1.The analysis of twelve patients with dermatomyositis complicated with neoplasma

Objective To improve the recognition of dermatomyositis (DM) complicated with intracranial neoplasma and to explore its pathogenesis aimed to collect the experience of diagnosis and treatment. Methods Twelve DM patients complicated with intracranial neoplasma were analysed and the literature of recent 23 years were reviewed. Results DM complicated with intracranial neoplasma was associated with the p53 genic mutation and it was rare because of blood-brain barrier and scanty of lymphocytes in brain tissue. Correct diagnosis depended on clinical manifestations and the computerized tomography or the magnetic resonance imaging of brain. Glucocorticoid was the first choice treatment.However, removal the neoplasma was necessary. Conclusion DM complicated with intracranial neoplasma is rare. Clinicians should make the diagnosis promptly and chosen the appropriate treatment.
2.Prognosis after fertility-sparing management with oralprogestin for women with complex endometrial hyperplasia and endometrial cancer

Objective To investigate the oncologic and reproductive outcomes after progestin treatment of complex endometrial hyperplasia(CEH) and grade 1 endometrial carcinoma(EC).Methods In a retrospective study, data were obtained for patients with CEH or grade 1 EC at presumed stage IA(without myometrial invasion) who wished to preserve fertility and were treated at the Peking Union Medical College Hospital, China, between January 1, 2000 and December 31, 2011.Patients had received oral medroxyprogesterone acetate(250-500 mg/d) or megestrol acetate(160-480 mg/d) for at least 6 months.Results Among 55 included patients, median age was 32 years(range 21-41 years).41(75%) achieved complete response after a median period of 6(3-24) months.Complete response was less frequent among obese than nonobese patients(4/12 [33%] vs 37/43 [86%];P=0.001).Disease recurrence was recorded in 10(24%) patients with complete response;the 5-year recurrence-freesurvival rate was 71%.Among the 33 patients who retained a desire to conceive, 17(52%) became pregnant.Conclusions Fertility-sparing management with oral progestin is effective and safe.Obesity is associated with a lower probability of long-term success.
3.Effect of IL-18 on expression of molecule on surface of 9 L in vitro and in vivo

Objective:To explore the influence of IL-18 on the expression of MHCⅠ,MHCⅡ,CD80,CD86 on the surface of 9L cells,to identify the effect of IL-18 on the immunogenicity and the efficiency of tumor antigen presentation of 9L.Methods:Retroviruses were used to transduce the mIL-18 gene into rat glioma cells and the cell clones (9L/IL-18) which steadily expressing mIL-18 gene were obtained ,and got the control cells of 9L/LXSN by the same method.The expression of MHCⅠ,MHCⅡ,CD80,CD86 on the cell surface were assessed by flow cytometry.The cell suspension of 9L/IL-18,9L/LXSN and 9L cells were inoculated into the brain of F344 rat to establish the animal models by the stereotactic technique ,got the tumor tissues to analyze the expression of MHCⅠ, MHCⅡ,CD80 and CD86 on the surface of tumor cells after 14 days.Results:The expression of MHCⅠ,MHCⅡ,CD80 and CD86 on the surface of 9L/IL-18 were (10.9±1.44)%,(0.61±0.14)%,(1.01±0.14)%,(0.57±0.11)% ; had no significant differences with the others in vitro (P>0.05);while the expression of MHCⅠ,CD80 and CD86 on the surface of 9L/IL-18 were(67.51± 1.40)%,(12.51±1.57)%,(6.95±0.56)%which were higher than 9L/LXSN and 9L cells (P<0.05),the expressions of MHCⅡwere no significant difference(P>0.05) in vivo.Conclusion: IL-18 did not affect the immunogenicity of 9L in vitro,but improve the immunogenicity and tumor antigen presentation in vivo.
4.Expression of serum AQP4-Ab and the significance in patients with optic neuritis

AIM:To investigate the serum antibody of aquaporin 4 ( AQP4 - Ab ) in positive expression rate and the significance in patients with neuritis.
●METHODS: A total of 98 cases ( 128 eyes ) of patients with optic neuritis were studied to detect the patient′s serum AQP4-Ab positive rate of antinuclear antibodies ( ANAs) from Jan. 2012 to Dec. 2015 in ophthalmology center of our hospital. According to the expression of AQP4 - Ab group, the best corrected visual acuity between the two groups, peripapillary nerve fiber layer thickness (pRNFL), the volume of the macula, macular RNFL ( mRNFL ) , macular core layer volume ( mlNL ) measurement were compared.
●RESULTS:Ninety-eight patients (128 eyes) with optic neuritis cases diagnosed through examination revealed AQP4-Ab positive in 22 patients ( 22%) , negative in 76 patients ( 78%) , ANAs positive in 21 patients ( 21%) , negative 77 patients ( 79%) . Optic neuritis patients with serum AQP4 - Ab positive rate and ANAs positive significant correlation ( r = 0. 707, P < 0. 05 ). After examination revealed AQP4-Ab patients and negative-positive patients with best corrected visual acuity difference was not statistically significance (P>0. 05). After inspection found pRNFL, macular volume measured value AQP4 - Ab positive patients were significantly less than the negative patients, the differences were statistically significant (P<0. 05). AQP4-Ab positive patients and negative patients the mRNFL, mlNL measured values were not significantly different (P>0. 05).
●CONCLUSION:AQP4-Ab and ANAs expression in optic neuritis patients is a significant correlation. AQP4-Ab positive patients with optic neuritis pRNFL thinning of macular volume are decreased compared with negative patients.
5.Gender Differences in Clinical Features of Acute Type 1 Diabetes Mellitus

