1.Research on expression of miRNA-21 in the peripheral blood of coronary heart disease and its clinical significance.

OBJECTIVETo study the expression difference in MicroRNA-21 (miRNA-21) levels of the plasma between the patients with coronary heart disease and the subjects without coronary artery lesions, and its clinical significance.
METHODSPlasma was obtained from the patients with coronary heart disease(trial group,56 cases) and the subjects without coronary artery lesions(control group, 10 cases), patients with coronary disease were divided into angina(AP, 39 cases) and acute myocardial infarction(AMI, 17 cases)subgroup, the contents of miRNA-21 were detected using qRT-PCR method, and the differential expression of miRNA-21 in each group was analyzed. The levels of creatine kinase isoenzyme (CK-MB), high sensitive troponin I(cTnI), B type natriuretic peptide urea (BNP), Gensini, left ventricular ejection fraction (LVEF), integral value of coronary left ventricular end diastole diameter (LV) and homocysteine (HCY) were determined and the correlation between miRNA-21 and these clinical indexes was analyzed.
RESULTSCompared with control group, there was a significant difference in expression of miRNA-21 in patients with angina and AMI (P < 0.05), and miRNA-21 expression in AMI group was much higher than that in AP group. There was statistical significance in CK, CK-MB, cTnI, Genisis integral comparison between AMI group and control group (P < 0.05). The correlation analysis showed that there was a positive correlation between expression of CK, CK-MB, cTnI and the level of circulating miRNA-21 in patients with acute myocardial infarction. And there was a negative correlation between BNP, Gensini integral, LVEF value of coronary artery, LV and circulating miRNA-21.
CONCLUSIONmiRNA-21 was significantly elevated in acute myocardial infarction subgroup than the control group. The level of miRNA-21 associates with the degree of coronary artery stenosis, and might be a potential marker for the diagnosis of acute myocardial infarction. miRNA-21 may play an important role in protecting myocardium from ischemia/reperfusion injury.

2.Effect of Orem self-care pattern on quality of life of patients undergoing radiotherapy after breast cancer operation

Objective To explore the effect of Orem self-care pattern on quality of life of patients undergoing radiotherapy after breast cancer operation.Methods One hundred patients undergoing radiotherapy after breast cancer operation were randomly divided into control group and treatment group in equal number.The control group was treated by conventional nursing.Orem self-control pattern was used in the treatment group besides conventional nursing.QOL scale was employed to observe their quality of life after radiotherapy.Result The self-care ability and quality of life of the treated group were improved significantly comparing with the control group(P<0.05,for both).Conclusion Orem self-care pattern can effectively improve the quality of life of patients undergoing radiotherapy after breast cancer operation by way of helping them actively completing function exercises on the diseased limbs and propelling their early rehabilitation.
3.Value of TNR and TLR of 18F-FDG PET/CT Tumor Metabolic Imaging in Diagnosis of Lung Cancer

Purpose To evaluate the role of 18F-FDG PET/CT tumor metabolic imaging in the detection of lung cancer lesions,using receiver operating characteristic (ROC) curves analysis and investigate the diagnostic value of TNR and TLR in lung lesions.Materials and Methods A total of 82 patients who were confirmed with lung space-occupying lesionsby 18F-FDG PET/CT tumor metabolic imaging were retrospectively analyzed.Based on the pathological gold standard,evaluate the diagnostic value of 18F-FDG PET/CT tumor metabolic imaging in lung space-occupying lesions by t-test and ROC curve.Results There was a significant difference of TNR and TLR between benign and malignant lesion (P<0.01).The AUC on ROC curve of TLR ratio was 0.943 in the lung lesions while the TNR was 0.927.The sensitivity,specificity and the accuracy were 86.36%,62.50% and 81.71%,respectively with the TNR cutoff of 6.87.The sensitivity,specificity and the accuracy were 90.91%,68.75% and 86.58%,respectively with the TLR cutoff of 2.86.Conclusion 18F-FDG PET/CT tumor metabolic imaging is useful for differentiating lung space-occupying lesions.But for cases of atypical 18F-FDG uptake,it is suggested to combine with clinical information.
4.The clinical value of activated protein C in pediatric leukemia patients with acute lung injury

