2.Prognostic analysis of 45 cases of nasal natural killer/T-cell lymphoma

Objective To analyze the clinical characteristics, short-term and long-term outcome, overall survival and prognosis of nasal-type,natural killer T-cell lymphoma(NK/TCL) patients. Methods Clinical data of 45 patients with nasal NK/T cell lymphoma from Jan 2001 to Dec 2008 were analyzed retrospectively. Results The complete remission rate was 53.3% (24/45)and the partial remission rate was 17. 8% (8/45), the overall response rate was 71.1% (32/45). At a median follow-up of 15 months, the 1-year,3-year and 5-year survival rates were 68. 3% ,57. 1% ,and 43. 7% respectively. Multivariate analysis showed that B symptoms was independent prognostic factors of NK/TCL( P = 0. 001 ). Conclusions NK/TCL showed specific characteristics, and the long-term survival rate is still poor. Investigation of optional treatment is needed.
3.The Value of Reading Report in Training of Medical Students' Integrated Faculties

Objective:To explore the value of reading report in upbringing the integrated faculties of medical students.Methods:To give the training of reading report to the Seven-year and the five-year program medical students in their last year.Results:Such a training stirred the students' learning interests,improved their abilities in independent study,promoted their ability in collecting and dealing with information and widened their knowledge.Medical students grasped of the essential methods in writing reading report,which settled the groundwork for research abilities in the future.Conclusions:Reading report writing is instrumental in de- veloping students' integrated faculties and worthy advocating in medical teaching.
4.Effects of different cavosurface margins on color matching of the resin composite

Objective: To evaluate effects of color matching of different cavosurface margins on the resin composites in vitro.Methods:Twenty extracted human premolars with an A 2 shade buccal surface were used in this study .Rectangular shaped cavities (3.0 mm depth, 2.0 mm width, 2.0 mm length) were prepared in the center of the buccal surfaces .The gingival and occlusal cavosurface margins were prepared to be either shoulder or bevel;the other cavosurface margins remained vertical .Ten teeth were filled with Clearfil AP-X (AP), the other ten with Clearfil Majesty (MJ) and light cured.The color difference at the cavosurface margin area was measured using a spectrophotometer ( CrystalEye ) and evaluated by 3 observers subjectively .The data were statistically analyzed using repeated measures ANOVA and Chi-square test .Results:When measured by CrystalEye , the color difference between the tooth and resin composite was reduced from the center of restoration to the cavosurface margin area .Both objective and subjective evaluations showed that for AP , the color difference at the cavosurface margin area had no statistical difference among 3 types of the margins; for MJ, the color difference at bevel margin area was significantly smaller than that at the vertical margin area .Conclusion: The resin com-posite restorations produced the color matching at marginal area .The color matching of resin composites with higher diffused light transmission property is more susceptible to the type of cavosurface margins . Preparing bevels may reduce the color difference between the restoration and tooth surface .
5.Dynamic Regulation of Histone Acetylase on Cardiac Development Gene NKX2.5 During Cardio-genesis in Fetal Mice

