1.Retrobulbar injection with triamcinolone acetonide for macular edema caused by retinal vein occlusion

Objective To evaluate the therapeutic efficacy of retrobulbar injection with triamcinolone acetonide (TA) for macular edema caused by retinal vein occlusion (RVO). Design Retrospective case series. Participants 21 eyes of 21 patients with macular edema caused by RVO. Methods Retrobulbar injection with TA 40 mg was performed on 21 eyes. The visual acuity, intraocular pres-sure, fundus fluorescein angiography (FFA) and macular central thickness under the optical coherence tomography (OCT) were recorded and analyzed before and after treatment. The follow-up duration was 6-11 months. Main Outcome Measures The visual acuity and the macular central thickness of OCT. Result Visual acuity was improved at the end of follow-up in 16 eyes (76.2%) and unchanged in 5 eyes (23.8%). Macular central thickness was (287.55±121.70) μm at the end of follow-up and (617.23±185.58) μm before treatment. The difference was statistically significant(P=0.000). After treatment, the comparison of visual acuity and macular central thickness be-tween CRVO patients and BRVO patients, as well as between ischemic RVO patients and non-ischemic RVO patients had no significant difference (P>0.05). In patients of course less than 6 months, the visual prognosis after treatment is better than that of before treatment (P=0.011, 0.01), while in patients of course more than 6 months, no difference of vision could be detected before and after treatment (P= 0.583). The result of FFA showed decrease of fluorescein leakage. During follow-up, intraocular pressure of all patients was in the nor-real range. Recurrence of macular edema was found in 8 eyes 2 to 6 months after treatment. Conclusions Retrobulbar injection with TA is a safe, effective, and easy method to treat macular edema caused by RVO. 5.Clinical Effects of Sodium Hyaluronate Combined with Arthroscopic Debridement on Knee Osteoarthritis

Objective:To investigate the clinical effects of arthroscopic debridement combined with intra-articular injection of sodi-um hyaluronate in the treatment of knee osteoarthritis. Methods:One hundred and seventy-two cases with knee osteoarthritis were ran-domly divided into the treatment group and the control group. The control group received arthroscopic debridement, while the treatment group received intra-articular injection of 2. 5ml sodium hyaluronate after arthroscopic debridement. Lequence score, Lysholm score and visual analogue scale for pain were compared between the two groups before the treatment and after the 1-week and 3-month treatment, and the clinical effects were compared after the 3-month treatment as well. Results: Lequence score, Lysholm score and visual ana-logue pain scores after the treatment were lower than those before the treatment with significant differences (P <0.01) in the two groups. After the treatment,Lequence score, Lysholm score and visual analogue pain scores of the treatment group was lower than those of the control group with significant differences (P<0. 05 or 0. 01). After the 3-month treatment,the clinical efficacy of the treatment group was better than that of the control group (P<0. 05). Conclusion:Arthroscopic debridement combined with intra-articular injec-tion of sodium hyaluronate in the treatment of knee osteoarthritis has better efficacy than arthroscopic debridement alone. The combina-tion can effectively improve symptoms, which is worthy of clinical promotion.
6.Optimal Rectal Dosage of Misoprostol for Dilation of Cervix Before Painless Electrical Vacuum Abortions

Objective To evaluate the optimal dosage of misoprostol administered in the rectum for dilation of the cervix. Methods Two hundred and forty women at 40-60 day gestational age without vaginal delivery history were randomly divided into three groups,with 80 cases in each group. Patients received 200,400 or 600μg of misoprostol rectally one hour before electrical vacuum abortions in group A,B and C,respectively. Cervical dilation,blood loss,and drug side effects in the three groups were compared. Venous blood samples were taken before vein anesthesia,and misoprostol acid concentration in the serum was tested by liquid chromatography-mass spectrometry. Results The analgesic rate was 100. 00%in all three dose groups, and cervical dilation rate was 23. 75%,46. 25%and 70. 00%in groups A,B and C,respectively. The severity of drug side effects such as vaginal bleeding and abdominal pain is dose-dependent. Blood concentration of misoprostal acid was(117±65),(206± 98),and(303±149)pg·mL-1 ,in groups A,B and C,respectively. Conclusion The recommended dose of misoprostol is 400 μg administered in rectum. Rectal administration of misoprostol is cheap,safe,and convenient,and therefore could be widely applied.
7.Expression of Heat Shock Protein 70 and Bag-1 Protein in Gastroenteric Cancer

