1.Effect of naloxone on neuronal cells apoptosis induced by repeated febrile seizures

Objective To investigate the effect of naloxone on neuronal cells apoptosis induced by repeated febrile seizures(FS).Methods Warm water was used to induce 70 rats FS model 15 days after birth in this study; each rat was induced 7 times febrile seizures at one- day interval . Seventy rats were randomly divided into naloxone-treated group and FS control group, receiving injection of naloxone or saline at 5, 30, 60 min and 2 hours after FS each day respectively. The rats were sacrificed 24 hours after the last seizure. Neuronal cell apoptosis was determined by TUNEL methods in situ cell death kit. TUNEL positive cells(TPC) were stained and counted as apoptosis in hippocampus and cortex. Ultrastructural changes of apoptosis neurons were observed under the electron microscope(EM). Results Compared with the FS control group, naloxone treatment could significantly relieve neuron apoptosis induced by repeated FS when it was used at 5, 30, 60 min after the last FS. However there was no significant difference in neuron apoptosis between 2 groups when naloxone was used at 2 hours after FS. The comparison of different naloxone administration time showed that the earlier naloxone was injected,the fewer apoptosis neurons were induced by FS.Conclusion Naloxone,as early used in proper dosage,may significantly alleviate apoptosis after repeated FS ,and protect neurons.
2.Effect of Cluster Needling of Scalp Acupuncture Combined with Rehabilitation on Spasticity after Stroke

ObjectiveTo explore the effect of cluster needling of scalp acupuncture combined with rehabilitation(Tang's Approach) on the spasticity after stroke.Methods39 stroke patients with spasm were divided into observation group and control group. The observation group received anti-spasm acupuncture combined with rehabilitation therapy, while the control group received baclofen combined with rehabilitation therapy. Spasm and activities of daily living(ADL) and therapeutical effect were assessed with the Modified Ashworth Scale and Barthel Index.ResultsThere was no significant difference between the two groups (P>0.05). ConclusionTang's Approach has the same effect as baclofen to improve the spasm after stroke without serious side-effect.
3.Studies on chemical constituents in dried buds of Lonicera confusa.

OBJECTIVETo investigate the chemical constituents of the dried buds of Lonicera confusa.
METHODChromatography and spectral analysis were respectively used to isolate and identify the constituents.
RESULTSeven compounds were isolated from the dried buds of L. confusa, and identified as rutin, quercetin, luteilin-7-O-beta-D-galactoside, lonicerin, chlorogenic acid, beta-sitosterol and tetratriacontane.
CONCLUSIONRutin was isolated from the genus for the first time, and the others were isolated from the species for the first time.

4.The expression of glutamate receptor in hippocampus of neonatal rats with hypoxic-ischemic brain damage

Objective To study the expression of Ca-A/K channel-related molecules glutamate receptor 2 and 1(GluR2/1) in hippocampus tissues of neonatal rats with hypoxic-ischemic brain damage (HIBD). Methods A total of 60 7-day-old Sprague-Dawley rats were randomly divided into sham operation group and HIBD group. Hippocampal tissues were obtained at 0 h, 1 h, 6 h, 24 h, 48 h and 72 h after HIBD. The expression of GluR2, GluR1 and autophagy marker protein Beclin-1, LC3 were detected by Western blot assay. Results Edema and focal softening and necrosis were observed 6 h after HIBD in the brains of neonatal rats. Compared with Con group, at each time point, the expression levels of GluR2 were lower while the levels of GluR1, Beclin-1 and LC3 were higher significantly in HIBD group (P<0.05). The protein levels of LC3, Beclin-1, GluR1 and GluR2 in hippocampus tissues of HIBD group were significantly different among different time points after the estab-lishment of HIBD model (F=10.65~701.14, P<0.01). The protein level of GluR2 was decreased from 1 h to 24 h after HIBD and reached the lowest level at 24 h. The levels of GluR1, Beclin-1 and LC3 were increased at 6 h, plateaued at 24 h and remained there until 48 h. The levels of these proteins returned back to the initial level at 72 h. Conclusions Ca-A/K channel-related mol-ecules GluR2 and GluR1 play important roles in the autophagic cell death of hippocampus tissues in neonatal rats with hypoxic-ischemic brain damage.
5.Gender mismatch, use of anti-thymoglobulin, and occurrence of graft-versus-host disease in preparative regimen are associated with high incidence of post-engraftment hemorrhagic cystitis in allogeneic hematopoietic stem cell transplantation

