2.Efficacy and safety of glucocorticoids in community-acquired pneumonia: A systematic review and meta-analysis of randomized controlled trials

Objective To evaluate efficacy and safety of glucocorticoids for community-acquired pneumonia (CAP).Methods We searched relevant randomized controlled trials (RCT) from Pubmed,EMBASE,Cochrane Library,Chinese Journal Full-text Database,Chinese Biomedical Literature Database and traced the related reference to obtain the information that had not been found by using key words of steroids,glucocorticoids,cortisol,corticosteroids,community acquired pneumonia and CAP.The quality of RCT included was evaluated.Meta-analysis was performed with RevMan 5.0 provided by the Cochrance Collaboration.Results Seven RCTs involving 944 patients were included in the meta-analysis.Mean length of hospital stay in glucocorticoids treatment group was significant shorter than that in conventional treatment group (WMD =-1.70,95% CI:-2.01-1.39,Z =10.81,P <0.01).No significant differences were found regarding mortality (RR =0.77,95% CI 0.46-1.27,Z =1.03,P =0.30),mean length of ICU stay (WMD =1.17,95% CI:-1.68-4.02,Z =0.81,P =0.42),the risk of double infection (RR =1.32,95% CI 0.66-2.63,Z=0.79,P=0.43),the incidence of hyperglycemia (RR=1.84,95% CI0.76-4.41,,Z=1.36,P=0.17),the incidence of upper gastrointestinal bleeding (RR =1.98,95% CI O.37-10.59,Z =0.80,P =0.42) between conventional treatment group and glucocorticoids treatment group.Conclusions Glucocorticoids treatment used in patients with community-acquired pneumonia may shorten the course of illness and possess better safety,but could not reduce the mortality.

4.Drug susceptibility of wild-type and mutant H7N9 neuraminidase to zanamivir and oseltamivir.

This study aimed to investigate the drug susceptibility of wild-type and mutant avian influenza A (H7N9) virus neuraminidase (NA) to oseltamivir and zanamivir. Codon optimized DNA of H7N9 (A/ Hangzhou/1/2013) NA was synthesized and constructed into the pcDNA3.1/His vector (NA(H7N9-WT)). Mutant NA(H7N9-H274Y) and NA(H7N9-R292K) plasmids were constructed by directed mutagenesis PCR using NA(H7N9-WT) plasmid as the template followed by sequencing. NA plasmids were transfected into 293T cells and cell lysates containing NAs were collected 48 h post-transfection. Wild-type and mutant NAs were analyzed by Western blotting and their activities were tested by the 4-MUNANA-based assay. All three NAs were expressed and enzymatic activities were confirmed. The effects of oseltamivir and zanamivir on all three NAs were then tested. It showed that the half maximal inhibitory concentrations (IC50s) of oseltamivir carboxylate on NA(H7N9-WT), NA(H7N9-H274Y) and NA(H7N9-R292K) were 1.6 nM, 15.1 nM, and > 1 000 nM with fold changes of 9 and > 625, respectively. The IC50 values of zanamivir on NA(H7N9-WT), NA(H7N9-H274Y), and NA(H7N9-R292K) were 1.1 nM, 1.4 nM, and 38.0 nM with fold changes of 1.3 and 34, respectively. These results indicated that oseltamivir and zanamivir could significantly inhibit NA(H7N9-WT). NA(H7N9-R292K) showed high-level resistance to both drugs (34-fold and 625-fold) and NA(H7N9-H274Y) was sensitive to both (1.3-fold and 9-fold). These results indicated that both oseltamivir and zanamivir could be used for patients infected with the H7N9 virus. However, when patients carried the H7N9 virus with a NA R292K mutation, other medications would be preferred over oseltamivir or zanamivir.

5.Preparation and self-assembly process of fibrillar collagen sponges

BACKGROUND:An ideal scaffold material needs appropriate degradation rate and certain mechanical properties, but the traditional colagen sponge scaffold has rapid degradation velocity and low mechanical strength, which is easy to colapse and difficult to maintain its natural form. Traditional cross-linking methods also have the problems of cytotoxicity or colagen denaturation, severely limiting the application of colagen. OBJECTIVE:To design a new cross-linking method for colagen and to optimize the self-assembly process so as to develop a colagen sponge scaffold with good mechanical properties and resistance to degradation. METHODS:Colagens were modified by self-assembly technology to prepare colagen fibrils which were then freeze-dried into fibrilar colagen sponges. Meanwhile, we optimized the conditions of self-assembly by using orthogonal experiment based on univariate analysis of the effect of initial colagen mass concentration, final phosphate concentration and pH value on the conversion yield of colagen self-assembly. RESULTS AND CONCLUSION:We optimized the conditions of self-assembly revealed that the optimum conditions to prepare colagen fibrils were determined as pH=8.0, initial colagen concentration=2 mg/mL, and final concentration of phosphate=15 mmol/L. The results of scanning electron microscope showed that fibrilar colagen sponges were characterized by refined porous structure which was connected by colagen fibrils. In addition, the fibrilar colagen sponges showed better equilibrium-sweling ratio, water retaining property and mechanical strength compared with unmodified colagen spondages (P < 0.05), to solve the problems in rapid degradation.
6.Therapeutic effect analysis of MSM after highly active antiretroviral therapy in Chongqing

