1.Relationship between Helicobacter Pylori and Pathological Lesiens in 458 Bioptic Specimens from gastric Mucosa in children

Objectives To search for the relationship between HP and lesions of gastric mucosa of children.Methods Four hundred and fifty-eight bioptic specimens-from eastrie mueosa in children were strained with hematoxylin-eosin(HE)and Warthin-Starry for light microscopic study.Results HP was detected in 152 specimens. HP deteetable rate-in specimens with normal mucosa was 1. 1%, in compari-on with 42.3% in those with chronic superficial gastritis and 14. 3% in those with chronic atrophic gastrins .Specimens with chronic active gastritls, mostry manitesting local active lesions,had HP detectable rate of as high as 80.2%、69. 7% of the specimene with HP were found to have obvious lymph follicles reaction within lamina propria in their gastric mucosa.Conclusions There is a close relationship between HP infection and Pediatric chronic gastritis(especially chronic active gastritis). Histopathologically, HP-associated gastritis is characterized by chronic and local active inflamations, and tbe formation of lymph follicles within lamina propria.
2.Role of phosphatidyl-inositol 3-kinase-Akt signal pathway in attenuation of ischemia-reperfusion injury by sevoflurane preconditioning in isolated rat hearts

Objective To investigate the role of phosphatidyl-inositol 3-kinase-Akt (PI3k-Akt) signal pathway in the attenuation of ischemia-reperfusion (I/R) injury by sevoflurane preconditioning in isolated rat hearts. Methods Ninety-six adult male SD rats weighing 220-280 g were randomly divided into 6 groups ( n = 16 each): sham operation group (group S); I/R group; sevoflurane preconditioning group (group SP); wortmannin group (group W); dimethyl sulfoxide (DMSO) group (group D) and sevoflurane preconditioning + wortmannin group (group SW) . Their hearts were excised and perfused in a Langendorff apparatus with K-H solution saturated with 95%O2-5%C02 at 37 ℃ . The hearts were continuously perfused for 180 min in group S. After 15 min of equilibration, the isolated hearts were subjected to 30 min of ischemia followed by 120 min of reperfusion in SP, W, D and SW groups. Croups SP, W, D and SW received 10 min of perfusion with K-H solution containing 2. 4% sevoflurane, 100 nmol/L wortmannin, 20 μmol/L DMSO, and 2.4% sevoflurane + 100 nmol/L wortmannin, respectively, followed by 5 min washout before I/R. Eight hearts in each group were selected and HR, left ventricular end-diabetic pressure (LVEDP), left ventricular developed pressure (LVDP), and ± dp/dtmax were recorded at the end of equilibration and at 15 min of reperfusion, Myocardial tissues were obtained at 15 min of reperfusion for determination of apoptosis (by TUNEL) and phosphorylated Akt (p-Akt) expression (by Western blot) . Another 8 hearts were selected at 120 min of reperfusion for determination of myocardial infarct size by TTC staining. Result Compared with group S, LVDP and ± dp/dt,^ were significantly decreased and LVEDP was significantly increased in groups I/R, SP, W, D and SW, and myocardial p-Akt expression was up-regulated in groups I/R, SP and D ( P < 0.05). Compared with group I/R, LVDP and ± dp/dtmax were significantly increased, LVEDP and apoptosis index were significantly decreased, myocardial p-Akt expression was up-regulated, and myocardial infarct size was significantly reduced in group SP (P <0.05) . Conclusion Activation of PI3K-Akt signal pathway is involved in the attenuation of I/R injury by sevoflurane reconditioning in isolated rat hearts.
3.Study on measurement of aortic annulus diameter by real-time three-dimensional transesophageal echocardiography

