2.Diagnostic value of computer-aided diagnosis with three-dimensional volumetric measurement in solid pulmonary nodules on follow-up CT

Objective To evaluate the diagnostic value of computer-aided diagnosis with three-dimensional volumetric measurement in solid pulmonary nodules on follow-up CT. Methods Forty-six patients with 58 solid nodules were evaluated.Each patient underwent chest CT at least twice.The volume, doubling times(DTs) and volumetric growth rates(VGRs) of nodules were calculated with three-dimensional volumetric measurement software. If there were significant statistical differences in DTs between malignant and benign nodules, Nonparametric test was used to make the analysis.If there were significant statistical differences in upper DTs'values between malignant and benign nodules, X2 test was used to make the analysis.Final diagnosis of all nodule were confirmed by pathology or by response to diagnostic therapy or by clinical follow-up.Results The DTs of 13 malignant nodules were <400 days in 12 nodules, with a range from 45 to 465 days (median, 159 days) ; the VGRs ranged from 8% to 329% (median, 30.0%).The VGRs of 45 benign nodules ranged from -38% to 24% (median, 0%).The DTs of 24 benign nodules that increased or not changed in size were >400 days in 22 nodules, with a range from 302 days to 55 158 days (median, 2 704 days); the VGRs ranged from 0 to 24% (median, 6.5%. Twenty-one nodules that decreased in size had the VGRs ranging from -38% to -4% (median, - 16.0%), and the DTs of these nodules could not be calculated by software.There were significant statistical differences in DTs between malignant and benign nodules(P <0.05).Taking 500 days, 400 days and 300 days as the upper value for malignancies there were statistically significant differences in these values between malignant and benign nodules(P < 0.01).Their sensitivities were 100.0%, 92.3%, 76.9% respectively and specificities were 87.5%, 91.7%, 100.0% respectively.Accuracies were 91.9%, 94.6% ,91.9% respectively and positive predictive values were 8,11,0.Conclusions Taking 400 days as the upper DTs'value for malignancies has important diagnostic value. Computer-aided diagnosis with three-dimensional volumetric measurement is sensitive to the volume changes of solid pulmonary nodules on follow-up CT, and is useful in the differential diagnosis of pulmonary nodules.
3.Role of HO-1 in acute rejection after orthotopic liver transplantation in rats

Objective To investigate the effect of HO-1 on acute rejection after orthotopic liver transplantation (OLT) in rats. Methods Taking dark agouti (DA) rats as donors and Lewis rats as recipients, OLT was performed. Before OLT, the DA rats and Lewis rats were matched in pairs, and divided into control group, ZnPP group and CoPP group, the paired rats of which intraperitoneally received normal saline, 20 mg/kg ZnPP and 5 mg/kg CoPP respectively 24 h before OLT. Ten pairs of rats were in each group. On day 3 or 7 after OLT, recipient rats were killed, then blood sample and liver specimen were collected. Plasm levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bilirubin (TBIL), albumin (ALB) were measured; HE staining and MPO staining were performed; The expression of HO-1 was detected by the fluorence real time RT-PCR. Results On day 3 and 7 after OLT, AST, ALT and TBIL showed significant statistical significance among the three groups; ALB was highest in CoPP group, then in control group, lowest in ZnPP group. On day 3 after OLT, control group had acute rejection of degree one to degree two, ZnPP group had acute rejection of degree two; CoPP group mainly had acute rejection of degree one; On day 7 after OLT, control group mainly had acute rejection of degree two; ZnPP group had acute rejection of degree two to degree three; CoPP group had acute rejection of degree one to two. The expression of HO-1 was highest in CoPP group, then control group, and lowest in ZnPP group. Conclusion The expression of HO-1 in OLT with acute rejection in rats can palliate the acute rejection.
4.The clinical recent effects of two neoadjuvant chemotherapies in the treatment of advanced gastric cancer

Objective To observe the recent effects and toxicity in advanced gastric carcinoma by two kinds of neoadjuvant chemotherapies ( PF and FOLFOX).Methods A total of 60 patients with advanced gastric cancer ( Ⅲ ~ Ⅳ ),they were randomly divided into 2 groups; PF group ( 30 cases) and FOLFOX group ( 30 cases).All patients were given 3 cycles of docetaxel or L-OHP chemotherapy preoperatively,to observe the recent effects and toxicity in advanced gastric carcinoma.Results The total effective rate in PF group was 77.67%,while it was 50.00% in FOLFOX group,it had significant difference between in two groups ( x2 =4.59,P < 0.05 ).There is no difference in the incidence of neutropenia,transaminase elevation,nausea and diarrhea two groups.The incidence of hair loss in PF group is higher than in FOLFOX group,but there is no difference between two groups; The incidence of peripheral nerve paresthesia in FOLFOX group is higher than that in PF group( x2 =5.08,P <0.05).Conclusion PF as neoadjuvant chemotherapy is a very effective and well-tolerated regimen in patients with advanced gastric cancer.
5.Analysis of the clinical effect of whole boby γ-knife in treatment of non-small cell lung cancer:a report of 42cases

ObjectiveTo evaluate the effect and complication of whole γ-knife stereotactic radiotherapy on the non-small cell lung cancer and explore the value of γ-knife stereotactic radiotherapy in treatment of non - small cell lung cancer. Methods42 patients with non -small cell lung cancer:γ-knife radiotherapy patients ,4 ~ 8Gy once a day,five times a week,4 ~ 10times and 40 ~50Gy in total. ResultsThe overall response rate (CR + PR) in γ-knife was 71.4%. The1-,2-and 3-year survival rates were 95.5%, 59.5%, 33.3%. ConclusionCompared with conventional radiation,γ-knife stereotactic radiotherapy could achieve tumor dose escalation,and give the normal tissue better protection at the same time. It could improve the tumor control rate of non-small cell lung cancer,improve long survival rate,and so could reduce the radiation reaction.
6.Surgical revascularization of patients with chronic total coronary occlusion combined with diffuse distal atherosclerosis