Objective To analyze whether there are any gender differences in clinical features of acute type 1 diabetes mellitus.Methods Clinical data of 193 patients with acute type 1 diabetes mellitus were collected in our hospital from 2002 to 2007,and subjected to statistical analysis by SPSS.Results The ratio of male to female in all the subjects was 1.6:1.There was no significant gender difference in such clinical aspects as age,fasting blood glucose level,HbAlc concentration,positive percentage of serum autoantibodies against pancreatic islet or morbidity of diabetic neuropathy.In contrast to the males,the onset age was significantly younger,the diabetic duration longer,fasting serum C peptide level lower and the incidence of ketotic acidosis higher in female patients.The patients with onset age less than 14 years (puberty) did not show any significant gender differences in the above-mentioned clinical aspects.Among those with onset age more than 14 years,the females had significantly longer duration and lower fasting serum C peptide level than males.The male patients with onset age more than 14 years had significantly higher fasting serum C peptide level than those less than 14,whereas it was the same for total positive percentages of pancreatic islet autoantibodies and morbidity of diabetic neuropathy in females.Conclusion Gender differences may exist in some clinical features of type 1 diabetic patients with acute onset after puberty, which may partially result from the effects of sex hormones.
6.Spindle and chromosome configuration of human in vitro matured oocytes after slow freezing-fast thawing

Objective To evaluate the development of immature oocytes after freezing-thawing by conventional cryopreservation method for mature oocytes.Methods Immature oocytes were collected from stimulated ovaries of intracytoplasmic sperm injection(ICSI)cycles.Immature oocytes were in vitro matured directly or after slow freezing-fast thawing and immunostained for tubulin and chromatin and at last visualized by confocal microscopy.Results No statistical difference was found in maturity rate between freezing groups and the controls.There was a statistically significant increase in abnormalities of chromosome(23.7% vs. 50%)and spindle(28.9% vs.53.9%)in the GV freezing group compared with the GV control(P

Two Pseudomonas strains, K80 - B and K80 - A, can use hydrocarbon as carbon source by two different mechanisms. Different effects can be observed when they are applied to 69-8 oil of Dagan Oil Field. The strain K80-B producing both hydrocarbon pseudosolubilizing factor and hydrocarbon emulsifying factor can decrease the viscosity and solidifying point of the 69-8 oil 51.6% and 2℃ respectively. Whereas the other can only decrease 43.5% and 3℃ respectively. When the two strains are mixed, synergistic effect on crude oil is obvious.
8.Clonal evolution of abnormal Philadelphia chromosome-negative cells after imatinib mesylate therapy in patients with Philadelphia chromosome-positive chronic myelogenous leukemia.

OBJECTIVETo investigate clonal evolution of abnormal Philadelphia chromosome-negative cells (Ph- CE) after imatinib mesylate therapy in patients with Philadelphia chromosome-positive chronic myelogenous leukemia (Ph+ CML).
METHODSBone marrow cells G-banding karyotype was evaluated every 3 months in 100 patients with Ph+ CML after achieving hematologic responses on the course of imatinib therapy. There were 54 patients in chronic phase (CP), 37 in accelerated phase (AP) and 9 in blast phase (BP).
RESULTSAfter a median follow-up of 32 months (ranged 25-34 months), 11 patients, including 5 cases in CP, 5 in AP and 1 in BP, developed transient, interrupted or continuous Ph- CE after 3 - 29 months on imatinib therapy. Ph- CE emerged at the beginning of Ph+ cells decreasing or after Ph+ cells disappearing. The proportion of Ph- CE, was negatively correlated with the proportion of Ph+ cells (P < 0.05). Ph- CE commonly included +8 (45.5%) and +Y (27.3%). Five patients had additional cytogenetic abnormalities besides Ph+ in Ph- CE. Seven of the patients with Ph- CE achieved a major cytogenetic response while 9 of them achieved a complete hematologic response. One patient with Ph- CE in AP progressed to BP 20 months after the initiation of the therapy while the rests remained in hematologic or cytogenetic responses.
CONCLUSIONPh- CE occurred in about 11% of the patients with Ph+ CML who achieved major or minor cytogenetic responses on imatinib therapy. After a median follow-up of more than 2 years, most of the patients with Ph- CE were in a stable status with no disease progression.

9.Fibrinogen is a predictor for progressive proteinuria in type 2 diabetes

The association of coagulation function with progressive proteinuria in type 2 diabetic patients was retrospectively analyzed.With increasing microalbuminuria,fibrinogen level was increased significantly.Fibrinogen was an independent risk factor of microalbuminuria. In patients as the early-stage diabetic nephropathy (DN)progressed to clinical-stage DN,the baseline level of fibrinogen was also increased [ ( 3.5 ± 0.9 vs 3.0 ± 0.6 ) g/L,P<0.05 ].Fibrinogen may serve as a useful predictor of progressive proteinuria in type 2 diabetes.
10.Correlation of blood lipids and insulin resistance in patients with impaired glucose tolerance

Blood lipid level and its associations with insulin resistance were studied in patients with impaired glucose tolerance (IGT).Two hundred and twenty first degree relatives of type 2 diabetes mellitus were grouped into normal glucose tolerance (NGT) and IGT groups according to results of oral glucose tolerance test.Compared with the NGT group,the IGT patients had higher serum levels of total triglyceride (TG),total cholesterol (TC),low density lipoprotein-C (LDL-C) but a lower serum level of high density lipoprotein-C (HDL-C).Homeostasis model of assessment for insulin resistance index (HOMA-IR) and area under curve of insulin (AUCI) also increased.A positive relationship was found between TG and HOMA-IR (or AUCI),but a negative relationship existed between HDL-C and HOMA-IR.In conclusion,abnormal blood lipid metabolism is present in IGT patients and it has a close correlation with insulin resistance.
