1.Diagnosis and treatment of hypocalcemia in critical illness children

Hypocalcemia often exists in critical illness children. Most of the sick children do not have an underlying disease of calcium homeostasis, and the mechanism of the hypocalcemia is complicated. Abnormal values of blood calcium may be a sign of critical illness and normalize spontaneously with resolution of the primary disease process. Hypocalcemia may be a autoprotection , and it may be harmful if we try to correct it. This article focuses on the theory and clinical treatment of hypocalcemia in critical illness children.
2.Statistics analysis of tri-allelic patterns in STR loci.

OBJECTIVE:
To investigate the statistical method of tri-allelic patterns of STR loci.
METHODS:
The DNAs of venous blood and blood stain samples from 8,846 unrelated individuals were extracted using magnetic bead method, and STR genotypes were determined by multiplex fluorescent amplification and capillary electrophoresis and analyzed using GeneMapper ID v3.2 software. The genotype frequency and allele frequency of trizonal were determined by direct counting and formula, respectively, in order to deduce the formula of trizonal in paternity test and individual recognition.
RESULTS:
Of the 8,846 individuals, four tri-alleles and three tri-genotypes were detected. The multiplying of allele frequency and actual rate showed significantly statistical difference. The formula of trizonal in paternity test and individual recognition was successfully deduced.
CONCLUSION
The frequency of the two alleles inherited as a whole in the population could be calculated by multiplying the frequencies of each allele in the tri-allelic patterns.

Remote ischemic postconditioning can induce cerebral ischemic tolerance and have neuroprotective effects.It has provided a novel strategy for the treatment of ischemic cerebrovascular disease.In recent years,the mechanism research of remote ischemic postconditioning and clinical applications have made significant progress and have shown encouraging prospects of clinical transformation.
4.Progress in the investigation of cardiovascular protective effects of DPP-4 inhibitors

Dipeptydil-peptidase-4 (DPP-4) inhibitors are designed as oral anti-hyperglycemic drugs for the treatment of type 2 diabetes owing to their effects on the incretin system through sparing incretin hormones including glucagon-like peptide-1 (GLP-1) from rapid degradation by DPP-4.It has been shown that GLP-1 signaling may exert beneficial effects on cardiovascular system.In addition to GLP-1,DPP-4 physiologically cleaves cytokines,chemokines,and neuropeptides involved in inflammation,immunity,and vascular function.It means that DPP-4 inhibitors hold promise for cardiovascular protection independent of GLP-1.And its modulation of endothelial progenitor cells,infiammatory pathway,ischemic response,and lipid metabolism emerges as the major cardiovascular target of DPP-4 inhibitors.
5.Quality of life in Chinese women treated surgically for breast cancer with one of three different procedures

Objective To determine the quality of life of patients treated with one of three different types of surgery for breast cancer.Methods This was a cross-sectional study using a questionnaire survey completed by Chinese patients without active disease after at least 2 years of follow-up after breast cancer surgery.Results This study totally included 139 breast cancer patients:44 ( 31.6%) had undergone modified radical mastectomy with reconstruction, 41(29.5%)had a quadrantectomy with axillary lymph node dissection, and 54(38.9%)had a modified radical mastectomy without reconstruction.The EORTC QLQ-C30 and EORTC QLQ-BR23 question-naires were used;their reliability was>0.82.Global health status(94.30 ±12.04, P=0.028)and role functio-ning(85.16 ±17.23, P=0.138)were highest in the quadrantectomy group.Pain score was highest in the modi-fied radical mastectomy with reconstruction group(26.13 ±30.15, P =0.042).The breast symptom score (22.56 ±22.30, P=0.009)and body image perception(85.56 ±19.72, P=0.025)were highest in the conser-vative treatment group.The overall health of patients given modified radical mastectomy without reconstruction was lower(72.61 ±20.89, P=0.014) in women older than 50 years compared with younger women.Conclu-sions The quadrantectomy with axillary lymph node dissection procedure had better acceptance, but the overall health status did not differ between groups.Overall health status is lower in women older than 50 years receiving a modified radical mastectomy without reconstruction.

7.The study of infection situation and antibacterial resistence of Pseudomonas aeruginosa in patients with lower respiratory tract infection in respiratory intensive care unit

Objective To investigate the infection situation of Pseudomonas aeruginosa (PA) in patients with the lower respira‐tory tract infection in the department of respiratory intensive care units (RICU ) in recent five years ,and to analyze the changing trend of antibacterial resistence ,in order to guide rational selection of antimicrobial agents .Methods Strains of bacteria were isola‐ted and identified from sputum specimen of patients in the department of RICU and common ward from Jul .2008 to Jul .2013 .The situations of PA infection and antibacterial resistence were analyzed ,and differences of infection rates of PA and antibacterial resis‐tence were compared between RICU and common ward .Results 517 strains of bacteria were detected from sputum specimens of patients in the department of RICU from 2008 to 2013 ,including 141 strains of PA (accounted for 27 .3% ) ,and ticarcilli/clavulanic acid(61 .0% ) was with the highest rate of resistance among 14 drugs and colistin B(9 .2% ) was with the lowest rate of resistance . 378 strains of bacterias were detected from sputum specimens of patients in common wards ,including 125 strians of PA(accounted for 33 .1% ) ,and ticarcilli/clavulanic acid(28 .0% ) was with the highest rates of resistance and colistin B (4 .0% ) was with the low‐est rate of resistance .Conclusion In recent 5 years ,PA might be one of the main pathogenic bacterias of respiratory tract infections in the depatment of RICU in this hospital ,and antibiotic resistance may increse gradually .The antibiotic resistance in the depatment of RICU may be higher than that in cionmon wards .Clinical effective measures should be taken to prevent nosocomial infection ,and rational use of antibiotics should be taken to reduce the emergence of resistant strains .
8.The clinical analysis of 27 cases of polycythemia vera complicating stroke