Objective To research the feasibility and accuracy on echocardiography measurement aortic annulus diameter(AAD)at the plane of three Junctions of aortic valve leaflets by real-time three-dimensional transesophageal echocardiography(RT-3D-TEE).Methods Twenty-three patients underwent echocardiography and aortic valve replacement because of acquired aortic valve disease.The AAD was measured in parasternal left ventricle long axis view(TTE-AAD)by transthoracic echocardiography(TTE)preoperative.The AAD was measured in left ventricle long axis view(TEE-AAD)and the distance of three iunctions of aortic valve leaflets (N,L,R) was measured in aortic short axis view by RT-3D-TEE intraoperative.The three-dimensional full volume images were analyzed by online QLAB 7.0 software.The AAD was measured by standard cylindrical valve sizer (OP-AAD)before aortic valve replacement surgery,too.Results Comparing with the three aortic valve leaflets of acquired aortic valve disease,the lines between their junctions constituted an approximate equilateral triangle regardless of the cause and degree of disease,and the length of the three groups was no significant difference(P>0.05).There was no significant difference between TTE-AAD and OP-AAD group(P>0.05),and the correlation was good(r=0.84).The N,L and R group compared separately with the OP-AAD group, there was no significant difference(P>0.05)and correlation was superior to TTE-AAD group(r=0.94, 0.97, 0.96).The difference between TEE-AAD and OP-AAD were significant(P<0.01).Conclusions Echocardiography measurement of AAD at the plane of three junctions of aortic valve leaflets is feasible,and it is better accuracy and repeatability comparing with parasternal left ventricle long axis view.
4.The relationship between serum high-mobility group box-I and prognosis of coronary heart disease in old patients

Objective To investigate the correlation of serum high-mobility group box-1 (HMG-B1) with the severity of lesion of coronary artery disease (CAD) and its prognosis in elderly patients.Methods A total of 180 CAD patients with coronary stenosis exceeding 50 percent by coronary angiography were divided into three groups:one branch stenosis;two branches stenosis and three branches stenosis.The control group included 50 patients without coronary stenosis.The degrees of coronary stenosis were diagnosed as mild stenosis,moderate stenosis and severe stenosis based on improved Gensini scores.The severity of decrements of left ventricular ejection fraction (LVEF) by echocardiogram were divided into three groups:mild,moderate and severe LVEF.Levels of HMGB1,hs-CRP and glucose were measured in all the patients.According to whether there was a complication of type 2 diabetic mellitus (T2DM),the 180 patients were classified as two groups.The patients were also divided into two groups according to whether there were adverse events.Results The HMGB1 levels of the CAD group were increased along with the number of affected vessels [three bunch group (40.5±6.0) ng/ml,double bunch group (33.1±4.9)μg/L,single bunch group (20.5±3.3)μg/L and control group (6.2±1.4)μg/L (all P<0.05)].And the HMGB1 levels of the CAD group were increased along with the degrees of CAD stenosis [severe stenosis group (43.0±5.8)μg/L,medium stenosis group (32.1±4.5)μg/L,mild stenosis group(19.3±2.0)μg/L] (all P<0.05).Meanwhile,the levels of HMGB1 were increased along with the decrement of left ventricular ejection fraction [left ventricular severe dysfunction group (41.0 ± 5.5) μg/L,medium dysfunction group(33.1± 4.3)μg/L,mild dysfunction group (21.3± 2.0)μg/L] (all P<0.05).CAD with T2DM had a higher HMGB1 level than non-T2DM group[(35.7±5.0) (C)/L vs.(23.3±3.0) (C)/L,P<0.05].The adverse events group had a higher HMGB1 level than non-adverse events group[(38.7±5.5) (C)/L vs.(25.3±3.3)μg/L,P<0.05].Besides,HMGB1 had a positive correlation with levels of hs-CRP and glucose(r=0.680,0.571,P<0.05).Conclusions Serum HMGB1 change is closely related to morbid change degree of elderly CAD patients as well as prognosis.As a new type of inflammatory factor,HMGB1 may serve as a new target for disease treatment.
5.Hypoxia-induced changes of retinal progenitor cells migration by chemotaxis factor 4

