1.Affect evaluation of a new medical posture mat in the prevention of pressure ulcer in pediatric orthopedics

Objective To evaluate the application effect on preventing pressure ulcers of modern medical position pad in orthopaedic. Methods A total of 124 patients with congenital dislocation of the hip, pelvis fractures and femoral fractures were selected between January 2013 and December 2013. They were divided into 2 groups by random digits table method with 62 cases each. The observation group was given modern medical position pad nursing after operation, and the control group was given routine postoperative nursing. Postoperative 2 hours, 4 hours after operation, postoperative 1 day, 2 days, 3 days and before discharge pressure situation, children crushed area skin comfort, families of children with pain on pressure parts skin care satisfaction and turn the situation were analyzed and compared. Results The observation group postoperative 2 hours did not appear the skin red and bruised, there was no crushed in 4 hours, only 4 cases of skin red, no 1 case skin crushed before leaving hospital. Twelve cases of the control group in postoperative 2 hours appeared red skin pressure, 4 hour after crush skin in 4 cases, 6 cases of skin crushed before leaving hospital. The comfortable children for 56 cases, mild discomfort for 6 cases in the observation group,and 31, 23 cases in the control group. The satisfaction was 100.0%(62/62) in the observation group, and 93.5%(58/62) in the control group. The degree of turn the situation pain in the observation group was lower than that in the control group. Conclusions New medical position mat can effectively extend the pediatric orthopaedic congenital dislocation of the hip, pelvis fracture and femoral fracture with roll over time, reduce the stimulation of the skin, sweat for pressure parts effectively prevent the occurrence of pressure sores, improve the crushed area comfort children and families of children with satisfaction, greatly reduce the postoperative pain when rolled over, worth clinical promotion.
2.Involvement of Caspase-dependent Mitochondrial Pathway in CORM-2 Induced Apoptosis in Neuronal Cells and Mechanism of Intervention Effect of Xingnaojing Injection

This study was aimed to investigate the mechanism of CO-releasing molecules (CORM-2) induced apoptosis and the intervention effect of Xingnaojing (XNJ) Injection in neuronal cells of rats. Optical microscope was applied to observe morphologic changes of neuronal cells. MTT assay was performed to assess the survival rates of CORM-2 on neuronal cells. Apoptosis was examined by flow cytometric analysis and the expression of relative proteins was measured by western blotting analysis. At the same time the morphologic changes, survival rates and expression of relative proteins of neuronal cells were also checked after XNJ treatment. The results showed that CORM-2 can influence survival rates in a time- and concentration-dependent manner. Survival rates decreased gradually after the cultures subjected to 24 h with 100 μmol·L-1, 200 μmol·L-1, 400 μmol·L-1 and 800 μmol·L-1 of CORM-2. It can induce neuronal apoptosis and activate Caspase-3, Caspase-9 and Cytochrome C in a concentration-dependent manner. The neuronal cells were treated with 200 μmol·L-1 of CORM-2 and then incubated with 10 mL·L-1 and 20 mL·L-1 XNJ injection for 20 h. It turned out early neuronal apoptosis decreased and the expression of Caspase-3, Caspase-9 and Cytochrome C also decreased. To sum up, CORM-2 may induce neuronal apoptosis through Caspase-dependent mitochondrial pathway, which can be intervened by XNJ Injection through inhibiting Caspase-dependent mitochondrial pathway.

4.Diffusion tensor imaging to assess recovery of white matter injury in periventricular leucomalacia with cerebral palsy after surgery

