3.Association of pre-transplant serum level of anti-endothelial cell antibody with acute rejection in kidney transplant recipients

Objective To study the pre-transplant serum level of anti-endothelial cell antibody(AECA)in kidney allograft recipients and its impact on the episode of acute rejection (AR) within 6 months after transplantation. Methods A total of 495 kidney allograft recipients with pre-transplant serum between December 1998 and August 2003 in our center and 40 healthy controls(negative controls)were enrolled in the study.Clinical data including AR within 6 months after transplantation were analyzed retrospectively.The serum AECA level was measured by cyto-ELISA using EA.hy926 cells as substrate,which was shown as the ratio of P (patient)/N (negative control)=(A_(petient)-A_(blank contrnal)/(A_(negative contral)-A_(blank contral).AECA was considered positive when P/N value Was greater than the average A_(negative control)value plus two times the standard deviation.Results Positive rate of AECA was 18.8%(93/495).AECA level in hemodialysis patients who had been on hemodialysis more than 12 months was 1.43±0.37,greater than those less than 12 months(1.27± 0.32,P=0.013)and those of non-dialyzed patients(1.31±0.32,P=0.029).Correlation coeffieient between AECA level and hemodialysis duration was 0.218 (P=0.018).AR incidence in AECA positive recipients was 38.7%,greater than that in AECA negative recipients (23.4%,P=0.002). Incidence of acute T cell-mediated rejection and acute antibody-mediated rejction increased significantly (P=0.035,P=0.002 respectively).Multifactor logistic regression analysis indicated that AECA positive,PRA greater than 1 0%and high CDC level were risk factors of AR with odds ratio of 2.056,1.751 and 1.764 respectively(P=0.004,0.029,0.050). Conclusions The AECA positive in pre-transplant serum indicates the elevated risk of acute allograft rejection.The AECA level increases with prolonged hemodialysis duration.
4.Bacterial Distribution and Drug Resistance in Patients in Intensive Care Unit

OBJECTIVE To investigate the bacterial distribution and drug resistance in patients in intensive care unit(ICU) and provide theoretical bases for rational usage of antibiotics.METHODS The distribution and drug resistance of 372 strains isolated from patients in ICU collected from Jul 2007 to Jun 2008 were investigated and studied retrospectively.RESULTS Among them,the Gram-negative bacilli covered 59.14 %,the Gram-positive cocci 28.49%,and the fungi covered 12.37%.Pseudomonas aeruginosa,Klebsiella,Stenotrophomonas maltophilia and Ancinetobacter were the main Gram-negative bacilli.Staphylococcus aureus,coagulation-negative Staphylococcus and Enterococcus were the main Gram-positive cocci.The resistance rate of P.aeruginosa,S.maltophilia and Acinetobacter to imipenem was over 10%,and the S.maltophilia was 96.7%,the resistance rate of three main Gram-positive cocci to vancomycin and teicoplanin was zero,and the isolated bacteria showed serious multidrug-resistance.CONCLUSIONS Periodic monitoring should be done to learn the drug resistance and bacterial distribution in ICU in order to rationally use antibiotics to avoid the generation of new drug resistant strains and control the infection of patients in ICU.
5.Drug-resistance of Pseudomonas aeruginosa Isolated from Patients in Hospital

OBJECTIVE To investigate the change of antibiotics resistance of the Pseudomonas aeruginosa isolated from patients′s sputum in our hospital from Jan 2006 to Dec 2008,and offer basis for prevention of clinical infection and the reasonable use of drugs.METHODS The culture,identification and sensitivity to antibiotics of P.aeruginosa from the clinical sputum specimens were analyzed using USA VITEK-32 system.RESULTS Totally 196 strains of P.aeruginosa were isolated and characterized during the three years.The rates of resistance to cefoperazone/sulbactam were 18.37%,piperacillin/tazobactam 16.84%,netilmicin 17.35%,trimethoprim/sulfamesoxazole 100.00%,ampicillin 99.49%,cefazolin 99.49%,cefotetan 88.78%,and to ceftriaxone were 79.08%.The resistance rate to cephalosporins showed rising tendency.But the resistance rate to ?-lactam antibiotics showed deereasing tendency.CONCLUSIONS P.aeruginosa has single and multi-resistance to antibiotics seriously,but sensitive to ?-lactam antibiotics and aminoglycosides.Using antibiotics reasonably based on bacteria identification and sensitivity test is the best way to reduce the resistance of the pathogens.
6.Influence of glutamate and NMDA-receptor antagonist MK-801 on the electric activities of pain-excitation neurons in the nucleus accumbens of rats.