Objective To observe the expression of bag-1 and heat shock protein (HSP) 70 in clinical malignant tumors and the correlation between them. Methods RT-PCR and Immunohistochemistry were employed to examine the expression of bag-1 and HSP70 in 69 cases with gastroenteric cancer (54 cancer tissues and 15 non-cancer tissues). Results The expression of Bag-1 was detected in 96.3% of the cases, much higher than that in the control group (P0.05), and no correlation was observed between histological grade and HSP 70 expression. Conclusion There exists definite relationship between the gastroenteric cancer and the expression of bag-1 and HSP70, and there are close correlation between the tumor progress and the expression of bag-1.
9.The effect of HLA matching on patient/graft survival rate of renal transplant recipients positive for PRA

Objective To elucidate the effect of human leukocyte antigen(HLA)cross-reactive groups(CREGs)matching on the patient/graft survival rate of renal transplant recipients positive for penal-reactive antibody(PRA).Methods The PRA level and the HLA antibody specificity of renal transplant candidates were assayed using the LAT1240,LM720R,and SSP2LB kits.The appropriate donors were selected according to the CREGs matching standard.Results 112 recipients included 43 positive cases of HLA classⅠantibody,39 positive cases of HLA classⅡantibody,and 30 positive cases of bothⅠandⅡ.The HLA mismatching cases of 0,1,2,3,4 and 5 allele(s)were 6,39,38, 21,7 and 1,respectively.Hyper-acute rejection(2 cases),acute rejection(18 cases),chronic rejec- tion(5 cases),delayed graft function(4 cases),graft excision(one case)due to rejection and deaths (13 cases,5 with a functioning graft)occurred after transplant.Ninety-nine recipients and 96 grafts survived.The 1-,3-,and 5-year graft survival rate was 91.96%,86.96%,and 86.21%,respec- tively.Conclusion The HLA matching rate of patient/graft was elevated significantly and the effect of high PRA level was decreased according to the principle of CREGs,which increased the patient/graft survival rate remarkably in sensitized recipients.
10.Effect of Ranibizumab intravitreal injection before 23 G - vitrectomy surgery in the treatment of patients with proliferative diabetic retinopathy

AIM: To analyze the effect of preoperative intravitreal injection of Ranibizumab ( lVR ) in patients undergoing pars plana vitrectomy ( PPV ) for proliferative diabetic retinopathy ( PDR) .
●METHODS:ln this retrospective research, 62 patients (67 eyes ) with PDR in the First Affiliated Hospital of Xinjiang Medical University from Jun. 2014 to Jun. 2015 were recruited. They were assigned to an experimental group ( n=37 eyes ) and a control group ( n=30 eyes ) . The patients in experimental group were given 1 injection of lVR ( Lucentis 1. 0mg/0. 1mL ) 1wk before surgery, whereas those in control group went down to surgery directly. The average operation time, iatrogenic breaks, the use of tamponade, electric coagulation, and vision increase rate were comparatively analyzed between two groups.
●RESULTS:The average operation time was (91. 7±20. 8) min in the experimental group vs (117. 6±18. 6)min in the PPV group(t = -5. 314,P<0. 05). The rate of electric coagulation (11%) vs (47%), (χ2 =8. 963, P=0. 006);iatrogenic breaks (5%) vs (33%), (χ2=8. 789, P=0. 004) and silicone oil or inert gas tamponade ( 27. 0%) vs (53. 3%), (χ2 = 4. 828, P = 0. 043 ). The rate of visual improvement (75. 7%) vs (46. 7%), (χ2 = 5. 960, P =0. 022 ) . There was no severe complication associated with surgery, such as choroidal detachment, retinal detachment and endophthalmitis.
●CONCLUSION:lVR before microincision vitrectomy can effectively shorten the operation time, reduce the use of electric coagulation and intraocular tamponade, and improve the rate of visual improvement for PDR patients.
