1.Application of high frequency ultrasound in knee joint lesions of elderly patients with rheumatoid arthritis

Objective To investigate the application of high frequency ultrasound in knee joint lesions of elderly patients with rheumatoid arthritis.Methods Fifty patients with rheumatoid arthritis were collected in Yangpu Hospital Affiliated to Tongji University from March 2015 to December 2016 as test group.Fifty healthy elderly people without knee joint lesion were randomly selected as control group.High frequency ultrasound was used to examine the patients in the test group and the Results of this examination were used to compare with those of the control group and observe four indexes: patellofemoral effusion,synovial thickness,cartilage thickness and blood flow in the lesion area.Results There was only a small amount of synovial fluid in the articular cavity in the control group,the thickness was (2.11± 0.94) mm,while the thickness of the test group was (10.33±2.74) mm,the difference was statistically significant (t=17.03,P<0.05);in the control group,there was no synovial hyperplasia,the thickness was (1.11±0.24) mm,while the thickness of the test group was (5.48±2.74) mm which was significantly higher than that of the control group (t=13.18,P<0.05);in the control group,the cartilage tissue was striated without echo band,the thickness was (1.76±0.29) mm,while the cartilage tissue of the experimental group had lesion,the thickness was (1.12±0.12) mm lower than that of the control group and the difference was statistically significant (t=6.44,P<0.05);in the control group,there was no blood flow signal in the joint cavity,while low-speed blood flow signal could be seen in the joint cavity of the test group,the difference was statistically significant (0 vs.96%,χ2=6.44,P<0.05).Conclusion High frequency ultrasonography is effective in the treatment of senile rheumatoid arthritis with high specificity,high sensitivity,low cost,high efficiency and simple operation and it is not subject to external conditions.Therefore,it is suitable for early examination of senile rheumatoid arthritis and clinical promotion.

3.Expression and significance of bcl-2, bax and ER in foci of adenomyosis

Objective To investigate the expression and signification of B-cell lymphoma/leukemia-2(bcl-2),bax and ER proteins in epithelial cells in different depth of adenomyosis.Methods Expression and correlation of bcl-2,bax and ER proteins were detected by immunohistochemistry staining in 36 adenomyosis cases with superficial (< 1/2 depth of penetration) and deep (≥ 1/2 depth of penetration)ectopic endometrium and eutopic endometrial tissues matched with 30 cases with benign ovarian tumor,uterine septum,pelvic floor dysfunction without adenomyosis as controls.Results (1) The expression of ER protein in superficial and deep ectopic tissues (5.04 ±0.24,4.91 ±0.16) were found significantly lower than 6.06 ± 0.36 in eutopic tissues (P < 0.01) and higher than 3.70 ± 0.58 in control group (P < 0.05).There was no statistical difference of ER expression in superficial and deep ectopic endometrium (P > 0.05).(2) The level of bcl-2 protein of 5.6 ± 0.4 in superficial and 6.0 ± 0.3 in deep myometrium of ectopic tissues were significantly higher than 3.6 ± 0.4 in eutopic tissues and 1.9 ± 0.4 in control group (P < 0.01).(3) The level of bax protein of 3.50 ± 0.28 in superficial and 4.80 ± 0.29 in deep myometrial ectopic and 4.43 ± 0.37 in eutopic tissues were significantly lower than 6.18 ± 0.65 in control groups (P <0.05).The expression of bax in superficial myometrium is significantly lower than deep myometrium in ectopic tissues (P <0.01).(4) The positive correlation between the expression of ER and bcl-2 protein at superficial myometrium of ectopic tissues (r =0.720,P < 0.01).And there was no significant correlation with the expression of ER and bax protein at superficial myometrium (r =0.008,P > 0.05).As well as,there was not significant correlation with the expression of bcl-2,bax and ER protein at deep myometrium (r =0.089,r =-0.023,P > 0.05).The expression of bax protein in ectopic endometrium was positive correlation with the depth of adenomyosis penetration (r =0.736,P < 0.01).There was positive correlation between the expression of ER and bcl-2 protein at normal endometrium (r =0.453,P < 0.05).And there was negative correlation between the expression of ER and bax protein at control group (r =-0.514,P =0.05).Conclusion The bax protein expression of ectopic endometrium in deep adenomyosis was higher than superficial adenomyosis.
4.A respective analysis on 462 cases of leukemia typing