Background In vitro study showed that chemotaxis consist of chemotaxis factor 4(CXCR4)and stromal cells derived factor-1(SDF-1)and may play a role in the orientation and migration of retinal progenitor cells (RPCs)toward lesion.Overexpression of CXCR4 in RPCs can enhance the chemotaxis activity.Objective This work was to explore the feasibility and underlying mechanism of up-regulation of CXCR4 on RPCs induced by hypoxia.Methods RPCs were retained in an incubator with normal O2volume(16%)or hypoxia condition(10% O2)for 12 hours and 24 hours respectively.Flow cytometer cell analysis screening(FACS)was conduced to measure the proportion of CXCR4-expressing cells,and CXCR4,HIF-1 mRNA were analyzed by reverse transcription-polymerse chain reaction(RT-PCR).The chemotical effect of 30 mg/L SDF-1 to RPCs cultured under the hypoxia condition was assessed using Boyden chamber.Results The expression level of CXCR4(CXCR4 mRNA/β-actin mRNA)inRPCs cultured by 10% O2 for 12 and 24 hours were 0.28+0.07and 0.48+0.17 and increased by 1.75 and 3.00 fold more than that of 16% O2 culture group(0.16+0.02)(P<0.01).The expression level of HIF-1 mRNA(HIF-1 mRNA/β-actin mRNA)in RPCs cultured by 10% O2 for 12 and 24 hours were 0.18 ±0.07and 0.38 ±0.13 and increased by 3.00 and 6.30 fold more than that of 16% O2 culture group(0.06±0.01)(P<0.01).The chemotical effect of 30 μg/L SDF-1 to RPCs increased from 13.00% in 16% O2 culture group to 36.00% and 46.00% in the cells cultured by 10% O2for 12 and 24 hours.FACS revealed that the proportion of CXCR4+ cells in hypoxia-exposure for 12 and 24 hours were 26.90% and 46.10%,respectively,but that in 16% O2 culture group was 9.10%,showing a statistically significant difference(P < 0.01).Conclusions RPCs induced by hypoxia can enhance the expression of CXCR4 in RPE cells and the chemotaxia to SDF-1.The overexpression of H1F-1 in RPCs may be involved in the up-regulation of CXCR4 expression.
6.Experimental Study of Antitoxic Effect of Sodium Selenite on Guinea Pig Cochlea Injury Induced by Gentamicin in Vivo

ObjectiveTo explore the effect of sodium selenite on gentamicin-induced ototoxicity.MethodsTwenty guinea pigs were randomly divided into the experimental group and control group with 10 animals in each group. The animals were treated with i.m injection of gentamicin (200 mg/kg/d for 8 consecutive days, and at same time, the experimental group was added with p.o tablet of sodium selenite (1 mg/kg/d). Before and after the administration, hearing function was evaluated by examination for auditory brain stem responses (ABR). The cochlear outer haircells were observed by scanning electronic microscope (SEM) and transmission electronic microscope (TEM).ResultsABR threshold of the control group was about 30 dB above that of the experimental group (P<0.01). The latency of waveⅠof the control group was about 0.23 ms above that of the experimental group (P<0.01). Under SEM, the cilia of the majority outer hair cells of the control group lodged even disappeared, while that of the experimental remained regular. Under TEM, in outer hair cells of the control group, mitochondrial crests were obscure, out-membrane was damaged and local protruding, the number of secondary lysosomes was increased, myeloid bodies appeared, but in the experimental group, outer hair cells basically remained normal.ConclusionSodium selenite has antitoxic effect on guinea pig cochlea injury induced by gentamicin in vivo.
7.Clinical research of tear film stability after pterygium excision with amniotic membrane transplantation

