1.Comparison and enlightenment of the cultivation models for nursing postgraduate of two schools of nursing in Hong Kong

Through comparing of the cultivation models for master and doctoral postgraduate of two schools of nursing in Hong Kong,the viewpoint that the relatively perfect cultivation models for nursing postgraduate in Hong Kong had been formed was pointed out.Reformation of exam-oriented admission requirements,rationalization of cultivation types,enrichment of cultivation directions as well as making cultivation objectives explicitly,curriculum optimized and evaluation system improved should be suggested and referenced by domestic nursing educational innovators.
2.Expression and Clinical Significance of miR-504 in Primary Hepatocellular Carcinoma

Background:Dysregulation of microRNA expression is involved in the carcinogenesis of many tumors,expression of miR-504 has been observed in many tumors,but its expression in primary hepatocellular carcinoma(HCC)has not been reported. Aims:To explore the expression and clinical significance of miR-504 in HCC tissues and cells. Methods:Tumor and para-cancerous tissue in 85 HCC patients were collected. Five liver cancer cell lines and immortalized liver cell line were conventionally cultured. Effect of miR-504 expression on HepG2 cells proliferation was determined by CCK8 assay. The expression of miR-504 was determined by real time fluorescent quantitative PCR. Relationships between expression of miR-504 and clinicopathological features and prognosis of HCC patients were analyzed. Results:Compared with normal liver cells,expression of miR-504 was significantly downregulated in five liver cancer cells(P < 0. 05). Up-regulation of miR-504 inhibited proliferation of HepG2 cells,and down-regulation of miR-504 promoted proliferation of HepG2 cells. miR-504 expression in tumor tissue was significantly decreased than that in corresponding para-cancerous tissue. Expression of miR-504 was correlated with tumor differentiation(P = 0. 002),TNM stage(P = 0. 021),alpha fetoprotein(P = 0. 012)and portal vein tumor thrombus(P = 0. 003),but not with gender,age,tumor size and tumor number. Survival in HCC patients with lower miR-504 expression was significantly shorter than that in patients with high expression(P < 0. 05). Conclusions:Expression of miR-504 is downregulated in HCC,and is correlated with the prognosis of patients,suggesting that miR-504 can be used as an independent predictor for prognosis of HCC patients.
3.Clinical observation of docetaxel combined with capecitabine chemotherapy and psychological intervention after radical mastectomy for breast cancer

Objective To investigate the effect of docetaxel combined capecitabine chemotherapy and psychological intervention after radical mastectomy. Methods 80 cases of radical mastectomy for breast cancer treated in our hospital from January 2015 to January 2017 were treated with docetaxel combined with capecitabine chemotherapy. According to different nursing methods, the patients were divided into 2 groups. The observation group was treated with psychological nursing, while the control group received routine nursing care. The anxiety and depression scores in two groups before and after nursing were observed and compared. Results There were no significant difference in the scores of anxiety and depression between the two groups before and after treatment. After the intervention group, patients in observation group were significantly improved, and better than the control group patients; The nursing satisfaction rate in the observation group was 97.5%,which was significantly higher than that in the control group (72.5%) (P<0.05). Conclusion After radical mastectomy of breast cancer with docetaxel combined with capecitabine chemotherapy and psychological clinical observation of dry pre found in breast cancer resection and chemotherapy as the basis, to patients with psychological intervention can improve the clinical situation of patients, promote their mental status improved, improve nursing satisfaction rate of patients, so it is worthy of reference.
4.Cerebrespinal fluid ferritin in the child patients with acute lymphatic leukemia

Objcetives To sparch for the change of cerebrospinal fluid ferritin (CSF-Ft) content and its clinical significance.Methods The 42 patients with acute lymphatic leukemia(ALL)diagnosed by bone marrow biopsy were divided into 3 groups. There were 14 cases in Ⅰ group [introduced therapy stage without central nervous system leukemia(CNSL)7, 24 cases in Ⅱgroup (complete remission stage without CNSL) and 18 cases in Ⅲ grotip (with CNSL). There were 17 patients with viral encephalitis in viral encephalitis group and 15 patients without central nervous system oisease in control group. The CSF-Ft and SFt were determined by radioimmunoassay.Results The CSF-Ft contents of Ⅰ、Ⅱ、Ⅲ 、viral encepbalitis and control groups are 7.03 ?2.21 ?g/L,6.75 ?1.94?g/L, 31.06 ? 8.85?g/L, 7.26?1.83?g/L and 6.52 ?1.57?g/L. 5.Research on the relationship between obstructive sleep apnea hypopnea syndrome and nocturnal laryngopharyngeal reflux