Objective To study the relationship between FAB‐classifying standard and immunophenotype of leukemia in clinical diagnostic and treatment .Methods There were 462 cases of leukemia according to FAB‐classifying standard and 34 cases by immu‐nophenotype in our hospital from 2011 to 2013 .The results of FAB‐classifying standard and immunophenotype were statisticed and compared .Results Among the 462 cases ,there were 171 acute myeloid leukemia (AML) ,50 acute lymphoblastic leukemia (ALL) , 76 chronic leukemia ,3 rare types ,56 unclear types and 27 mismatched according the FAB‐classifying standard .The diagnostic rate was 82 .03% .34 cases of immunophenotype included 29 single phenotype ,4 double phenotypes and 1 unknown .The diagnosis rate was 97 .06% .CD13 and CD33 have high positive express in AML ;CD34 and HLA‐DR do not express in M3 ;CD7 which was known as a lymphatic antigen also was founded in AML .Conclusion The combing use of FAB‐classifying standard and immunophenotype can improve diagnostic rate ,and immunophenotype has an important role in differentiating different subtypes of leukemia .
5.Factor analysis and treatment strategies of post-operative soft-tissue defect in leg trauma

Objective To analyze risk factors for soft-tissue defects after surgery on leg trauma,to discuss methods for prevention of soft-tissue defects and its treatment.Methods A retrospective analysis was conducted on 217 cases of soft-tissue defects with bone exposure and / or internal fixation exposure after surgery on leg trauma from January,1999 to December,2012.Soft-tissue defects with various flaps were used in 201 cases,including neurocutaneous flap(89 cases),random flap(21 cases),free flap(75 cases),cross-leg flap(16 cases),and by skin grafting in 16 cases.For the 117 internal plate-fixated fractures,96 removed the plate and 21 cases did not remove the plate.For the 13 intramedullary nail fixations,4 cases of intramedullary nails were removed,9 cases were not removed.For the 56 cases of external fixation of the fractures,39 patients had unadjusted external fixation,17 cases re-adjusted external fixations.Results Of the 217 cases of soft-tissue defect after surgery on legs,201 cases were conducted flap surgery.The flap was completely necrosis in one case,and partial distal necrosis were in 14 cases,of which 6 cases healed after changing the dressing,5 underwent debridement and skin grafting,3 cases underwent flap surgery again.In cases of fracture patients,177 cases healed within 1 year,delayed union 23 cases,nonunion eight cases.Nonunion healing after bone grafting and re-fixation surgery.In 9 cases of uncontrollable osteomyelitis,6 patients underwent bone removal and bone transport surgery,3 patients underwent amputation.Conclusion Soft-tissue defects after surgery on leg trauma is a serious complication whose prevalence is associated,in a certain degree,with incorrect surgical time and methods.It is crucial for early repair of soft-tissue defects by using various kinds of proper flaps whenever soft-tissue complication occurs.In patients with early stage internal fixation,flap surgery without internal fixation removing is feasible if little soft-tissue defect,little exposed internal fixation and mild infection,otherwise it is necessary to remove internal fixation and re-fix by external fixation before flap surgery.
6.Preparation Technique of Fluconazole for Injection and Its Stability Study

OBJECTIVE:To select a technique for preparation of fluconazole for injection and to establish a method for determination of its content.METHODS:The formula was selected on the basis of pH value and species of solution adjuvant.The content was determined by HPLC.RESULTS:The preparation was stable when pH value was between6.5~8.5,and its clarity could be increased by propylene glycol.The detectable concentration of fluconazole showed a good linear correlation in the range of40~200?g/ml.The average recovery was100.37%,RSD=1.37%(n=3).CONCLUSION:The fluconazole for injection prepared by the present technique is stable in quality and the method for content determination is accurate and practicable.
7.Expression of exogenous interferon-? gene in human hepatocellular carcinoma cells mediated by liposome

Objective To establish a recombinant eukaryotic expression vector containing human interferon-?(IFN-?) cDNA,and investigate the expression of IFN-? gene in transfected hepatocellular carcinoma (HCC) cell lines. Methods pcDNA3-IFN-?,established by subcloning IFN-? cDNA into a mammalian expression vector pcDNA3,was mediated into HCC cell lines SMMC-7721 and QGY-7701 by lipofectamine. The transfected HCC cells were selected in RPMI1640 containing G418(400~700?g).RT-PCR analysis and ELISA assay were used for mRNA transcription and protein expression of IFN-? gene. Results IFN-? mRNA was detected only in pcDNA3-IFN-? transfected cell lines, and high levels of IFN-? protein were detected only in the supernatants of IFN-? gene-modified cell lines. Conclusions pcDNA3-IFN-? can stably expressIFN-? cDNA in the transfected HCC cell strains
8.Expression and Antigenic Characterization of the Epitope-G1 of the Bovine Ephemeral Fever Virus Glycoprotein in Pichia pastoris

