1.Clinical Observation of Budesonide,Azithromycin Combined Terbutaline in the Treatment of Infantile Acute Bronchitis

OBJECTIVE:To observe the efficiency and safety of budesonide,azithromycin combined with terbutaline in the treatment of infantile acute bronchitis. METHODS:150 children with acute bronchitis were randomly divided into control group(75 cases) and observation group(75 cases). All children were given electrolyte supplement,antiasthmatic,antispasmodic and other conventional treatment;based on it,control group was treated with 10 mg/kg Azithromycin injection,adding into 250 ml 5% Glu-cose injection,by intravenous infusion with time of more than 60 min,once everyday+2 mg Terbutaline sulfate spray solution,add-ing into 2 ml 0.9%sodium chloride injection,aerosal inhalation,twice a day,15 min for every times. Observation group was addi-tionally given 1.0 mg Budesonide aerosol;3 times a day,10 min for every times. The treatment course for both groups was 10 d. FEV1,MEF50,PEF before and after treatment,total effective rate and disappearance time of clinical symptoms,hospitalization time and incidence of adverse reactions were recorded. RESULTS:The total effective rate in observation group were significantly higher than control group,disappearance time of clinical symptoms and hospitalization time were significantly shorter than control group,the differences were statistically significant(P<0.05). After treatment,FEV1,MEF50 and PEF in 2 groups were significant-ly higher than before,and observation group was higher than control group,the differences were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Based on conventional treament,budesonide,azithromycin combined with terbutaline is effective in the treatment of infantile acute bronchi-tis,and it can significantly improve lung function,with good safety.

3. Effects of chest pain center certification on the treatment of patients with acute ST-elevated myocardial infarction

ObjectiveTo compare the treatment of acute ST-elevated myocardial infarction (STEMI) before and after the certification of chest pain center model, so as to provide reference for the construction of chest pain center. Methods We retrospectively analyzed the clinical data of patients with acute STEMI before and after certification of the chest pain center of Shanghai General Hospital of Shanghai Jiao Tong University. Totally 119 patients were included in the pre-certification group from January 2017 to December 2017, and 160 patients were included in the post-certification group from January 2018 to April 2019. The baseline data, first medical contact to electrocardiogram (ECG) time, first ECG confirmation time, troponin reporting time, cath lab activation time, door-to-balloon dilation time, hospitalization stay, hospitalization drug cost, hospitalization inspection cost, hospitalization examination cost, total hospitalization cost, and outcomes were compared between the two groups. Results There was no significant difference in the gender, age, history of hypertension or history of diabetes between the two groups (all P>0.05). The cath lab activation time was not significantly different between the two groups (P>0.05). The first medical contact to ECG time (Z?-7.247, P<0.001), first ECG confirmation time (Z?-5.072, P<0.001), troponin reporting time (Z?-3.210, P?0.001) and door-to-balloon dilation time (Z?-7.025, P<0.001) were significantly shorter in the post-certification group than those in the pre-certification group. There were no significant differences in hospitalization stay, hospitalization drug cost, hospitalization inspection cost, hospitalization examination cost, total hospitalization cost, or mortality between the two groups (all P>0.05). Conclusion Through the certification and standardized operation of chest pain center, the time of treatment for acute STEMI can be significantly shortened.
4.Research on transmission risk exposure of pathogenic microorganisms in anesthesia procedures

Objective To raise risk exposure awareness for spreading pathogenic microorganisms in anesthesia procedures and normalize aseptic technique of anesthesiologists,thus minimizing postoperative infection.Methods Choose 45 cases of general anesthesia.Respectively before anesthesia induction (Time0),five minutes after induction (Time1) and two hours after anesthesia (Time2),make bacteriological tests on seven spots vulnerable to contamination,including the three-way stopcock,screwtype hose coupling,oxygen flux knob,pressure-release valve surface,exhaled breath entry of the anesthesia machine,oxygen intake of the breathing loop of the anesthesia machine,and operating desktop of the anesthesia cart.Results Contamination risk exposure of the stopcock extends with the operative time.At Time 0,it is sterile; at Time 1,84.4%of the 45 cases are found with bacteria growth,with colony count of 1~2CFU/ml; at Time2,colony counts at various monitoring points range 7~21 CFU/ml,of which the differences between Timel group and Time0,as well as Time2 and Time0 are statistically significant(P<0.05).Colony counts at other monitoring points comply with the regulations.Conclusion Three-way stopcocks are highly vulnerable to contamination during operative anesthesia.Regular sterile operations can effectively prevent and lower postoperative infection of the patients.
5.Mitochondrial encephalopathy, lactic acidosis and stroke-like episodes / myoclonus epilepsy with ragged-red fibers /Leigh overlap syndrome caused by mitochondrial DNA 8344A>G mutation.

