OBJECTIVE:To optimize the parameters of the extraction and purification technologies of total flavonoids from Di-ospyros kaki Thunb. leaves, and then to compare the contents of flavonoids in fresh and dried D. kaki Thunb. leaves. METHODS:Taking the yield of total flavonoids as the investigated index, the influences of liquid-solid ratio, the volume fraction of ethanol and ultrasonic extraction time on the extraction effect were discussed by single-factor and orthogonal test. With the purity of total flavo-noids as the investigated index, the purification effects of such three kinds of methods as ethyl acetate extraction method, alcohol deposition method and macroreticular resin purification method combined with petroleum ether degreasing on the extracted solution were compared. The optimal extraction technology was adopted to extract the total flavonoids from fresh and dried D. kaki Thunb. leaves and their contents were compared. RESULTS:The optimal extraction technology was as follows as liquid-solid ratio of 25∶1 (ml/g),volume fraction of ethanol of 70%,ultrasonic extraction time of 30 min, extraction temperature of 30 ℃. The results of the verification tests showed the average content of total flavonoids was 1.75%(RSD=2.00%,n=3). The total flavonoids in the extracted solution purified by the above-mentioned three purification methods had a purity of 24.92%,15.94% and 35.52% respec-tively,in which the macroreticular resin purification method with petroleum ether degreasing combined with AB-8 resin purification was optimal. The content of flavonoids in fresh D. kaki Thunb. leaves(1.75%) was about twice as much as that in dried leaves (0.87%). CONCLUSIONS:The optimal extraction and purification technologies are simple with good effect, and suitable for large-scale production. Fresh D. kaki Thunb. leaves should be used as raw materials for extracting flavonoids.

Objective To evaluate the reliability and validity of a medication dependence questionnaire in mi?graineurs of the Chinese version. Methods One hundred fifteen migraine patients with drug dependence syndrome were included and their clinical data were then collected. All the subjects were interviewed with the medication dependence questionnaire in migraineurs of the Chinese version. The reliability and validity of the questionnaire were evaluated ac?cording to test-retest reliability and internal consistency reliability, and the content validity and construct validity respec?tively. Results The test-retest reliability of the questionnaire items was 0.725~1.000 except item 11 (0.558) and item16 (0.443). The internal consistency coefficient of the questionnaire was 0.820, internal consistency coefficient of each di?mension was above 0.7. Every item score was positively correlated with corresponding dimension score (r=0.595~0.962). Seven factors with latent roots greater than 1 could explain 76.3%of total variance, and these factors were relatively pow?erful in related items. Conclusions The medication dependence questionnaire in migraineurs of the Chinese version shows preferable reliability and validity. Thus, this is a promising assessment tool to evaluate the medication dependence in Chinese migraineurs in future clinical practice.

Abstract: Objective To observe the curative effect of thread-hanging combined with cotton plug on stage Ⅲ paronychia. Methods Sixty-one patients with stage Ⅲ paronychia were selected and randomly divided into a treatment group and a control group. The treatment group (n=31) was treated with thread-hanging and tampon under local infiltration anesthesia, and changed dressing and tampon every day after operation. After the wound healed, the patient soaked his feet in warm water every day and changed the tampon himself until the symptoms subsided, and the knot did not receive special treatment, and the nail plate would naturally shed as it outgrew the paronychia. The control group (n=30) was treated with thread-hanging and nail groove reconstruction under nerve block anesthesia, and the dressing was changed every day after operation. After thread removal, the patients soaked their feet in warm water every day until the symptoms subsided, and the knot was not specially treated, and it naturally fell off with the growth of the deck beyond the nail groove. The postoperative Visual Analog Scale (VAS) pain score, pain duration, wound healing time, cure rate, effective rate and recurrence rate of paronychia, and patients' satisfaction with the operation were compared between the two groups. Results Compared with the control group, the treatment group had lower VAS pain scores on the first and third postoperative days (2.1±0.3) and (0.2±0.1) vs. (6.3±0.1) and (3.2±0.2), respectively, shorter duration of pain and wound healing time (3.3±0.3) days and (10.1±0.5) days vs. (5.2±0.3) days and (15.2±0.3) days, respectively, higher cure rate (87.1% vs. 66.7%), lower failure rate (12.9% vs. 33.3%), lower recurrence rate (7.4% vs. 20.0%), and higher patient satisfaction (97.0% vs.75.3%). The treatment group showed significant superiority over the control group in all outcomes. Conclusion For patients with stage Ⅲ paronychia, thread-hanging combined with cotton tampon without nail groove reconstruction is advantageous as it avoids additional skin trauma, and does not affect the nail appearance and normal periungual barrier after healing, , reduces patient discomfort, and shortens the time off work, resulting in a higher cure rate. This treatment approach is therefore worth promoting in clinical practice.
