Objective To investigate the effects of peroxisome proliferator-activated receptor γ (PPARγ) agonist on serum adiponectin (ADP),urine microalbumin (mALB) and kidney pathology of obese mice,and discuss the significance of PPARγagonists preventing obesity-related glomerulopathy (ORG).Methods Sixteen male ob/ob mice and 8 male C57BL/6 mice which were 8 weeks old were selected in this study.Ob/ob mice were dividing into 2 groups according to body mass:obesity group (M group) and pioglitazone intervention group (T group) which were fed with high lipid chow.C57BL/6 mice were control group (C group) which were fed with ordinary chow for 12 weeks.Body mass,blood glucose,serum ADP,urine mALB were assayed in each group and compared.The morphological changes of kidney were observe by HE staining,glomerular diameters were measured and compared.Zonula occludens-1 (ZO-1) and Wilms tumor 1 (WT1) were positioned by immunohistochemistry,the level of ZO-1 expression in podocyte and podocyte number which was signed with WT1 in each group were evaluated and compared.And then the correlations between serum ADP,urine mALB,body mass,blood glucose,kidney mass,glomerular diameter,the level of ZO-1 expression,podocyte number were analyzed.Results The serum ADP of obese mice decreased (P ＜ 0.05),and the urine mALB increased (P ＜ 0.05),when compared with that of control group,and there was a negative correlation between ADP and mALB (r =-0.538,P ＜ 0.01).Renal pathology showed glomerular hypertrophy,in part associated with focal segmental glomerulosclerosis (FSGS),the expression of ZO-1 in podocyte and podocyte number were lower than that of control group (P ＜ 0.05).After the intervention with pioglitazone,the urine mALB of obese mice reduced (P ＜ 0.01),and the serum ADP level of them was higher than that of obese mice without intervention (P ＜ 0.01).There was no glomerular hypertrophy and glomerulosclerosis in kidney of obese mice,the expression of ZO-1 in podocyte and podocyte number were higher tban that of obese mice without intervention (P ＜ 0.05).Conclusions The kidneys of obese mice have clinical and pathological changes indicating that obesity can lead to kidney damage.Pioglitazone can make the low serum ADP level of obese mice ameliorated,and make the urine mALB of them reduced.Renal pathological change significantly is alleviated,the expression of ZO-1 in podocyte and podocyte number are increased,indicating pioglitazone can improve renal injury related to obesity.

Objective To evaluate the effect of telmisartan on serum adiponectin and urine microalbumin(mAlb) level by administrating it to mice with simple obesity, so as to explore new therapies for obesity-related kidney diseases.Method A total of 24 8-week-old male OB mice and 8 8-week-old male C57 mice were selected for this study.The genetic background of OB mouse was C57 mouse, but the lepin gene was deleted in OB mouse.OB mice were randomly divided into 3 groups by body weight and fed with high-fat diet for 12 weeks: model group (M group), telmisartan group (T1 group) and losartan group (T2 group).C57 mice acted as control and were fed with general diet for 12 weeks.Serum adiponectin and blood glucose levels were measured before and after treatment.24 h urine was collected to measure urine mAlb.Quantitative real-time PCR (qRT-PCR) was used to measure the expression level of peroxisome proliferators-activated receptors gamma mRNA(PPARγ mRNA).HE stain was used to observe the morphological changes of kidney and measure the glomerular diameter.Body weight, serum adiponectin level, blood glucose level, urine mAlb level, expression level of kidney PPARγ mRNA, kidney wet weight and glomerular diameter of 4 groups were compared and a correlation analysis was carried out by Person correlation coefficient.Results Compared with C group, the urine mAlb level in M group increased (P＜0.01), serum adiponectin level and the expression level of kidney PPARγ mRNA in M group decreased (P ＜ 0.01);The urine mAlb level was negatively correlated with serum adiponectin level and expression of kidney PPARγmRNA (r=-0.773,P ＜ 0.01;r=-0.469, P ＜ 0.01).The urine mAlb level in T1 group was lower than M group (P ＜ 0.05),serum adiponectin level and expression of kidney PPARγmRNA in T1 group were higher than M group (P ＜ 0.05).Compared with T2 group, the urine mAlb level in T1 group decreased, serum adiponectin level and expression of kidney PPARγ mRNA in T1 group increased (P ＜ 0.05);Compared with M group, the urine mAlb level in T2 group decreased (P=0.01).The morphological changes of kidney:glomerular volume increase and focal segmental sclerosis were found in some mice in M group and T2 group.No glomerular volume increase and focal segmental sclerosis were observed in T1 group.Conclusions Telmisartan can reduce urine microalbumin, whose mechanism might be that telmisartan can active the PPARγand promote the level of serum adiponectin.

Objective To explore the relationship between serum creatinine ( Scr ) , blood urea nitrogen ( BUN) and lung image changes in uremic patients .Methods According to the chest CT ,X-ray and related findings , 256 patients with uremia were divided into 84 cases of uremia ( uremic lung group ) and non-uremic lung 172 cases ( non-uremic lung group ) .The chest CT and X-ray examination ,Scr and BUN levels were observed .Results Uremic lung group had five types of image findings,including alveolar edema (67.86%),pulmonary interstitial edema (16.67%),pulmonary congestion syndrome(8.33%),pleural effusion(4.76%) and pulmonary interstitial fibrosis (2.38%).The Scr[(1 145.6 ±236.4)μmol/L]and BUN [(38.9 ±6.1)mmol/L]levels in uremic lung group were significantly higher than those in the non-uremic lung group[(905.3 ±138.9)μmol/L,(23.8 ±5.6)mmol/L](t=8.128,4.782,all P<0.05).Conclusion The image of uremic lung is mainly pulmonary edema ,and radiological and pathological changes are closely related with Scr and BUN levels .