The experiment explored the influence of glutamic acid (Glu) and the NMDA-receptor antagonist dizocilpine maleate (MK-801) on the pain-evoked responses of pain-excitation neurons (PEN) in the nucleus accumbens (NAc) of rats. The trains of electric impulses applied to the sciatic nerve were used as noxious stimulation. The discharges of PEN in NAc were recorded by glass microelectrode. We observed the influence of intracerebroventricular (icv) injection of Glu and microinjection of MK-801 into the NAc on the noxious stimulation-evoked activities of PEN in NAc. The results showed that the noxious stimulation potentiated the electric activities of PEN in NAc. Intracerebroventricular injection of Glu (10 nmol/10 microl) increased the frequency of the discharge of PEN evoked by the noxious stimulation in NAc, the Glu-induced response was blocked by the injection of MK-801 (1.0 nmol/0.5 microl) into NAc. MK-801 partly inhibited the response of PEN upon the noxious stimulation. It is therefore suggested that the facilitatory effect of Glu on PEN response in NAc to the noxious stimulation is mediated by NMDA receptors, and that Glu and NMDA receptors are involved in the modulation of nociceptive information transmission in the NAc.

7.A survey on reproductive health related sexual behavior among middle school students in Luoyang city

Objective To explore the appropriate ways and contents of reproductive health education for middle school students and to understand reproductive health related sexual behavior and influencing factors among middle school students. Methods Reproductive health related sexual behavior was evaluated among junior and senior middle school students in Luoyang by cluster sampling. The statistical software of SAS 8.1 was adopted for data analyses. Sexual behavior and influencing factors were analyzed by logistic regression. Results Critical sexual behaviors were found significantly higher in senior students,including masturbation, sexual fantasy and sexual intercourse than that in junior students (P<0.05), and boys had higher prevalence than girls (P<0.05). Results from multivariate logistic regression model analyses indicated that incidence rate of sexual behavior among those who ever having had experiences was higher than those who were inexperienced ( OR = 2.62,95 % CI:1.21-5.66). Incidence rate of sexual behavior was related to access of reproductive health and STD/AIDS knowledge ( OR = 3.09,95 % CI:1.43-6.51). In addition, incidence rate of sexual behavior was related to attitude and relation of amour between boys and girls ( OR = 2.24,95 % CI:1.32-3.75). Conclusion Awareness on reproductive health knowledge among middle school students was not enough. Marginal sexual behaviors as masturbation and sexual fantasy had not been correctly and openly discussed to avoid inappropriate sexual activities.

Objective To study the sero-epidemiological starus regarding Rickettsia (R.) typhi,Bartonella (B.) henselae and Orientia (O.) tsutsugamushi in farmers from rural areas of Tianjin.Methods Field epidemiological surveys were performed in 8 districts (county) of Tianjin city from 2007 to 2009.886 farmers were randomly recruited and their serum samples collected to detect the specific antibodies of R.typhi,B.henselae and O.tsutsugamushi by micro-indirect immunoflorescence (IFA).Results The total antibody positive rates of R.typhi increased from 5.0% to 58.2% while B.heaselae had an increase from 2.6% to 14.5% and O.tsutsugamushi increased from 1.8% to 39.8%.Geographic distribution showed that farmers living in the central and southeast areas were higher than that in other areas.Conclusion Infections of both R.typhi,B.henselae and O.tsutsugamushi in farmers from Tianjin areas were popular and the antibody positive rates ofR.typhi,B.henselae and O.tsutsugamushi had an annual increase.

10.Familial aggregation of metabolic syndrome in adolescents with paternal metabolic syndrome

Objective To analyze the impact of parental Metabolic Syndrome(MS)on adolescents,and to explore the familial aggregation of MS with its components.Methods Using a 1:3 case-control familial study design to choose 26 MS male patients as proband and 78 healthy men as controls.Data regarding phenotype of their adolescence offspring were collected.Height,weight,waist circumference(WC),blood pressure,body mass index(BMI),waist-to-hip ratio(WHR)and waist-to-height ratio(WHtR)were measured or calculated.FPG,TC,TG,HDL-C and LDL-C were detected by automatic biochemical analyzer and hs-CRP was detected.Results WC,WHtR,WHR,DBP and hs-CRP of those adolescents with paternal MS were significantly higher than in controls(P<0.05).Rates of MS,Obesity depend WC,low level of HDL-C of adolescent with paternal MS were significantly higher than in controls(P<0.05).The rate of number on MS was significantly higher in case group than in control(r=0.231,P<0.05).Conclusion The phenotypes of MS were different between adolescents with or without parental MS,indicating that the familial aggregation of MS had been existed in their adolescent offspring,and mainly presented in central obesity,increased blood pressure and inflammation.