Objectlve To study the relationship between obstructive sleep apnea hypopnea syndrome(OSAHS) and nocturnal laryngopharyngeal reflux(LPR),and discuss the possible mechanism.Methods Nineteen OSAHS patients had PSG,nocturnal pH and introesophageal pressure monitoring simultaneously. The sleep stage,body position and pressure change were recorded during every reflux event.The relationship between obstructive sleep apnea(OSA)and LPR was analyzed. Results Among 19 OSAHS patients,6 cases were found to have LPR,and 63 reflux events were recorded. There was a negative relationship between the severity of OSAHS and nocturnal mean laryngopharyngeal pH value,but relationship between OSA and reflux events is not statistically significant. The introesophageal pressure(t=3.211,P=0.007) and upper esophageal sphincter(UES) pressure(t=2.234,P=0.046) were statistically different between the patients with and without LPR. There was a positive relationship between the mean value of introesophageal pressure and the whale night mean laryngopharyngeal pH value(r=0.592,P=0.033).Conclusions There was significant relationship between severity of OSAHS and nocturnal laryngopharyngeal pH value. OSAHS patients are more likely to have esophageal reflux which could increase LPR,Compared with OSAHS patients without LPR,UES pressure during the night decreased significandy in those cases with LPR,it suggested this could be another mechanism of LPR.
6.Preliminary study of semi-quantitative and quantitative dynamic contrast-enhanced MRI in evaluating the response to concurrent chemoradiotherapy in patients with non-small cell lung cancer.

OBJECTIVETo investigate the capability of semi-quantitative and quantitative parameters of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) to predict the response to concurrent chemoradiotherapy( CCRT) in patients with non-small cell lung cancer (NSCLC).
METHODSA total of 24 patients with stage III or IIIB NSCLC, who underwent 3.0T DCE-MRI before CCRT, were enrolled in this study. Semi-quantitative and quantitative parameters were calculated by Funtool and Omnikinetics software. The relationship between these obtained parameters and tumor response was evaluated by Spearmen' s correlation analysis. The patients were classified into two groups according to the tumor regression rate after treatment, as response group (group A) and non-response group ( group B). Mann-Whitney U test was used to compare the parameters of responders and non-responders. The value of the parameters on predicting response was calculated by receiver operating characteristic curve (ROC).
RESULTSThe tumor regression rate after treatment was negatively correlated with time to peak (TTP) and the extravascular-extracellular volume fraction (Ve), and was positively correlated with signal enhancement ratio (SERmax) and volume transfer constant (Ktrans) (P < 0.05 for all). Statistical significant differences were found between group A and group B both in semi-quantitative and quantitative parameters (P < 0.05). Group A had a lower TTP value [(34.66 ± 16.37) s vs. (44.09 ± 17.41) s] and Ve value [(0.19 ± 0.03) vs. (0.25 ± 0.05)] than group B, whereas group A had a higher SERmax [(166.50 ± 44.95)% vs. (113.57 ± 46.62)%] and Ktrans [(0.41 ± 0.17) min(-1) vs. (0.28 ± 0.12) min(-1)] than group B (P < 0.05 for all). The ROC analysis indicated that when setting the threshold of Ve on ≤ 0.21 for predicting response, the specificity, sensitivity and accuracy were 85.7%, 80.0% and 83.3%, respectively, with an area under curve of 0.875 (P < 0.001).
CONCLUSIONSBoth the semi-quantitative and quantitative DCE-MRI parameters are helpful for predicting the response after CCRT of NSCLC. Quantitative parameters seem to be more meaningful than semi-quantitative parameters.

