1.Screening analysis of irregular antibodies from random donor population in Shaoguan area.

The study was purposed to analyze the frequency and distribution of irregular antibodies in Shaoguan area. Screening 15 033 random donor antibodies in Shaoguan area by screening cells, polybrene and antiglobulin tests. The results indicated that the irregular antibodies were found in 42 samples. The frequency of irregular antibodies in female was higher than that in male (P < 0.001), and Rh blood group antibodies such as anti-D, anti-E, and anti-Ec C were common (47.6%). 2 samples of Le antibodies were failed to be found by polybrene test. 2 samples of irregular antibodies with titer 2 were undiscovered by screening test of 10 pooled samples. In conclusion, because of irregular antibodies resulting in hemolytic transfusion reaction, the investigation of frequency and distribution of irregular antibodies is very important for safe transfusion. Antibody screening must be done for female donors, and especially for massive plasma transfusion of patients with severe and dangerous illness and infants so as to ensure safety.

2.Dermoscopic features of alopecia areata and their correlation with clinicopathological manifestations

Objective To observe the microstructural changes in lesions of alopecia areata (AA) with dermoscopy and to evaluate their correlation with clinicopathological manifestations. Methods The area of alopecia of 62 patients with AA and 44 patients with other types of hair loss were observed by using a noncontact polarized dermoscope (Dermlite, USA). Clinical data on and laboratory findings from these patients were collected. Pathological examination was carried out with scalp biopsy specimens from the alopecia area of 15 AA patients. Results Characteristic dermoscopic signs of AA included yellow dots, black dots, broken hairs, exclamation mark hairs, short vellus hair and newly-grown short hairs. Among these signs, yellow dots showed the highest prevalence (83.9%). Exclamation mark hairs, black dots and broken hairs were rather specific signs for AA, and the prevalence of the three signs was positively correlated with disease activity and positivity rate of hair-pull test. A positive correlation was also noted between the prevalence of elevated thyroid peroxidase antibody levels and positivity rate of hair-pull test (r = 0.269, P < 0.05 ) as well as prevalence of broken hairs (r = 0.445, P < 0.05), and between the prevalence of yellow dots and that of keratinous plug in follicular orifice. There was a negative correlation between the prevalence of newly-grown short hairs and perifollicular mast cell infiltration and between the prevalence of black dots and the anagen/catagen ratio. Conclusions Yellow dots can serve as a preliminary screening marker for AA. Exclamation mark hairs, black dots and broken hairs are highly sensitive for the confirmation of diagnosis of AA, and often predict progressive AA.Dermoscopic signs are well correlated to the histopathology features of AA, and may be useful for the evaluation of disease severity and guidance on the treatment of AA.
3.Abnormal expressions of inflammatory cytokines and apoptosis-related factors in lesions of early alopecia areata

Objective To detect the expressions of apoptosis-related factors and inflammatory cytokines in superficial and deep layers of as well as anagen hair follicles in lesions of early alopecia areata (AA).Methods Scalp biopsy samples were collected from 25 patients with early AA and 15 healthy human controls.Fluorescence-based quantitative PCR was performed to detect mRNA expressions of apoptosis-related genes p53,caspase 3,Fas,survivin and bcl-2,as well as those of inflammatory cytokines interleukin (IL)-4,IL-10,IL-12 and interferon (IFN)-γ.An immunohistochemical assay was conducted to measure the expression of p53 protein in anagen hair follicles.Results Compared with control skin samples,anagen hair follicles in AA lesions showed significantly increased mRNA expression levels (expressed as 2-△△Ct) of pro-apoptotic factors caspase 3,p53 and Fas (6.78,8.01,9.74,respectively,all P < 0.05),but decreased mRNA expression levels of antiapoptotic factors bcl-2 and survivin (0.08 and 0.03 respectively,both P < 0.01),and similar mRNA expression levels of inflammatory cytokines.There was a significant increase in mRNA expression levels of Th1 cytokines IFN-γ and IL-12 (2.75 vs.1.00,P < 0.05; 85.67 vs.1.00,P < 0.01),but a significant decrease in the expression level of the Th2 cytokine IL-10 (0.002 vs.1.000,P < 0.01) in superficial layers of AA lesions compared with those of normal control skin.The degree of changes in mRNA expression levels of IL-10 and IL-12 was significantly higher in superficial layers than in deep layers of AA lesions (P<0.01 and 0.05 respectively).The immunohistochemical assay showed that the number of p53-positive cells per 100 cells in anagen hair follicles of AA lesions was higher than that in those of control skin (t =23.79,P < 0.01).Conclusions Anagen hair follicles in AA lesions exhibit high expressions of pro-apoptosis factors,but low expressions of antiapoptotic factors,suggesting that apoptotic factors play a role in the occurrence of AA.
4.Efficacy of maggot debridement therapy in the treatment of diabetic foot ulcers

Maggot debridement therapy(MDT)and conventional therapy were applied to diabetic foot ulcers(DFUs)with Wagner 2-4 respectively. The results showed that the prognosis was better in MDT group than in conventional therapy group. The reduction of circumference, square and volume of DFUs, healing rate of DFUs, decrease in number of bacterial species, and cost of antibiotics were all improved in the former group. The results suggest that MDT offers a fresh and effective method in the treatment of DFUs.

