The effect of taurine on IL-1?induced inhibition of insulin release was investigated with cultured neonatal rat pancreatic islet cells.The results indicated that:①Insulin release, both the basal and glucose-stimulated,could be significantly inhibited by addition of IL-1?(5~20U/ml).②The inhibitory effect of IL-1? on insulin release could be reversed by taurine and this effect was directly related to the dose and action time of taurine.
2.Video-assisted thoracoscopic mini-invasive lobectomy: A clinical analysis of 35 cases

Objective To assess the value of video-assisted thoracoscopic mini-invasive lobectomy in the treatment of pulmonary benign lesions or malignancies. Methods A retrospectively analysis on clinical data of 35 cases of video-assisted thoracoscopic mini-invasive lobectomy, which were performed from February 2002 to June 2006, was made. The pathological diagnosis included 31 cases of peripheral lung cancer (T_1N_0-1M_0), 3 cases of bronchiectasis, and 1 case of atelectasis combined with pulmonary abscess. The procedures included 30 cases of anatomical lobectomy and 5 cases of non-anatomical lobectomy. Results No conversion to open surgery was required. The operation time was 85~210 min (115?35 min) and the intraoperative blood loss was 150~450 ml (210?55 ml). Postoperatively, acute respiratory failure was encountered in 2 cases, and mechanical ventilation was given for 3 and 5 days, respectively. Acute myocardial infarction occurred in 1 case, which was cured by early-stage thrombolysis. No postoperative complications were seen in the remaining 32 cases. The closed chest drainage was maintained for 2~10 days (mean, 5.4 days). In 3 cases of pulmonary benign lesions, follow-up checkups for 41, 33, and 13 months, respectively, found no long-term complications. In 25 cases of lung cancer, follow-up observations were made for 3~53 months (mean, 15.2 months), including 3~12 month in 6 cases, 12~24 months in 9 cases, 24~36 months in 7 cases, and 36~53 months in 3 cases. There were 4 cases of death due to cancer recurrence or metastasis (16%), 2 cases of with-tumor survival (8%), and 19 cases of disease-free survival (76%), respectively. Conclusions Video-assisted thoracoscopic mini-invasive lobectomy is a feasible and safe procedure for peripheral lung cancer and benign lung diseases.
3.RP-HPLC in determination of dextromethorphan and dextrophan in human urine:phenotype analysis of CYP2D6

Objective To establish a RP-HPLC method for determination of the concentrations of dextromethorphan and its metabolites dextrorphan in human urine.Methods Phenacetine was used as internal standard,and the urine sample was hydrolyzed by enzyme,alkalified and extracted with hexane-butanol(91).The separation was carried out on DiamonsilTM C18 column(250 mm?4.6 mm,5 ?m) with mobile phase of acetonitrile-1% triethylamine buffer solution(pH adjusted to 2.2 with H3PO4).Gradient elution was done for 0-15 min(20%-35% A).The flow rate was 1.0 ml/min.The detection wavelength was set at 280 nm and the column temperature was 40℃.Results The linear ranges of dextromethorphan and dextrorphan were 0.05-2.0 ?g/ml(r=0.999 9,n=5) and 0.5-20.0 ?g/ml(r=0.999 9,n=5),respectively,and their lowest detecting concentrations were 0.04 ?g/ml and 0.4 ?g/ml,respectively.The intra-day and inter-day precision were both less than 10%.The low,middle and high extraction recoveries were between 94%-108%.Conclusion Our method is accurate and sensitive,and is suitable for the CYP2D6 phenotype analysis and pharmacokinetic studies of dextromethorphan and its metabolites in human.
4.Effects of miR-122a on blood-spinal cordbarrier after spinal cord ischemia-reperfusion injury in rats

Aim To investigate the effects of miR-122a on blood-spinal cord barrier after spinal cord ischemia-reperfusion injury in rats.Methods Thirty-six SD rats were randomly divided into three groups:group of sham(S group),group of control(C group)and group of miR-122a antagomir(M group).Rats in S group were subjected to exposure of aorta arch but without occlusion.Spinal ischemia-reperfusion injury was induced by clamping the aorta arch for 14 min in C group and M group.Rats in M group and C group were intrathecally injected with miR-122a antagomir or antagomir control daily for three times after injury.The miR-122a expression in injured spinal cord tissue was detected by real-time PCR.The occludin expression in injured spinal cord tissue was detected by Western blot.The permeability of blood-spinal cord barrier was examined using evans blue as a vascular tracer.The neurological motor function was evaluated by Basso Beattie Bresnahan score.Results Compared with S group,the expression of miR-122a was increased,the expression of occludin was decreased,the permeability of blood-spinal cord barrier was increased,and neurological motor function score was decreased significantly in C group(P<0.05).Compared with C group,the expression of miR-122a was decreased,the expression of occludin was increased,the permeability of blood-spinal cord barrier was decreased,and neurological motor function score was increased significantly in M group(P<0.05).Conclusion miR-122a can regulate the expression of occludin and change the permeability of blood-spinal cord barrier.
5.Expression of PTPN13 in human gastric cancer and gastric cancer SGC-7901 cell line and its effect on cell proliferation and invasion