Objective To know basic characteristics of MSM infected with HIV treated by highly active antiretroviral thera‐py(HAART) ,and evaluate therapeutic effect based on the level of CD4+ T cell counts .Methods Basic database of patients were collected from the national AIDS comprehensive data information system ,patients were observed by the retrospective cohort study , and effect indicators were analyzed based on the level of CD4+ T cell counts .The data was analyzed by SPSS18 .0 software .Results A total of 1 758 cases were recruited in this study and the average age was (33 .5 ± 11 .1)years .The component ratio of the CD4+T cell counts <50/μL ,50 to 199/μL ,200 to 350/μL ,> 350/μL were 7 .7% ,27 .6% ,54 .9% ,9 .7% ,respectively .The CD4+ T counts increased and kept among 150 to 250/μL ,250 to 350/μL ,350 to 450/μL ,450 to 550/μL ,respectively after 12 months ,treat‐ment .The rates of viral inhibition were 70 .2% ,81 .7% ,84 .2% ,83 .3% ,respectively after 6 months′treatment .And the cumulative survival rates were 0 .81 ,0 .93 ,0 .98 ,1 .0 after 36 ,months treatment ,respectively .Conclusion HAART on MSM achieves remarka‐ble results .And the early treatment was more beneficial to improve immune function ,reduce AIDS mortality and control the HIV/AIDS epidemic of MSM .
7.The relationship between shear wave velocity and the ratio of typeⅠ/typeⅢ collagen fiber in the carotid plaques

Objective To assess the correlation between the shear wave velocity(SWV) and the ratio of type Ⅰ /type Ⅲ collagen fibers in plaques using virtual touch tissue imaging quantification(VTIQ).Methods Fifty four patients (54 plaques) who scheduled to carotid endarterectomy(CEA) were enrolled in this study.Their carotid plaques were preoperatively examined using VTIQ.The value of SWE of each plaque was obtained.The SWV of the plaque was evaluated by VTIQ.The endarterectomy specimens were transferred to the pathological department immediately after surgery to evaluate the plaque types and the distribution of collagen fibers in plaques.According to HE staining,the 54 plaques were divided into stable and unstable group.Results Among 54 plaques,14 were stable plaque and 40 were unstable plaques.The thickness and stenosis rate had no significant difference between stable group and unstable group (both P > 0.05).The mean value of SWV and the ratio of type Ⅰ/typeⅢ collagen fiber in stable plaques were higher than those in the unstable plaques (both P <0.05).Based on ROC analysis,a cut-off value of SWE of 3.725 m/s (AUC=0.936) showed a sensitivity of 92.9%,a specificity of 80.0% and an accuracy of 83.3% for predicting stable plaques (95% CI 0.818-0.982,P <0.05).The value of SWVs was closely correlated to the ratio of type Ⅰ/typeⅢ collagen fiber in plaques(r =0.882,P <0.05).Conclusions SWV of the carotid atherosclerotic plaque can be used to reflect the ratio of the type Ⅰ/typeⅢ collagen fiber in the plaques,and to evaluate the stability of plaque.
8.Progress in Ginsenosides Biosynthesis and Prospect of the Secondary Metabolic Engineering for the Production of Ginsenosides

Ginsenosides,which belongs to triterpenoid saponins of plant terpenoids,are the main active components of the valuable medicinal herbs ginseng and American ginseng.Recent studies show that ginsenosides have a variety of beneficial effects,including anti-inflamatory,antioxidant,and anticancer effects.Ginsenosides are synthesized by complicated modification of triterpenoid skeleton after cyclization of 2,3-oxidosqualene through triterpene saponin synthesis pathway in which they share common early steps with phytosterols synthesis.It outlines recent advances for the biosynthesis of ginsenosides,the cloning and characterization of genes encoding key enzymes in the pathway and the basal framework of ginsenosides biosynthesis pathway.The prospects of secondary metabolism engineering in the biosynthesis of plant natural products and its application in ginsenosides biosynthesis are also discussed.

Objective To investigate the therapeutic effects of Smad7 and urokinase-type plas- minogen activator(uPA)co expression on CCl_4-induced rat liver fibrosis.Methods Forty SD rats were subcutaneously injectied of 40% CCl_4 every three days for 8 weeks.The rats were then divided into model group,AdSmad7/uPA group(injected with AdSmad7/uPA via tail vein),AdSmad7 group(injec- ted with AdSmad7 via tail vein)or AdGFP group(injected with AdGFP via tail vein).Ten healthy rats were served as control.The serum levels of procollagenⅢ(PCⅢ)and laminin(LN)were determined by radioimmunoassay,and the hydroxyproline level in liver tissues were examined by alkaline hydrolysis.