Objective: To explore the dynamic regulation of histone acetylases p300 and p300/CBP associated factor (PCAF) on cardiac development gene NKX2.5 during cardio-genesis and to provide the new theoretical basis to clarify the regulatory mechanism for cardio-genesis in fetal mice.
Methods: Our research included 4 groups of cardiac tissues: Embryo (EB) 14.5 days group,n=10, EB 16.5 days group, n=10 and Neonatal 0.5 day group,n=5, Neonatal 7 days group,n=3. Immunoprecipitation was performed in myocardial tissues using anti-p300, anti-PCAF and anti-H3K9ac antibodies to retrieve p300, PCAF and H3K9ac binding DNA, the speciifc DNA sequences were ampliifed by real-time PCR to detect and the binding levels of p300, PCAF and the acetylation level of H3K9ac in NKX2.5 promoter sequence. In addition, the mRNA expression of NKX2.5 was examined by RT-PCR.
Results: The binding levels of p300 and PCAF had the timing consequence at different stage of cardio-genesis. The binding level of p300 in EB 16.5 days group (0.063 ± 0.021), Neonatal 0.5 day group (0.019 ± 0.008), Neonatal 7 days group (0.011 ± 0.003) were all lower than that in EB 14.5 days group (0.231 ± 0.033), and in Neonatal 0.5 day group and Neonatal 7 days group were lower than EB 16.5 days group, allP<0.05. The binding level of PCAF in EB 16.5 days group (0.063 ± 0.021), Neonatal 0.5 day group (0.019 ± 0.008), Neonatal 7 days group (0.011 ± 0.003) were all lower than that in EB 14.5 days group (0.185 ± 0.023), allP<0.05. The H3K9ac acetylation level and NKX2.5 mRNA expression level had the timing consequence at different stage of cardio-genesis. H3K9ac acetylation level in EB 16.5 days group (0.098 ± 0.014), Neonatal 0.5 day group (0.074 ± 0.010), Neonatal 7 days group (0.045 ± 0.014) were all lower than that in EB 14.5 days group (0.119 ± 0.020), and in Neonatal 7 days group was lower than EB 16.5 days group, allP<0.05. The NKX2.5 mRNA expression level in EB 16.5 days group (0.701 ± 0.181), Neonatal 0.5 day group (0.502 ± 0.159), Neonatal 7 days group (0.529 ± 0.13) were all lower than that in EB 14.5 days group (1.000 ± 0.130), allP<0.05.
Conclusion: Histone acetylases p300 and PCAF may dynamically regulate H3K9ac acetylation in NKX2.5 promoter sequence, and the mRNA of NKX2.5 was dynamically expressed during cardio-genesis in experimental fetal mice.
6.The design and optimization of a microfluidic device for sperm motility screening

Objectiye To optimize the depth of the microchannel and the time point for sperm collection,and improve the efficiency of sperm screening on a microfluidic device. Methods Microchannels with four different depths of 25, 50, 100 and 200 μm were tested. Mice sperm were added to the inlet of the microchannel. The relative quantity and motility of sperm in the outlet were recorded at different collection times, i.e. ,5, 15, 30 and 60 min. Statistical method one-way ANOVA and appropriate post-hoc testing were applied to analyze differences between different groups, and further to select the best-fit depth of the microchannel and the time point for collection. Results In microchannels with depths of 25, 50, 100 and 200 μm, the sperm motilities measured in each outlet were (85.4 ± 2.3)%, (85.8 ± 5.8)%,( 87. 2 ± 2. 8 ) %, (76. 5 ± 2. 8 ) % respectively with statistical significance ( F = 5.8, P < 0. 05 ). No obvious differences were found among 25-100 μm channels, however the motility dramatically decreased in the 200 μm group. The relative sperm quantities were (5.2 ±2.0)%, (7.2 ±2.5)%,(12.3 ±2.0)%,(7. 7 ± 1.1 ) % respectively with statistical significance ( F = 6. 9, P < 0. 05), which increased with channel depth from 25 to 100 μm,while it decreased in the 200 μm channel Taking 2 indexes into account, 100 μmwas the most fit channel depth for sperm motility screening. The sperm motility in the outlet gradually decreased with time. At the time points of 5, 15, 30 and 60 min after adding sperm, the sperm motilities were (99. 6 ±0. 7)%, (87.2 ±2. 8)%, (79. 3 ±2. 2)% and (62. 6 ±8.0)% respectively with statistical significance ( F = 37. 3, P < 0. 01 ). Yet the relative quantities of sperm in the outlet increased almost three times in this process. At the time points mentioned above, the relative quantities of sperm were (5.8±1.1)%, (10.6 ± 0.9)%, (12.1 ± 1.7)%, (17.9 ± 3.4)% respectively with statistical significance ( F = 17.8, P < 0. 01 ). Thus 15-30 min was the ideal screening time. Conclusion An effective microdevice for sperm screening with optimized depth and collection time period is developed,which may contribute significantly for the screening of healthy sperm on microfluidic chips.
7.Comparsion of pathological differences between cervical high-grade intraepithelial lesions positive resection surgical margin after cold knife conization and hysterectomy

