1.Preliminary study of Conbercept injected intravitreally for the treatment of wet age-related macular degeneration

AIM:To observe the preliminary efficacy of conbercept injected intravitreally for the treatment of wet age-related macular degeneration(wAMD).METHODS:Seventeen wAMD patients (18 eyes) were selected to receive conbercept injection.All patients were given a single conbercept injection every month,3 times.Before and after 1,2,3mo of the injection,the best corrected visual acuity (BCVA),intraocular pressure (IOP,measured by Non-contact tonometer),fundus photography,fundus fluorescein angiography(FFA),indocyanine green angiography(ICG),optical coherence tomography(OCT) examination and the complications incidence were compared.RESULTS:Three months after conbercept injection,the BCVA improved in 15 eyes (83%),stable in 3 eyes (17%).Before treatment,the average central macular thickness was 421.72±54.43μm,at 1 and 2 and 3mo after treatment,the average central macular thickness was 337.89±25.88μm,293.56±26.87μm,266.89±19.10μm respectively.There were significant differences compared with before and after injection(P<0.05).In the final follow up,FFA and ICG showed that the leakage in macular area disappeared in 15 eyes (83%),still existed in 3 eyes (17%),in those 3 eyes the injection was given for one or two times till the leakage disappeared.Elevated intraocular pressure occurred in 2 cases (26mmHg,23mmHg),after 1d down to normal.Another patient showed postoperative envy,given left ofloxacin eye drops after 2d,then back to normal.There was no serious ocular adverse reactions.CONCLUSION:Intravitreal injection conbercept for wAMD can significantly improve the visual function,reduce the macular edema and the leakage with higher safety and less complications.However the prolonged efficacy needs further observation.
2.Relationship between clinicopathologic features and neoplasm recurrence,prognosis in hepatocellular carcinoma after liver transplantation

Objective To study the risk factors affecting neoplasm recurrence and survival of hepatocellular carcinoma(HCC)following liver transplantation(LT).Methods The clinicopatholo gic data,neoplasm recurrence and survival results of 118 patients with HCC receiving LT were retro- spectively analyzed and various clinicopathologic risk factors for neoplasm recurrence and survival were evaluated by univariate and multivariate analysis.Results The follow-up time ranged from 1 to 32 months.The recurrence rate was 37.3% and the mortality was 35.5%.The 12-,18-,24-month survival rate was 84.55%,70.30% and 62.24%,respectively.The 12-,18-,24-month neoplasm free survival rate was 69.05%,66.93% and 61.38%,respectively.In the univariate analysis,por- tal vein neoplasm thrombus(PVTT),Milan-criteria,neoplasm size,histological differentiation and pTNM stage were associated significantly with neoplasm recurrence,and PVTT,Milan-criteria,pre- operative alpha-fetoprotein(AFP),histological differentiation and pTNM stage were associated signif- icantly with survival rate;In the multivariate Logistic regression analysis,PVTT and histological dif- ferentiation were independent predictive factors of neoplasm recurrence,and multivariate Cox regres- sion analysis showed that PVTT and AFP independently associated with prognosis.Conclusions PVTT and histological differentiation are the most important predictive factors of neoplasm recur- rence,and PVTT and AFP independently predict the survival of patients undergoing LT.
3.Value of transrectal ultrasonography in the diagnosis of female traumatic urethral stricture

Objective To investigate the diagnostic value of transrectal ultrasonography (TRUS)for female traumatic urethral stricture.Methods Fifteen female patients with traumatic urethral stricture were examined by using TRUS and urethrography and the examination results were compared with op-eration findings.Results The ultrasonography results of all patients were in consistency with their operation findings,while the urethrography results of six patients were different from their operation findings.Conclusion TRUS is of great value in the diagnosis of female traumatic urethral stricture.
4.Application of Transnasal Gastroscope in Children With Upper Gastrointestinal Tract Disorders

Objective To explore the property and diagnostic significancec of transnasal gastroscope(EG-470N5)in children with upper gastrointestinal tract disorders.Methods Sixty children with upper gastrointestinal tract disorders were examined by using EG-470N5.The scope was inserted either through nose(n=28)or via mouth(n=32).Beside the routine preparations,nasal spray of lidocaine or nasal drib of ephedrine was added to those who received nasal intubation.Results Examination of upper gastrointestinal tract subjects in all succed with no obvious side response in 28 cases(46.7%)who received the examination via nose,27 cases(45%)had mild nausea and 5 cases(8.3%)had serious nausea and dysphoria.Children with esophagostenosis and pyloristenosis and 2-day-old neonate were examined without adverse effect.Conclusions Transnasal gastroscope is preponderant to children due to its slim diameter and supple body,especially to those having swallow difficulty and confined tract.It is more favorable practical prospect than routine gastroscope.
5."Efficacy Theory" may help to explain characteristic advantages of traditional Chinese medicines.

