2.Prognostic factors of 152 cases with Ⅲ/Ⅳ stage nasopharyngeal carcinoma

Purpose:To compare the survival rates of hyperf ractionated radiotherapy plus concurrent chemotherapy with hyperfractionated rad iotherapy alone in the treatment of stage Ⅲ/Ⅳ nasopharyngeal carcinoma (NPC) a nd analyze the prognostic factors. Methods:Between December 1992 and December 1995, 144 NPC patien ts were randomized into hyperfractionated radiotherapy plus concurrent chemother apy (R +C) and hyperfractionated radiotherapy alone (R alone).Radiotherapy were similar in the two groups: 1.2 Gy/f, twice a day. Chemotherapy was given to R + C patients before and during the course of radiotherapy. Results:The 5-year overall survival rates of the R + C and R alone groups were 63.3% and 50.7%,repectively . The factors influencing the resu lts were N stage,chemotherapy and the peripheral blood hemoglobin concentration . Conclusions:Hyperfractionated radiotherapy plus concurrent chem otherapy can improve the survival rate, especially for the patients with N2-N3 stage.The negative prognostic factors for Ⅲ/Ⅳ NPC are N2-N3 stage, radiothera py alone and the reduction of hemoglobin during treatment.
3.Surgical treatment of cervical ossification of posterior longitudinal ligament with high-signal intensity zone and analysis of the curative effect

[Objective]To observe and analyze the clinical results of ossification of posterior longitudinal ligament(OPLL) combined with high-signal intensity zone(HIZ) change. [Methods]Forty patients with OPLL combined with HIZ were treated from March 2004 to March 2008.Anterior approach surgery was given to 19 cases as group A and posterior approach surgery was given to 21 cases as group P.All cases were followed up.The pre-and post-operative JOA17 scores and improvement ratio were evaluated.[Results]The mean follow-up time were(19.5?3.4) months in group A and(20.3?4.5) months in group P.The mean improvement ratio were(56.2?8.4)% in group A and(52.2?12.2)% in group P.There were no statistical differences between those of groups A and P.Mean JOA scores at last follow-up were(12.8?0.9) in group A and(11.5?1.6) in group P,with significantly difference.[Conclusion]Treatment of OPLL combined with HIZ is a high risk procedure with limited recovery in neoro-function.Good surgical result could be got with correct judgement of type and location of OPLL,occupation ratio of spinal canal and selection of approach.
5.The artificial antimicrobial peptides having potential application to preventing oral infection

Objective To observe the antibacterial activity of artificial antimicrobial peptides (AMPs) to Blastomyces albicans, E. coli and Staphylococcus aureus in oral infection in vitro. Methods The AMPs were dissolved into PBS or sterilized saliva, then the inhibi-tion of different concentration AMPs to E. coil was evaluated by tube testing. The inhibition effect of different concentration of AMPs to Staph-ylococcus aureus and Blastomyces albicans were evaluated by antibiotic-susceptive test paper. Results When the concentration of AMPs added to broth of E. coli is from 150μmol/L to 300μmol/L , the bacterium count is 0 while the number in control is 6700 ~ 9200cfu/ml. At 100μg/tablet AMPs, the diameter of inhibitor zones of Staphylococcus aureus and Blastomyces albicans are 13mm and 11 mm. At 300μg/ tablet, the diameter are 16 mm and 17mm. AMPs dissolved in PBS and saliva have similar antimicrobial activity. Gonclusions The AMPs is effective to prevent Blastomyces albicans, E. coli and Staphylococcus aurcus in oral infection in vitro.
7.Effects of internal and external biliary drainage on inducible nitric oxide synthase mRNA expression of Kupffer cell and serum tumor necrosis factor-alpha in rats with obstructive jaundice

Objective To study the effects of internal and external biliary drainage on inducible nitric oxide synthase (iNOS) mRNA expression of Kupffer cells and serum tumor necrosis factor-alpha (TNF-α) in rats with obstructive jaundice. Methods Forty-eight male Sprague-Dawley rats were randomly assigned to obstructive jaundice (OJ), sham operation (SH), internal biliary drainage (ID) and external biliary drainage (ED) groups with 12 each. Kupffer cells were isolated by in situ hepatic perfusion and digestion with collagenase type Ⅳ, and purified by cell culture attachment. The expression of iNOS mRNA of Kupffer cells was detected by reverse transcription polymerase chain reaction (RT-PCR) and the serum TNF-α concentration was measured using ELISA method. Results The serum TNF-α level was increased in OJ group[ (110.84±26.3) pg/ml ]compared with SH group [-(88.4±17.9) pg/ml, (P=0.045)]. The TNF-α level in ID group[ (89.8±28. 3) pg/ml ]was significantly lower than that in ED group[ (118.64±22.7) pg/ml, (P=0. 011) ]and OJ group (P=0. 059). Expression of iNOS mRNA of Kupffer cells was significantly higher in OJ group (0. 824± 0. 24) compared with SH group (0. 384±0.35,P=0. 005). After relieving the OJ, the iNOS mRNA expression in ED group (0. 974± 0.48) was not suppressed (P=0. 321). On the contrary, the iNOS mRNA expression in ID group was suppressed and significantly lower than that in ED group (0. 59±0. 35) (P=0. 016). Conclusions Internal biliary drainage is superior to external drainage in terms of reversing the elevated serum TNF-α and in suppressing the iNOS mRNA expression of Kupffer cells in rats with obstructive jaundice.

