4.An prospective observation on clinical therapeutic effect of Lund program combined with Angong Niuhuang pill for treatment of patients with severe traumatic craniocerebral injury

Objective To observe the therapeutic effect and prognosis of patients with severe traumatic craniocerebral injury treated by Lund program combined with Angong Niuhuang pill. Methods A prospective study was conducted. Forty-nine patients with severe traumatic craniocerebral injury were divided into two groups:combined treatment group (25 cases) and control group (24 cases). All the patients in the two groups received conventional therapy. Additionally, the combined treatment group was treated by the Lund program combined with Angong Niuhuang pill, the patients after admission into the intensive care unit (ICU), indwelling of gastric tube was immediately given (the tube inserted and retained through mouth in cases with cranial base bone fracture), one pill of Angong Niuhuang was fed nasally twice a day for consecutive 15 days. Before and after treatment, the intracranial pressure (ICP), cerebral perfusion pressure (CPP), Glasgow coma scale (GCS) score, the mean flow velocity of middle cerebral artery (MCA), and the incidence of complication, clinical efficacy and prognosis in the two groups were observed. Results There were no statistically significant differences in the levels of ICP, CPP, GCS score and the average flow velocity of MCA before treatment and the first day after treatment between the two groups. Beginning from 3 days after treatment, the degrees of improvement in the above indexes in the combination treatment group became much better than those in the control group [ICP (mmHg, 1 mmHg=0.133 kPa):16.78±4.50 vs. 18.80±5.67, CPP (mmHg):71.35±1.63 vs. 58.11±1.47, GCS score:9.53±0.38 vs. 8.23±0.44, the mean blood flow velocity (cm/s):152.23±3.33 vs. 178.86±5.23, all P<0.05]. The incidences of complications such as hyperpyrexia [8.0%(2/25) vs. 41.7%(10/24)], hyperspasmia [12.0%(3/25) vs. 45.8%(11/24)] and cerebral hernia [8.0%(2/25) vs. 33.3%(8/24)] in the combined treatment group were significantly lower than those in the control group (all P < 0.05). The rates of renal failure, electrolyte imbalance and pulmonary infection in combined treatment group were also lower than those in the control group, but the differences were not statistically significant (all P>0.05). The rate of good therapeutic effect in combination treatment group was obviously higher than that of the control group [72.0% (18/25) vs. 33.3% (8/24), P < 0.05], while the mortality was markedly lowered [8.0% (2/25) vs. 33.3% (8/24), P < 0.05]. Conclusions The combination of Lund program and Angong Niuhuang pills for treatment of patients with severe traumatic craniocerebral injury can reduce the degree of increase of ICP at early stage, ameliorate cerebral edema and its development, elevate CPP and improve cerebral ischemia and hypoxia. In the aspects of controlling hyperpyrexia, decreasing limbs twitches and the incidence of hernia, elevating the quality of patients' life, reducing mortality and improving prognosis, the therapy has relatively good therapeutic effects.
5.Design and application of digital manometer for air-pocket in manual air-way

This paper discusses the design of a special digital manometer for air-pocket in manual air-way. Integrated silicon pressure sensor MPX5010 with high precision and sensitivity is adopted in the device. It can not only measure and display air pressure of air-pocket,but also give the alarm for excessive pressure,thus providing a timely and convenient way for clinician to control air-pocket pressure,which ensures the safety and precision for clinic.

In natural condition, two years were needed for Panax quinquefolius seeds to break dormancy. In the first year, the embryo developed slowly, and the activities of amylase and peroxidase were low. In the second year the embryo developed rapidly, and the activities of both enzymes also increased gradually. But with the treatment of hormones enzymatic activ ities accelarated remarkably and speeded up the decomposition of stored nutrients in endos perm, so that starch and non-reducing sugar decreased while reducing sugar incrcased rapidly, resulting in earlier germination.
7.The value of peripheral blood and pleural effusion T-SPOT.TB combined with pleural effusion adenosine deaminase in diagnosis of tuberculous pleurisy