This article proposes the "Efficacy Theory" hypothesis of the traditional Chinese medicines (TCMs): TCMs take effects and weaken toxicities through the additive effects of numerous effective forms (including their constituents or/and metabolites) on a same target, the synergistic effects based on the overall action of the additive effects on individual targets and their toxicities scattering effects. A TCM may include approximately 1000 constituents and each constituent may produce about 100 metabolites in vivo after oral administration. Numerous effective forms of incalculable constituents and their metabolites could work like a "army group" together. When the quantity of a specific target molecule is larger than the pharmaceutical molecules, the molecules of different kinds of effective forms could combine with the target molecules successively, to exert the additive effects. When the target molecules are mostly occupied ("target most spaces occupied"), this TCM begins to work. The additive effects maybe exert not only in concentration but also in a time order way, which gives a sustained efficacy of TCM. The additive effects and the toxicities scattering effects are resulted from the same effective groups and not identical toxic groups among different effective form molecules. The "toxicities scattering effect" can be used to explain the non-toxic TCMs, but not fit for toxic TCMs. The efficacy theory showed that the variety of constituents and metabolites may participate in the process of pharmacodynamic actions, including the additive effects, synergy effects and toxicities scattering effects, which may be useful for explaining and developing the characteristic advantage of the TCMs. The questions we need to study or confirm are as follows: What are the TCMs' pharmacodynamic substance basis and mechanism made up of Why are toxicities of most TCMs' smaller How is the TCMs' "Efficacy Theory" which reflects characteristic advantage of TCMs applied in the research and development of new drugs.

As the preparation process from Salvia miltiorrhiz herbs to S. miltiorrhiz injection involves complicated technology and has relatively more factors impacting quality safety, the overall quality control is required for its effectiveness and safety. On the basis of the component structure theory, and according to the material basis of S. miltiorrhiz injection, we discussed the multi-dimensional structure and process dynamic quality control technology system of the preparation, in order to achieve the quality control over the material basis with safety and effectiveness of S. miltiorrhiz injection, and provide new ideas and methods for production quality standardization of S. miltiorrhis injection.

7.Re-evaluation of overnight low-dose dexamethasone suppression test in the diagnosis of Cushing syndrome

Objective To reinvestigate the value of overnight low-dose dexamethasone suppression test in the diagnosis of Cushing syndrome.Methods Fifty-two patients with Cushing syndrome and 153 patients with simple obesity or essential hypertension in whom Cushing syndrome was excluded were studied retrospectively in order to compare the sensitivity and specificity of different serum cortisol cut-off levels in overnight 1 mg dexamethasone suppression test in the diagnosis of Cushing syndrome.Results The sensitivity of 50% of basal serum cortisol level at 8:00 and of the serum cortisol cut-off levels of 275,200,138,50 nmoL/L at 8:00 after the overnight 1 mg dexamethasone suppression test was 92.3%,92.3%,92.3%,92.3% and 100.0% respectively, and the specificity was 90.8%,98.7%,96.1%,91.5% and 78.4%,respectively.Conclusion The serum cortisol cut-off level of 50 nmol/L in the overnight 1 mg dexamethasone suppression test has very high sensitivity and can be used as a screening test for Cushing syndrome.
8.PCNA, Bcl-2 and TERT expression in epiretinal membrane of rat traumatic proliferative vitreoretinopathy

· AIM: To explore the dynamic expression and correlation among telomerase catalytic subunit (TERT), proliferating cell nuclear antigen ( PCNA) and antiapoptosis protein Bd-2 which relate to cell proliferation in epiretinal membrane of rat traumatic proliferative vitreoretinopathy(PVR).· METHODS: S-P technique was applied for immunohistochemical staining of epiretinal membrane of traumatic PVR with TERT, PCNA and Bcl-2 antibody. HE staining was also carried out. The staining results were analyzed with image analysis system.· RESULTS: The positive rate and average A of PCNA protein were upregulated at first and then down-regulated, with the peak value in 14d Group, which was significantly different from those in 7d Group and 28d Group.The positive rate and average A of TERT and Bcl-2 were also upregulated at first and then down-regulated, with the peak value in 14d Group and 21d Group, which were significantly different from those in 7d Group. There was significant correlation among PCNA, Bcl-2 and TERT protein expression (P≤0.01).· CONCLUSTON: TERT and Bcl-2 take part in the regulation of proliferative cells in epiretinal membrane of traumatic proliferative PVR, with high correlation with the dynamic changes of cell proliferation.
9.Optimal position of locking compression plate for proximal humeral fractures:choice of lateral deltoid splitting approach?

BACKGROUND:Classical deltopectoral approach leads to a great injury to soft tissues. It is difficult to lay the steel plate on the lateral side. The steel plate is frequently laid on the anterolateral side of humerus. Fixed position is not ideal.
OBJECTIVE:To evaluate the safety and feasibility of lateral deltoid-splitting approach in treatment of proximal humeral fractures.
METHODS:From August 2008 to August 2013, 114 patients with fracture of surgical neck of humerus, who were treated in the Department of Orthopedics, Yan’an Hospital Affiliated to Kunming Medical University in China, were enrol ed in this study. They were treated with locking compression plate, and assigned to deltoid splitting approach group and deltopectoral approach group (n=57). Surgical trauma, fracture healing, curative effects, functional recovery and Constant shoulder score were compared between the two groups.
RESULTS AND CONCLUSION:Al patients were fol owed up for at least 8 months. Compared with the deltopectoral approach group, the proximal humeral fractures were healed as scheduled in the deltoid splitting approach group, and the anteflexion, abduction, shoulder lifting of the shoulder joint, and sensation of lateral skin of the upper arm were not impacted. No significant difference in Constant shoulder score was detectable between the two groups (P>0.05), which suggested that deltoid splitting approach for treating the proximal humeral fractures was safe and feasible. In addition, in accordance with the Neer’s classification system, the excellent and good rate in the deltoid splitting approach group (91%) was significantly higher than that in the deltopectoral approach group (75%) (P<0.05). Therefore, the deltoid splitting approach can be used as the first choice for the treatment of proximal humeral fractures.
