4.Observation on efficacy of dexmedetomidine combined with butorphanol in preventing brachial plexus block tourniquet reaction

Objective To observe the clinical effect of dexmedetomidine combined with butorphanol in preventing tourniquet reaction.Methods Fifty patients(ASA Ⅰ-Ⅱ)with upper limb operation and satisfactory effect of brachial plexus block were randomly divided into the dexmedetomidine group(A)and dexmedetomidine combined with butorphanol group (B),25 cases in each group.The anesthesia drug preparation method in the group A:dexmedetomidine 200μg was added with normal saline 50 mL;in the group B:dexmedetomidine 200 μg,butorphanol 1 mg and normal saline 50 mL altogether.The group A used dexmedetomidine 1 μg/kg load for 15 min and maintained by 0.4 μg · kg-1 · h-1 until the end of operation;the group B was given 0.015 mg/kg butorphanol,then was given medication according the scheme of group A.The VAS score,BIS value,Remsay sedation score,HR,MAP and SPO2 were recorded at the time points:before medication (T0),immediately after the tourniquet inflation (T1),15 min (T2),30 min (T3),45 min (T4),60 min after tourniquet inflation (T5) and 10 min after deflation (T6).The subjective response classification count,respiratory depression,bradycardia and hypotension in the two groups were assessed.pH,PaO2,PaCO2,HCO3-and LAC at T6 were evaluated by theblood gas analysis.Results The VAS score,BIS value,Ramsay sedation score,HR and MAP at T4,T5 had statistical difference between the two groups (P<0.05).In the group A,the good and poor subjective responses had 9 cases and 4 cases respectively,which in the group B had 13 cases and 1 case,the difference was statistically significant (P<0.05);there was no respiratory depression,bradycardia and hypotension in the two groups;the blood gas analysis showed no statistical difference between the two groups.Conclusion Applying dexmedetomidine combined with butorphanol in upper and lower limb surgery under brachial plexus anesthesia can significantly prevent the tourniquet reaction,its effect is superior to single use of dexmedetomidine.
5.Acute hypervolemic hemodilution for reducing perioperative blood use

Objective To study the feasibility of preoperative acute hypervolemic hemodilution for decreasing perioperative blood use. Methods Thirty two patients(ASAI-II) undergoing spinal surgery were randomized into AHH group and control group. After anesthesia 6% HES(200/0.5) was given through internal jugular vein for AHH. The blood volume was increased by about 25%. During the operation, blood loss was replaced by HES in equal volume. Blood was transfused when the Hct dropped below 25%. The HR, MAP, CVP, SPO2, Hb, Hct., Plt, FIB, APTT, PT, amount of blood loss, blood transfused and urine output, volume of drainage 24h after operation and VAS score (0 means no effect, 10 means maximal effect) were observed before AHH, at the end of AHH, at the end of operation, and 24h after the operation. Objective To investigate the influence of Levetiracetam(LEV) and Topiramate(TPM) on P-glycoprotein(P-gp) expression at brain in the epileptic rats.Methods The epileptic rat models were made with 1.5 ?g(3 ?l) Kainic acid stereotaxic injected into male SD rats' hippocampi(epilepsy group) and the control group rats were injected with 3 ?l normal saline(NS).The rats in epilepsy group(18 rats) and control group(15 rats) were randomized into TPM,LEV and NS subgroups(6 or 5 rats for each subgroup) to receive once-daily orally feeding of TPM(50 mg/kg),LEV(40 mg/kg) and NS(same amount NS) for 30 d.Immunohistochemistry(EnVision) was used to measure the level of P-gp exprssion in the rats brain.The level of P-gp expression was semi-quantitatively analyzed by mean integrated blue(MIB) value in Leica Qwin imaging analysis & measuring system.Results The expression levels(MIB value) of P-gp in hippocampus and temporal lobe in epilepsy subgroups were significantly higher than those in the corresponding control subgroups(P

Trans-cinnamaldehyde, the main component of volatile oil from cassia twig or Cinnamomum cassia, which is a traditional Chinese herbal medicine. Trans-cinnamaldehyde is a kind olefine aldehyde of organic compounds and has many pharmacological properties, such as anti-inflammatory, anti-tumor, anti-bacterial, antidiabetic, anti-obesity, and neuroprotection etc. The compound has preventive and therapeutic effects on the nervous system, cardiovascular, cancer, diabetes and other diseases. Trans-cinnamaldehyde, as a preventive care of nature medicine, has great clinical and market potential. This paper gives a review about the pharmacological effects and mechanism of trans-cinnamaldehyde researched in the latest five years. We hope to provide some basic information for further research on trans-cinnamaldehyde.

9.Consistency study of PowerPlex 21 kit and Goldeneye 20A kit and forensic application.

To ensure the consistency of genotype results for PowerPlex 21 kit and Goldeneye 20A kit.
METHODS:
The STR loci were amplified in DNA samples from 205 unrelated individuals in Beijing Han population. And consistency of 19 overlap STR loci typing were observed. The genetic polymorphism of D1S1656 locus was obtained.
RESULTS:
All 19 overlap loci typing showed consistent. The proportion of peak height of heterozygous loci in two kits showed no statistical difference (P > 0.05). The observed heterozygosis of D1S1656 was 0.878. The discrimination power was 0.949. The excluding probability of paternity of triplet was 0.751. The excluding probability of paternity of diploid was 0.506. The polymorphism information content was 0.810.
CONCLUSION
PowerPlex 21 kit and Goldeneye 20A kit present a good consistency. The primer design is reasonable. The polymorphism of D1S1656 is good. The two kits can be used for human genetic analysis, paternity test, and individual identification in forensic practice.

10.The up-regulation of p-p38 MAPK during the induction of brain ischemic tolerance induced by intermittent hypobaric hypoxia preconditioning in rats.

OBJECTIVETo explore the expression of p-p38 MAPK protein and the number of astrocytes expressing p-p38 MAPK in CA1 hippocampus in rats during the induction of brain ischemic tolerance induced by intermittent hypobaric hypoxia (IH) preconditioning.
METHODSThirty healthy adult male Wistar rats were randomly divided into 6 groups (n = 5 in each group): sham 0 min group, IH + sham 0 min group, sham 7 d group, IH + sham 7 d group, Ischemia (Is) 7 d group, and IH + Is 7 d group. Neuropathological evaluation was performed by thionine staining in CA1 hippocampus in rats. The expression of p-p38 MAPK in CA1 hippocampus was observed by immunohistochemical staining. And the number of astrocytes expressing p-p38 MAPK was observed by immunofluorescent double labeling.
RESULTSThe results showed that IH preconditioning induced brain ischemic tolerance successfully. At the same time, IH preconditioning obviously up-regulated the expression of p-p38 MAPK protein in CA1 hippocampus, and also increased the number of astrocytes expressing p-p38 MAPK.
CONCLUSIONIt might be concluded that IH preconditioning induced brain ischemic tolerance by up-regulating the expression of p-p38 MAPK protein in pyramidal neurones and astrocytes.