Abstract: Objective To explore the relationship between peripheral blood and pleural effusion tuberculosis (TB)
infection effector T cells, and to further evaluate the value of combined pleural effusion adenosine deaminase (ADA) for rapid
diagnosis of tuberculous pleurisy. Methods The test data of 80 cases of tuberculous pleurisy and 70 cases of nontuberculous
pleurisy treated in the Sixth People's Hospital of Nantong City from January 2017 to December 2020 were analyzed. The TBinfected effector T cells were also detected simultaneously in the peripheral blood and the pleural effusion by the T-SPOT
technique, and the pleural effusion ADA was detected by the rate method. The subject operating characteristic curve (ROC) was
applied to take the optimal pleural effusion ADA threshold to compare the sensitivity and specificity of different critical values.
Person phase analysis was applied to analyze the correlation between peripheral blood and pleural effusion T-SPOT.TB. Data of
peripheral blood, pleural effusion T-SPOT.TB and ADA were integrated. Results When pleural effusion ADA>45 U/L, the
sensitivity and specificity for the diagnosis of tuberculous pleurisy were 50.0% and 94.3%, respectively; when ADA > 25.15 U/
L, the sensitivity and specificity were 80.0% and 72.9%. When ADA > 45 U / L, pleural/ blood T-SPOT.TB spot ratio (spot
forming cells, SFCs) > 2 times, the specificity for the diagnosis of tuberculous pleurisy was 100% (highest); when 25.15 U/L<
pleural effusion ADA ≤ 45 U/L, pleural/blood T-SPOT.TB spot ratio > 2 times, the specificity for the diagnosis of tuberculous
pleurisy was 92.3% (second). When pleural effusion ADA ≤ 25.15 U/L, and the pleural effusion/blood T-SPOT.TB spot number
ratio > 2 times, with 83.3% specificity (the lowest of the three groups). Conclusions The level of pleural effusion ADA is one
of the most used methods for diagnosing tuberculous pleurisy. Further combination of pleural effusion and blood T-SPOT.TB, if
the ratio of pleural effusion / blood T-SPOT. TB spots is greater than 2 times, it can further improve the diagnosis rate of
tuberculous pleurisy.
8.Effects of tirofiban combined with nitroprusside injection through the rapid exchange balloon catheter on no-reflow phenomenon during elective PCI

Objective To investigate the effects of tirofiban combined with nitroprusside injection through the rapid exchange balloon catheter with self-made side holes on no-reflow (NR)phenomenon during elective percutaneous coronary intervention (PCI).Methods A total of 49 patients with NR phenomenon were randomly divided into the balloon catheter group (n=25)and the guiding catheter group (n=24).Tirofiban combined with nitroprusside were used in both groups.Thrombolysis in myocardial infarction (TIMI)grade and corrected TIMI frame count (CTFC)of target coronary artery at 10 minutes after administration were assessed.Results At 10 minutes after drugs injection,there were 21 (84.0%)patients with target of coronary TIMI 3 grade in the balloon catheter group,and 13 (54.2%)patients in the guiding catheter group.The CTFC was (33.5±12.5)frame in the balloon catheter group,and (41.9±1 5.3)frame in the guiding catheter group.The difference in TIMI and CTFC between the two groups was significantly (P <0.05).Conclusion Injection tirofiban combined with nitroprusside through the rapid exchange balloon cathe-ter with self-made side holes in NR patients during elective PCI might be feasible and effective.
9.Comparative study on intestinal metaplasia in Barrett's esophagus, cardia and gastric antrum

Objective To compare the differences in mucin-histochemistry and etiology of Barrett's esophagus (BE) intestinal metaplasia (IM) , cardiac intestinal metaplasia (CIM) and gastric antrum intestinal metaplasia (GA-IM). Methods Alcian blue /periodic acid-schiff (AB/PAS) and high iron diamine / alcian blue (HID/AB) histochemical methods were used to classify IM in BE, cardia and gastric antrum, and IM were classified into three subtypes: complete small intestinal type (type Ⅰ ) , incomplete small intestinal type (type Ⅱ) and incomplete colonic type (type Ⅲ). Compared the prevalence of different subtypes of IM in above-mentioned sites, and investigated their relationships among the symptoms of gastroesophageal reflux disease (GERD) and Helicobacter pylori (Hp) infection. 10.In vitro activity of micafungin against Candida albicans biofilms

Objective To explore the susceptibility of Candida albicans biofilms to micafungin. Methods In vitro model of C. Albicans biofilm was established in 96-well microtiter plates with 30 C. Albicans isolates from the Department of Mycology, Changhai Hospital, Shanghai. The susceptibility of C. Albicans biofilms to fluconazole, amphotericine B and micafungin was evaluated by colorimetric XTT-reduction assay. Sessile MIC80 (SMIC80), defined as the lowest antifungal concentration at which an 80% reduction in fungal growth was achieved, was determined. Results Of the 30 C. Albicans isolates grown in sessile states, all were resistant to fluconazole (SMIC80≥64 μg/mL), 4 sensitive to amphotericine B (SMIC80≤1 μg/mL), 26 resistant to amphotericine B (SMIC80 > 1 μg/mL), 27 sensitive to micafungin (SMIC80 < 16 μg/mL), 3 resistant to micafungin (SMIC80 >16 μg/mL). Statistical analysis revealed a significant difference in the activity against C. Albicans biofilms between micafungin and fluconazole (χ2=736.36, P<0.01), micafungin and amphotericine B (χ2=529.95, P<0.01), but not between anphotericine B and fluconazole (χ2=2.29, P>0.05). Conclusion C. Albicans biofilms are resistant to routine antifungal agents such as fluconazole and amphotericine B, but relatively more sensitive to micafungin.
