1.The study on core competency of cardiovascular specialist nurses in military hospitals

Objective To learn the status of core competency of cardiovascular specialist nurses in military hospitals in order to supply evidence to establish contrapuntal training protocol.MethodsThe Competency Inventory for Registered Nurse Scale (by LIU Ming) was distributed to investigate the core competency of 156 registered nurses.The data underwent analysis.Results The overall score of core competency level was (3.17±0.69),the highest mean score was the dimension of legal and ethical practice,and the lowest mean score was the dimension of critical thinking and nursing research apititude.the core competency varied with professional title and staffing.Evident difference existed in different dimensions and total scores.Conclusions The overall core competency is at the upper middle level for cardiovascular specialist nurses.They are competent enough to provide healthcare.The cultivation of nurses' critical thinking and nursing research ability should be strengthened.The nursing administrators should carry out pertinent training for nurses according to their different professional levels.

3.Prognostic value of post-mastectomy radiation therapy in node-positive breast cancer patients treated with breast conservation therapy

Objective:To evaluate the prognostic value of post-mastectomy radiation therapy (PMRT) in patients with axillary lymph node-positive breast cancer treated with breast conservation surgery and to establish the candidates for PMRT based on different pN stages and lymph node ratios (LNR). Methods:A retrospective analysis of the clinical data of the patients was conducted. The pa-tients had positive lymph nodes (n=152) between 1998 and 2007 and underwent breast conservation surgery. A comparison of the dis-ease-free survival (DFS) and overall survival (OS) rates was conducted based on LNR and pN staging and with PMRT as a prognostic factor. Results:A total of 152 cases were studied, of which 114 were pN1, 23 were pN2, and 15 were pN3. Among these cases, 114 had an LNR ranging from 0.01 to 0.20, 26 had an LNR from 0.21 to 0.65, and 12 had an LNR>0.65. Univariate analysis showed that the number of dissected lymph nodes, LNR, pN stage, estrogen and progesterone receptor status, and radiotherapy were the prognostic fac-tors for DFS and OS rates (P<0.05). Age and chemotherapy were prognostic factors only for OS rate (P<0.05). Multivariate analysis in-dicated that PMRT and LNR were independent prognostic factors of DFS and OS (P<0.05). The pN staging had no significant effect on DFS or OS (P>0.05). In the subgroup analysis, PMRT had significant effect on DFS and OS (P<0.05) in patients with pN1 and LNR<0.21. Conclusion:LNR is an independent prognostic factor in axillary lymph node-positive breast cancer patients treated with breast conservation surgery, and a candidate for PMRT should be established based on different LNR risks.

5.The correlation of plasma homocysteine and intima-media thickness with incipient cerebral infarction

Objective To explore the relationship between the level of plasma homocysteine(HCY)and intima-media thickness(IMT)with incipient cerebral infarction.Methods According to the clinical diagnostic criteria,112 patients with incipient cerebral infarction were randomly selected as the cerebral infarction group,and 102 healthy people were randomly selected as the control group.All carotid arteries of both groups were checked by carotid artery ultrasound and IMT was tested.The levels of plasma HCY and plasma folate as well as vitamin B12 were measured in both groups by chemiluminescence immunoassay.Results The plasma HCY level in the cerebral infarction group [(24.89 ± 1.96) μmol/L] was significantly higher than (8.58 ± 2.34) μmol/L in the control group (P < 0.01).The levels of plasma folate [(4.79 ± 2.35) μg/L] and vitamin B12 [(228.67 ± 114.75) ng/L] in the cerebral infarction group were significantly lower than [(8.65 ± 2.64) μg/L and (320.53 ± 154.78) ng/L] of the control group (P < 0.01).The IMT of the cerebral infarction group [respectively the left common carotid artery fork (1.12 ± 0.25) mm,the right common carotid artery fork(1.09 ± 0.28) mm,the left internal carotid artery(1.18 ± 0.23) mm,the right internal carotid artery(1.03 ± 0.24)mm] were obviously higher than those of the control group[respectively the left common carotid artery fork (0.65 ± 0.18) mm,the right common carotid artery fork (0.61 ± 0.24) mm,the left internal carotid artery(0.58 ±0.22)mm,the right internal carotid artery(0.61 ±0.19)mm] (P <0.05).Significant positive correlation between IMT and HCY was found in patients with cerebral infarction(r =0.834,P < 0.01),but no corre lation was found in the control group(r =0.081,P > 0.05).Conclusion The increase of plasma HCY and the thickening of IMT are closely related with incipient cerebral infarction.Examination of plasma HCY and IMT has important clinical significance for the secondary prevention of incipient cerebral infarction.
6.Value of transrectal ultrasonography in the diagnosis of female traumatic urethral stricture

Objective To investigate the diagnostic value of transrectal ultrasonography (TRUS)for female traumatic urethral stricture.Methods Fifteen female patients with traumatic urethral stricture were examined by using TRUS and urethrography and the examination results were compared with op-eration findings.Results The ultrasonography results of all patients were in consistency with their operation findings,while the urethrography results of six patients were different from their operation findings.Conclusion TRUS is of great value in the diagnosis of female traumatic urethral stricture.

Objective To examine the expression of Toll-like receptor(TLR)4 protein and the transcription of TLR4 mRNA in the peripheral blood mononuclear cells(PBMC)from patients with hepatitis B virus(HBV)infection and explore the relationship between TLR4 and chronic HBV infec- tion.Methods The expression level and transcription level of TLR4 were determined by flow cytometre and reverse transcriptase polymerase chain reaction respectively in PBMC from 37 chronic hepatitis patients,28 liver cirrhosis patients,31 severe hepatitis patients and 27 healthy controls. 10.Microsurgical treatment of tumors in cervical spinal canal

Objective To report effects of microsurgical treatment for tumors in cervical spinal canal. Methods A total of 32 cases of tumors in cervical spinal canal had received microneurosurgery through a posternmedial approach. 22 cases had received reconstruction of cervical spinal canal stability. Results A total tumor resection was performed in 26 cases (81.25%),a subtotal tumor resection was conducted in 6 cases(18.75%). A cemplete recovery was achieved in 23 cases(71.88%), an improvement of symptoms was achieved in 6 cases(18.75%), and no improvement in 3 cases(9.38%), no death was encountered. Follow up observations were carded out in 30 cases for 3 months to 3 years. MRI examinatious 3 months after operation in 26 cases found no residual or recurrent tumor,recurrence was seen in 3cases of astrocytoma, 1 case was invariable. X-ray radiography in 26 cases showed good vertebral stability ,spinal deformation was found in 4 eases. Conclusion Microsurgery in early period play a key role in treatment of cervical spinal tumors. Reconstruction of spinal canal stability is important to patients.