Objective To investigate the expression of PTPN13 in human gastric cancer and gastric cancer SGC-7901celllineanditsassociationwithproliferationandinvasion.Methods 106casesgastriccancertissuesamples and matched normal peritumorial tissues were collected .SGC-7901 cells were cultured and divided into two groups including pcDNA3.1-PTPN13 transfection group and without transfection group .Immunohistochemical technique was used to detecte protein expression of PTPN 13.The association of PTPN13 expression with tumor location , tumor size , depth of invasion and tumor metastasis were analyzed .The survival rate of patients with different PT-PN13 expression was calculated by Kaplan-Meier curves.CCK-8 assay was used to estimate the proliferation chan-ges.Using Trans-well assay analyzed the invasion of SGC-7901 cells.Moreover, Western blot was performed to de-tect the markers of EMT including E-cadherin, Snail and MMP9.Results Immunohistochemistry showed the expression rate of PTPN13 in gastric cancer tissues was lower than normal tissue (31 %vs 83%, P<0.05).The expression of PTPN13 in patients was related to with different tumor size , depth of invasion and tumor metastasis (P<0.05).The 2-year survival rate of patients with negative PTPN13 expression were lower.Overexpression reduced both proliferation and invasion in SGC-7901 cells.Up-regulated PTPN13 may increase E-cadherin level but decreases the level of Snail and MMP9 .Conclusions PTPN13 plays a role in gastric cancer tissue and cells as a tumor suppressor.Lower PTPN13 may predict a poor prognosis.PTPN13 may be used as therapy target in gastric cancer.
6.Pharmaceutical Care for Anti-infective Treatment of A Case of Septic Shock

OBJECTIVE:To investigate the role of clinical pharmacists in anti-infective treatment for the patient with septic shock. METHODS:Clinical pharmacists participated in anti-infective treatment for a patient with septic shock,assisted physicians to formulate individual regimen based on empirical anti-infective treatment,ESBLs producing Escherichia coli and anti-fungal treat-ment. Clinical pharmacists suggested using Itraconazole injection 200 mg,ivgtt,qd,for anti-fungal treatment;Itraconazole cap-sules 200 mg,po,bid,instead of fusidinic for anti-fungal treatment;meropenem 0.5 g,ivgtt,q6 h,for anti-infective treatment. RESULTS:Physicians adopted the suggestion of clinical pharmacists;the infection had been controlled after 16 d treatment and then the patient discharged from the hospital. CONCLUSIONS:Clinical pharmacists participate in clinical treatment,and assist phy-sicians to make a decision and formulate individual regimen,so as to promote rational drug use.

8.Transport efficiency and safety evaluation by process management during the handover of patients with tracheal intubation post-operation

Objective To evaluate transport efficiency and safety by process management during the handover of patients with tracheal intubation post-operation. Methods Prospective studies were performed between patients with or without process management during the handover. The time of handover were recorded and compared. The difference between systolic and diastolic blood pressure, heart rate, and arterial oxygen saturation were also recorded and compared. The adverse events during the handover were also investigated. Results It costed less time in the handover of patients under process management with significant difference[(4.75±0.54) min vs. (7.05±0.88) min, t=-17.21, P<0.01]. The incidence rate of harmful cases in the handover of patients under process management was significantly declined than that without process management[ 1.67%(1/60) vs. 13.33%(8/60),Χ2=4.324 3,P<0.05 ]. Conclusions Process management may facilitate the handover of patients with tracheal intubation post-operation and improve its safety.
9.Efficacy of Jian'ganle () versus Hugan Pian (), glucuronolactone and reduced glutathione in prevention of antituberculosis drug-induced liver injury.

Evidence-based medicine is advocated by WHO and adopted by developed countries for many years. In China, however, the selection of essential medicine and various medical insurance reimbursement schemes medicine is usually based on experts' experience of prescription practice which is under heavy critics resulting from the lack of related comparative efficacy and evidence-based research. The efficacy of Jian'ganle in prevention of drug-induced liver injury (DILI) caused by antituberculotics was evaluated in this study by comparison with Hugan Pian, glucuronolactone and reduced glutathione. Evidence was provided for relevant sectors such as Ministry for Human Resources and Social Security of the People's Republic of China and National Health and Family Planning Commission of the People's Republic of China to select and renew the Essential Medicine List (EML), the new rural cooperative medical scheme in China (NRCMS) list or the reimbursement list of industrial injury insurance. A total of 189 patients with initial pulmonary tuberculosis were divided into four groups who took antituberculotics combined with Jian'ganle, Hugan Pian, glucuronolactone and reduced glutathione respectively. Their liver function profile including alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), direct bilirubin (DBIL), total protein (TP), albumin (A) and globulin (G) were detected at admission as baseline and after treatment. The Jian'ganle group was compared with the three others by chi-square tests. In an aspect of maintaining bilirubin indexes normal, Jian'ganle was more efficacious than glucuronolactone. And Jian'ganle had a little more efficacy than reduced glutathione to maintain protein indexes normal as well. And the therapeutic regimen of antituberculotics combined with Jian'ganle was the best in treating tuberculosis and preventing DILI at the same time. The study showed that among the four hepatinicas which demonstrated similar prevention of DILI caused by antituberculotics, Jian'ganle has more advantages over the three others to some extent, which provides a reliable basis for health sectors to select and renew the EML, NRCMS List or the reimbursement list of industrial injury insurance.
10.Anti-bacterial Drugs Applying in Liver Transplanting Patients During Perioperative Period:A Survey and Analysis

OBJECTIVE To find out the situation of the application of anti-bacterial drugs in liver transplantation perioperative patients and to improve the quality of their usage.METHODS According to the criteria of DDD and DUI recommended by WHO,making a random sampling survey of the application of anti-bacterials in 206 liver transplantation perioperative patients who discharged hospital during Jan-Dec 2006,and then to analyze the rationality of their application.RESULTS Among the 206 patients,100.0% patients had received anti-bacterial drugs involving 13 kinds,137 patients in them used one,31.1% used two and 2.4% used three,and there were twelve in thirteen kinds of drugs whose DUI was no more than one.CONCLUSIONS The survey has proved that the liver transplantation perioperative patients have received rational therapy.
