1.Constructing ICU scheduling mode by two-way selection and decentralized management

Objective To explore nursing scheduling mode in ICU. Methods Grouping was carried out on the basis of nursing decentralized management and the two-way selection between group heads and group members. The research target was the 60 nurses in ICU. Degrees of the nurses' job satisfaction and changes of nursing quality before and after grouping were compared and analyzed. Results The general degree of the nurses' satisfaction towards scheduling was (3.75±0.70) points before grouping while (4.30±0.50) points six months after grouping. The difference had the statistical significance (t=6.5645, P < 0.01);the scores of nursing quality check on classification management, ward management, nursing security, emergency management and hospital infection management were (91.97±1.47), (93.07± 1.66), (93.43 ± 1.50), (94.03 ± 1.10), (94.07 ± 1.39) points respectively before grouping and (95.08 ± 1.63), (95.77±1.55), (96.07±1.41), (97.37±1.56) and (97.07±1.48) points respectively six months after grouping. The differences had the statistical significance (t=6.5192-10.9934, P<0.01). Conclusions The two-way selection among nurses and decentralized management grouping can improve the degree of nurses′job satisfaction and nursing quality.

Objective To investigate the change of intestine mucosal permeability during cardiopulmonary bypass (CPB) and its effect on gut barrier in rats model. Methods The rats model of CPB was established and samples of tissue and plasma were collected at different intervals. Plasma D-lactate and LPS concentrations were determined and gut tissue was examined microscopically. Results The plasma concentration of D-lactate and LPS increased gradually at 1h CPB, and increased markedly 1h after CPB. They recovered to normal 8h after CPB, respectively. 4.Study on Main Active Components in the Extract of Dunaliella Salina

Objective To provide a basis for further exploration of Dunaliella Salina by determining the main active components in the extract of Dunaliella salina. Methods The carotenoids and vitamins were determined by HPLC or colorimetric method, amino acids were analyzed by amino acid analyze. Unsaturated fatty acid was determined by GC. Atomic absorption spectrophotometry was used to detect K, Na ,Ca, Mg, Cu, Zn, P, Fe, Mn and SPF was used to detect Se. Results There were affluent carotenoids, vitamins, amino acids, unsaturated fatty acid and trace elements in the extract of Dunaliella salina. Conclusion The extract of Dunaliella salina could be used as natural raw material to develop the supplement food.

6.Surgical treatment of congenital subaortic stenosis

Objective To review retrospectively the clinical experience in surgical treatment of congenital subaortic stenosis in 21 cases. Methods A total of 21 cases of congenital subaortic stenosis, including 12 cases of diaphragmatic type of stenosis and 9 cases of cast constriction, underwent intracardiac surgery from January 1999 to June 2003. Simple resection of stenosis membrane was performed in 12 cases, simple resection of stenosis membrane plus resection of myocardium of left ventricle outflow tract in 9 cases, simple resection of stenosis membrane plus resection of myocardium of left ventricle outflow tract plus treatment of united cardiac monstrosity in 16 case. Results No operative death and complications were found in all patients. Disappearance of cardiac murmur was found in 15 cases and diminishment of cardiac murmur in the rest of the patients after operation. A follow up time for 6 months to 2 years revealed that there were no obvious clinical symptoms, and the surgical outcomes were satisfactory. The cardiac ultrasound suggested that the left ventricular outflow tract was fluent without recurrent constriction. Conclusion Early treatment of congenital subaortic stenosis should be conducted when the disease was diagnosed. Better knowledge of pathological anatomy, proper operation, and prevention of complications are essential.
7.Therapeutic roles of parenteral and enteral nutrition in patients with disphagia complicating stroke

Objective To evaluate therapeutic roles of parenteral and enteral nutrition in patients with dysphagia complicating stroke. Method Seventy-two patients with dysphagia after stroke were randomly divided into group A (parenteral and enteral nutrition in sequence) and group B (parenteral nutrition) who were respectively given nutritional support 48 hours after the illness,measured hospital nutrition parameters,and observed the incidence rate of complications including tympanites,diarrhea and stress-induced ulcer,aspiration pneumonia.Death rate were compared between two groups at the end of the third week after admitted in hospital. Results Nutrition parameters in group A were higher than that in group B.There was significant difference (P0.05). Conclusion Parenteral and enteral nutrition in sequence for patients with dysphagia after stroke are safe and effective.
8.Change of gene rstn expression in brain tissues after traumatic brain injury

Objective To investigate the change of gene rstn expression in brain tissues following traumatic brain injury (TBI). Methods A total 90 SD rats were involved in the study and divided into normal control group (5 rats), sham operation group (10 rats), mild, moderate and severe trauma groups (25 rats per group). Rat model was made with sap pressure clash method and RT-PCR assay was employed to detect expression change of gene rstn at 3, 6, 24, 72 h and 1, 2, 4 weeks after TBI. The change of peripheral blood glucose concentration was measured in moderate trauma group to observe its relationship with gene rstn expression in brain tissue. Results Postoperative expression of gene rstn was increased in severe trauma group at 24 hours, in moderate trauma group at 72 hours and in mild trauma group at four weeks (P<0.05). The expression of gene rstn was increased in hippocampus, thalamus and cortex of all trauma groups at four weeks postoperatively, when the hippocampus showed the highest expression while the thalamus showed the least (P< 0.05). Moreover, the expression of gene rstn showed high level in injury side but low level in contralateral side in different districts (P < 0.05). The expression of gene rstn was increased the most obviously in severe trauma group (P <0.05). Peripheral blood glucose concentration showed a linearity positive correlation with gene rstn expression in brain tissue (R=5.32,P<0.05). Conclusions Expression of gene rstn shows obvious increase after TBI, and the time course correlates with the injury severity. The gene rstn expresses the most in the ipsilateral hippocampus. There shows a certain correlation between gene rstn expression and peripheral blood glucose concentration in brain tissues after TBI.
9.Clinical analysis on tumorectomy under extracorporeal circulation for 352 patients with primary cardiac tumors

Objective To summarise the clinical characteristics and surgical treatment experience of primary cardiac tumours treated by tumorectomy under extracorporeal circulation .Methods The clinical data of 352 patients with primary cardiac tumors in our hospital from January 1980 to December 2015,who received surgical treatment of tumorectomy under extracorporeal circulation were retrospectively ana -lyzed.Results Among the 352 cases in all, there were 338 cases (96%) of benign tumor, including 309 cases (87.8%) of myxoma and 14 cases (4.0%) of malignant tumor.There were 4 cases of perioperative death, and all the 4 cases were malignant tumor.Conclusion Myxoma is the most common primary cardiac tumor ,and the best solution for the cardiac benign tumor is surgery .However , the effect of sur-gical operation on malignant cardiac tumors was poor .

Objective To study the effects of polypeptide from Chlamys farreri(PCF) on PI3K/Akt and ASK1-JNK signal transduction pathway in thymocytes after ultraviolet B radiation.Methods Murine thymocytes were exposed to UVB radiation.The reactive oxygen species(ROS) level in thymocytes was detected by colormatching.The activation of Akt was investigated by western-blot.ASK1,JNK activation,mitochondria memberine potential and DNA ladder were also investigated after pretreatment with or without PI3K/Akt pathway inhibitor LY294002.Results PCF could activate Akt,inhibit the activation of the ASK1 apoptosis pathway in murine thymocyte radiated by UVB.Conclusion PCF decreased ROS level,increased Akt activity and inducing ASK1 degradation leading to the inhibition of ASK1-JNK induced apoptosis.