OBJECTIVE:
Mitochondrial deoxyribonucleic acid (mtDNA) 8344 A>G (m.8344A>G) mutation is the common mutation associated with mitochondrial myoclonus epilepsy with ragged-red fibers (MERRF) syndrome. Herein we report a rare case with mitochondrial encephalopathy, lactic acidosis and stroke-like episodes/MERRF/Leigh (MELAS/MERRF/Leigh) overlap syndrome caused by m.8344A>G mutation.
METHODS:
The clinical and imaging data of the patient were collected and an open muscle biopsy was carried out. We further employed molecular genetic analyses to detect mtDNA mutation in the proband and his mother. And then a clinical and neuroimaging follow-up was performed.
RESULTS:
This patient was a 25-year-old male, who developed exercise intolerance since the age of 6. At age 10, he suffered from acute episodes of hemianopia, and cranial magnetic resonance imaging (MRI) showed occipital stroke-like lesions and cranial magnetic resonance spectroscopy (MRS) revealed a lactate peak corresponding to the lesion. After that the patient presented slowly progressive psychomotor decline. He had myoclonic seizures and cerebellar ataxia since the age of 12. At age 21, he was admitted to our hospital because of confusion and cranial MRI revealed symmetrical lesions in bilateral posterior putamen, thalami and midbrain. Then repeated MRI showed progression of original lesions and new frontal multiple stroke-like lesions. Symptomatic and rehabilitation treatment relieved his condition. Follow-up cranial MRI at age 24 showed the lesions in basal ganglia and thalami diminished, and the midbrain lesions even completely vanished. Muscle pathology indicated the presence of numerous scattered ragged-red fibers (RRF), suggestive of a mitochondrial disorder. Polymerase chain reaction-restricted fragment length polymorphism (PCR-RFLP) detected the m.8344A>G mutation of the MT-TK gene encoding mitochondrial transfer RNA for lysine in the patient's blood. Next generation sequencing (NGS) of the whole mitochondrial genome identified that the proportion of m.8344A>G was 90%, and no other mtDNA mutation was detected. Sanger sequencing further identified this mutation both in the proband and his mother's blood, although the mutation load was much lower in his mother's blood with approximately 10% heteroplasmy.
CONCLUSION
The present study is the first to describe a patient with m.8344A>G mutation in association with the MELAS/MERRF/Leigh overlap syndrome, which expands the phenotypic spectrum of the m.8344A>G mutation.

6.Colposcopy image database design and application of retrieval algorithm.

Due to the insufficiency in colposcopy image database collection in china, a novel image database is developed and it will be the basis of a computer-aided-diagnosis system for colposcopy. An improved search algorithm of color is designed based on the current content-based image retrieval algorithms for the characteristics of colposcopy images. This algorithm is authenticated by more than one hundred of clinical pictures with primary satisfactory result.

7.Polymorphism at Position - 318 in Promoter of Cytotoxic T Lymphocyte Associated Antigen 4 Gene in Children with Primary Nephrotic Syndrome

Objective To investigate the correlation between cytotoxic T lymphocyte associated antigen 4 (CTLA - 4 ) and primary nephrotic sysdrome(PNS) with the pathologic type of mesangial proliferative glomerulonephritis(MsPGN) of glucocorticoid(GC) resistance. Methods The polymerase chain reaction - restriction fragment length polymorphism( PCR - RFLP) analysis was used to investigate the genotype of position - 318 promoter of CTLA- 4 gene of 36 patient children and 30 healthy controls. Results The frequences of genotypes at position - 318 promoter of CTLA- 4 gene in patients were 38.9% for CC,61.1 % for TC and 0% for TT. The frequences of alleles at this position were 69.4% for C allele and 30.6% for T allele. The frequences of genotypes and alleles in children were not significantly different from those in controls. Conclusion CTLA-4 promoter ( -318)C/T dimorphism was not associated with PNS- MsPGN of GC resistance, which hinted that the polymorphism may be not inlolved in pathogenensis of PNS-MsPGN and the mechanism of GC resistance.
8.Cisplatin inhibites HeLa cell proliferation by suppressing activation of metastasis suppressor gene 1-extracellular signal-regulated kinase/serine-threonine kinase