Objective To investigate the changes of plasma activated protein C(APC)in the pediatric leukemia patients with acute lung injury(ALI),and the relationship with the outcome.Methods During the study period(from Jan to Dec 2009),17 pediatric leukemia patients with ALI were selected.They were divided into neutropenic(n =10)and non-neutropenic(n =7)group.We collected the basic data including the age,gender,stage of chemotherapy,pediatric critical illness score,mechanical ventilation time,ICU time.The blood gas and plasma APC levels were detected on day 1 and day 4 of ALI onset.Results Compared with the non-neutropenic group,the neutropenic group had a significantly longer mechanical ventilation time [(16.60 ± 1 0.83)d vs(3.79 ± 4.08)d,P =0.009 6],lower PaO2 level on day l[(54.90 ± 17.05)mm Hg vs (92.70 ± 27.53)mm Hg,P =0.009 7],and lower APC level on day 4[(193.06 ± 63.19)pg/mlvs(286.28 ±25.12)pg/ml,P =0.007 7].Conclusion The APC generation is severely impaired in the neutropenic group,especially in those who died.The lower APC level is,the longer is the mechanical ventilation time,the worse is oxygenation,the poorer is the prognosis.APC replacement therapy may be promising in these patients.
5.Multicenter study on the molecular characteristics of community-associated Methicillin-resistant Staphylococcus aureus in children with skin and soft tissue infection

Objective To explore the molecular characteristics of community-associated Methicillin-resistant Staphylococcus aureus (CA-MRSA) from children with skin and soft tissue infection (SSTIs) in China.Methods CA-MRSA was collected from the outpatients in Department of Dermatology in 13 Children's Hospitals of China.Genotypic characteristics of CA-MRSA isolates were tested by adopting mnltilocus sequence typing (MLST),staphylococcal protein A (spa) typing and Staphyloccoccal cassette chromosome mec (SCCmec) typing.And the presence of Panton-Valentine leukocidin genes(pvl) was determined also.Results Overall,1 705 strains of Staphylococcus aureus were isolated from SSTIs children,and CA-MRSA accounted for 2.6% (44/1 705 strains).Two types of SCCmec were detected in all the CA-MRSA strains,and the main types were SCCmec Ⅳ and SCCmec Ⅴ,accounting for 45.5 % (20/44 strains) and 54.5 % (24/44 strains),respectively.Thirteen MLST types (STs) and 15 spa types were detected among CA-MRSA.The most prevalent MLST was ST121 (18/44 strains,40.9%),followed by ST59 (9/44 strains,20.5%).Additionally,t437 was predominant,accounting for 40.9% (18/44 strains).ST121 strain had 8 spa types,and t2086 was the most common type (6/18 strains,33.3%);while ST59 had only 1 spa type,t437.No ST121,ST59 and t437 strains were found in Central and Eastern region of China.Only 4 (9.1%) of the 44 CA-MRSA strains harbored pvl genes which were all from Southern region.Conclusions The most common clone of SSTIs CA-MRSA in children is MRSA-ST121-SCCmec Ⅳ/Ⅴ,as the molecular epidemiology of CA-MRSA strain has changed,ST121 has replaced ST59 to become the main epidemic strains.
6.Expression of p63 and p73 Protein in Capillary Hemangioma

Objective To investigate the relationship between the expression of p63, p73 proteins and the development of hemangioma. Methods The immunohistochemical technique and quantitative image analysis were used to detect the expression of p63 and p73 proteins in 40 cases of capillary hemangioma and 20 specimens of normal skin. Results The absorbance value (mean ? SD) of p63 and p73 expression in normal skin tissue, proliferative phase of hemangioma and involuting phase of hemangioma were 0.923 ? 0.191 and 0.953 ? 0.120, 8.271 ? 1.953 and 6.408 ? 2.151, 0.920 ? 0.187 and 1.073 ? 0.516, respectively. The expression of p63 and p73 in proliferative phase of hemangioma was significantly increased as compared with those in involuting phase of hemangioma and normal skin tissue (P 0.05). Conclusions It is suggested that p63 gene is not a tumor suppressor gene but an oncogene in hemangioma and may contribute to the proliferation of endothelial cell and be associated with angiogenesis, and p73 may play an important role in the proliferation of hemangioma.
7.Analysis of distribution characteristics and drug resistance of pathogens isolated from ascites of patients in Children's Hospital Affiliated to Zhengzhou University, 2015-2021