9.Anti-infective immunity of recombinant Bifidobacteria bifidum-Eg95 vaccine of Echinococcus granulosus in mice

Objective To investigate the protective immunity in mice immunized with recombinantBifidobacteria bifidum(Bb)-Eg95 vaccine of Echinococcus granulosus (Eg) and challenged with Eg protoscoleces.Methods Fifty-six female BALB/c mice 12-14 weeks old and weighed 20-25 g were vaccinated with the recombinant Bb-Eg95 vaccine subcutaneously,intramuscularly,intranasally and orally,respectively,with blank vector,Bb and medium of solution(MRS) as control,8 mice in each group.Mice were challenged with Eg protoscoleces on week 8 after immunization and killed on week 25 after infection.The weight of hydatid cyst was measured and the decreased larva rate was calculated.Sera were collected to determine the levels of IgE,IgG and its subclasses by enzyme linked immunosorbent assay(ELISA).Splenocytes were collected and cultivated to test the proliferation of splenocytes using methyltetrazolium (MTT) assay under EgAg and concanavalin A (ConA) stimulation.The results were compared with analysis of variance and the comparison between two groups was performed with LSD-t test.Results There was significant difference in the weight of hydatid cyst between groups (F =11.062,P < 0.05).Compared with MRS control group[(0.075 ± 0.019)g],the hydatid cyst weight decreased in subcutaneous group [(0.050 ± 0.013)g],intramuscular group[(0.050 ± 0.019)g],intranasal group[(0.028 ± 0.016)g] and oral group [(0.031 ± 0.018)g,all P < 0.01).Compared with subcutaneous and intramuscular groups,the hydatid cyst weight decreased in intranasal and oral groups(all P < 0.05).The decreased larva rate was inversely proportional to the weight of hydatid cyst.There was significant difference in the levels(obsorbancy,A) of IgG,IgG2a,IgG2b,IgG1,IgG3 and IgE between these groups(F =21.774,36.977,27.071,14.746,10.131,9.444,P < 0.05 or P < 0.01).Compared with MRS control group (0.015 ± 0.002,0.002 ± 0.001,0.003 ± 0.001),the levels of IgG,IgG2a and IgG2b increased in subcutaneous group(0.022 ± 0.004,0.007 ± 0.002,0.008 ± 0.002),intramuscular group (0.023 ± 0.003,0.008 ± 0.002,0.007 ± 0.002),intranasal group(0.032 ± 0.007,0.012 ± 0.002,0.013 ± 0.004)and oral group(0.028 ± 0.006,0.010 ± 0.003,0.010 ± 0.002,P < 0.05 or P < 0.01).Compared with subcutaneous and intramuscular groups,the levels of IgG,IgG2a and IgG2b increased in intranasal and oral groups(P < 0.05 or P < 0.01).Compared with MRS control group(0.009 ± 0.001,0.009 ± 0.002,0.009 ± 0.001),the levels of IgG1,IgG3 and IgE decreased in subcutaneous group(0.022 ± 0.004,0.007 ± 0.002,0.008 0.002),intramuscular group(0.004 ± 0.001,0.004 ± 0.001,0.004 ± 0.002),intranasal group(0.005 ± 0.002,0.005 ± 0.003,0.005 ± 0.002)and oral group(0.005 ± 0.001,0.004 ± 0.002,0.004 ± 0.003,all P < 0.01).There was significant difference in the proliferation of splenocytes in the supernatant of cultured splenocyte,of cultured splenocyte + EgAg and of cultured splenocyte + ConA(F =63.975,359.833,167.399,P < 0.01).There was significant difference in the proliferation of splenocytes inside groups(F =6741.955,4953.667,869.320,201.235,175.413,139.653,169.994,all P <0.01).Compared with the cultured splenocyte the proliferation of splenocytes increased in the cultured splenocyte +EgAg and splenocyte + ConA (all P < 0.01).Compared with the cultured splenocyte + EgAg,the proliferation of splenocytes increased in the cultured splenocyte + ConA(P < 0.01).Conclusion An effective and protective immunity is induced by the recombinant Bb-Eg95 vaccine of Eg in mice.
10.Analysis of risk factors for cerebral infarction resulting from transient ischemic attack

Objective To evaluate the risk factors for cerebral infarction (CI) resulting from transient ischemic attack (TIA).Methods The clinical data of 133 patients with TIA were evaluated.The condition of CI happening was recorded.Seizure frequency,seizure duration,onset to treatment time,the number of seizure before treatment time,blood pressure,fasting blood glucose,total cholesterol,triglyceride and other factors were collected.The clinical data of TIA with CI and without CI were evaluated.Results Among the 133 patients,there were 48 patients suffering from CI (36.09%).Among 108 patients with TIA in the anterior circulation system,there were 39 patients (36.11%) suffering from CI.Among 20 patients with TIA in the posterior circulation system,there were 7 patients (35.00%) suffering from CI.There was no significant difference in the rate of CI between TIA in the anterior circulation system and TIA in the posterior circulation system (P > 0.05).Among 5 patients with TIA in both the anterior and posterior circulation systems,there were 2 patients suffering from CI.In patients of TIA with seizure frequency > 3 times/d,seizure duration > 30 min,onset to treatment time > 24 h or the number of seizure before treatment >5 times,the incidence of CI was significantly higher,and the difference was statistically significant (P < 0.05 or <0.01).The onset to treatment time,systolic pressure,diastolic pressure,fasting blood glucose,total cholesterol and triglyceride levels in TIA with CI were significantly higher than those in TIA without CI (P<0.05).Conclusion There is highly danger for patients to develop CI if patients with TIA has longer seizure duration,much more seizure frequency,longer onset to treatment time,especially those accompany with high blood pressure,hyperglycaemia and hyperlipoidemia.
