1.Analysis of human brucellosis monitoring results in Baotou city of Inner Mongolia in 2010

ObjectiveTo find out the prevalence of brucellosis in Baotou,and to provide a scientific basis for integrated control of brucellosis.MethodIn 2010,high-risk people 7 to 70 years of age engaged in livestock farming,grazing,slaughtering,processing and selling were investigated in 9 districts (banners,counties)of Baotou city.In accordance with the principle of informed consent,blood samples were collected,serum titer was tested using the rose bengal agglutination test and tube agglutination test(SAT),and serum titer of 1:100(+ +) was considered as positive brucellosis,and results were evaluated based on brucellosis monitoring standards(GB 16885-1997).ResultsA total of 5832 copy blood samples were collected,and 250 cases were tested positive,the positive rate was 4.25%.The number of new cases was 164,accounting for 2.81% (164/5832).Age was mainly concentrated in the 40 to 60 years,accounting for 66.0%(165/250).Brucellosis infection in the livestock industry accounted for 80.87% (4757/5882).From the regional distribution,DAMAOQI had the most detected cases,and the detection rate was 12.10% (73/603).ConclusionsContinue to increase the brucellosis prevention knowledge propaganda,carry out brucellosis surveillance of high-risk people,quarantine,and strengthen immunity is the only measure of effective control of brucellosis epidemic.
2.Analysis of monitoring results of drinking water-borne endemic arsenic poisoning in Tuyouqi, Baotou of Inner Mongolia in 2010

Objective To understand the arsenic level in drinking water and the current situation in prevention and control of endemic arsenic poisoning in Tuyouqi,Baotou of Inner Mongolia.Methods Through door-to-door survey,we selected patients with arsenic poisoning in order to monitor the arsenic poisoning condition of seven villages(Gangfangying,Laohuyao,Shilaozhang,Youfangying,Wanglaosi,Dajing and Xiaojing) in Tuyouqi,Baotou in 2010,and the disease was diagnosed based on Standard of Diagnosis for Endemic Arsenism(WS/T 211-2001).We also investigated the operation of Salaqi Water Company,Maodai Water Company and the Laohuyao water improvement project.We collected two samples per place in both low water period and high water period to measure water arsenic with silver diethyldithiocarbamate spectrophotometry(WS/T 28-1996).Results Three water improving projects were operating in normal conditions.Water arsenic in Salaqi Water Company and Maodai Water Company was level less than 0.01 mg/L,which met the National Safety Standards of drinking water with arsenic.Water arsenic in Laohuyao improvement project was 0.053 mg/L,which was higher than the drinking water safety standards.The seven villages in Tuyouqi had improved their water conditions.Among them,the arsenic level in Laohuyao was 0.052 mg/L,and the remaining six villages were < 0.05 mg/L.There were 6 suspected cases,14 mild cases,4 moderate cases and 10 severe cases of the 2095 people monitored and no new cases found.The age of patients with arsenic poisoning was in the range of 40 to 70.Conclusions The monitoring results show that drinking water arsenic level in some villages in Tuyouqi is still excessive,but no new arsenic poisoning patients in arsenic poisoning areas are found.
3.Effect of Comprehensive Rehabilitation on Breast Cancer Related Lymphedema

Objective To observe the effect of comprehensive rehabilitation on breast cancer related lymphedema. Methods 70 patients with breast cancer related lymphedema were divided into experiment group (n=35) and control group (n=35). Both groups received intermittent pneumatic compression, while the experiment group received comprehensive rehabilitation including manual lymphatic drainage, muscle stretching, expanded the range of motion of shoulder, gradual resistance training, high frequency electrical therapy and health education. They were assessed with Disabilities of Arm, Shoulder and Hand Scale (DASH) and development of lymphedema before and 8 weeks after treatment. Results DASH scores and the development of lymphedema improved in both groups after treatment (P<0.05), and improved more in the experiment group than in the control group (P<0.05). Conclusion Comprehensive rehabilitation can further relieve breast cancer related lymphedema, and improve the upper extremity function.
4.Effects of lung protective ventilation strategy on inflammatory mediators in bronchial alveolar lavage fluid in pulmonary and extrapulmonary acute respiratory distress syndrome

