1.Clinical characteristics of cerebral blood perfusion and metabolism of galactoside in type 2 diabetic patients

Objective To investigate clinical characteristics of cerebral blood perfusion in patients with T2DM and T2DM accompanied with hypertension.A part of the patients were studied on the metabolism of cerebral galactoside. Methods 99mTc-ECD was injected into 77 patients diagnosed as T2DM,and SPECT imaging of the cerebral cortex was done. And also 18 F-FDG imaging of the cerebral cortex was done in twelve of them. Results In T2DM patient the radioactivity was decreased in the temporal lobes of the cerebral cortex. Conclusion Our investigation demonstrates that although some older T2DM patients do not suffer from brain vascular accidents, all of them have a decreased cerebral blood perfusion and decreased cerebral metabolism. The early checked cerebral blood perfusion and metabolism may be important for these patients.
2.Effects of Mosapride on capsule endoscopy in elderly patients

Objective To explore the effect of oral mosapride on gastrointestinal transit time in elderly patients undergoing capsule endoscopy.Methods A total of 61 patients referred for capsule endoscopy during 2010 September to 2012 January were involved in this study.40 patients over 65 years old were prospectively randomized into mosapride citrate group (n=19) or non-mosapride citrate group (n=21).Patients under 65 years old were in control group (n=21).Mosapride citrate group took 10 mg mosapride citrate orally before endoscopy.The gastrointestinal transit time was calculated.Results Gastric emptying time and small intestinal transit time of patients over 65 years old were significantly shorter in mosapride citrate group than in non-mosapride citrate group [(48.6± 21.1) minand (64.3±22.4) min,t=2.274,P=0.029; (302.2±67.6) min vs.(347.14±51.18) min,t=2.384,P=0.022].There were on significant differences in gastric emptying time and small intestinal transit time between non-older group [(45.4 ± 28.4) min and (284.8 ± 78.3) min] and mosapride citrate group (t=0.407,P=0.686;t=0.751,P=0.457).There was a difference in the image score between medication group and no medication group (4.94±0.63 versus 4.50±0.68; t=2.137,P=0.039) in over 65 years old patients.Conclusions Mosapride may decreased the gastric emptying time and small intestinal transit time,improves the capsules endoscopy image quality and testing completion rate.
3.Efficacy of phacoemulsification auxiliary hookinterred the anterior chamber treated failure of glaucoma combined with cataract surgery

AIM: To explorethe therapeutic efficacy of treating cataract phacoemulsification remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.
METHODS:Twenty-seven cases ( 27 eyes ) of glaucoma combined cataract surgery(3mo) in the early failure were treated with that the phacoemulsification auxiliary hook into anterior chamber under the original sclera flap, open from the inside have adhesion of the sclera flap. Postoperative follow-up 12mo, the intraocular pressure and best corrected visual acuity were observed.
RESULTS: The postoperative follow- up was 12mo. There were functional filtering bleb in 14(52%) eyes; and the IOP was below 21mmHg in 17 eyes(63%) without any anti- glaucoma drug, and in 7 eyes ( 26%) with 1 - 2 categories of lower intraocular pressure( IOP) drugs, and 3 eyes(11%), failure and the success rate was 89% after 12mo. Average intraocular pressure after 1wk, 1mo, 3mo, 6mo and 12mo were(7. 1±4. 3) mmHg,(10. 5±5. 1) mmHg,(15. 1±4. 8 ) mmHg, ( 16. 8±5. 2 ) mmHg, ( 17. 3±5.1 ) mmHg, significantly lower than the IOP preoperatively(30. 2±6. 8) mmHg (P<0. 01). Average visual acuity ( all of the following were best corrected vision) after 1wk, 1mo, 3mo, 6mo and 12mo was(0. 32±0. 52),(0. 52±0. 42),(0. 55±0. 39),(0. 53±0. 47),(0. 54±0. 42 ). Postoperative 1wk visual acuity deeper than preoperative( 0. 46 ± 0. 44 ) ( P < 0. 05 ), more than all improved compared with preoperative ( P < 0. 05 ). Postoperative anterior chamber bleeding in 5(19%) eyes, all absorbed itself in 3 to 5 days; ciliochoroidal detachment in 7 ( 26%) eyes, except one case need operate, others were conservative treatment to heal. Postoperative continuous low intraocular pressure concurrent macular edema was 1 eyes(4%), 4mo healed after treatment.
CONCLUSION: It is a safe and effective method thattreating phacoemulsification cataract remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.
5.An evaluation of status of depression using CES-D and analysis of its influencing factors among male nursing undergraduates