Several kinds of column chromatography method were used to investigate the chemical constituents of the leaves of Lophatherum gracile. The structures of the isolated compounds were identified based on their physicochemical properties and spectral data. A new flavone C-glycoside was isolated and its structure was identified as 3'-methoxyl-luteolin 6-C-beta-D-galactopyranosiduronic acid (1 --> 2) -alpha-L-arabinopyranoside (1).

Objective To observe the effect of inhibition of polo like kinasel (Plk-1) gene expression on 5-Fu and CDDP induced apoptosis of gastric cancer cell line-MKN45 in vitro. Methods The Plk-1 expression was inhibited by chemically synthesized siRNA, the Plk-1 mRNA and protein level were measured by real-time PCR and Western blotting, MKN45 cells proliferation was measured by MTT method, the apoptosis rate was detected by flow-cytometry. Results After treatment by siRNA targeting Plk-1, Plk-1 mRNA level decreased by 51.91% .89.45%.91.03% at 24 h.48 h and 72 h, and Plk-1 protein level decreased by 38.43% and 60.66% at 48 h and 72 h, compared with control group. 7.Comparison of clinicopathological features between gastric polyps and colorectal polyps

Objective To compare of clinical and pathological features between gastric polyps and colorectal polyps.Methods The clinical characteristics and pathological features of 2 125 patients with gastric polyps and 4 924 patients with colorectal polyps admitted in Wenzhou People's Hospital from 2004 to 2013 were compared.The detection rates,the indication of endoscopy,distribution,number,size and pathological type of gastric polyps and colorectal polyps were analyzed.Results The detection rate of gastric polyps was 3.1% (2 125/69 575) and that of colorectal polyps was 24.5% (4 924/20 124) (χ2 =9 886.401,P <0.01).The detection rates of gastric polyps in age groups < 20,20-39,40-59 and ≥60 were 1.8% (23/1 302),1.8% (399/22 600),3.4% (1 137/33 087) and 4.5% (566/12 586),respectively (χ2 =238.867,P < 0.01);and those for colorectal polyps were 6.6% (14/211),11.6% (623/5 385),26.0% (2 622/10 070) and 37.3% (1 665/4 458),respectively (χ2 =934.724,P < 0.01).The detection rates of gastric polyps in age groups 20-39,40-59 and ≥60 were lower than those in colorectal polyps(all P < 0.01).The detection rate of gastric polyps in males was lower than that in females(2.3%,848/36 447 vs.3.9%,1 277/33 128,χ2 =273.807,P <0.01),while the detection rate of colorectal polyps in males was higher than that in females (28.8%,3 239/11 230 vs.18.9%,1 685/8 894,χ2 =262.518,P < 0.01).Diarrhea and gastrointestinal bleeding in patients with colorectal polyps was more common than those in patients with gastric polyps (23.5%,1 156/4 924 vs.2.5%,54/2 125,χ2 =558.080,P<0.01;12.1%,597/4 924 vs.2.4%,51/2 125,χ2 =168.150,P <0.01).Single polyps were more common in gastric polyps than colorectal polyps(80.7%,1 714/2 125 vs.67.6%,3 331/ 4 924,χ2 =186.337,P <0.01).Polyps with size ≥ 1.0 cm were more common in colorectal polyps than those in gastric polyps (24.3%,1 197/4 924 vs.13.6%,289/2 125,χ2 =102.333,P < 0.01).The proportion of inflammatory and hyperplastic types in gastric polyps was higher than that in colorectal polyps (67.1%,1 378/2 125 vs.27.5%,1 273/4 924,χ2 =934.394,P <0.01;26.9%,552/2 125 vs.9.9%,459/4 924,χ2 =319.588,P <0.01);while the proportion of adenoma and canceration in gastric polyps was lower than that in colorectal polyps (1.7%,34/2 125 vs.62.4%,2 893/4 924,χ2 =2 135.743,P <0.01;0.4%,9/2 125 vs.4.8%,221/4924,χ2 =80.362,P<0.01).Conclusions In comparison with gastric polyps,the detection rate of colorectal polyps is higher.The detection rate of gastric polyps is higher in females,w hile that of colorectal polyps is higher in males.The detection rates of gastric polyps andcolorectal polyps increase with the age.The main pathological type of gastric polyps is inflammatory,while that of colorectal polyps is adenoma.The canceration of colorectal polyps is more common than that of gastric polyps.
8.Expression of filaggrin in alopecia areata lesions of patients