Objective To observe the effects of lung protective ventilation strategy (LPVS) on inflammatory mediators in bronchial alveolar lavage fluid (BALF) in pulmonary and extrapulmonary acute respiratory distress syndrome(ARDS).Methods All of 62 extrapulmonary ARDS(ARDSexp) cases and 60 pulmonary ARDS (ARDSp) cases were divided into observation group and control group by table of random digit.ARDSexp and ARDSp observation group were received LPVS treatment [tidal volume 6-8 ml/kg,positive end expiratory pressure (PEEP) 5-15 cm H2O (1 cm H2O =0.098 kPa)],and ARDSexp and ARDSp control group were received the routine ventilation strategy (tidal volume 10-12 ml/kg,PEEP 4-8 cm H2O).All groups were treated with the same conventional therapy of ARDS,same model and parameter of mechanical ventilation.The level of tumor necrosis factor (TNF)-α,interleukin (IL)-6,IL-8 in BALF were tested by enzyme linked immunosorbent assay on the 1st,4th,7th day in each group.The oxygenation index was tested at 8 o'clock each morning.Results There were 4 ARDSexp cases and 6 ARDSp cases who were expelled,because of death within 7 days.ARDSexp observation group was in 28 cases,ARDSexp control group was in 30 cases,ARDSp observation group was in 28 cases,ARDSp control group was in 26 cases.The oxygenation index had no significant difference between ARDSexp and ARDSp observation group and corresponding ARDSexp and ARDSp control group on the 1st and 2nd day (P > 0.05).But the oxygenation index in ARDSexp and ARDSp observation group were significantly higher than that in ARDSexp and ARDSp control group from 3rd to 7th day (P < 0.05).The oxygenation index in ARDSexp observation group was significantly higher than that in ARDSp observation group except 1st day (P <0.05).The oxygenation index in ARDSexp and ARDSp observation group were significantly better with the time passing (P < 0.05).The BALF levels of TNF-α,IL-6 and IL-8 in ARDSexp and ARDSp observation group were significantly lower than those in corresponding ARDSexp and ARDSp control group on the 4th and 7th day (P< 0.05).The BALF levels of TNF-α,IL-6 and IL-8 in ARDSexp observation group and ARDSexp control group were significantly lower than those in corresponding ARDSp observation group and ARDSp control group (P< 0.05).The BALF levels of TNF-α,IL-6 and IL-8 in ARDSexp and ARDSp observation group were decreased significantly with the time passing (P <0.05).Conclusion It is more reasonable to decrease the BALF levels of inflammatory mediators,increase oxygenation index for ARDS patients to with LPVS treatment,then for ARDSexp patients obviously.
5.Study on synthesis and liberation of NO and IL-1? from fascia tissue cells of rat under pressure stimulation in vitro

Objective:To provide some experimental basis of cell biomechanics for clinic treatment of massage by exploring synthesis and liberation of NO and IL-1? from fascia tissue cells of rat under pressure stimulation in vitro.Methods:Fascia tissue cells of rat were cultured in vitro.After morphological identi cation,50kPa pressure was forced on these cells,then the contents of NO and IL-1? in culture medium were detected.Results:The fascia tissue cells are mainly made up of broblast cells.The contents of NO and IL-1? in culture medium were both increased signi cantly under pressure stress.Conclusion:Fascia tissue cells can receive the mechanical stimulation,and transform the mechanical signals into biological signals.The reaction to mechanical stimulation may be one of the cell biomechanical principles of massage.
6.Human bone marrow derived mesenchymal stem cells regulate Th17 cells of peripheral blood in vitro

Objective To investigate the in vitro effects of human bone marrow derived mesenchymal stem cells (hBMSC) on Th17 cells of the human peripheral blood.Methods The density gradient centrifugation combined with lymphocyte separation medium was used to isolate hBMSC,which were then cultured.Cytokine IL-17 in the peripheral blood from a healthy person was measured by enzyme-linked immunosorbent assay (ELISA).Proportion of Th17 cells was evaluated by flow cytometry.Results The expression level of IL-17 in spent culture supernatant of the healthy person PBMC and AML hBMSC was (292.32±37.25) pg/ml,and was significantly higher than that of the healthy person PBMC and healthy hBMSC [(169.64±17.47) pg/ml,P < 0.01].There was no significant difference between the expression level of IL-17 in spent culture supernatant of the healthy person PBMC and ALL hBMSC [(159.89±23.71) pg/ml] and the level of the healthy person PBMC and hBMSC.The percentages of Th17 cells of co-culture systems from hBMSC,ALL-hBMSC,and AML-hBMSC and PBMC were (10.13±2.19) %,(13.77±4.04) % and (21.53±5.05) %,respectively,with the result between AML patients and healthy people being statistically different (P < 0.01).ALL patients and healthy people showed no difference (P > 0.05).Conclusion AML-hBMSC promotes the CD~ T cells to generate Th17 cells,which suggests that the MSC from AML marrow may play a role in the regulation of immune suppression.
7.Application of 3% hypertonic saline in patients with severe craniocerebral injury after operation