Objective To understand the status of depression and compare with the same age group of Chinese norms of CES-D in urban areas and analyze its potential influencing factors among male nursing undergraduates.Methods In this cross-sectional study,the CES-D and self-made questionnaire were administrated to 102 male nursing undergraduates from two schools of nursing through two-stage random sampling.Results The prevalence of depression was 14.0%,and the score of nursing undergraduates was (11.00±6.37) points,which had no significant difference from the scores of the same age group of Chinese norms in urban areas.Univariate analysis showed that coming from countryside,worse interpersonal relationship,worse career prospect,getting no attention from parents had some significant association with depression.Multivariate analysis showed that worse career prospect and getting no attention from parents associated with lower CES-D scores.Condusions For male nursing undergraduates,efforts should be given from the aspects of families and career prospects to improve their psychological health condition,and reserve qualified resources for nursing profession.
6.Left atrial linear ablation of paroxysmal atrial fibrillation guided by three-dimensional electroanatomical system

Objective To investigate the safety and efficacy of Left atrial linear ablation of paroxysmal atrial fibrillation guided by three-dimensional electroanatomical system. Methods 29 patients with paroxysmal atrial fibrillation in this study. A nonfluoroscopic mapping system was used to generate a 3D electroanatomic LA mapping, and all pulmonary vein ostia were marked under the help of pulmonary veins angiography on the 3D map. Radiofrequency (RF) energy was delivered to create continuous linear lesions encircling the pulmonary veins. It was delivered with a target temperature of 43℃, a maximal power limit of 30W and applied for ≥20 seconds until the maximal local electrogram amplitude decreased by ≥50%. The ablation was completed by finishing the circular line. Results The mean procedure duration was 180?18 minutes, with mean fluoroscopy time of 80?20 minutes. The average number of RF pulses was 120?15. After a follow-up of 6.0 months, 24 patients maintained sinus rhythm. 3 patients suffered from less frequent paroxysmal atrial fibrillation during the first 3.0 months after the ablation and remained Af free after 6 months. 1 patient had atrial fibrillation episodes and 1 patient had atrial fibrillation attacks unchanged. No pulmonary vein narrowing was observed. Conclusion Left atrial linear ablation of paroxysmal atrial fibrillation guided by three-dimensional electroanatomical system was safe and effective.
8.Clinical Observation of Yinhe Gargle for Disinfection of Infected Root Canals

Objective To evaluate the clinical efficacy of Yinhe gargle on infected root canals. Methods Forty-seven cases of root canal infection ( involving 48 affected teeth) were randomly divided into two groups. The experimental group (22 cases involving 22 affected teeth) was given intracanal medicament with Yinhe gargle, and the control group ( 25 cases involving 26 affected teeth) was given root canal disinfection with camphor phenol. After intracanal medicament for one week, therapeutic effect and safety were evaluated in the two groups. Results ( 1) During root canal disinfection, none of the subjects had adverse reaction such as hypersensitiveness or cauterization of soft tissue. Canal obturation was available after the first intracanal medicament for the infected root canals. (2) After intracanal medicament for one week, the effective rate in the experimental group was 95.45%, and was 100.00%in the control group, the difference being in significant (P>0.05). Conclusion Yinhe gargle has similar disinfection effect to camphor phenol on infected root canals, and it can be used as disinfection medicament for root canals.
9.Titanium plate combined with bone graft and internal fixation for calcaneal fractures:influencing factors for lateral L-shaped incision healing