The epitope-G1 gene of Bovine ephemeral fever virus (BEFV) glycoprotein was synthesised by PCR and cloned into expression vector pPIC9K to construct recombinant plasmid pPIC9K-G1. Then the pPIC9K-G1 was linearized and transformed into Pichia pastoris GS 115. The recombinant P. pastoris strains were selected by a G418 transformation screen and confirmed by PCR. After being induced with methanol, an expressed protein with 26 kDa molecular weight was obtained, which was much bigger than the predicted size (15.54 kDa). Deglycosylation analysis indicated the recombinant G1 was glycosylated. Western blot and ELISA tests, as well as rabbit immunization and specificity experiments indicated that the target protein had both higher reaction activity and higher immunocompetence and specificity. The recombinant G1 protein could be used as a coating antigen to develop an ELISA kit for bovine ephemeral fever diagnosis.
9.Effect of Zhibai Dihuang Decoction on Expressions of TRPV1 and TRPV5 in Spermatogenic Cells of UU-infected Rats.

OBJECTIVETo observe the effect of Zhibai Dihuang Decoction (ZDD) on mRNA and protein expressions of transient receptor potential family vanilloid subtype 1 (TRPV1) and transient receptor potential family vanilloid subtype 5 (TRPV5) in Ureaplasma urealyticum (UU)-infected rat semens and spermatogenic cells, and to explore the pathomechanism of UU-infected infertility and the intervention of ZDD.
METHODSTotally 45 were randomly selected from 60 4-5 months old SD rats. UU testicular infected animal models were set up after bladder inoculation of UU suspension. The remaining 15 rats were simultaneously injected with normal saline as a normal control group. After a successful modeling, UU infected model rats were randomly divided into the model group, the azithromycin group, and the ZDD group, 15 in each group. Rats in the ZDD group were administered with ZDD at the daily dose of 1 g/kg by gastrogavage. Rats in the azithromycin group were administered with azithromycin suspension at the daily dose of 0. 105 mg/kg by gastrogavage. Equal volume of normal saline was administered to rats in the normal control group and the model group. All medication was performed once daily for 21 successive days. Testes and epididymis were extracted after rats were killed and UU positive rates were compared among all groups. Sperm cells were separated using a mechanical separation technique. Sperm motility parameters were detected using color sperm motion detection system. mRNA and protein expressions of TRPV1 and TRPV5 in spermatogenic cells were determined by real-time quantitative PCR and Western blot.
RESULTSThe UU positive rate was obviously higher in the model group than in the normal control group [(86.7% (13/15 cases) vs. 0] P < 0.05). It was lower in the ZDD group [33.3% (5/15 cases)] and the azithromycin group [26.7% (4/15 cases)] than in the model group (P < 0.05). Compared with the normal control group, class A and B sperms were reduced, the linear velocity and the average velocity were significantly lowered, mRNA and protein expressions of TRPV1 and TRPV5 in spermated genic cells significantly decreased in the model group (P < 0.05). Compared with the model group, class A and B sperms were increased, linear and curve velocities and the average velocity were significantly elevated, mRNA and protein expressions of TRPV1 and TRPV5 significantly increased in the ZDD group and the azithromycin group (P < 0.05, P < 0.01). Compared with azithromycin group, class A and B sperms were increased, the linear velocity and the average velocity were significantly elevated, mRNA and protein expressions of TRPV1 and TRPV5 significantly increased in the ZDD group (P < 0.05, P < 0.01).
CONCLUSIONZDD could fight against UU infection and elevate semen quality, which might be associated with up-regulating mRNA and protein expressions of TRPV1 and TRPV5 in spermatogenic cells.

10.Characteristics of clinical presentation and neuropathology in idiopathic hypereosinophilic syndrome encephalopathy

Objective To discuss the mechanism and characteristics of clinic presentation and neuropathology in idiopathic hypereosinophilic syndrome (IHES) encephalopathy. Methods IHES encephalopathy was diagnosed by clinical presentations,lab examinations,neurologic images,marrow and brain biopsy,then treated with corticosteroids and continuous follow-up. Results The IHES patient presented progressive limbs weakness and cognitive deficit with elevated eosinophil count. Results of lab examinations and bone marrow biopsy ruled out secondary eosinophilia and clonal eosinophilia such as eosinophilic leukemia.Brain magnetic resonance imaging (MRI) study showed multiple lesions in right frontal lobe, bilateral parietal and occipital lobe, presenting hypointensity in T1 weighted images,hyperintensity in T2 and fluid-attenuated inversion recovery weighed images. Brain biopsy showed proliferation of vascular membrane,small vessels stenosis,ischemia-induced morphological change and necrosis of neurons in the lesions.The patient was continuously treated with corticosteroids,and the situation was stabilized with follow-up.Conclusions IHES encephalopathy should be paid more attention in clinical practice of neurologists for its rarity. Brain vascular damages caused by elevated eosinophil may be an important pathophysiological mechanism of IHES encephalopathy,and corticosteroids or hydroxycarbamide should be used for the treatment.