7.Analysis of the echocardiographic characteristics of ventricular diverticula

Objective To retrospectively analyzed the echocardiographic characteristics of ventricular diverticula in fetuses ,children and adults ,and discuss the main points of diagnoses and prognoses of ventricular diverticula . Methods Echocardiographic characteristics ,clinical data and intraoperative findings of ventricular diverticula of 8 fetuses ,3 children and 14 adults were summarized . Results The ventricular diverticula in 8 fetuses and 3 children were all single . Twenty five diverticula were found in the 14 adult patients ;in other words ,there were 3 multiple diverticula found in adult group . A statistical difference was found in ratio of diverticula area and corresponding ventricular area ( P = 0 .021 ) ,and corresponding ventricular fractional shortening ( P =0 .003) between fetal and single adult ventricular diverticula . There were 2 cases with ventricular septal defects ,1 with pericardial effusion ,1 with single umbilical artery ,and 2 with atrioventricular valvular regurgitation in fetal cases . Congenital complex cardiac disease and dextrocardia were found in 1 child . There were 1 case with supravalvular aortic stenosis , 1 with nonobstructive hypertrophic cardiomyopathy ,1 with left ventricular noncompaction ,and 1 with mitral reguigitation in adult group . All the 7 diverticula with ventricular arrhythmias ,ST‐T changes and abnormal Q wave were fibrous type . And 2 multiple diverticula were accompanied with ventricular dysfunction . Among the fetal cases ,2 cases were born ,3 cases were terminated pregnancy ,and 3 cases were lost to follow up . Three children and 2 adults were underwent cardiac operations for diverticula with good outcomes . No such complications as cardiac rupture ,thromboembolism ,and sudden death were found . Conclusions Echocardiography is a useful method to diagnose ventricular diverticulum ,especially for fetuses ,and provide the basis for prognosis consultation .
8.The research on the immunoprotection difference among five EV71 virus

Objective To screen the 5 EV71 vaccine candidates which were isolated from MRC-5 cells to find one as the vaccine virus. Methods The ICR mother mouse were immunized by intraperitoneal injection with the 5 vaccine candidates which were made from monoclonal EV71 virus. Two weeks after booster immunization, the animals were allowed to mate, another booster was given after 2 weeks, and then attracted the milk mouse within 24 h with different types of virus by cranial cavity injection. The survival condition were recorded everyday, and the antibody titre(IgG) were detected by ELISA, the virus titre of intestine were detected by nest-PCR, and neutralizing antibodies were determined using a microassay with MRC-5 cells, and then the data were analyzed by SPSS16.0. Results The antibody titre of 5 virus immunized ICR mouse were improved with the increase in the immune times, and they got difference in neutralization capacity, the survival rate after fatal attract and the virus titre of the intestine. Conclusion It proved that the five vaccine candidates were different at the molecular level, cellular level and individual level. 123 strain was the best one in immunogenicity and immunoprotective property, which agreed with the vaccine requirement.
9.Value of conventional fetal echocardiography combined with spatiotemporal image correlation in the prenatal diagnosis of total anomalous pulmonary venous connection

Objective To explore whether the use of spatiotemporal image correlation (STIC) can supply additional information with respect to conventional fetal echocardiography in the prenatal diagnosis of total anomalous pulmonary venous connection(TAPVC).Methods Twenty-five cases diagnosed as TAPVC received fetal echocardiography examination.Four-dimensional volumes from 19 cases of suspected TAPVC were compared to conventional echocardiography.Echocardiographic characteristics were compared with the results of postnatal work-up and pathology.Results TAPVC was found in 25 cases by fetal echocardiography,four cases were isolated TAPVC,21 TAPVC had associated cardiac anomalies.Among them,nine cases were supracardiac types,six cases were infracardiac types,nine cases were intracardiac types and one was indefinite.Three true positivecases of TAPVC were confirmed after birth.Twenty-two choose to terminate pregnancy,9 of which refused to autopsy,and 10 true positive cases of TAPVC were confirmed at autopsy while 3 were considered false-positive cases.4D ultrasound with STIC clearly visualized the anomalous PV confluence and/or the draining vertical vein in all nineteen cases examined.Conclusions STIC may be proposed to identify abnormal venous drainage at the screening level,thus supplying additional information over that provided by 2D fetal echocardiography.
10.Isolated peripheral neuropathy as an unusual presentation for an extramedullary relapse of acute leukemia

A 23-year-old man in remission from acute myeloblastic leukemia after allogeneic peripheral blood stem cell transplantation developed peripheral neuropathy presenting as sciatic and peroneal nerve deficits. Electrophysiological tests localized the lesions to the left sciatic and common peroneal nerve. Magnetic resonance imaging revealed nerve thickening and enhancement, while a positron emission tomography-computed tomography scan demonstrated increased fluorodeoxyglucose uptake tracking along the nerve, suggesting peripheral nerve infiltration. This report demonstrates an unusual presentation of acute leukemia relapse presenting as focal neuropathy