Abstract: Objective To investigate the distribution and drug resistance of pathogenic bacteria separated from ascites of patients in Children’s Hospital Affiliated to Zhengzhou University from 2015 to 2021, and to provide a basis for rational clinical antimicrobial agents. Methods Bacterial culture, bacterial identification and drug sensitivity analysis were performed on 1 058 non-duplicate ascites culture specimens from January 2015 to December 2021. The clinica1 and microbiologica1 data were ana1yzed by WHONET 5.6 and SAS 9.4 Results Of the 1 058 specimens, 586 (55.39%) were positive for pathogenic bacteria, with a total of 781 strains isolated. There was no significant trend of increase or decrease in the positivity rate over different years. Male children (63.99%) were more prevalent than female children. Appendicitis (59.22%) was the most common disease and Escherichia coli was the most common causative bacteria. Among neonates (≤28 d), the bacteria with the highest detection rate were Klebsiella pneumoniae (23.50%) and Enterococcus faecium (23.50%), while among children (>28 d), the highest detection rate was Escherichia coli (35.98%). Gram-negative bacteria accounted for 64.79% of the 781 strains, mainly Escherichia coli (38.28%), Klebsiella pneumoniae (8.58%), and Pseudomonas aeruginosa (5.89%); Gram-positive bacteria accounted for 29.45%, mainly Enterococcus faecium (8.58%), Streptococcus constellatus (2.69%), and Enterococcus avium (2.43%); fungi accounted for 1.66% and anaerobic bacteria accounted for 4.10%. The resistance rates of Escherichia coli to cefoperazone/sulbactam, piperacillin/tazobactam, imipenem and meropenem were 6.02%, 4.35%, 4.35%, and 3.68%, respectively. The resistance rates of Klebsiella pneumoniae to these drugs were 59.70%, 59.70%, 50.75% and 53.73% respectively. Linezolid-resistant strains of Enterococcus faecium were found. Conclusion Appendicitis is the most common abdominal infection in children, and the distribution of ascites pathogens varies with ages and diseases. The pathogenic bacteria are mainly Gram-negative bacteria, and the drug resistance of Klebsiella pneumoniae was more serious. It is particularly important to use antibiotics correctly and rationally to reduce the emergence of drug resistant bacteria.
8.Investigation of A New Economic and Practical Modality:Six-in-One Rehabilitation Approach for All-round Rehabilitation of Children with Cerebral Palsy:50 Cases Report

Objective To investigate an economic and practical modality for the all-round rehabilitation of children with cerebral palsy(CP).Methods The six-in-one rehabilitation approach was designed according to the principal of modern rehabilitation and conductive education.It was mainly conducted through physical therapy,occupational therapy,speech therapy,sensory integration,supplemented with individual education and a self-originated "movement rehabilitation exercise".50 CP children were treated with this approach.Results After one period of treatment by six-in-one approach,the movement function,language ability and cognitive ability of those CP children improved tremendously.The children also got much progress in sociality and activities of daily living(ADL).The effective rate of rehabilitation reached 90%~96%.Conclusion The CP children have benefited variously in areas of language,movement,cognition,behavior,sociality and ADL from six-in-one approach,which requires small financial contribution from family and is cost-effective to the rehabilitation center.It is a new approach for all-round CP children rehabilitation,which can be adopted by small and middle sized rehabilitating institution and community rehabilitation,especially for those poor families CP children more than 3 years old.
9.Study on effect of four traditional Chinese medicines distributed along lung meridian on TLR2 and NF-κB expressions in mice with lung heat syndrome.

OBJECTIVETo investigate the effect of four traditional Chinese medicines distributed along lung meridian, namely Ephedrae Hebra, Zingiberis Rhizoma, Scutellariae Radix and Mori Cortex, on TLR2 and NF-κB expressions in mice with lung heat syndrome, in order to study the intervention effect of the four traditional Chinese medicines (TCMs) on the lung heat syndrome.
METHODOne hundred KM mice were randomly divided into the normal control group, the model group, the Ephedrae Hebra group, the Zingiberis Rhizoma group, the Scutellariae Radix group and the Mori Cortex group (20, 10 g x kg(-1)), nasally dripped with streptococcus pneumoniae to establish the mouse lung heat syndrome model, and then administered with different TCMs. The expressions of TLR2, NF-κB p65 proteins in lung tissues were analyzed by the immunohistochemical method. The expressions of TLR2, NF-κB p65 mRNA were measured by real time PCR.
RESULTCompared with the normal control group, the expressions of TLR2 and NF-κB p65 proteins in lung tissues in the model group were higher (P < 0.01), and the expressions of TLR2 and NF-κB p65 mRNA in lung tissues were up-regulated (P < 0.01 or P < 0.05). Compared with the model group, Ephedrae Hebra high and low dose groups, the Zingiberis Rhizoma low dose group and the Scutellariae Radix high dose group showed decreased expression of TLR2 protein (P < 0.05 or P < 0.01); Ephedrae Hebra high and low dose groups, the Zingiberis Rhizoma low dose group, Scutellariae Radix high and low dose groups and Mori Cortex high and low dose groups showed reduced expression of NF-κB p65 protein (P < 0.05 or P < 0.01). Ephedrae Hebra high and low dose groups, Zingiberis Rhizoma high and low dose groups, Scutellariae Radix high dose group and Mori Cortex high dose group showed down-regulated expression of TLR2 mRNA (P < 0.01 or P < 0.05).
CONCLUSIONEphedrae Hebra, Zingiberis Rhizoma, Scutellariae Radix and Mori Cortex can induce the TLR2/NF-κB inflammatory signal pathways by down-regulating the expressions of TLR2 and NF-κB p65 in protein and mRNA, so as to alleviate the lung tissue injury in mice with lung heat syndrome.