Objective To evaluate the risk factors of developing post-engraftment hemorrhagic cystitis (HC) in patients receiving allogeneic stem cell transplantation (allo-HSCT).Methods Retrospective data was collected from 92 patients with acute leukemia (acute myeloid leukemia 41 and acute lymphoblastic leukemia 51) who underwent allo-HSCT from 2000 to 2010,and the association of pre-transplantation parameters with the incidence of post-engraftment HC was analyzed.Results Forty-three patients had HLA-matched donors and 49 had unrelated donors.Of these patients,25 developed HC at a median of 35 days (day +20 to +65) after allo-HSCT.In the univariate analysis,unrelated donor,gender mismatch (female donor to male recipient),conditioning containing busulfan,graft-versus-host disease (GVHD),prophylaxis with cyclosporine (CSA) + methotrexate (MTX) + mycophenolate mofetil (MMF),use of anti-thymoglobulin (ATG) and development of GVHD were associated with increased incidence of HC.In the multivariate study,gender mismatch (P =0.001),use of ATG (P < 0.001),and GVHD (P =0.007) remain as independent factors for the increased risk of HC.More importantly,with these 3 factors,it is able to classify patients into 4 groups with risk of postengraftment HC at (7.7±4.6) %,(22.9±7.1) %,(48.2±10.5) %,and 100.0 %,respectively.Conclusion This retrospective study identified the gender mismatch,use of ATG in the preparation regimen,and aGVHD as important risk factors to predict the development of post-engraftment HC.Based on these risk factors,it is possible to classify patients into different risk groups for post-engraftment HC.Prospective study with a large cohort of patients is warranted to confirm the findings.Future clinical trial for HC prevention and treatment must be carried out on the intermediate and high-risk patients.
6.Clinical observation on electroacupuncture combined with medicine for treatment of acute gouty arthritis.

OBJECTIVETo observe clinical therapeutic effect of electroacupuncture combined with medicine on acute gouty arthritis and effect on blood uric acid content.
METHODSOne hundred cases of acute gouty arthritis were randomly divided into 3 groups, an electroacupuncture group treated by acupuncture at Zusanli (ST 36), Fenglong (ST 40) and Ashi points, a Western medicine group by oral administration of indometacin and benzbromarone, and a combined electroacupuncture and medicine group by electroacupuncture combined with the Western medicine. The changes of their blood uric acid levels before and after treatment were detected and their therapeutic effects were compared.
RESULTSThere was a significant difference among the three groups in clinical therapeutic effect (P < 0.05), and the effective rate was respectively 97.5%, 90.0% and 86.7% in the combined electroacupuncture and medicine group, the electroacupuncture group and the Western medicine group, the combined electroacupuncture and medicine group being significantly higher than the electroacupuncture group and the Western medicine group; after treatment, the blood uric acid level in all the 3 groups had significant change (P < 0.05), the blood uric acid in the combined electroacupuncture and medicine group had more obvious decrease as compared with those in the electroacupuncture group or the Western medicine group.
CONCLUSIONElectroacupuncture combined with the Western medicine has a definite therapeutic effect on acute gouty arthritis.