Objective To study the application of 3% hypertonic saline (HS) in patients with severe craniocerebral injury after operation.Methods Sixty-eight cases with severe craniocerebral injury after operation were divided by random digits table method into HS group and control group with 34 cases each.The patients in control group were treated with 125 ml 20% mannitol every 6 hours or 8 hours.The patients in HS group were given conventional mannitol and added with 130 ml 3% HS every 12 hours or 8 hours through fast intravenous drip altemated with mannitol.The levels of intracranial pressure (ICP),mean arterial pressure (MAP),central venous pressure (CVP) were recorded within 6 hours,12 hours,24 hours of the 1st day,24 hours of the 2nd day,24 hours of the 3rd day,24 hours of the 5th day.The Na+ level of blood serum and the plasma osmotic pressure were recorded and calculated within 12 hours,24 hours of the 1st day,24 hours of the 2nd day,24 hours of the 3rd day,24 hours of the 5th day.The Glasgow coma scale( CCS) score was accounted within 24 hours of the 1 st day,24 hours of the 3rd day,24 hours of the 5th day.Results Compared with control group,the level of ICP in HS group decreased at every time point (P < 0.05).The tendency of ICP in two groups was not significant on the 1st and 2rid day (P > 0.05 ),but the level of ICP within 24 hours of the 3rd and 5th day obviously increased compared with those of the 1st and 2nd day (P<0.05).The levels of MAP and CVP in HS group were significantly higher than those in control group within 6 hours,12 hours and 24 hours of the 1 st day and 24 hours of the 2nd day [ ( 87.98 ± 5.03 ),(88.56 ± 5.36),(87.04 ±6.90),(90.03 ±5.19) mm Hg (1 mm Hg =0.133 kPa) vs.(77.98 ±5.09),(79.98 ±6.09),(80.98 ± 5.27),(81.98 ± 4.32) mm Hg and (9.23 ± 1.24),(9.67 ± 1.35),( 10.21 ± 1.38 ),( 10.56 ± 1.96)mm Hg vs.(7.15 ± 2.01 ),(8.32 ± 1.53),(8.67 ± 1.89),(9.22 ± 2.03) mm Hg] (P < 0.05),but the values within 24 hours of the 3rd and 5th day between two groups had no significant differences (P> 0.05 ).The Na+ level of blood serum and the plasma osmotic pressure in HS group at every time point was obviously higher than that in control group (P< 0.05).The GCS scores within 24 hours of the 1st,3rd and 5th day in HS group were (4.21 ± 2.31 ),(5.44 ± 2.46 ),(7.23 ± 1.64 ) scores,respectively,while the scores in control group were (4.14 ± 2.10),(5.15 ± 2.31 ),(7.31 ± 2.12) scores,respectively ;the score within 24 hours of the 5th day in two groups was obviously higher than that of the 1st day respectively (P< 0.05 ),but the scores between two groups had no statistical significance (P> 0.05 ).Conclusions Adding 3% HS is more effective to decrease ICP,improve the brain perfusion and reduce the adverse reactions.3% HS can be used as the first-line therapy for patients with severe craniocerebral injury after operation.
8.Changes in default mode network functional connectivity of resting-state functional magnetic resonance imaging in partial epilepsy

Objective To explore changes of brain function among patients with partial epilepsy in resting state by using the blood oxygenation level dependent functional magnetic resonance imaging (fMRI) techniques.Methods fMRI scanning was performed in resting state among 60 patients with partial epilepsy and 60 gender,age and educational levels matched normal controls.The functional connectivity analysis was applied to calculate the default mode network ( DMN ) functional connectivity in resting-state fMRI.SPM5 was used to analyze differences in functional connectivity between the two groups( P <0.001,cluster >50).Results Left precuneus and adjacent posterior cingulate cortex ( Pcu/PCC),angular gyrus,and cingulate gyrus were involved in the DMN of epileptic patients.By contrast,the DMN of controls included left Pcu/PCC,right angular gyrus,bilateral medial frontal lobe and temporal lobe.Compared with normal controls,patients with partial epilepsy showed a significantly decrease in functional connectivity of DMN region such as left inferior parietal lobule,supramarginal gyrus,parahippocampa gyrus and superior temporal gyrus,and bilateral uncus,while no regions were found increased functional connectivity in patients group.Conclusions Patients with partial epilepsy show abnormal changes in functional connectivity of DMN in resting state by fMR],which may associate with the potential pathophysiological mechanisms of epilepsy.The findings demonstrate that the resting-state fMRI might detect the extensive changes of brain function in partial epilepsy with negative results of conventional MRI,suggestive of fMRI as an effective and non-invasive method to explore brain function in epilepsy.
9.A pilot study of the ERCC1 and XPF genes in forensic age estimation