OBJECTIVE To study the molecular mechanism of cisplatin(DDP)by which HeLa cell growth and proliferation are inhibited. METHODS Cultured HeLa cells were treated with DDP 0.02-75 μmol · L-1 for 24 or 48 h. CCK-8 assay was used to determine the cell proliferation. The wound scratch assay was used to detect the cell migration and invasion. Flow cytometry was used to detect the cell cycle arresting. q-PCR was used to test the expression of metastasis suppressor gene 1 (MTSS1)mRNA. Western blot was used to determine protein levels of MTSS1,phosphorylated-extra?cellular signal-regulated kinase(p-ERK) and phosphorylated-serine-threonine kinase(p-AKT). RESULTS Following the treatment with DDP for 24 or 48 h,the proliferation of HeLa cells was inhibited significantly (P<0.05),the value of the half inhibitory concentration (IC50) of cells was 4.14 and 11.82 μmol · L-1. Migration and invasion activity of HeLa cells were reduced according to the wound scratch assay(P<0.05). Flow cytometry results showed that the cell cycle was arrested at S phase. q-PCR results showed that MTSS1 mRNA expression changed with DDP in a concentration-dependent manner (r24 h=-0.965,P<0.01;r48 h=-0.953,P<0.01). Western blot showed that the protein levels of MTSS1,p-ERK and p-AKT expression declined significantly with the increase in DDP concentrations(p-ERK:r24 h=-0.875,P<0.01;r48 h=-0.966,P<0.01. p-AKT:r24 h=-0.831,P<0.01;r48 h=-0.863,P<0.01. MTSS1:r24 h=-0.969,P<0.01;r48 h=-0.988,P<0.01). CONCLUSION DDP treatment inhibits HeLa growth and proliferation by interfering with the MTSS1 expression and disturbing the activation of ERK and AKT signaling pathways.
9.Sleep quality research of cancer patients before and after cancer

Objective To investigate the sleep quality of cancer patients before and after cancer so as to take effective measures to improve sleep quality of cancer patients.Methods In Xinjiang autonomous region,72 cancer patients in the Department of Oncology and Radiotherapy were surveyed for the study group and 61 benign patients as control group.The instruments including self-made general status and sleep status survey before cancer,and the Pittsburgh sleep quality index scale were used respectively.Results In the study group,40 people had sleep disorders (55.5%) before the cancer,significantly higher than control group people in 15 (24.5%),and the total Pittsburgh sleep quality index (PSQI) scores on average (14.00 ±2.39) were significantly higher than the control group (11.19 ± 2.45).The PSQI had seven sleep quality factors,with the statistical differences between two groups (P < 0.05).Conclusions Chronic insomnia patients are more prone to cancer,cancer patients sleep quality is poorer,and the adjustment of the sleep disorders has more importance.Paying more attention to the regulation of cancer patients to sleep helps to improve the patient's quality of life.
10.Effects of SAHA combined with PTX on survival and apoptosis of human paclitaxel-resistant ovarian cancer cells

Objective To evaluate the effect of SAHA or/and PTX on survival and apoptosis of human paclitaxel-resist-ant ovarian cancer OC3/P cells, and explore whether the combination of two drugs has a synergistic effect .Methods The morphology of OC3/P cells in different drug-groups was observed by inverted microscope .Cell viability was evaluated by MTT assay.The apoptosis rate was analyzed by Annexin V-FITC/PI assay.Results The morphology change of OC 3/P cells treated with different drug was observed by inverted microscope , and the change in combination group was more signif-icant than one drug alone group .The result of cell survival measured by MTT assay showed that inhibition rate of combina -tion group was more higher than one drug alone group (P<0.05).The analysis of factorial design and gold formula method all proved that the two drugs had synergy .Further the result of flow cytometry showed that apoptosis rate in combination group was significantly higher than SAHA or PTX alone group (P<0.05).Conclusion SAHA and PTX can inhibit the survival and induce apoptosis of OC 3/P cells, and two drugs have synergistic antitumor effects .