We sought to determine the efficacy of ramosetron in the prevention of nausea and vomiting during emergency cesarean delivery under spinal anesthesia with strict controls of causative factors.A total of 206 parturients participated in a randomized,single-blind and placebo-controlled trial.They received an intravenous injection of either ramosetron 0.3 mg or normal saline immediately after cord clamping.The primary outcome was the presence of postdelivery nausea and vomiting.Secondary outcomes included the need for rescue antiemetic,hypotension,pain and adverse effects.The incidence of postdelivery nausea and vomiting was 10.7％ in the ramosetron group vs.28.2％ in the control group (P ＜ 0.01 ).The incidence of intraoperative hypotension and postdelivery was similar in both groups.The incidence of postdelivery pain and the requirement for rescue antiemetic were similar in both groups.Ramosetron 0.3 mg is effective in the prevention of postdelivery nausea and vomiting during cesarean delivery.

OBJECTIVE: To observe the neuro-protective effects of harpagide on acute cerebral ischemic injury in mice and its mechanism involving mitochondria. METHODS: Acute cerebral ischemia were achieved by operation of MCAO in the left brain, random allocation was taken to divide ICR mice into sham group, model group, nimodipine group and harpagide (5,10,15 mg·kg^-1) groups. Mice were intraperitoneal injected harpagide immediately after surgeiy. Nerve function score, content of brain water, brain index and the common changes of brain pathological structure in HE staining were measured: Ability of mitochondria Ca²⁺-Mg²⁺-AT-Pase and protein expression level of caspase-3 in the MCAO mice' brains was determined: Ultrastructure change of mitochondria under the TEM was observed. RESULTS: Compared with model group, the harpagide groups could decreased the nerve function score, the content of brain water, brain index and the volume of ischemia in mice with different degrees in MCAO mice (P<0.05, P<0.01). 10 mg·kg^-1 of harpagide could increased the activity of Ca²⁺-Mg²⁺-ATPase obviously (P<0.01): And significantly decreased the protein expression level of caspase-3 (P<0.01): harpagide groups could protect the pathogeny structure and the ultrastructure of mitochondria with different degrees in MCAO mice, decrease edema of mitochondria obviously. CONCLUSION: Harpagide could obviously protect acute cerebral ischemia in mice, its therapeutical effects are approached to protecting the activity of brain mitochondria and decreasing protein expression level of caspase-3.

The lecture, role play, combination of team learning and self-directed learning, group presentation,these multiple pedagogies were adopted in the teaching of selective medical English course.The questionnaire result showed this multiple teaching strategy can foster the students' interest in medical English, enhance their awareness of active learning and increase the ir learning efficacy, which is an effective teaching model, and can be applied and promoted in the medical English teaching.

Rat hepatic oval cell model was induced by hyperglycemia and streptozotocin in vivo. Expression of insulin and mRNA expression of pancreatic transcription factors(Nkx6.1, PDX-1) were carried out. It showed that under high glucose, liver tissues were positive for insulin staining, the expression of Nkx6.1 and Pdx-1mRNA was significantly enhanced, and insulin-1mRNA was found to be expressed. The result suggests that under high glucose, hepatic oval cells can differentiate into insulin-producing cells.

Objective To survey the current status of job stressor and exhaustion among nursing staff in the catheter room of grade Ⅲ-A general hospitals and to look for regularities.Methods 178 nursing staff coming from catheter room,operating room,and outpatient department of Shenzhou Hospital and Fengtian Hospital accepted questionnaires of Chinese Nursing Stressor Scale and Exhaustion Scale in Spring 2010.Results Chinese Nursing Stressor Scale indicated that scores of subitem and total score of job stressor in the catheter room were significantly higher than those in the outpatient department; however,there were no significant differences in scores of subitem and total score of job stressor between catheter room and operating room.On the other hand,Exhaustion Scale showed that scores of emotional exhaustion and job unconcerned feeling in the catheter room were significantly higher than those in the outpatient department,but scores of job achieved feeling in the catheter room were significantly lower than those in the outpatient department.There were no significant differences in Exhaustion Scale between catheter room and operating room nurses.Conclusions Job stressor and exhaustion were serious among nursing staff in the catheter room of grade Ⅲ-A general hospitals,which should cause the attention of management department.

Transforming growth factor-?1 (TGF-?1) was reported to play an important role in the pathogenesis of asthma in many stu-dies.It can stimulate the growth of fibroblast and give rise to fibrosis of lung tissues.Many scholars think that TGF-?1 is closely related to airway remodeling.It is a breakthrough to aim at TGF-?1 to find a way to intervene remodeling in asthma.Glucocorticoid (Dexamethasone and Budesonide) can have an impact in some extent to airway remodeling.In addition,interferon-? and some traditional chinese medicine such as ligustrazine,tripterygium wilfordii and ginkgo maybe effective,they all may ameliorate the progression of airway remodeling following redu-cing partial airway TGF-?1 expression.
