Objective To investigate the protective effect of sevoflurane on the brain against focal ischemia-reperfusion(I/R)injury and its mechanism.Methods Twenty-four male SD rats weighing 250-300 g were randomly allocated into 3 groups(n=8 each):group Ⅰ sham operation;grouop Ⅱ I/R and group Ⅲ I/R + sevoflurane.The rats were anesthetized with intraperitoneal chloral hydrate 300 mg?kg~(-1).Middle cerebral artery occlusion(MCAO)was produced by insertion of a 4-0 mono-filament nylon thread with rounded tip at bifurcation of right common carotid artery into internal carotid artery.The nylon thread was advanced cranially until resistance was felt.The depth of insertion was 18-20 mm.After 3 h MCAO the thread was withdrawn to allow reperfusion.In group Ⅲ the animals inhaled 1.0 MAC sevoflurane for 30 min at 30 min before reperfusion.The rectal temperature of the animals was kept at 36.5-37.5℃.At the end of 24 h reperfusion the animals were weighed again.The animals'neurological deficit was evaluated using Zea Longa score(0=no defcit,4=unable to walk and unconscious).The animals were then killed.The neuronal apoptosis in striatum was assessed(TUNEL)and the PKC protein expression in striatum was determined by immunocyto-chemistry.Results The body weight of the animals in I/R group was significantly reduced after 24h reperfusion as compared to the body weight before ischemia (P＜0.01),while in control group and sevoflurane group there was no significant difference in the body weight before and after sham operation or I/R.The neurological deficit scores were significantly higher in I/R group than in sevoflurane group.The number of apoptotic neurons in striatum was significantly higher in I/R group than in sevoflurane group.The PKC expression in striatum was significantly higher in sevoflurane group than in I/R group (P＜0.01).Conclusion 1.0 MAC sevoflurane inhalation has protective effect on the brain against I/R injury. Upregulation of PKC expression in striatum decreased by I/R is involved in the mechanism.

Objective To strengthen special knowledge training in newly engaging-system nurses in laminar air flow cleaning ward so as to improve nursing quality.Methods Those measures such as establishing prospective target,standardizing training content,enhancing clinic technician and theoretic knowledge tutoring by experienced nurses,paying attention to looking ward and proposing nursing questions,strengthening rehearsing in clinic and periodical examination were put forward so as to ensure all-sided training in newly engaging-system nurses in laminar air flow ward.Results The newly engaging-system nurses have poor speciality knowledge on laminar air flow word.Mter three months' training,their speciality knowledge had improved a lot,and the score on speciality knowledge was significantly higher than that of pre-training.Conclusions Speciality knowledge training should be strengthened in newly engaging-system nurses so as to ensure improvement of nursing quality.

Permeabilized cells containing trehalose synthase from Meiothermus sp.CBS-01 was immobilized by the integration of entrapment in alginate beads with cross-linking and electrostatic self-assembly coating technique.Coating on alginate beads with diazo-resin and poly(styrene sulfonate) can protect the beads from degradation in phosphate buffer,whereas cross-linking with carbodiimide improves the thermostability of trehalose synthase entrapped in alginate beads.By co-immobilization of permeabilized cells using entrapment-cross-linking- coating method,the enzyme recovery was 32%,and the optimum temperature of the enzyme was still 60℃,but the optimum pH was shifted from 6.5 to 7.In batch manner,a maximum conversion yield of 60% trehalose from maltose could be reached by the immobilized cells.Within 4 times repeated use of the immobilized cells,and each time was operated at 50℃ for 24h,more than 50% conversion yield of trehalose could be maintained with less than 20% loss of the enzyme activity.