BACKGROUND:Calcaneus has its special anatomy. Moreover, it is difficult to expose the inside to the posterior side of the joint surface. Lateral calcaneal soft tissue was less. The joint surface can be clearly exposed after incision, so lateral L-shaped incision is conventionaly used in calcaneal surgery. OBJECTIVE:To analyze the suspicious factors influencing the open reduction titanium plate for calcaneal fracture and internal fixation of L-shaped wound healing during bone graft. METHODS:A total of 84 cases (94 sides) of calcaneal fractures, who were treated in the Disabled Rehabilitation Center in Liaoning Province from June 2011 to November 2014, were included in this study. They were grouped according to the source of bone graft. Alograft group contained 52 sides (44 cases). Autogenous iliac bone graft group contained 42 sides (40 cases). Operation time of calcaneus was 7-10 days after injury. L-shaped incision was made for open reduction and internal fixation with titanium plate and bone graft. The patient’s age, the type of bone graft, type of drainage and drainage time that may cause theincision complications were investigated and analyzed. RESULTS AND CONCLUSION:(1) Among 84 cases of 94 calcaneal fractures after operation, 16sides affected complications. (2) Significant differences in the incidence of complications were detected between negative pressure drainage and skin flap drainage (P< 0.05). Negative pressure drainage had a significant effect on reducing the incidence of incision complications. (3) Significant differences in the incidenceof incision complications were determined between the alograft group and autogenous iliac bone graft group (P< 0.05). Implantation of autogenous iliac bone could significantly reduce the incidence of incision complications. (4) No significant differencewas detected between the complication group and non-complication group in age and drainage time (P> 0.05). (5) These findings indicate that 7-10 days after injury, operation after the sweling subsided completely, the negative pressure drainage, and planting autogenous iliac crest can reduce the incidence of complications related to lateral calcaneal L-shaped incision.
10.The Clinical Significance of Ghrelin Level in Hp Related Upper Gastrointestinal Diseases

Objective To investigate the impact and clinical significance of helicobacter pylori (Hp) elimination on ghrelin. Methods Forty patients with chronic superficial gastritis (CSG), 42 patients with chronic atrophic gastritis (CAG), 41 patients with peptic ulcer (PU) and 17 patients with gastric adenocarcinoma (CA) were included in this study. All of pa-tients in four groups were Hp-positive. Forty patients with Hp-negative were used as control. The Hp elimination were only performed in CAG,CSG and PU groups. The serum ghrelin and pepsinogen (PG) levels before and after Hp elimination were detected with ELISA assay in CSG, CAG and PU groups. The correlation between PG and glrelin was also detected. The ex-pression of ghrelin in gastric mucosa was detected by RT-PCR. Results Comparing with control group (30.41 ± 8.97), the ghrelin level was increased in PU group (35.42±9.87), but which were decreased in CAG group (18.59±8.19) and CA group (18.33±6.88). There was no significant difference in ghrelin level between CSG group (26.08±9.14) and control group. After Hp elimination, the serum and gastric mucosa ghrelin levels were significantly increased in CSG group (P<0.01), but both serum and gastric mucosa ghrelin levels were significantly decreased in PU group (P<0.01). And no significant difference in the level of ghrelin after Hp elimination in CAG group (P>0.05). A positive correlation was found between serum PGⅠ/PGⅡand serum ghrelin level in CSG, CAG and CA groups (r=0.668,P<0.01). Conclusion Hp elimination has an impact on ghrelin level in patients with upper gastrointestinal diseases. The changes of ghrelin level related to PGⅠ/PGⅡ. Ghrelin can be used as one of the indexes of diagnostic and prognostic evaluation in Hp related upper gastrointestinal diseases.
