1.CT perfusion imaging in quantitative monitoring for tumour microvascular responses to anti-angiogenesis therapy in rabbits

Objective To investigate the value of CT perfusion imaging in quantitative monitoring on microcirculation hemodynamics,differences between tumor and normal tissues,differences between mass center and edge,and on tumour microvascular responses to anti-angiogenesis therapy. Methods Fifteen male New Zealand white rabbits were established to VX2 tumor-bearing model. The animals were divided into 2 groups,anti-angiogenesis therapy (n=11,TNP-470,20 to 30 mg/kg),and control (n=4). DEC-CT scanning (80 kV,120 mA,20 cm VOF) was performed at 1 week after tumor implantation,1 week after anti-angiogenesis therapy,and 2 week after anti-angiogenesis therapy. Perfusion parameters,including blood flow (BF),blood volume (BV),mean transit time (MTT),and permeability surface (PS) were calculated for tumor mass,tumor edge,tumor center and normal tissue. Pediatric advanced life support (PALS) training program development has been more than 20 years.PALS program provides a systematic,organized approach for the evaluation and management of acutely ill or injured children.It plays an active role in guiding treatment of critically ill children.In this paper,we reviewed the PALS training purpose,importance and training content,and introduced the current situation and promotion of PALS training in China.
5.PARNEM-A PARALLEL DISCRETE EVENT NETWORK EMULATION SYSTEM

Objective Network emulation system constructs a virtual network environment which has the characteristics of controllable and repeatable network conditions. This makes it possible to predict the availability and performance of new protocols and algorithms before deploying to Internet. Methods PARNEM, a parallel discrete event network emulation system described in this paper has the following characteristics: ① BREEN - a BSP based real-time event scheduling engine; ② application transparent flexible interactive mechanism; ③ legacy network model reuse. Conclusion PARNEM allows detailed and accurate study of application behavior. Comprehensive case studies covering bottleneck bandwidth measurement and distributed cooperative web caching system demonstrate that network emulation technology opens a wide range of new opportunities for examining the behavior of applications.
6.Clinical Isolate of Escherichia coli Producing Extended-spectrum AmpC ?-Lactamases(ESACs) with ClassⅠ Integron

OBJECTIVE To detect the genes of plasmid-mediated AmpC and ESBL ?-lactamases and classⅠ integron in clinical isolate of Escherichia coli.METHODS The bacterial susceptibility to antimicrobial agents for strain E50 was determined by agar dilution method.The genes of AmpC,ESBLs,and intⅠ1 were analyzed using PCR and verified DNA sequencing.Conjugation experiment was used to study the transfer of drug resistance.RESULTS The strain was only susceptible to imipenem.CTX-M-15,SHV-1,TEM-1,DHA-1 and intⅠ 1 genes were positive.CONCLUSIONS It has complicated mechanism of drug resistance.Some of clinical strains producing plasmid-mediated AmpC ?-lactamase are accompanied by extensively producing ESBLs.Carbapenems are the best choice for treatment infection caused by strains producing ESACs.More attention should be paid to integron which plays an important role in multi-resistance.
7.Combined treatment of rosiglitazone and metformin for blood glucose, blood lipid, blood pressure and body mass of type 2 diabetic patients complicated by blood lipid disturbance