AlM: To study the clinical effect of the application of microscopic pterygium resection combined with different concentration of mitomycin C ( MMC) .
METHODS:A total of 110 cases of pterygium patients (120 eyes) were randomly divided into control group (58 eyes) and observation group (62 eyes) according to the odd and even number method. The control group adopted the pterygium resection combined 0. 3mg/mL MMC, and the observation group was given pterygium resection combined 0. 2mg/mL MMC. The cure rate and the recurrence rate, eyesight before and after the treatment, two groups of cornea and sclera wound healing situation, the incidence of postoperative complications were compared.
RESULTS: The cure rate and recurrence rate of the control group was 84. 5% and 15. 5% respectively, and the observation group was 93. 6% and 6. 5% respectively, the differences were statistically significant (P<0. 05). There were statistical differences of vision of the two groups before and after treatment (P<0. 05), and there were no statistical differences of the two groups between the two groups after treatment (P>0. 05). The cornea, sclera, wound healing time of the observation group were less than the control group, and there were statistical differences between the two groups ( P < 0. 05 ). The incidence of complications was 13. 8% in the control group and 3. 2% in observation group, with statistically significant difference (P<0. 05).
CONCLUSlON: The application effect of microscopic pterygium resection combined with MMC is remarkable, and the joint of 0. 2mg/mL concentration of MMC is more safe and effective, and is worth popularizing in clinical application.

Objective To study the effect of minimal enteral feeding and mechanic ventilation on motilin(MTL) in premature infants and find a new way to promote gastrointestinal motility especially in critical illness,so as to develop their nutritional condition.Methods From Nov.2004 to Jun.2006, 60 premature infants were recruited at NICU of Xi′an children′s hospital,who were assigned to minimal enteral feeding and normal feeding groups,mechanic ventilationor non-mechanic ventilation was conducted based on premature infants′ illness.Their peripheral vein blood was collected at different time respectively: before their first breast-feeding,3 and 7 days after their birth and their MTL levels was measured by radioimmunoassay.The effect of 2 factors on hormones were factorially analyzed.The difference of gastrointestinal motility between 2 feeding groups.Results Mechanic ventilation could not alter MTL levels.But minimal feeding could significantly promote motilin′s secretion(P0.05).None of patients suffered from necrotizing enterocolitis.Conclusion Minimal enteral feeding can significantly increase gastrointestinal motility of premature infants,as well as those infants who are conducted mechanic ventilation.

Objective To explore the therapeutic effects of canal wall-down tympanoplasty(CWdT)on chronic otitis media and evaluate the effects of simultaneous auditory rehabilitation.Methods 13 cases on chronic otitis media with cholesteatoma and/or granulation tissue were treated by CWdT,the effects were contrast with radi- cal mastoidectomy.Results After 6 months to 6 years of follow-up,all the cases by CWdT had dry ear without re- currence.The rate of hearing restoration and improvement was 84.6% from the cases.In same times,the rate of re- currence,hearing restoration and improvement was 52.6%,31.0% from the mastoidectomies.Conclusion The CWdT has advantage in indication,removal of lesions and the operation could be adjusted appropriately for simulta- neously auditory rehabilitation.

The purpose of this study was to investigate the existence and extent of cognitive impairment in adult diabetes mellitus (DM) patients with episodes of recurrent severe hypoglycemia,by using meta-analysis to synthesize data across studies.PubMed,EMBASE and Cochrane library search engines were used to identify studies on cognitive performance in DM patients with recurrent severe hypoglycemia.Random-effects meta-analysis was performed on seven eligible studies using an inverse-variance method.Effect sizes,which are the standardized differences between the experimental group and the control group,were calculated.Of the 853 studies,7 studies met the inclusion criteria.Compared with control subjects,the adult DM patients with episodes of recurrent severe hypoglycemia demonstrated a significantly lowered performance on memory in both types of DM patients,and poor performance of processing speed in type 2 DM patients.There was no significant difference between adult DM patients with and those without severe hypoglycemia in other cognitive domains such as general intelligence,executive function,processing speed and psychomotor efficiency.Our results seem to confirm the hypothesis that cognitive dysfunction is characterized by worse memory and processing speed in adult DM patients with a history of recurrent severe hypoglycemia,whereas general intelligence,executive function,and psychomotor efficiency are spared.

OBJECTIVETo explore the roles of granulocyte colony-stimulating factor in the pathogenesis of moyamoya disease.

METHODSSerum G-CSF concentrations were measured using enzyme linked immunosorbent assay (ELISA) in 20 children with moyamoya disease and 20 healthy children.

RESULTSSerum G-CSF concentration (35.7+/-10.3 pg/mL) in children with moyamoya disease was significantly higher than that in healthy controls (23.5+/-3.8 pg/mL) (p<0.01).

CONCLUSIONSThe elevated serum G-CSF concentration in children with moyamoya disease suggests that G-CSF may play an important role in the pathogenesis of moyamoya disease.
