1.Diagnosis and treatment of pheochromocytoma in pregnancy:a case report

SUMMARY Pheochromocytomaisrareinpregn’ancy.Clinicalfeaturesofacaseofpheochromocytoma during pregnancy in the Peking University People’s Hospital was investigated and the literature reviewed to discuss the diagnosis and treatment of this disease.The patient manifested with hypertension and pro-teinuria,who was easily misdiagnosed with gestational hypertension disease.When she was transferred to our hospital,the symptoms such as,paroxysmal palpitation,dizziness,vomiting were noticed,and the possibility of pheochromocytoma was considered due to the accompanying abdominal mass.An emergent cesarean section was performed successfully due to preterm labor during the treatment of the disease.Af-ter the delivery the drug preparation continued.And the laparoscopic resection of pheochromocytoma pro-ceeded when the blood pressure was steady.The patient recovered fully after the surgery.The final diag-nosis of pheochromocytoma was confirmed with the pathology.Its diagnosis and treatment experiences could improve our understanding and treatment of secondary hypertension due to pheochromocytoma in pregnancy.
2.Early response and acute adverse effect after particle radiation therapy for adenoid cystic carcinoma of the head and neck

Objective To study the early response and acute/subacute adverse effects after particle radiation therapy for adenoid cystic carcinoma (ACC) of the head and neck.Methods Between May 2015 and March 2016,a total of 8 patients with ACC of the head and neck were treated using proton and/or carbon-ion radiation therapy.Three patients had early stage and 5 had locally advanced disease.Five patients had an R2 and three achieved an R1 resection.Results Seven patients received intensitymodulated proton therapy (IMPT) followed by intensity-modulated carbon-ion therapy (IMCT) boost.One patient received IMPT only.Among the five patients who had an R2 resection,2/3 patients had partial response (PR)/stable disease (SD) at the end of radiation,0/3 achieved PR/complete response (CR) at 1-month follow-up,and 1/2 achieved PR/CR at 3-months' follow-up after the completion of radiation,respectively.Two patients experienced Grade Ⅲ mucositis during radiation therapy.No patient experienced moderate or severe skin reactions.At the time of this analysis,all patients are alive and no patient had disease progression or recurrence.Conclusions The short-term outcomes indicated that particle therapy is safe and potentially efficacious in the management of head and neck ACC.However,longer follow up is needed to assess late toxicities and long-term efficacy.
3.The validity of different multiple comparison correction methods in the analysis of brain function im-age data

Objective To explore the effectiveness of different multiple comparisons correction methods by comparing the detection rate and false positive rate of brain activation analysis using functional magnetic resonance imaging ( fMRI) data. Methods On the basis of task-based fMRI dataset ( including low-intensity and high-intensity stimuli condition,n=20) and resting-state fMRI dataset( n=32),brain acti-vation results were corrected by multiple comparsion correction methods in SPM and SnPM13 software,and the activation detection rate and false positive rate were compared with different correction methods. Results Voxel-or peak-based correction methods had relatively low false positive rate. When P<0. 05 after correction,the proportion of the subjects with false-positive were 0. 19 and 0. 16,and the number of false-pos-itive voxels were 404 and 2 448,respectively. But the two methods had low detection rate,which were more suitable for detecting strong activation. While cluster-based correction methods had relative high detection rate and high false positive rate. When P<0. 05 after correction,the proportion of the subjects with false-posi-tive were 0. 34 and 0. 38,and the number of false-positive voxels were 7 870 and 8 320,respectively. And thus they were more suitable for detecting weak activation. Group-level analysis could effectively reduce false positive rate. Conclusion In practice,researchers should choose a suitable correction method based on their specific research objectives and data to achieve a balance between the detection rate and false positive rate.

OBJECTIVES:
To explore the mechanism of action of Anemarrhenae Rhizoma in treatment of Alzheimer's Disease (AD) through network pharmacology analysis and experimental validation.
METHODS:
The active ingredients and targets of Anemarrhenae Rhizoma for treatment of AD were screened with network pharmacological methods, the protein-protein interaction (PPI) network was constructed and the core targets were analyzed, enriching Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathways analysis was performed. The peripheral blood lymphocytes were extracted and lymphoblast like cell lines (LCL) were constructed and an in vitro cell model of LCL-SKNMC was established. MTT/CCK-8 method was used to detect the activity of SKNMC/LCL cells, 2 ´, 7 ´-dichlorodihydrofluorescein diacetate (DCFH-DA) probe was used to detect reactive oxygen species (ROS) generated, and immunofluorescence staining was used to detect the generation of Aβ1-42 in the co-cultured model; Western blotting was used to detect protein expression in the co-culture model. The lifespan of N2 nematodes was observed under oxidative stress, normal state, and heat stress; ROS generated by N2 nematodes was detected by DCFH-DA probes. The paralysis time of CL4176 N2 nematodes was evaluated by paralysis assay, and Aβ deposition in the pharynx was detected by Thioflavin S (Th S) staining.
RESULTS:
Through network pharmacology, 15 potential active ingredients and 103 drug-disease targets were screened out. PPI analysis showed that the Anemarrhenae Rhizoma might play anti-AD roles through albumin, Akt1, tumor necrosis factor, epidermal growth factor receptor (EGFR), vascular endothelial growth factor (VEGFA), mammalian target of rapamycin (mTOR), amyloid precursor protein (APP), glycogen synthase kinas (GSK) 3β and other related targets. KEGG analysis showed that the pharmacological effects of Anemarrhenae Rhizoma might involve the biological processes of Alzheimer's disease, endocrine resistance, insulin resistance; and neuroactive ligand-receptor interaction, phosphatidylinositol 3-kinase (PI3K)-Akt signaling pathway, calcium signaling pathway, AGE-RAGE signaling pathway in diabetes complications, neurotrophic factor signaling pathway and others. The in vitro cell experiments showed that Anemarrhenae Rhizoma was able to reduce the production of ROS and Aβ1-42 (all P<0.01), inhibit the expression of β-secretase 1 (BACE1), APP and Aβ1-42 proteins (all P<0.05), up-regulate the expression of p-PI3K/PI3K, p-AKT/AKT, p-GSK3β/GSK3β in SKNMC cells (all P<0.05). Studies further confirmed that Anemarrhenae Rhizoma prolonged the lifespan of C. elegans under stress and normal conditions, reduced the accumulation of ROS and the toxicity of Aβ deposition.
CONCLUSIONS
Anemarrhenae Rhizoma may reduce the production of Aβ in AD and inhibit its induced oxidative stress, which may be achieved by regulating the PI3K/AKT/GSK-3β pathway.
6. Operating room management strategy for orthopedic patients in Third Hospital of Hebei Medical University during epidemic of corona virus disease 2019

In December 2019, the corona virus disease 2019 (COVID-19) broke out in Wuhan, Hubei Province. Although the number of newly confirmed COVID-19 cases in provinces outside of Hubei Province has declined continuously since February 4, the epidemic of COVID-19 remains serious. As companies resume work, it is still inevitable that some high-energy orthopedic trauma patients and elderly patients with low-energy fractures will need surgical treatment. The operating room, as a place for close contact between doctors, patients and nurses, increases the risk of infection and transmission. Based on the current needs of orthopedic trauma patients and the situation of the country's resistance to the epidemic of COVID-19, the authors expound the operating room management, preparation of medical materials, transfer of patients needing surgery, intraoperative protection and post-operative end disinfection in Third Hospital of Hebei Medical University so as to provide reference for prevention and control management of the operating room during the epidemic period.