Objective To discuss the value of DTI on recovery of white matter injury in periventricular leucomalacia (PVL) with cerebral palsy after surgery.Methods 15 childrens suffered from PVL and 15 age-matched normal controls with normal MRI findings and no neurological abnormalities were recruited in this study.Conventional plain MRI scans and DTI sequence of brain were preformed in all subjects.FA maps of eight selected regions,which included the corpus callosum genu,splenium of corpus callosum,bilateral anterior limb of internal capsule,bilateral posterior limb of internal capsule,bilateral optic radiation,bilateral cerebral pedunculusand superior longitudinal fasciculus were compared between two groups.Results Significant differences of FA values were observed among the three group before and after the treatment and control groups(P<0.05).FA value of the caudate nucleus and lenticular nucleus among three group was not statistically significant difference.Further pairwise comparison,FA values of the posterior limb of internal capsul,genu of corpus callosum and splenium of corpus callosum in three groups showed statistically significant differences,there was no statistically significant difference for FA value of the genu of capsulae internae,cerebral peduncle and pons among the three group.Compared to the contralateral normal areas,FA value of the genu of capsulae internae,cerebral peduncle and pons showed statistically significant differences (P<0.05).Conclusion Quantitative DTI may show subtle changes in the structure of white matter,which has a certain reference value of FA values of white matter in periventricular leucomalacia with cerebral palsy after surgery,and help clinicians further reasonable treatment.
5.The application effect of using utility model patient information sign in pediatric surgical ward

Objective To explore the practical new type of patient information sign on the application effect of pediatric surgical ward. Methods Divided 200 cases of patients in our department into the control group and the observation group according to random number table, there were 100 cases in each group. The control group and the observation group in A and B ward respectively, A ward (control group) used the traditional end of the bed, B ward (observation group) used the new information sign. The incidence rate of adverse events, patients′ families health mission efficiently, satisfaction of patients or medical staff were compared between the groups. Results The incidence rate of adverse events in the observation group was significantly lower than that of in the control group [1%(1/100) vs 8%(8/100),χ2=5.701, P < 0.05]. The effectiveness of health education, satisfaction of patients or medical staff and patients′families to health mission satisfaction in the observation group were significantly higher than that of in the control group [91% (91/100) vs 79% (79/100), 97% (97/100)vs 84% (84/100), 98% (98/100)vs 86% (86/100),the value of χ2 was 5.647,9.828 and 9.783 respectively, P < 0.05)]. Conclusions This new utility model patient information sign by the clinical application effect is satisfied, the head of a bed, cue CARDS unification and standardization of management, and reduced the occurrence of nursing adverse events, which can effectively improve the continuity of care services, the effectiveness of health education and medical, nursing, and satisfaction. Objective To study formulation and characteristics of self-microemulsifying drug delivery system for breviscapine(BRV-SMEDDS).Methods The optimum formulations of BRV-SMEDDS were screened by solubility tests,formula compatibility,and pseudo-ternary phase diagrams.And the physicochemical characters,dissolution in vitro and in situ rat's intestine absorption of BRV-SMEDDS were also observed.Results The optimum formulation of SMEDDS was composed of Maisine 35-1-Cremophor RH40-PEG400-TEA=25∶40∶35∶7.The particle diameter was 88.6 nm.The percent of accumulated dissolution of BRV in SMEDDS in vitro was up to 97.8% at 1h,which was 8.0 times as much as that of BRV powder,and 5.1 times as BRV tablets.In the tests of in situ rat's intestine absorption,the permeability coefficient of BRV-SMEDDS was increased by 3.4 times as much as BRV powder,and 3.3 times as BRV tablets.Conclusion The dissolution and absorption of BRV is improved by formulation of SMEDDS.It could provide reference for the new dosage form of BRV.

Objective To explore the expression of tumor necrosis factor(TNF) ligand-related molecule-1A(TL1A) in mice with viral myocarditis(VM) and the role of astragaloside.Methods Fifty-five male Balb/c mice were randomly divided into 4 groups:control group,model group,low-dose intervention group and high-dose intervention group.Mice in model group,low-dose intervention group and high-dose intervention group were inoculated with 0.1 mL coxsackie B3 virus intraperitoneally.Then,mice in low-dose intervention group and high-dose intervention group were treated with 10 g?L-1 and 90 g?L-1 astragaloside solution,respectively.Mice in control group and model group were treated with 0.1 mL carboxymethycellulose solution.All mice were killed on the 15thday.Histological cross sections of heart were stained with hematoxylin-eosin and myocardial histopathologic scores were counted under optical microscope.The expressions of myocardial TL1A mRNA and protein were detected by reverse transcription polymerase chain reaction and immunohistochemistry.Results The mortality were 0,46.7%,40.0% and 13.3% in control group,model group,low-dose intervention group and high-dose intervention group,respectively.Compared with model group and low-dose intervention group,the mortality was significantly lower in high-dose intervention group(?2=9.46,8.95,Pa
