HIV complicating TB has no correlation with CD4+ T cells count.