Objective To evaluate the effect of off-pump coronary endarterectomy (CE) plus off-pump coronary artery bypass grafting (off-pump CABG) on patients with chronic total occlusion (CTO) combined with diffuse distal atherosclerosis. Methods From October 2006 to August 2009,65 CTO patients with 176 angiographically confirmed vascular stenosis or occlusive lesions, 70 of which were complete occlusion, underwent off-pump CABG. During the operation, diffuse intimal thickening distal to occlusive lesion was found, and blood flow of the bridges was unfavorable.Results Therefore endarterectomy was performed, followed by CABG. The blood flow in the bridges were 2-10 ml/min versus 14-37 ml/min before versus after endarterectomy. Pulsatility index (PI) was 5.1-15.6 versus less than 5 before versus after endarterectomy. Left ventricular ejection fraction was also improved significantly [before operation: (0.47±0.12)%, after operation: (0. 52±0.15)%, t=2.17, P<0.05]. Peri-operative myocardial infarction occurred in 2 cases, but without significant cardiac homodynamic changes. And 23 patients underwent coronary angiography to evaluate graft patency 3-18 months after operation, all of them had favorable blood flow. Conclusions It is feasible to perform off-pump CABG plus coronary endarterectomy for patients of chronic coronary total occlusion combined with diffuse distal atherosclerosis. This treatment is safe and effective.
7.Multi-level identification and analysis about infrared spectroscopy of lophatheri herba.

Based on the infrared spectra of Lophatheri Herba and Commelinae Herba, one-dimensional infrared spectra, second derivative spectra and two-dimensional correlated spectra were used to find out the differences between Lophatheri Herba and its imitations, respectively. The common peak ratio and variant peak ratio dual-indexes sequential were calculated and established according to infrared spectra of eleven batches of herbs. Infrared spectral data of Lophatheri Herba cluster analysis was applied to explore the similarity between each sample. The grouping results trend of sequential analysis of dual-indexes and cluster analysis was accordant. The results showed that the differences could be found by multi-level identification, and the source and the quality of the herbs could be effectively distinguished by the two analysis methods. Infrared spectroscopy, used in the present work exhibited some advantages on quick procedures, less sample required, and reliable results, which could provide a new method for the identification of traditional Chinese medicine with the imitations and adulterants, and the control of quality and origin.

9.Effect of gamma knife surgery on treatment of craniopharyngiomas

Objective To evaluate the adaption,effectiveness,dose,complication of the treatment for cranio pharygiomas with gamma knife surgery.Methods Gamma knife surgery was performed in 41 patients.Patients with mixed solid and cystic tumors were treated with stereotaxic aspiration while six cases were treated with divergence surgery prior to gamma knife therapy.The central dose ranges from 17.1 ~40 Gy(29.6Gy in average).The patients were treated at 30%~50% equal dose curve with 6 ~14Gy of tumormargin dose(9.5Gy in average).The exposure dose of the optic nerve and optic tract is less than 10Gy.Results Of twenty-nine patients who were followed up from 6 to 100 months,sixteen had disappeared or decreased tumor,six had unchanged,two was performed craniotomy one year or three years after gamma knife surgery,and the remaining five were dead one year to three years.The tumor control rate was 75.9%(22/29).Conclusion The treatment of stereotaxic radiation with single and high dose is sensitive to most of the solid craniopharyngioma,and the treatment of stereotaxic resection combined with gamma knife surgery may be feasible for the recurrent mixed solid and cystic craniopharyngioma.
10.Influence of radiofrequency ablation on dendritic cells in rats with liver carcinoma

Objective To investigate the change of dendritic cells (DCs) in rats with hepatic carcinoma treated by radiofrequency ablation (RFA),and to explore the mechanisms of anti-tumor immune response to RFA. Methods Forty healthy SD rats with established animal model of hepatic carcinoma were randomly divided into control group (n = 10) ,RFA 7 d group (n = 16) and RFA 14 d group (n = 14). The rats of control group were killed without treatment. The other rats were killed in 7 d and 14 d after RFA treatment respectively. Peripheral blood, liver tissue around the ablation area and spleen were taken out. The OX62,OX6,CD86 of DCs were analyzed with flowcytometry. Results ①OX62 cells accounted for (0.45 ± 0.19)% of mononuelear cells in peripheral blood in control group. The account of OX62 cells increased to (0.78 ± 0.30)% and (1.53 % 0.80)% in RFA 7 d and 14 d groups respectively. There were significant differences between control and RFA 7 d group, control and RFA 14 d group (P<0.05). ②OX62 cells accounted for (18.91 ± 4.58)% of mononuclear cells in liver tissue around the tumor in control group. The account of OX62 cells increased to (24.49 ± 4.59)% in RFA 7 d group (P<0.05). The expression of OX6 on DCs in liver tissue was (15.29 ± 4.59)% and increased to (34.2 ± 11.62)% and (39.18 ± 9.14)% in RFA 7 d and RFA 14 d group respectively (P<0.05). ③OX62 cells accounted for (11.69 ± 4.39)% of mononuclear cells in spleen in control group which increased to (15.10±2.37)% in RFA 14 d group (P<0.05). Conclusions The precursor DCs in peripheral blood and DCs in liver and spleen increased significantly after RFA. The expressions of OX6 on DCs in liver and spleen increased after RFA. RFA can promote the differentiation and maturation of DC. The increased function of antigen presenting may contribute to the anti-tumor responses after RFA.