Purpose To compare the pathological differences between cervical high-grade intraepithelial lesions (CHGIL) positive resection surgical margin after cold knife conization and total hysterectomy,and to explore how to take further clinical treatment measures.Methods The pathologic results of 74 patients with CHGIL positive surgical margin after cold knife conization and hysterectomy were analyzed retrospectively.Results There was a correlation between positive cervical incision margin and residual uterine lesion (P < 0.01).Age,menopause status,gland involvement did not related to residual uterine lesions(P > 0.05).There was significant correlation between the lesion-related quadrant and the residual rate of uterine lesion (P < 0.01).Conclusion Cold knife conization and hysterectomy are safe and effective treatments for CHGIL.However,for the positive resection surgical margin after cold knife conization cases,hysterectomy should be carefully chosen and may choose conservative treatment or re-cone cutting.
8.Studies on Detecting Brugia malayi Larva in Mosquitoes by Polymerase Chain Reaction

Objective To establish a specific, sensitive and simple assay for the detection of Brugia malayi larva in Anopheles sinensis .Methods Using a new DNA purification technique (Microcon 100) and two pairs of oligonucleotide primers (p1, p2 and p3,p4) suitable for detecting B malayi in seven areas in our country, the mosquito vectors infected by B malayi were detected by polymerase chain reaction(PCR).Results This PCR method could amplify separately a 322 basepair(bp) and a 155 bp DNA fragment and detect as few as 1/64 of one L 1 in 1 mosquito,the detectable limit was nearly 4 pg DNA of filarial larvae, and it could also detect 1 infected mosquito with one L 3 of B malayi in pools of up to 200 mosquitoes. In contrast,no such specific 322 bp or 155 bp DNA band was detected in Dilofilaria immitis and normal mosquito.Conclusion This PCR techique established for supervision of mosquito vector in B malayi endemic areas is specific,sensitive,and simple.
9.The clinical diagnostic value of high-frequency ultrasound in diagnosis of thyroid benign and malignant tumors

Objective To detect and analyze thyroid tumor by two-dimensional sonogram and Doppler parameter, and evaluate the value of high-frequency ultrasound in diagnosing thyroid benign and malignant tumors. Methods The ultrasonic images of 104 thyroid tumor from 80 patients with typical features were collected. Thyroid tumor was classified into benign and malignant nodules, based on the shape, border, or the rear wall echo, echo attenuation loss, internal echo, and microcalcifications in two-dimensional sonogram and systolic blood peak velocity (Vmax) and resistant index (RI) in Doppler examination. The expected results of high frequency ultrasound were compared with pathological results on consistency and error rate. Results Prediction of benign tumor by high frequency ultrasound was 66, and pathology 61, consistency rate of the two was 92.4%. Prediction of malignant lesions was 38, and pathological examination 32, consistency rate of the two was 84.2%. The total coincidence rate was 89.4%(93/104) and the error rate was 11.6%( 11/104). Conclusions The typical features of thyroid tumor on high-frequency ultrasound are helpful in diagnosis of benign or malignant nodules, which is valuable in guiding clinical treatment.
10.Role of community service as a curriculum delivery tool in the outcome-based curriculum of the International Medical University, Malaysia

Background:
The International Medical University
(IMU) has an outcome-based curriculum defined
by eight major curriculum outcome domains.
The attributes, qualities and competencies expected of a
health care professional form the basis for these outcome
domains. Community service is an effective curriculum
delivery tool widely practised by medical universities
around the world. We present the results of a survey
among IMU students to explore the effectiveness of
community service as a curriculum delivery tool in
enabling activities defined within the major curriculum
outcome domains of IMU.
Methods:
A self-administered 6-point Likert scale
questionnaire was used to survey student participants
of 20 community service events held in a rural village
between 2007 – 2012. The survey tool included
questions on demographic data as well as the perception
of the students on whether participation in the events
enabled them to experience activities defined under
the eight major curriculum outcome domains of IMU.
The one sample Student t-test was used to test for
statistical significance while regression analysis was done
to look for significant predictors.
Results:
A total of 255 students were surveyed, of
which 229 (90.5%) were medical students while the rest
were nursing students. Most of the students were in the
3
rd
(48.2%) and 4
th
(43.8%) year of their studies and
have completed the surgery, internal medicine and
family medicine posting. Six out of the 8 curriculum
outcomes domains were achieved through participation
in the community service programme.
Conclusion:
Community service is an effective
curriculum delivery tool for the outcome-based
curriculum of IMU where activities defined in six out of
eight outcome domains were achieved.