Objective To explore the clinical features and treatment of polycythemia vera(PV)complicating stroke.Methods Clinical data of 27 cases of PV complicating stroke were analysed retrospectively.Results(1)Among all the cases,PV complicating cerebral infarction were 22 cases(81.5%),among the 22 cases,18 cases(66.7%)of multiple cerebral infarction,2 cases(7.4%)of transient ischemic attack,2 cases(7.4%)of cerebral hemorrhage,1 case(3.7%)of subarachnoid hemorrhage(SAH).The patients's clinical manifestation,physical sign,hemogram,marrow characteristic were consistent with that of PV diagnosis criteria.(2)Imageology check show that small infarction focus were common(66.7%).Lobe of brain,basal ganglia,capsula interna were common place of cerebral infarction.The density of hemorrhagic focus was asymmetrical,there was obvious edema around the hemorrhagic focus.(3)Applied vein blood-letting and small dose chemotherapy,8 cases were recovery,18 cases were obviously improved,1 case of stroke was invalid.Conclusion The cerebral infarction are common in PV complicating stroke.Among all the PV complicating stroke cases,multiple cerebral infarction are the most common,cerebral hemorrhage and subarachnoid hemorrhage are few,vein blood-letting and small dose chemotherapy has a good curative effect for the patients.
9.The relationship between serum Sirt1 and EGR1 and other inflammatory cytokines in type 2 diabetic patients with different stages of diabetic nephropathy

Objective To investigate changes of serum silent information regulator 1 (Sirt1) and inflammatory cytokines in type 2 diabetes patients with different stages of diabetic nephropathy.To explore the relationship between serum Sirt1 level and inflammatory cytokines in type 2 diabetic patients with different urinary albumin excretion rates.Methods A total of 436 cases with type 2 diabetes were divided into three groups:normoalbuminuric [D1,n =168],microalbuminuric [D2;n =152],and macroalbuminuric [D3,n =116].Serum Sirt1,hypoxia-inducible factor-1α (HIF-1α),early growth response protein 1 (EGR1),insulin-like growth factors-Ⅰ (IGF-Ⅰ),and monocyte chemotactic protein-1 (MCP-1) were determined by enzyme-linked immunosorbent assay (ELISA).Results The levels of serum Sirt1 in type 2 diabetes patients were significantly lower than that in control group,and with the increase of urinary protein excretion rate,the levels of serum Sirt1 in group D1,D2 and D3 were decreased gradually (P < 0.01).Compared to control,serum inflammatory cytokines (HIF-1α,EGR1,IGF-Ⅰ,and MCP-1) levels were significantly increased in type 2 diabetic patients and gradually increased in the patients of D1,D2 and D3 groups (P <0.01).Furthermore,Serum Sirt1 was negatively correlated with the levels of inflammatory cytokines.Age,duration,fasting blood glucose (FBG),fasting insulin (FINS),homeostasis model assessment insulin resistance index (HOMA-IR),glycosylated hemoglobin (HbA1c),low density lipoprotein (LDL),total cholesterol (TC),triglyceride (TG),serum creatinine (Scr),blood urea nitrogen (BUN),uric acid (UA),HIF-1α,EGR1,IGF-Ⅰ,and MCP-1 were positively correlated with Ln Koc of urinary albumin to creatinine ratio [Ln(ACR)] (P < 0.05);and Sirt1 were negatively correlated with Ln(ACR)(P < 0.01).HIF-1α,MCP-1,IGF-Ⅰ,duration,BUN,Sirt1,UA,LDL,and EGR1 were independent factors that significantly influenced Ln (ACR) (P < 0.05).Conclusions Serum Sirt1 might be a new target for the treatment of diabetic nephropathy.Enhancing serum Sirt1 levels might have a role in delaying the progression of diabetic nephropathy.
10.Clinical value of neuro-electrophysiological measure in Guillain-Barre syndrome

ObjectiveTo explore the characteristics of neuro-electrophysiology and clinical value in patients with Guillain-Barre syndrome.MethedsWe performed retrospective analysis of the clinical date of 46 patients with Guillain-Barre syndrome in our hospital.Hughes grades and neuro-electrophysiological measure including motor conduction velocity (MCV),complex muscle action potential (CMAP),sensory conduction velocity (SCV) and F wave,were investigated as the related factors with the prognosis of GBS.Results The rate of abnormal F wave was higher than the rate of abnormal MCV.The rate of abnormal SCV was lower than the rate of abnormal F wave and MCV( P <0.05).The prognosis of the acute inflammatory demyelinating polyneuropathy (AIDP) group was superior to the acute motor axonal neuropathy (AMAN) group,and there had significant difference( P <0.05).ConclusionsThe neuro-electrophysiological investigation is an important way to diagnose the GBS.F wave is a sensitive and specify index in the early diagnosis of GBS.The neuro-electrophysiological observation of MCV and CMAP may be served as cues for diagnosis,determination of the subtype and evaluation of the treatment effect in GBS.