ObjectiveTo assess the relationship of filaggrin expression with atopic diathesis and disease severity in patients with alopecia areata (AA).MethodsThirty-seven patients with AA aged (26.3 ± 10.6) years were enrolled in this study.Atopic diseases were noted in 8 of these patients.Clinical data and laboratory test resuhs were reviewed.Immunohistochemical staining was performed to quantify the expression of filaggrin protein in scalp biopsy specimens from all of the 37 patients with AA and from 10 human controls,and fluorescence-based semiquantitative reverse transcription-PCR to detect the expression of filaggrin mRNA in scalp biopsy specimens from 22 patients with AA and 13 healthy controls.Data were statistically analyzed by Mann Whitney U test,chi-square test,and Spearman's rank correlation test.ResultsThe expressions of filaggrin protein and mRNA were significantly lower in patients with AA than in the controls(P < 0.05 or 0.01 ),and the decrease seemed more obvious in patients with large areas of lesions,long duration of disease,and nail abnormalities,but the degree of decrease was unrelated to the complication with atopic diseases.No significant differences were observed in sex ratio,age at onset,disease duration,area of hair loss,the prevalence of family history or incidence of nail abnormalities and increase in serum IgE and eosinophils,between patients with atopic diseases and those without.ConclusionsThe expressions of filaggrin protein and mRNA are decreased in patients with AA,suggesting that filaggrin may participate in the development of AA and is correlated with the severity of AA.
9.The MR diagnosis and clinical significance of bone contusion of knee

Objective To evaluate MRI in the diagnosis of the bone contusion of the knee joint and its clinical significance.Methods Using special coil for knee joint,coronal,sagittal,axial and oblique sagittal plane scanning with fast spin-echo sequence(T_1WI,T_2WI,PDWI+FS)was performed on knee joint in 205 patients in three days after injury.According the distributing bone marrow edema and injury mechanism,bone contusion were classified five types as pivot shift injury,clip injury,dashboard injury, hyperextension injury and lateral patellar dislocation.Results One hundred and forty-five cases of the 205 patients were found bone marrow edema without fracture on X-ray films.Among them,pivot shift injury was found in 43 cases accompanied with anterior cruciate ligament rupture in 30 cases,tear of the posterior horn of the lateral or medial meniscus in 12 and tears of the medial collateral ligament in 8 cases;clip injury in 53 cases accompanied with anterior cruciate ligament rupture in 10 cases,tear of the posterior horn of the lateral or medial meniscus in 15 and tears of the medial collateral ligament in 38 cases;dashboard injury 40 cases accompanied with posterior cruciate ligament rupture in 16 cases,hyperextension injury 9 cases accompanied with anterior cruciate ligament rupture in 2 cases,posterior cruciate ligament rupture in 5 cases.No lateral patellar dislocation was found.Forty-eight of 145 patients had undergone arthroscopy, 43 cases(89.6%)of them were in accordance with MRI diagnosis.Bone contusion were defined as geographic regions of abnormal signal intensity,that is,low signal intensity in T_1-weighted images and high signal intensity in PD-weighted or T_2-weigeted images with fat saturation.Conclusion MRI can accurately display the location and area of bone contusion of the knee joint as well as its adjunctive structure injury and deduce their injury mechanism.MRI should be used routinely for knee trauma.
10.Therapeutic effects of different doses of botulinum toxin A in the treatment of spastic cerebral palsy with different location ways

Objective To observe the clinical effects of different doses of botulinum toxin A (BTX-A) and different lacation ways on children with spastic cerebral palsy. Methods One hundred and twenty patients with BTX-A were employed in our study;these patients were performed locations of electromyography (n=60) and reverse stretching hand (n=60), and then, they were equally sub-divided into 6 groups, respectively: receiving injection of BTX-A at doses of 3, 4 and 5 U/kg, respectively (n=20).After injection, they were commenced functional training for 3 months. The gross motor assessment scale (GMFM) was used to assess the motor function of functional areas in these children 3 months after the training. The modified Ashworth scale was used to assess the degree of muscle spasticity. Results The scores of GMFM in patients received treatment with location of electromyography increased as compared with those with reverse stretching hand (P< 0.05), but the scores had no statistically significances among different doses of BTX-A treatment groups (P> 0.05). The Ashwort grade in patients received treatment with location of electromyography alleviated as compared with those with reverse stretching hand (P<0.05), but no statistically significance of grading was noted among different doses of BTX-A treatment groups (P>0.05). Conclusion All parameters in patients received treatment with location of electromyography improve better than those with reverse stretching hand after injection of same dose of BTX-A.