7.Research on the relationship between disease composite index and the prognosis of allogeneic hematopoietic stem cell transplantation

Objective To assess the impact of a composite index which combines the prognosis of specific hematologic malignancies and the disease remission state pre-transplant on the efficacy of allogeneic hematopoietic stem cell transplantation.Methods A total of 148 patients who underwent allogeneic hematopoietic stem cell transplantation from Jan,2007 to Feb,2012 in the Blood and Marrow Transplantation Center of Ruijin Hospital were included in this retrospective analysis.According to the composite score,patients were classified into low-risk group (n =17),medium-risk group (n =100) and high-risk group (n =31).The overall survival (OS),event free survival (EFS),treatment related mortality (TRM) and relapse rate (RR) were analyzed.Results Significant difference had been found on OS (76.5 % vs 66.0 % vs 41.9 %,P =0.002),EFS (70.6 % vs 57.0 % vs 32.3 %,P =0.001) and RR (41.9 % vs 27.0 % vs 5.9 %,P < 0.001) among the three groups.However,there was no impact on treatment related mortality (23.5 %,17.0 %,29.0 %,P =0.190).Multivariate analysis suggested that the composite index affecting the OS,EFS and RR of allogeneic hematopoietic stem cell transplantation (P =0.005,P =0.001,P < 0.001),but not the TRM (P =0.666).To some extent,it was an independent prognosis index on RR.Conclusion The composite index is closely related to the efficacy of allogeneic hematopoietic stem cell transplantation.
8.Chronic continuous massive pericardial effusion

A 77-year-old man was admitted to our hospital at July 5th,2010 with an unexplained massive pericardial effusion for 10 years.With dyspnea for one month and normal vital signs without pulsus paradoxus,other physical examination included a small amount of moist rale,normal heart sounds,jugular vein engorgement,positive hepatojugular reflux,hepatosplenomegaly and pitting edema of the extremities.The patient had a complex past history with lymph node tuberculosis,primary artertial hypertension,polycythernia vera,chronic renal insufficiency and hypothyroidism (Hashimoto's thyroiditis),and moreover,received a high dose radiation of 31p in 1967. Family history is negative.The patient had no cardiac tamponade or pericardial constriction during 10 years,he received pericardiocentesis twice,and pericardial effusion was exudative with a high proportion of monocyte.There was no evidences of tuberculosis infection,hypothyroidism,malignant tumor,severe heart failure,uremia,trauma,severe bacterial or fungus infection,chronic myeloid leukemia or bone marrow fibrosis during the admission. The patient refused anti tuberculosis,indwelling catheter drainage or surgical therapy.In this rare case,the aetiology of chronic massive pericardial effusion is most probably chronic idiopathic recurrent pericarditis.
10.Effect of ossicular chain reconstruction without mastoidectomy on hearing in patients with chronic otitis media

OBJECTIVE To investigate a better surgical treatment for chronic otitis media.METHODS 106 patients with chronic otitis media were collected from hospital. We assessed effectss of ossicular chain reconstruction without incision of mastoid (method 1) and ossicular chain reconstruction with incision of mastoid (method 2).We compared therapeutic effect, complication, and surgical treatment between these two groups. Furthermore, we performed stratification analysis by normal or abnormal function of stapes, Eustachian and tendon of tensor tympani, using logistic regression model. RESULTS The improvement of hearing by method 1 and 2 were 85.71%, 68.00% (P=0.0296). The recovery rate of drying ear were 92.86%, 88.00% (P=0.3931). The proportion of continued tinnitus were 10.71%, 18.00% (P=2827). There were 85.71% and 36.00% samples with numeric pain score from 1 to 3 (P<0.0001). There were 89.29%, 24.00% samples with operation hour <2.0 hour (P<0.0001). For those patients with normal function of stapes, Eustachian and tendon of tensor tympani, the OR of method 1 were 2.91, 3.08 and 3.33 For those patients with abnormal function, the OR was 3.27, 2.89 and 1.33. CONCLUSION Ossicular chain reconstruction without incision of mastoid is a suitable surgical treatment for chronic chronic otitis media,it could improve hearing more effectively with entire debridement of the focus and shorter operation time.