AIM:To assess changes of tear film function in patients after pterygium excision combined with amniotic membrane transplantation.
METHODS:Totally 126 patients with pterygium excision with amniotic membrane transplantation from January 2011 to November 2013 were entered in the study. The tear breakup time ( BUT) , the Schirmer I test ( SⅠt) and tear ferning test ( TFT ) were elevated in the patients before and after pterygium excision combined with amniotic membrane transplantation. The examnation times were 1d before surgey, 1wk, 1, 2mo after surgery. Operation eyes were studied group, while opposite healthy eyes as control group.
RESULTS: Compared with the control group, BUT and TFT were significantly different in the eyes with pterygium (P<0. 05); However, no obvious difference was detected in the results of SⅠt (P>0. 05). The results of BUT and TFT at 1mo after surgery in study group were significantly better than 1wk (P<0. 05), while no significant difference compared with 2mo (P>0. 05); The tear film stability in the study group at 1wk after surgery was still inferior to the control group (P<0. 05) and there was no significant difference at 1, 2mo after surgery (P all>0. 05). SⅠt results did not differ between the different examination times(P>0. 05).
CONCLUSION:Tear film stability was broken in the eyes with pterygium. Pterygium excision combined with amniotic membrane transplantation can obviously restore the tear film function into normal state, and the tear film function could reach steady-state 1mo after surgery.
8.Effect and safety of intrachamberaI triamcinoIone acetonide injection during cataract surgery in diabetic patients

·AlM:To evaluate the effect and safety of intrachamberal triamcinolone acetonide ( TA ) injection during cataract surgery on controlling postoperative inflammation and macular edema on diabetic patients.
· METHODS: Three hundred patients ( 300 eyes ) with type 2 diabetes who scheduled for cataract surgery were randomly divided into three groups: group A: 0.3%tobramycin/0. 1% dexamethasone eye drops and pranoprofen eye drops treatment for 1mo postoperatively;group B:intrachamberal injection of TA 1mg after cataract surgery, and 0.5% levofloxacin eye drops treatment for one month postoperatively; group C: intrachamberal injection of TA 2mg after cataract surgery, and 0.5%levofloxacin eye drops treatment for one month postoperatively.The main measurements included visual acuity, intraocular pressure ( lOP ) , corneal endothelial cell density, anterior chamber inflammation and the thickness of macula of the three groups.
· RESULTS:All cataract surgeries were done successfully by a single surgeon.The best corrected vision of group B and C was better than that of group A 1d, 1wk and 1mo postoperatively (P<0.05).The inflammation of anterior chamber of group B and C was milder than that of group A 1d and 1wk postoperatively (P<0.05).The average lOP of group C 1d postoperatively was higher than that of group A (P<0.05).There was no significant difference between group B and group C on the best corrected vision, anterior chamber inflammation and lOP at any time point.No significant change in the macular thickness was found in patients of group B and C before and after cataract surgery, while there was thicker macula in patients of group A 3wk and 1mo postoperatively when comparing with that of the baseline (P<0.05).There was no significant difference among three groups on corneal endothelial cell density at any time point.
· CONCLUSlON: lntrachamberal TA injection during phacoemulsification can effectively control postoperative inflammation, reduce the macular edema and accelerate the recovery of visual acuity.lntrachamberal TA 1mg has good effect and safety.
9.Observation of Dynamic Changes in Ultra-Micro-Structure of Pulmonary Arteries and Endogenous Hydrogen Sulfide in Rats with Left-Right Shunt

Objective To explore the relationship between dynamic changes in ultra-micro-structural of pulmonary arteries and endogenous hydrogen sulfide in rats with left-right shunt.Methods Rats in shunt group were subjected to an abdominal aorta-inferior vena cava shunt to create an animal model of pulmonary artery structural remodeling. After 1 day, 3 days, 1 week, 4 weeks and 8 weeks of experiment, the ultra-micro-morphologic changes of pulmonary arteries of rats were observed under electronic microscope and H_2S concentration in serum was evaluated by modified sulfide electrode method.Results The changes of ultra-micro-structure of pulmonary arteries were progressively exacerbated, endothelial cells became swollen and large in size on 3 days, smooth muscular cells increased in size as well as the change of endothelial cells in 1 week, and they changed from contractile phenotype to synthetic phenotype in 4 weeks.Conclusions Shunt exhibited changes of ultra-micro-structure of pulmonary arteries are accompanied by the changes of endogenous H_2S. It is suggested that endogenous H_2S might play a protective role in changes of ultra-micro-structure of pulmonary artery.