BACKGROUND: Insulin resistance (IR) is associated with type 2 diabetes mellitus and its macrovascular complications. Euglycemic agent (rosiglitazone and metformin) can ameliorate IR, but its influences on other cardiovascular risk factors, including blood lipid, blood pressure and body mass need to be further studied.OBJECTIVE: To observe the effects of rosiglitazone and metformin on the blood glucose, blood lipid, blood pressure and body mass of patients with type 2 diabetes mellitus complicated by blood lipid disturbance.DESIGN: A clinical observation.SETTING: Department of Endocrinology, Nanjing First Hospital, Nanjing Medical University.PARTICIPANTS: Forty-two patients with type 2 diabetes mellitus complicated by dyslipideamia, who received treatment in the Department of Endocrinology, Nanjing First Hospital, Nanjing Medical University between June and September 2006 were recruited in this study. The involved patients, 27 male and 15 female, averaged (58±2)years ranging from 30 to 70 years old. Inclusive criteria:①Type 2 diabetes mellitus all met the diagnostic criteria of American Diabetes Association (2006).② Diagnostic criteria of blood lipid disturbance : total cholesterol(TC)> 4.68 mmol/L, triglyceride (TG) > 1.7 mmol/L, low density lipoprotein cholesterol (LDL-C) > 2.68 mmol/L, high density lipoprotein cholesterol (HDL-C) < 0.9 mmol/L (female), < 1.0 mmol/L (male). Informed consents were obtained from all the patients.METHODS:①Drug treatment:Dudng the 12-week treatment,all the patients received 0.75 g metformin(Lot No.051110 and 051214,0.25 g,lablel,Jiangsu Hengrui Medicine Co.,Ltd)and 4 mg rosiglitazone (Lot No.0511101,2 mg/tablet,Jiangsu Hengrui Medicine Co.,Ltd) daily.②Physical and laboratory measurements:Body mass index,blood pressure were taken,and blood lipid profile(TC,TG,C-LDL and C-HDL),fasting and postprandial blood glucose were measured by colodmetdc method before,8 and 12 weeks after treatment.MAIN OUTCOME MEASURES:①Fasting and postprandial blood glucose level change.②Blood lipid,blood pressure and body mass index change.RESULTS:Forty-two patients with type 2 diabetes mellitus complicated by dyslipideamia were recruited,and three dropped out the experiment,one patient suffered from severe watery stool,one patient from severe abdominal distention,one patient from urinary tract infection, and the other thirty-nine patients completed the observation in the final analysis.Compared with pretraatment data,fasting and postprandial blood glucose levels 12 weeks after treatment were decreased(P<0.01),TC increased(P<0.05),LDL-C decreased(P<0.01),HDL-C increased(P<0.01),blood pressure did not change(P>0.05) and body mass index increased(P<0.01).CONCLUSION:Combinatin of rosiglitazone and metformin significantly decreases blood glucose,decreases LDL-C,increases TC,HDL-C,body mass index,and has no obvious effects on TG and blood pressure.It is safe and bearable for type 2 diabetes mellitus patients with dyslipideamia.

9.Significance of standardized treatment for cancer pain

Objective:To discuss the significance of standardized treatment for cancer pain, according to the Cancer Pain Treat-ment Specification (2011 Edition) issued by the Ministry of Health, PR China. Methods:Clinical data of 126 patients with cancer pain, who were admitted to the Department of Medical Oncology, The Fourth Hospital of Hebei Medical University, were collected to com-pare the improvement of the Numeric Rating Scale (NRS) score, number of breakthrough pain, and quality of life score after treatment. The relationships between different pain grades, disease entities, and treatment effect were analyzed. The influence factors of pain relief were also analyzed by using Logistic regression analysis. Results:1). Following standardized treatment, the improvement rate of NRS score has shown significant differences in pain grading (P=0.001) and gender (P=0.000). However, no significant differences were ob-served between different diseases (P=0.112). The improvement rate of the life quality score and the number of breakthrough pain had no significant difference after standardized treatment. 2). The grading of cancer pain and the disease entities had little effect on cancer pain relief. 3). The anti-tumor treatment and“no distant metastasis”were the independent factors that brought about the decrease in NRS and quality of life scores.“No distant metastasis”(P=0.046) was the independent factor that reduced the number of pain out-breaks. Conclusion: The standardized treatment positively affects the NRS score, number of breakthrough pain, and quality of life score. Patients who received anti-neoplastic therapy and who had no distant metastasis showed significant treatment effectiveness in pain management.
10.Affection of Zidan decoction containing serum on endometrial receptivity in uterine immunity environment

Objective To investigate the effect of Zidan decoction containing serum on endometrial receptivity in uterine immunity environment,and then to discuss if it's effect on endometrial receptivity through its controlling the expression of cell factors which related to endometrial receptivity.Methods Mice's endometrium tissue samples and uterine Natural Killer (UNK) cells were obtained from pregnant mice on gd 4.5 days and gd 12.5 days.After endometrial glandular epithelial cells,endometrial stromal cells and UNK cells were separated through primary culture,epithelial cells and endometrial stromal cells at rate of 1 ∶ 1 a certain concentration UNK were mixed together and highly purified-cultured.Different concentrations of Zidan decoction was acted on the culture solution.FBS,black and aspirin control group were setup for contrast.The expression of LIF and VEGF which influenced by Zidan decoction in the immunity environment was assayed by Realtime-PCR.Results The expression of LIF and VEGF was significantly increased in the immunity environment of uterine (in 10% Zidan decoction group,10% aspirin group and 10% blank control group:LIF was 2.10±0.20,1.98±0.14,0.90±0.05 separately,and VEGF was 3.17±0.31,2.52±0.09,0.92±0.06respectively,P<0.05).Conclusion Zidan decoction could improve endometrial receptivity by increasing the expression of cell factor which related to endometrial receptivity with the help ofUNK paracrine.