Objective The aim of this study is to detect the mRNA and protein expression levels of ERCC1 and XPF genes among different age groups of healthy Chinese Han individuals,and to analyze the correlation between the mRNA and protein expression levels andthe age of individuals in order to find new molecular markers for forensic age estimation.Methods Peripheral blood samples were obtained from 150 unrelated healthy Chinese Han individuals.The plasma was centrifuged from the whole blood by gradient centrifugation,and the totalRNA was extractedwithTrizol fromperipheral blood mononuclear cells(PBMCs).Real-time quantitative reverse transcription polymerase chain reaction (RT-qPCR) was used to quantitatively analyze the mRNA relative expression levels of ERCC1and XPF in PBMCs.Enzyme linked immunosorbent assay (ELISA) was used to quantitatively analyze the protein expression levels of ERCC1and XPF in plasma.Results There were no significant differences in the mRNA relative expression levels of ERCC1 and XPF in PBMCs between males and females(P>0.05).Significant differences were found in the mRNA relative expression levels of ERCC1 and XPF between different age groups (P<0.05).Regression analysis showed thatthe mRNA relative expression levels of ERCC1 and XPF were both negatively correlated with age.The correlation coefficients(r) were-0.578 and-0.844,respectively.When the age was used as independent variable(x) and the mRNA expression relative level as dependent variable (y),the fitting curveswere Y=3.3E-5X2-0.0261X+1.9175 (R2=0.3244,P<0.01),Y=0.0003X2-0.0459X+2.0439 R2=0.729,P<0.01),respectively.There were no significant differences inthe protein expression levels of ERCC1 and XPF in plasma between different age groups or genders (P>0.05).Conclusion The mRNA relative expression levels of ERCC1 and XPF in PBMCsdeclined with the increase of age,however,the protein expression levels in plasma were unrelated to age.ERCC 1 and XPF genes can be used asnew molecular markers for forensic age estimation,so as to providetheoretical basis for establishing the mathematical model of ERCC1/XPF genesin concern ofindividual ages.
10.Clinical features, electroneurophysiology, neuroimaging and gene analysis of one juvenile dentatorubral-pallidoluysian atrophy pedigree

Objective To explore the clinical features,electroneurophysiology,neuroimaging and gene characteristics of one juvenile dentatorubral-pallidoluysian atrophy (DRPLA) pedigree with an onset of epilepsy.Methods The clinical data of the elder sister and younger brother in a family with juvenile DRPLA were collected.Furthermore,their clinical manifestations,electroneurophysiology results,neuroimaging characteristics and atrophin-1 gene CAG repeat numbers were detected and analyzed in detail.Results There were four patients in this family in total.The probands were two siblings,and they both had the onset manifestation of epilepsy.The younger brother had frequently epileptic seizure,marked cerebellar ataxia,involuntary movement and mental retardation.Compared with her younger brother,the sister had light symptoms such as mild memory deterioration without ataxia and involuntary movement,and she could undertake some simple work.The spike wave and sharp wave complex can be detected in electroencephalogram (EEG) examination,the cortical center segment lesions pathological changes were revealed in somatosensory evoked potentials (EP),and the latency period of P300 was prolonged in the both siblings.Magnetic resonance imaging (MRI) showed that the younger brother had marked atrophies in the cerebral cortex,brainstem and cerebellum.Furthermore,MRI showed that the elder sister had only mild atrophies in the cerebral cortex,brainstem and cerebellum,and that on the contrary some abnormally high signals were observed in cerebral cortex but not white matter.DRPLA gene detection revealed that the numbers of CAG repeats were 15/68 (the younger brother) and 15/64 (the elder sister),respectively.Conclusions Epilepsy,especially the myoclonus,is a common clinical manifestation for juvenile DRPLA,and many other types of epileptic seizures may arise with the development of DRPLA.DRPLA has diverse clinical heterogeneity.EEG,EP and brain MRI examination are great for DRPLA diagnosis and differential diagnosis,and the specific gene detection can be helpful for a definitive diagnosis.
